A smile is a symbol of beauty and wellbeing in our modern society, human facial expression transcending language, culture, race, gender, time, and socioeconomic differences. An esthetic smile consists of three main components: the teeth, the lip framework, and the gingival scaffold. In some patients, the altered relationship between the teeth, the alveolar bone, and the soft tissue may result in the clinical condition known as gummy smile. A single factor or a combination of factors may be present in patients with this clinical condition, including altered passive eruption (APE), vertical maxi

INTRODUCTION: Dentists traditionally have thought of periodontal treatment as a means of saving the teeth while leaving the patient with an esthetic problem. This no longer is true. The goal of this article is to show how esthetic crown-lengthening procedures, papillary regeneration and root coverage may enhance the overall esthetic results of periodontal treatment.
METHODS AND RESULTS: Esthetic crown lengthening aims not only to provide biological width for the healthy restoration of teeth, but also to permit esthetic gingival and prosthetic contours. Papillary regeneration aims to fill the

Extensive coronal destruction following trauma or caries presents a recurring challenge in prosthodontic rehabilitation, particularly in the esthetic zone. A multidisciplinary approach combining traditional post-core systems with contemporary surgical adjuncts ensures optimal long-term outcomes. This paper reports the management of a grossly fractured maxillary central incisor using a Richmond crown following diode laser-assisted crown lengthening. A 21-year-old male patient presented with pulpal necrosis and apical periodontitis in tooth 21. After nonsurgical endodontic therapy, post space pr

Surgical procedures for the prosthodontic patient have advanced greatly over the last decade. Both the aesthetics and function of the final restoration can be greatly enhanced by various surgical procedures. Some are very predictable, and others are still in the developmental stages. This review is divided into three basic categories: aesthetic root coverage; crown lengthening; and ridge augmentation procedures.

The gingival complex plays a vital role in the overall beauty of a smile. To predictably achieve a successful esthetic and functional result, the dentist must be able to precisely predict the treatment outcome based on biologic determinants. In this article, the biologic requirements for gingival health are discussed. In addition, a differential diagnosis for excess gingival display and treatment options for this condition are discussed.

In the 1980s, mucogingival surgery evolved into periodontal plastic surgery with various techniques designed to produce root coverage in areas of marginal tissue recession, to augment deficient ridges, and to lengthen crowns in cases of excessive gingival display. Periodontal plastic surgery not only enables the dentist to reconstruct but also to regenerate lost tissues.

Forced eruption offers a method of treatment of teeth fractured close to the alveolar crest. We introduce a modification of this common technique. The fractured fragment of a patient's crown is bonded to the adjacent teeth and used as anchorage, permitting the root extrusion while offering acceptable aesthetics. The bonded crown does not impede the eruption of the root so that the frequent occlusal adjustment of other methods is not required. The final result is acceptable and a low cost alternative to common techniques.

The biologic width (BW) includes attached epithelial cells and connective tissue attachment complex being very important in the periodontal health during prosthetic treatments as invading this zone can cause bone resorption and gingival recession. The present study investigated biologic width values in the normal periodontium in anterior and posterior teeth. 30 patients that referred from restorative department to periodontics department of Tehran University of medical sciences who need crown lengthening procedure on their teeth with no history of orthodontic, prosthodontic and periodontal tre

PURPOSE: The location of the gingival zenith in a medial-lateral position relative to the vertical tooth axis of the maxillary anterior teeth remains to be clearly defined. In addition, the apex of the free gingival margin of the lateral incisor teeth relative to the gingival zeniths of the adjacent proximal teeth remains undetermined. Therefore, this investigation evaluated two clinical parameters: (1) the gingival zenith position (GZP) from the vertical bisected midline (VBM) along the long axis of each individual maxillary anterior tooth; and (2) the gingival zenith level (GZL) of the later

One of the most frequent causes of gummy smile is an altered eruption, ie, the gingival margins are located at a more coronal position than normal. These cases can be successfully treated with a periodontal surgical approach. Several techniques may be used, depending on the different anatomic variations. The surgery must be carefully planned so as to avoid discrepancies between the soft tissues and the teeth. Clinical examination and radiographic evaluation will allow clinicians to determine the position of the gingival margin, bone crest, and cemento-enamel junction and thus elicit a correct

The purpose of this article is to discuss how the periodontist can aid the restorative dentist in achieving the best prosthetic result. The procedures discussed include: surgical crown lengthening to provide access to subgingival preparation margins, esthetic gingival contouring to blend gingival architecture with tooth shape, esthetic root coverage by gingival grafts, grafting to prevent loss of alveolar plates in extraction sockets and preservation or correction of the alveolar ridge.

OBJECTIVE: To describe papilla-preserving gingival fiber retention osseous surgery (PP-FibReOS), a biologically driven resective approach intended to eliminate periodontal pockets while maintaining interproximal connective tissue and papillary stability in esthetically demanding maxillary anterior sites.
CLINICAL TECHNIQUE: PP-FibReOS was implemented in two challenging anterior maxillary scenarios: functional crown lengthening and surgical pocket elimination in a stage IV, grade C periodontitis case. The key feature of this protocol uses a split-full-split buccal flap with selective coronal t

BACKGROUND: A clinical scenario in the esthetic zone is the presence of short clinical crowns, which, in the absence of occlusal wear and other injuries, is referred to as altered passive eruption (APE). The presence of APE can lead to esthetic issues, which can impact the patient's smile harmony. In esthetically demanding cases, APE must be addressed prior to or in conjunction with implant therapy, and failure to do so can result in esthetic complications and failure in prosthetic planning. This case report exhibits a digital workflow in treating such patients to achieve optimal outcomes.

Mucogingival surgery currently encompasses surgical management of many defects thought to be untreatable when the term mucogingival surgery was introduced in the 1950s. Mucogingival surgery has now expanded beyond the treatment of recession and problems associated with attached gingiva. Additionally, it would now include correction of ridge form, exposing unerupted teeth for orthodontic treatment, crown lengthening for esthetic purposes, and frenal surgery. With this in mind, mucogingival surgery has been redefined as "periodontal plastic surgery."

This case report describes endodontic, restorative, and periodontal treatment of a complicated crown-root fracture of the right maxillary fourth premolar tooth in a dog. A buccal portion ('slab') had separated from the tooth, which extended subgingivally into root structure. Following completion of standard root canal therapy, a periodontal flap was elevated. Alveolectomy and alveoloplasty were performed, and inflamed soft tissues were debrided. The fracture site was prepared, restored, and shaped to receive a dental bulge contour. A gingival collar expansion technique was utilized to allow fo
