Case Report: Cyclodialysis Cleft in a Case of Open-globe Injury and Role of Swept-source Anterior Segment Optical Coherence Tomography in Diagnosis.

SIGNIFICANCE: Cyclodialysis clefts can potentially develop secondary to open globe injury. The swept-source anterior segment optical coherence tomography (SS-AS-OCT) may be a valuable diagnostic tool for the identification and estimation of the circumferential extent of cyclodialysis clefts. It could be considered an alternative when ultrasound biomicroscopy cannot be performed successfully. PURPOSE: The purpose of this study was to report a case of open-globe injury associated with cyclodialysis cleft and the utility of SS-AS-OCT in its diagnosis. CASE REPORT: A 12-year-old boy presented to

[Ultrasound biomicroscopy as a criterium of functional assessment of the supra-choroidal cleft after chamber angle surgical interventions].

BACKGROUND: The new technique of ultrasonic biomicroscopy provides detailed information about the extension and width of suprachoroidal clefts during the follow-up period after cyclodialysis ab interno. PATIENTS AND METHODS: We examined a group of 9 patients suffering from dysgenetic glaucoma and secondary glaucoma. Ultrasonic biomicroscopy was performed before and after cyclodialysis ab interno. The results were compared to those of gonioscopy. RESULTS: These examinations revealed that even cyclodialysis clefts which appeared to be closed by gonioscopy were still open. This was proven by a

Ultrasonic biomicroscopic evaluation of cyclodialysis before and after direct cyclopexy.

OBJECTIVES: To investigate the clinical manifestations and surgical prognoses after direct cyclopexy in patients with traumatic cyclodialysis according to the cleft extent as determined by ultrasound biomicroscopy (UBM). METHODS: A detailed ophthalmologic examination, which included gonioscopy and UBM, was performed before and after direct cyclopexy in 32 eyes of 31 patients with traumatic cyclodialysis clefts. RESULTS: Cyclodialysis clefts were accurately diagnosed and delineated in all 32 eyes using UBM. Cyclodialysis resulted in hypotony with a mean intraocular pressure of 3.2 mm Hg irres

[Filtering bleb after surgical cyclodialysis].

CASE REPORT: A 35-year-old man with degenerative myopia suffered blunt ocular trauma due to being hit in the face by a ball. The injury caused four cyclodialysis clefts in his left eye which resulted in hypotony and chorioretinal folds. DISCUSSION: The persisting hypotony was unsuccessfully treated with topical therapy and with laser photocoagulation. Therefore, a surgical treatment with direct cyclopexy was performed. In one of the cyclodialysis clefts, incarcerated vitreous was found, and this was considered to be the reason for its incomplete closure. In the postoperative period complete c

Intraocular endoscopy for the evaluation and treatment of hypotony due to a traumatic cyclodialysis: a case report.

BACKGROUND: A cyclodialysis cleft often leads to direct communication between the anterior chamber and the suprachoroidal space. It is a rare condition that is encountered with blunt trauma, and less commonly, after surgery. Hypotony is the major sequelae that may lead to hypotonous maculopathy, optic disc edema, corneal folds, and astigmatism. These may cumulatively lead to visual loss. We describe how endoscopy in a cyclodialysis repair allowed us to accurately locate the cleft and guided its appropriate management avoiding unnecessary cryopexy. CASE PRESENTATION: A 41-year-old male experie

[Comparative experimental morphological study of the impact of various drainages for cyclodialysis to the adjacent tissues].

The investigation was made on 8 eyes (4 rabbits) undergone cyclodialysis with the hydrogen or collagenic drainage being introduced into the supraciliary cleft. Comparative pathohistological study indicated the evident advantages of a hydrogel implant ensuring the drainage function of the formed cyclodialysis cleft throughout the experiment (5 weeks). By contrast, subject to resolution, the collagenic drainage spurred the early development of young connective tissue in the cyclodialysis cleft just on day 14 after implantation. Complete atresia of the cyclodialysis cleft and cessation of its dra

Hypotony due to inadvertent cyclodialysis after intraocular lens implantation.

Six patients had hypotony from inadvertent cyclodialysis after intraocular lens implantation. Factors that may have contributed to the formation or persistence of the cyclodialysis are scleral cataract incision, prolonged postoperative course of corticosteroid treatment, intraocular lens weight, iridectomy, and maneuvers related to insertion and fixation of the intraocular lens. Closure of these cyclodialysis clefts may be a lengthy process involving various treatment modalities. Fortunately, cyclodialysis with hypotony, although increased in incidence with intraocular lens surgery over routin

Encircling Scleral Buckling for Traumatic Severe Hypotony.

