Impact of traumatic muscle crush injury as a cause of cardiomyocyte-specific injury: an experimental study.

BACKGROUND: Muscle crush injury commonly occurs after earthquakes, collapse of buildings and sjambok beatings, and it often induces crush syndrome if not treated promptly. The general manifestation of crush syndrome is the presence of myoglobinuria with acute renal failure. However, whether cardiomyocyte injury is induced after muscle crush injury has not been investigated. The aim of this study was to observe the effects of muscle crush injury on cardiomyocyte injury and its relationship to the changes of ANP and ET-1 levels after muscle crush injury in rats. METHODS: Muscle crush injury was

Crush injuries to the lower limbs at a major UK trauma centre: a retrospective observational study.

PURPOSE: Crush injuries result from the physical compression of muscles and may lead to crush syndrome. Early fluid resuscitation and surgical intervention is key. Few studies have reported the outcomes of crush injuries in the non-disaster setting. This retrospective study aims to characterise such cases. METHODS: Patients with lower limb crush injuries were identified from an internal database. Non-crush injuries and patients under the age of 18 were excluded. Types of injuries, management, and complications were extracted. RESULTS: 27 patients were included. The right leg (n = 10) was the

Reconstruction of skin and soft-tissue defects in crush-injuries of the lower leg in children.

Severe crush injuries with open comminuted fractures of the lower extremity often result in extensive tissue loss. In such a situation early free microsurgical tissue transplantation offers most effective possibilities of treatment. Among others, two of the most eminent advantages of free microsurgical tissue transplantation are: 1. The possibility of tissue replacement which cannot be achieved by traditional procedures such as functional muscle transplantation or the replacement of extremely extensive tissue loss; moreover immediate or early bone coverage by well vascularized soft-tissue is m

Tibial fractures associated with crush injuries to the soft tissues of the dorsal foot in children.

We retrospectively studied 15 children with tibial fractures associated with crush injuries to the soft tissues of the dorsal foot. The fractures, including six open fractures, were united with no complications within an average of 11.1 weeks. Wound closure to treat crush injuries of the dorsal foot was achieved using split- or full-thickness skin grafts in most patients. The outcomes of these grafts were acceptable, and all skin coverage was successful and remained viable with no breakdown. Extensor tendon injuries of the foot sustained by eight patients could not be sutured or repaired due t

Split-thickness skin excision: its use for immediate wound care in crush injuries of the foot.

Split-thickness skin excision (STSE) was used as an adjunctive modality in the treatment of eight crush injuries of the foot. Compartment syndromes were present in four feet and were treated with fasciotomy. Wound debridement, internal fixation of fractures, and STSE followed. This technique accurately determined the viability of the skin flap, simultaneously providing skin for local wound coverage. All flaps treated in this manner survived and all (100%) of the degloved STSE grafts healed. Additional procedures were performed in four patients (two free flaps and two split-thickness skin graft

Effects of exercise on muscle reinnervation and plasticity of spinal circuits in rat sciatic nerve crush injury models with different numbers of crushes.

INTRODUCTION/AIMS: Motor function recovery is frequently poor after peripheral nerve injury. The effect of different numbers of nerve crushes and exercise on motor function recovery is unknown. We aimed to examine how different numbers of crushes of the rat sciatic nerve affects muscle reinnervation and plasticity of spinal circuits and the effect of exercise intervention. METHODS: Single and multiple sciatic nerve crush models with different numbers of crushes were created in rats. Treadmill exercise was performed at 10 m/min for 60 min, five times a week. Muscle reinnervation and synaptic c

Predicting recovery trajectories and injury severity following partial crush spinal cord injury in mice.

The partial crush spinal cord injury (SCI) model enables preclinical testing of experimental therapies in mice, but substantial inter-animal variability in recovery outcomes confounds efficacy assessments. Here, we used open field behavioral data collected during the first 3 days post partial thoracic SCI to generate an Acute Functional Score (AFS) that defined three subgroups with divergent recovery trajectories. Applying latent class growth analysis and growth mixture modeling to open field and grid walk testing data, we demonstrated 83-92% prediction accuracy for AFS-defined recovery trajec

Plantar-to-plantar punch skin grafts for a wound caused by crush injury.

BACKGROUND: Skin grafting of the sole of the foot poses a special challenge because it is subjected to friction and shear. Various kinds of grafts have been used throughout history for this purpose based on the features of the donor site and graft thickness. Given these unique characteristics, as well as the need to withstand pressure bearing and friction, the best grafts should be obtained from identical skin. CASE REPORT: A patient with a crush wound of the plantar and dorsal region of the left foot underwent combined treatment with plantar-to-plantar punch grafting and split-thickness skin

Neuroregenerative effects of cardamonin in a rat model of sciatic nerve crush injury.

BACKGROUND AND AIMS: Peripheral nerve injuries (PNIs) cause motor and sensory dysfunction. This study assessed cardamonin's effectiveness in enhancing sciatic nerve regeneration and functional recovery in a rat model of nerve crush injury. METHODS: Male rats underwent unilateral sciatic nerve crush and received graded doses of cardamonin (5, 10, 20 mg/kg). The study also included a Sham surgery and a Crush control group. Motor function was assessed with the Sciatic Functional Index (SFI) and sensory function using the Hargreaves test. Electrophysiological measurements, including compound musc

Spontaneous resolution of traumatic bronchial tear after thoracic crush injury.

