Vesicouterine fistulas: imaging findings in three cases.

OBJECTIVE: The objective of our report is to present three cases of vesicouterine fistulas secondary to a cesarean delivery, a uterine rupture during labor, and radiation therapy. The delay between the onset of symptoms and the diagnosis varied between 3 and 7 years. Different techniques such as color Doppler sonography, excretory urography, cystography, CT, MRI, cystoscopy, vaginoscopy, and hysterography were performed with variable results, mostly negative and sometimes undefined. CONCLUSION: The definitive diagnosis was made with contrast-enhanced helical CT after cystography in one case,

Ureteral endometriosis: clinical and radiological follow-up after laparoscopic ureterocystoneostomy.

BACKGROUND: Ureteral endometriosis is a rare entity that may lead to progressive hydroureteronephrosis and renal loss. When the localization of ureteral stenosis is close to the ureterovesical junction, ureterocystoneostomy may be required. The aim of the present study was to evaluate post-operative complication rates and clinical outcomes at 1- and 6-month follow-up after laparoscopic ureterocystoneostomy. METHOD: Twenty patients who underwent ureterocystoneostomy for pelvic endometriosis in our tertiary referral centre for endoscopic surgery during 1 year were studied. A cystography was per

Comparison of radiographic methods for detecting radiolucent uroliths in dogs.

The purpose of this study was to evaluate and compare positive cystography techniques at 5%, 10%, and 20%, as well as three different double-contrast protocols for detecting radiolucent uroliths with a diameter of less than 3.0 mm in dogs. Six cadavers were used, one was selected at random to represent the negative control, and the others were submitted to urolith implantation in the bladder by urethral catheter. Three radiology professionals blindly accessed ventrodorsal and -lateral projections of each test. Contrast at 20% showed greater diagnostic sensitivity, but with greater difficulty i

The impact of bladder neck mucosal eversion during open radical prostatectomy on bladder neck stricture and urinary extravasation.

PURPOSE: To determine whether the bladder neck mucosal eversion (BNM-eversion) during radical retropubic prostatectomy (RRP) reduces the risk of bladder neck stricture (BNS) and of peri-anastomotic extravasation (PAE) in postoperative cystography. METHODS: Two hundred and eleven patients with clinically localized prostate cancer underwent RRP and were prospectively randomized into patients with BNM-eversion (group I) and without BNM-eversion (group II). All patients underwent an evaluation of PAE by retrograde cystography on postoperative day 8. We assessed BNS after 6 months. RESULTS: Ninet

Bladder diverticula in patients with prostatism.

The incidence of bladder diverticula was investigated in a patient material referred for prostatism. Out of 104 referred patients 69 were examined by cystography and urography. Fifty-one per cent had diverticula on cystography but only 30% on urography. Diverticula were significantly related to age. They were significantly associated with upper tract dilatation and tended to relate to residual urine volume and bladder instability. There was no relation to the bladder neck-verumontanum distance, maximal flow rate or bladder capacity.

Contrast material in posterior vaginal fornix mimicking bladder rupture: CT features.

Abdominopelvic CT of a 22-year-old woman who sustained blunt abdominal trauma revealed a collection of contrast material posterior to the bladder. This CT appearance was interpreted as evidence of bladder rupture, despite previously performed normal retrograde cystography. Subsequent urologic studies confirmed the absence of bladder injury. Retrospective review of CT revealed that collection was within the vagina. This case demonstrates an important false-positive CT diagnosis of bladder rupture and confirms the importance of retrograde cystography for the diagnosis of bladder rupture.

Assessment of the Anastomosis after Radical Prostatectomy: A Review of Available Diagnostic Methods.

