Neurocritical Care Coding for Neurologists.

Coding specifies the work performed when providing patient care. Critical care services mostly use code 99291, and other codes specify additional time and procedures. Current Procedural Terminology defines critically ill as "a high probability of imminent or life-threatening deterioration in the patient's condition," a condition necessary for use of the critical care code. A patient may be critically ill for neurologic reasons even when stable from a cardiorespiratory status. Rules govern who can use these codes, whether they can be used by more than one physician, the locations where the code

Classification of GLOSSECTOMIES: Proposal for tongue cancer resections.

BACKGROUND: Surgery of tongue tumors includes different procedures ranging from mucosal resection to complex combined resection. Numerous terms have been used to describe such procedures, but there is no consensus between the terminology and the extent of resection. METHODS AND RESULTS: We searched the medical literature and found a lack of published information. We undertook to describe a new classification of surgical procedures for tongue tumor resection. We based it upon the surgical anatomy of the tongue and the spread of the cancer. We posited that there were five major types of glossec

The application of a computerised diagnostic index for ophthalmic practice.

All 2792 patients seen in my private practice between July 1989 and September 1992 and receiving a diagnosis were coded according to the International Classification of Diseases-9 (Clinical Modification), 1979. Any procedures performed were coded according to the American Medical Association Current Procedural Terminology. Patient details and codes were filed by computer using a customised database in Foxpro 2.0 software. Those with minor refractive error or presbyopia alone and those who had no abnormality were excluded from the database. The set-up and maintenance of this diagnostic index is

Implementation and evaluation of a performance improvement intervention to address physician documentation deficiencies in abdominal ultrasound.

PURPOSE: For ultrasound reports to meet criteria for coding as abdomen complete (USABC), 8 elements are required: liver, bile ducts, gallbladder, spleen, kidneys, pancreas, inferior vena cava (IVC), and aorta. Failure to document all 8 results in coding as ultrasound abdomen limited. The purposes of our study were to identify deficiencies in documentation, implement a performance improvement intervention to address deficiencies, and evaluate the intervention. METHODS: In the first phase, 50 consecutive USABC reports performed as part of routine medical care were retrospectively analyzed for t

Optimizing identification of major adverse limb and cardiovascular events among adults with peripheral artery disease in administrative data.

OBJECTIVE: Atherosclerotic cardiovascular diseases (ASCVD) are the commonest cause of death. Peripheral artery disease (PAD) is an ASCVD that significantly increases risk of death and reduces quality of life; however, characterization of its consequences is limited because it requires manual adjudication of major adverse cardiovascular events (MACEs) and major adverse limb events (MALEs). Hence, we developed and validated a system to quantify MACE and MALE using administrative data. METHODS: In a multihospital single health care system (2016-2023), we identified adult index vascular surgery c

Subtypes and service utilization among opioid use disorder patients at a community health center: findings from a medically underserved urban area of the Northeastern United States.

BACKGROUND: Opioid use disorder often co-occurs with other mental health and substance use disorders. Identifying clusters of individuals receiving treatment for opioid use disorder based on co-diagnosed conditions, healthcare plans, and service utilization over a seven-year treatment period provides insight into service needs. Objectives included [1] characterizing the sample [2], examining subtypes of the sample using cluster analysis, and [3] identifying differences in Current Procedural Terminology by subtype to examine service utilization among identified clusters. METHODS: This study us

Comparison of Complication Risk Following Trigger Digit Release Performed in the Office Versus the Operating Room: A Population-Based Assessment.

PURPOSE: Trigger digit release (TDR) performed in an office-based procedure room (PR) setting minimizes surgical costs compared with that performed in an operating room (OR); yet, it remains unclear whether the rates of major complications differ by setting. We hypothesized that surgical setting does not have an impact on the rate of major complications after TDR. METHODS: Adult patients who underwent isolated TDR from 2006 to 2015 were identified from the MarketScan commercial database (IBM) using the provider current procedural terminology code 26055 with a concordant diagnosis on the same

Clinical Resource Use in Patients Undergoing Intradetrusor Abotulinum Toxin A Injections for Neurogenic Compared to Non-Neurogenic Indications-The Potential for Compensation Inequities for Urologic and Urogynecologic Practices.

INTRODUCTION: Intradetrusor abotulinum toxin A (BoNT-A) injections effectively manage urinary urgency, frequency, and urinary incontinence in patients with both overactive bladder (OAB) and neurogenic lower urinary tract dysfunction (NLUTD). However, NLUTD patients often require increased staffing support and extended procedure times compared to OAB patients. This study aims to quantify resource utilization differences between the two populations undergoing cystoscopic BoNT-A treatment where only one Current Procedural Terminology Code (CPT 52287) exists. METHODS: We prospectively collected d

Patient out-of-pocket expenses in major orthopedic procedures: total hip arthroplasty as a case study.

Total hip arthroplasty (THA) is a successful procedure for improving quality of life. There are few publications regarding out-of-pocket expenses incurred by individuals undergoing uncomplicated THA. Detailed billing and record reviews and patient phone surveys were conducted on 34 Medicare patients identifying charges and reimbursements recorded by the health care system and all out-of-pocket expenses incurred by patients undergoing uncomplicated THA (diagnostic related group [DRG] 209/current procedural terminology [CPT] 27130). Mean THA total billed charges were $46378, 89% of that from inp

Neurofibromatosis Type I: Consensus on Current Terminology for Neurofibromas, as a Basis for Recommendations on the Use of MEK Inhibitors.

Neurofibromatosis type 1 (NF1) is characterized by the development of neurofibromas, including cutaneous neurofibromas and plexiform neurofibromas (PNFs). Despite more than a century of clinical recognition, the terminology used to describe these tumors remains inconsistent across disciplines, particularly in pathology, radiology, and clinical practice. This lack of standardized nomenclature complicates communication, diagnostic assessment, risk stratification, and therapeutic decision-making. In this multidisciplinary consensus paper, experts from the German NF Working Group sought to harmoni

Comparative Analysis of Work Relative Value Units by Procedural Complexity in the Surgical Treatment of Ankle Fractures and Syndesmosis Injury: A Propensity Score-matched Study.

BACKGROUND: Unimalleolar, bimalleolar, and trimalleolar ankle fractures and syndesmosis injuries represent a spectrum of increasing instability and surgical complexity. Although the work Relative Value Unit (wRVU) system is designed to account for surgical time and technical complexity, previous studies suggest a mismatch between procedural complexity and proportional reimbursement rate. Our study aims to evaluate whether current wRVU allocation across ankle fracture types adequately reflects procedural complexity and to assess whether more complex cases are relatively undercompensated. Association of Neurosurgical Work Relative Value Units with Objective Markers of Operative Complexity.

OBJECTIVE: Relative value units (RVUs) form the backbone of health care service reimbursement calculation in the United States. However, it remains unclear how well RVUs align with objective measures of procedural complexity within neurosurgery. METHODS: The 2018 American College of Surgeons National Surgical Quality Improvement Program database was queried for neurosurgical procedures with >50 patients, using Current Procedural Terminology (CPT) codes. Length of stay (LOS), operative time, mortality, and readmission and reoperation rates were collected for each code and a univariate correlat