Traumatic bronchial tears are rare life-threatening injuries. Here, we report a 28-year old male who presented after sustaining a crush injury to his thoracic cavity, resulting in a spiral left mainstem bronchial tear secondary to high intraluminal pressure. While preparing for surgery, a preoperative bronchoscopy found that the bronchial tear had re-approximated and effectively sealed the laceration. No operative intervention was performed and the patient subsequently underwent a full recovery. While most bronchial tears undergo surgical intervention, our report describes the successful manag

Hyperbaric oxygen therapy for the treatment of a crush injury of the hand: a case report.

We describe a case of hyperbaric oxygen therapy (HBOt) as an adjunct to treatment of a crush injury to the hand. A 34-year-old male paramedic was involved in a motor vehicle accident and admitted for diagnosis and surgical treatment. He sustained a crush injury to his right hand and presented with significant muscle damage, including multiple fractures and dislocations, an avulsion injury of the flexor tendons, and amputation of the distal phalanx of the little finger. He underwent reconstructive surgery and received HBOt over the following days. In the following 2 months, he lost the distal a

The susceptibility of cardiac arrhythmias after spinal cord crush injury in rats.

High-level spinal cord injury (SCI) often interrupts supraspinal regulation of sympathetic input to the heart. Although it is known that dysregulated autonomic control increases the risk for cardiac disorders, the mechanisms mediating SCI-induced arrhythmias are poorly understood. Here, we employed a rat model of complete spinal cord crush injury at the 2nd/3rd thoracic (T2/3) level to investigate cardiac rhythm disorders resulting from SCI. Rats with T9 injury and naïve animals served as two controls. Four weeks after SCI, rats were implanted with a radio-telemetric device for electrocardiogr

The effects of 4-aminopyridine and methylprednisolone on recovery of the facial nerve crush injury.

OBJECTIVE: 4-Aminopyridine (4-AP) is a potassium channel blocker that enhances nerve excitability. In this study, rat models that have facial nerve crush injury (FNCI) were grouped and treated with methylprednisolone (MP), 4-AP, and a combination of these two drugs. Electrophysiologic and histopathologic outcomes of these groups will be compared with a control group. MATERIALS AND METHODS: Thirty healthy male Wistar rats (mean weight of 265 g) were used in this study. The rats were randomly divided into five groups with six subjects in each: Group 1 (sham group), Group 2 (control group), Grou

Electromyography and ultrasonography in the diagnosis of a rare double-crush ulnar nerve injury.

Reported here is a 46-year-old man who was seen for pain, numbness, and weakness in his left upper limb and hand. Electromyographic studies demonstrated denervation of ulnar-innervated muscle groups except for the flexor carpi ulnaris. A localized nerve conduction block could not be depicted because of severe axonal loss. Ultrasonographic evaluation showed enlargement of the ulnar nerve at 2 sites: at the level of the epicondylar groove and the inside of the flexor carpi ulnaris muscle. Herein, we would like to emphasize the complementary role of an ultrasound in peripheral nerve pathologies,

Nimodipine and acceleration of functional recovery of the facial nerve after crush injury.

OBJECTIVE: To establish whether nimodipine, a calcium channel blocker, accelerates or otherwise improves functional recovery of whisking after facial nerve crush injury in the rat. METHODS: Thirty rats underwent exposure of the left main trunk of the facial nerve followed by a standard crush injury and subsequent quantitative facial movement testing. Animals were randomized into an experimental group (n = 15) and a control group (n = 15). Four days prior to facial nerve manipulation, experimental animals underwent subcutaneous implantation of a nimodipine-secreting pellet. All animals were te

Interretinal transduction of injury signals after unilateral optic nerve crush.

In this study, we report that partial unilateral optic nerve crush in the rat affects the number of retinal ganglion cells of the contralateral eye still in continuity with the ipsilateral superior colliculus. The reduction in cell number of the uncrossed retinal projection was accompanied by a microglia response and could be prevented by the local intravitreal application of the anti-inflammatory agent dexamethasone. Interestingly, the level of neuronal activity after optic nerve crush as evidenced by thallium autometallography was enhanced in the termination area of the uncrossed projection,

Effect of severe crush injury on axonal regeneration: a functional and ultrastructural study.

In the present study, we performed sciatic nerve compression for 30, 45, and 60 minutes by the Yasargil aneurysm clip and observed recovery on postoperative days 14, 28, and 42. We observed that as compress time increases, functional recovery and morphological regeneration of crushed sciatic nerve needs longer time. Additionally, we showed which cells are durable in nerve regeneration after crush injury. Nerve regeneration or recovery depends on trauma durations in which a longer recovery time is needed after longtime pressure. Although Schwann cells are found to be resistant and this might be

Perforation of the lower thoracic oesophagus following crush injury to the chest and abdomen.

We report a case of perforation of the lower thoracic oesophagus following a crush injury to the chest and upper abdomen. A laparotomy was performed for abdominal injuries, and appropriately placed drains resulted in complete resolution of the oesophageal leak, 21 days following the injury. This case report demonstrates that a conservative approach to lower thoracic oesophageal perforations can be carried out successfully without the added morbidity of a thoracotomy, or risks of a direct repair.