INTRODUCTION: After radical prostatectomy, many institutions perform cystography to exclude vesicourethral anastomotic leakage before removing a urethral catheter. We reviewed diagnostic methods to exclude leakage compared to the reference standard cystography. METHODS: We performed systematic literature review to summarize the published options and outcomes for assessment of vesicourethral anastomotic leakage after radical prostatectomy. RESULTS: Of 2,137 publications, 45 full-text manuscripts underwent full-text screening, of which 9 studies contributing 919 patients were included. Seven s

Diagnostic performance of posterior: Anterior urethra ratio on postoperative micturating cystourethrogram for residual posterior urethral valves: A systematic review and meta-analysis.

BACKGROUND: Posterior urethral valves (PUV) remain an important contributor to childhood chronic kidney disease. After endoscopic valve ablation, residual valves may perpetuate obstruction and worsen renal and bladder outcomes. The posterior: anterior urethral ratio (PAR) on postoperative micturating cystourethrogram (MCUG) is a simple quantitative measure that may help detect residual valves, but reported thresholds and accuracies vary widely. We aimed to evaluate the diagnostic performance of PAR on postoperative MCUG for detecting residual PUV, using cystoscopy as the reference standard. M

Tamsulosin reduces the incidence of acute urinary retention following early removal of the urinary catheter after radical retropubic prostatectomy.

OBJECTIVES: To determine the efficacy of tamsulosin in preventing acute urinary retention following early catheter removal after radical retropubic prostatectomy. METHODS: Between February 2000 and October 2000, cystography was performed on postoperative day 7 after radical retropubic prostatectomy by a single surgeon (group 1). Between September 2001 and August 2002, cystography was performed on postoperative day 8 after radical retropubic prostatectomy by the same surgeon (group 2). The protocol for performing cystography and assessment of extravasation was similar for both groups. Tamsulos

[Evaluation of preoperative bladder tumor staging].

Twenty-eight out of 90 patients with bladder tumor seen from 1979 to 1982 were examined by CT, pelvic angiography, cystography and bimanual digital palpation for staging diagnosis preoperatively. Twenty-two patients underwent total cystectomy, and the other 6 patients underwent partial cystectomy. Preoperative staging (T) and postoperative histopathological staging (pT) were compared to define the diagnostic accuracy. The accuracy rate with CT was 20.0%, that with pelvic angiography, 29.4%, that with cystography 33.3%, and that with bimanual digital palpation 47.1%. Digital palpation had a bet

Urinary tract infection in premature infants: the role of imaging studies and prophylactic therapy.

BACKGROUND: The prevalence of urinary tract infection (UTI) in premature infants ranges from 4% to 25%. It is surprising, however, that scant information exists regarding management of UTI in premature infants, particularly the need for radiologic evaluation of the urinary tract and the use of preventive antibiotic therapy after the first episode of UTI occurs. The aim of this study was to answer these questions. PATIENTS AND METHODS: Twenty-seven (8%) premature infants (< 1750 gm birth weight) born during the period from 1990 through 1993 had UTI. Eleven of them were of extreme low birth wei

[Ultrasonographic diagnosis of massive bladder hernia at the inguinoscrotal level: report of a case].

We present a case of massive inguinoscrotal bladder hernia in a 58 years-old man with a left inguinal hernia. It was diagnosed by ultrasound during evaluation of a mass in the left inguinal region extending to the scrotum. Cystography later confirmed the ultrasound findings but provided no new information. Although filling cystography is considered to be the gold-standard radiological diagnostic procedure for this entity I the use of ultrasound should not be discarded. Firstly I because direct and indirect ultrasonographics signs characterising inguinal bladder hernias have been reported, and

An uncommon and easily overlooked case: Delayed intraperitoneal bladder rupture following blunt trauma: A case report and review of the literature.

INTRODUCTION: Delayed intraperitoneal bladder rupture is a rare clinical occurrence, frequently overlooked and misdiagnosed due to its nonspecific clinical manifestations. However, literature provides only a limited number of cases reporting delayed intraperitoneal bladder rupture resulting from blunt abdominal injury. PATIENT CONCERNS: A 72-year-old female pedestrian was struck by a vehicle and experienced sudden, severe abdominal pain on the 8th day following the injury. Abdominal B-ultrasound revealed a significant accumulation of peritoneal effusion. The abdominal puncture retrieved seros

Radiographic assessment of the vesicourethral anastomosis directing early decatheterization following nerve-sparing radical retropubic prostatectomy.