The authors report the use of an encircling scleral buckling procedure for the management of severe hypotony secondary to traumatic annular ciliochoroidal detachment (CCD) with cyclodialysis cleft. Medical records of patients with severe ocular hypotony were retrospectively reviewed. Four patients with traumatic annular CCD with cyclodialysis cleft were identified. Diagnosis of CCD was documented by ultrasound biomicroscopy and presence of cyclodialysis cleft was confirmed by gonioscopy or ultrasound biomicroscopy. All patients underwent scleral buckling surgery with an encircling band for ann

Direct cyclopexy for traumatic cyclodialysis with persisting hypotony. Report in 29 consecutive patients.

BACKGROUND: Traumatic or postoperative cyclodialysis frequently is associated with persisting ocular hypotony, causing morphologic changes and visual loss. METHODS: The authors retrospectively analyzed the data of 29 eyes of 29 patients who underwent consecutive direct surgical cyclopexy for hypotonus cyclodialysis between 1980 and 1993 at the authors' institution. Cyclopexy was performed by directly suturing the ciliary body to the scleral spur under a scleral flap. RESULTS: The cyclodialysis clefts were posttraumatic (26 eyes) or postsurgical (3 eyes), extended for 3.6 +/- 1.7 clock hours

Modified cyclodialysis suturing with sodium hyaluronate for traumatic choroidal avulsion and cyclodialysis: a retrospective study.

BACKGROUND: Traumatic choroidal avulsion combined with cyclodialysis cleft is a severe form of ocular trauma that frequently leads to profound vision loss and persistent hypotony. Conventional surgical approaches often fail to achieve stable anatomical reattachment. This study aimed to evaluate the efficacy and safety of a modified cyclodialysis suturing technique combined with sodium hyaluronate injection for repairing traumatic choroidal avulsion and cyclodialysis cleft. METHODS: This retrospective study included 12 patients (mean age: 47.17 ± 15.84 years) with choroidal avulsion and ciliary

Experimental use of space-retaining substances with extended duration: functional and morphological results.

BACKGROUND: In glaucoma surgery, e.g. cyclodialysis ab interno, to date no viscous substances have been available that stay in the cyclodialysis cleft long enough to prevent scarring. We examined the qualities of two different viscoelastic substances that stay longer in the place of instillation than hyaluronic acid. METHODS: We performed cyclodialysis ab interno in 12 eyes of six owl monkeys. The right eyes were treated with the gel with the higher molecular weight, the left eyes using the gel with the lower molecular weight. The baseline data of intraocular pressure and outflow facility wer

Trabeculectomy Augmented with Limited Deep Sclerectomy and Cyclodialysis with Use of Scleral Tissue as a Spacer.

Trabeculectomy remains the most commonly performed surgery for medically uncontrolled glaucoma. Its success in primary open angle glaucoma is approximately 82% in the initial year after surgery and 64% at the end of five years. Lower success rates have been found in secondary glaucomas like neovascular glucoma, uvietic glaucoma, post-traumatic glaucoma, and for repeat surgeries. To illustrate improvement of the efficacy of trabeculectomy, enhancement with cyclodialysis has been introduced. This involves the creation of a cyclodialysis cleft in a controlled manner to allow additional suprachoro

Cyclodialysis: an update.

Cyclodialysis is the result of the separation of the longitudinal ciliary muscle fibers from the scleral spur, which creates an abnormal pathway for aqueous humor drainage that may lead to ocular hypotony. For many years cyclodialysis was considered a treatment option for glaucoma. However, today it usually occurs as a complication of blunt trauma or more rarely as a complication of anterior segment ocular surgery. Ocular hypotony can lead to cataract development, optic disk swelling, refractive changes, and several retinal complications, making accurate identification and timely intervention

[Surgical treatment of cyclodialysis].

PURPOSE: Cyclodialysis is a rare complication following blunt traumatic bulb injuries or surgical interventions. When treatment with cycloplegics or steroids is ineffective in attaching the ciliary body, cyclodialysis should be surgically treated. However, for reliable diagnosis and surgical therapy, an exact identification (size and extent) of the cyclodialysis cleft is imperative. Ultrasound biomicroscopy (UBM) provides the appropriate information. CASE REPORT: Four patients (19 to 65 years old, mean 45 years) with a detached ciliary body are described. Three patients had suffered an injury

Surgical reconstruction of traumatic ciliary body dialysis: a case report.

BACKGROUND: A cyclodialysis cleft is a gap resulting from disruption of the longitudinal fibers constituting the ciliary body attachment to the scleral spur. The cyclodialysis cleft can be of traumatic or iatrogenic origin, and it may occur during intraocular surgery or as a result of a glaucoma operation. In this report we present a surgical technique to treat cyclodialysis: cyclopexy combined with phacoemulsification subluxation lens, transscleral suturing of Cionni ring, and intraocular lens implantation with iris cerclage suture. CASE PRESENTATION: A 44-year-old Polish woman experienced a