Early decatheterization directed by postoperative gravity cystography in 55 consecutive radical prostatectomy patients is described. The catheter-free status was 22 per cent by postoperative day 8, 62 per cent by postoperative day 11 and 80 per cent by postoperative day 14. Cystograms performed beginning on postoperative day 7 identified 3 groups of patients and dictated their management: 1) no extravasation resulting in immediate catheter removal (36 patients), 2) moderate extravasation requiring repeat cystography leading to decatheterization by postoperative day 15 (9 patients) and 3) sever

Isolated Ascites on CT After Blunt Trauma: A Sign of Intraperitoneal Bladder Rupture.

We report a case of intraperitoneal bladder rupture in a 24-year-old man who was struck by a motorcycle. Initial contrast-enhanced CT scan shortly after presentation to our emergency department demonstrated simple free fluid within the upper abdomen and pelvis. Delayed CT scan of the pelvis showed contrast extravasation into the perineal cavity. CT cystography showed rupture of the bladder dome with active contrast extravasation. This case illustrates that intraperitoneal bladder rupture should be considered as an etiology for otherwise unexplained ascites after blunt abdominal trauma. Delayed

IMAGING DIAGNOSIS-RADIOGRAPHY AND ULTRASONOGRAPHY OF BILATERAL CONGENITAL URETEROVESICAL JUNCTION STENOSIS CAUSING HYDRONEPHROSIS AND HYDROURETER IN A FERRET (MUSTELA PUTORIUS FURO).

An 8-month-old neutered male ferret was presented for vaccination and preventive treatment for adrenal gland disease. Abdominal ultrasound revealed severe bilateral hydronephrosis and hydroureter. Excretory urography and negative contrast cystography showed bilateral ureterovesical junction stenosis. Positive retrograde cystography demonstrated right ureteral reflux. A subcutaneous ureteral bypass was placed in the left kidney; however, it had to be removed 3 months later due to an obstruction and persistent urinary tract infection. Bilateral ureteroneocystotomy was performed. Although congeni

Bladder cancer arising in a spina bifida patient.

We report the case of a 52-year-old patient with spina bifida, neurologic bladder, and a history of recurrent urinary tract infections (UTIs) in whom a bladder cancer was incidentally discovered. Cytology, cystoscopy, and cystography showed nonspecific, extensive inflammatory lesions. Cystography demonstrated a complex of diverticulae and cellules. Pathologic examination of a diverticulectomy specimen revealed a grade III pT3b transitional and squamous cell carcinoma. Because of the similar disease causation (recurrent UTIs, stones, and indwelling catheterization), we suggest extension of the

[Imaging strategy for children after a first episode of pyelonephritis].

Pyelonephritis is a common bacterial disease in young children and is a serious infection because of its potential to produce renal scarring. One of the concerns of physicians is therefore the diagnosis of uropathy at risk for recurrence of pyelonephritis, especially high-grade reflux. There are no French recommendations on imaging evaluation after a first episode of pyelonephritis. Voiding cystography was systematically proposed years ago and recommended by the American Academy of Pediatrics until 1999. This systematic strategy exposed all children to a painful, irradiating exam, and exposed

Premedication with oral midazolam for voiding cystourethrography in children: safety and efficacy.

OBJECTIVE: Midazolam is a relatively short-acting water-soluble benzodiazepine that provides anxiolysis and anterograde amnesia and can be given orally with few adverse effects. We evaluated the benefit and safety of oral midazolam for sedation of young children during voiding cystourethrography or nuclear cystography. SUBJECTS AND METHODS: For 3.5 years, a highly selected group of 98 children, ages 23 months to 9 years (mean, 4 years), were given oral midazolam 0.6 mg/kg 20-30 min before cystourethrography or nuclear cystography. These children either had been frightened by a previous cathet
