Dantrolene inhibition of the hepatic mixed function oxidase system.

The effect of dantrolene sodium, a skeletal muscle relaxant, on the hepatic MFO system has been investigated. The dantrolene produces a dose related inhibition of the system. Single doses of 100 mg/kg and accumulative doses up to 125 mg/kg prolong pentobarbital sleeping time and decrease the rate of aminopyrine N-demethylation without altering cytochrome P450 content. However, single doses of 400 mg/kg and accumulative doses of 500 mg/kg do cause a decrease in cytochrome P450 content as part of the dantrolene induced inhibition of the hepatic MFO system.

The novel rapid formulation of intravenous dantrolene (NPJ5008) versus standard dantrolene (Dantrium®): A clinical part-randomised phase 1 study in healthy volunteers.

BACKGROUND: Delays in treating anaesthesia-induced malignant hyperthermia increase risks of complications and death. NPJ5008 is a novel formulation of the indicated treatment, dantrolene sodium, developed to shorten preparation and administration times compared with the reference formulation Dantrium®. The two formulations have been compared preclinically. OBJECTIVES: Assess bioequivalence of overall dantrolene (free acid) exposure of NPJ5008 versus Dantrium® and ascertain similarities in their pharmacokinetics and safety/tolerability profiles. Evaluate preparation/administration time savings

Cardioprotective effects of dantrolene in doxorubicin-induced cardiomyopathy in mice.

BACKGROUND: Doxorubicin (Dox) is a potent chemotherapeutic agent, but its usage is limited by dose-dependent cardiotoxicity. Intracellular calcium dysregulation has been reported to be involved in doxorubicin-induced cardiomyopathy (DICM). The cardioprotective role of RyR stabilizer dantrolene (Dan) on the calcium dynamics of DICM has not yet been explored. OBJECTIVE: To evaluate the effects of dantrolene on intracellular calcium dysregulation and cardiac contractile function in a DICM model. METHODS: Adult male C57BL/6 mice were randomized into 4 groups: (1) Control, (2) Dox Only, (3) Dan O

Role of dantrolene in dinitrophenol (DNP) overdose: A continuing question?

BACKGROUND: 2,4-Dinitrophenol (DNP) is a known uncoupler of oxidative phosphorylation that clinically results in hyperthermia, tachycardia, tachypnea, and metabolic acidosis. Overdoses of DNP are often fatal and there is no specific reversal therapy. Dantrolene interferes with calcium release in skeletal muscle and is traditionally used to treat malignant hyperthermia. There has been limited published data on its use in DNP toxicity. We present two cases of DNP toxicity that were treated with dantrolene. CASE 1: A 22-year-old male presented following an overdose of his bodybuilding supplements

Malignant hyperthermia with excellent response to small dose of dantrolene.

Malignant hyperthermia (MH) is an operative emergency associated with general anesthesia. Early diagnosis and prompt treatment are the keynotes in management of MH. Dantrolene is the only specific drug and all of the institutions where general anesthesia is a daily routine should have a stockpile of this drug for the rare occurrence of MH. Nonetheless, the enormous expenditure on stockpile and 3-year validity make a large reserve of the drug to forestall MH, a disorder of rare occurrence, seems disputable, especially in small hospitals where general anesthesia is seldom practiced. We herein re

Hemodynamic interactions of propofol and dantrolene in chronically instrumented dogs.

UNLABELLED: The hemodynamic interaction of dantrolene, a specific drug for malignant hyperthermia, and propofol which appears to be safe in malignant hyperthermia-susceptible patients, has not been investigated. We performed this study to examine the hemodynamic actions of dantrolene at a therapeutic dose during propofol anesthesia. Ten dogs were chronically instrumented for the measurements of systemic and coronary hemodynamics. The dogs were assigned to receive propofol with vehicle or dantrolene in a random manner on separate experimental days. Propofol significantly decreased mean arterial

The effects of dantrolene on the contraction, relaxation, and energetics of the diaphragm muscle.

UNLABELLED: Dantrolene is used in patients with muscle spasticity and is the only known effective treatment for malignant hyperthermia. However, its effects on muscle relaxation and energetics are unknown and may have important consequences in diaphragmatic function. We studied the effects of dantrolene (10(-8) to 10(-4) M) on diaphragm muscle strips (n = 12) in the hamster in vitro (Krebs-Henseleit solution, 29 degrees C, 95% oxygen/5% carbon dioxide) in response to tetanic stimulation (50 Hz). We studied contraction and relaxation under isotonic and isometric conditions, as well as energetic

Effect of dantrolene on leg metabolism in porcine malignant hyperthermia.

The effect of the intravenous administration of dantrolene 3.5 mg/kg body weight on leg metabolism during porcine malignant hyperthermia (MH) was investigated in six Pietrain pigs. Arterial pH improved only slowly after dantrolene and was associated with the continuing efflux of lactate from the leg. Oxygen uptake by the leg had returned to control values 30 minutes after dantrolene. Glucose production by the leg was observed during malignant hyperthermia but this was rapidly abolished by dantrolene. It is concluded that the most useful indices for assessing the adequacy of the treatment of MH

Dantrolene sodium in the treatment of malignant hyperthermia. A case report.

The pharmacogenetic syndrome of malignant hyperthermia (MH) in response to general anaesthesia, though rare, is usually fatal when it does occur. A case of anaesthetic-induced MH arising in a 10-year-old girl during intranasal antrostomy and successfully treated with dantrolene sodium (Dantrium; SKF) is described. A treatment protocol for MH is appended. The importance of an awareness on the part of the clinician together with prompt reaction to early tell-tale signs is stressed.

Interaction of dantrolene with the hepatic mixed function oxidase system.

Dantrolene pretreatment of rats (100 mg/kg/day for five days) causes a fifty percent decrease in hepatic mixed function oxidase (MFO) system activity and a fifty percent decrease in cytochrome P450 content. Recovery of hepatic MFO system activity after discontinuing dantrolene therapy is slow (only sixty-three percent recovery in ten days) and greatly exceeds the half-life of dantrolene in rats (thirty-one minutes). The inactivation of the hepatic MFO system and the slow-recovery of its activity is apparently caused by dantrolene binding and forming a stable complex with hepatic proteins. 14C-

Rationale for dantrolene vs. procainamide for treatment of malignant hyperthermia.

The use of procainamide or procaine for treatment of malignant hyperthermia is commonly recommended. The skeletal muscle relaxant dantrolene has also been indicated for treatment of this complication during anesthesia. In the present study, effects of procainamide and dantrolene were compared in malignant hyperthemia-susceptible (MHS) pigs in vivo and on MHS muscle from human patients in vitro. The ED50 for dantrolene block of indirectly evoked twitch tension was 0.85 mg/kg in MHS pigs. A final cumulative dose of 2 mg/kg resulted in 68 per cent block of the twitch response. In contrast, procai

Effects of intravenous dantrolene sodium on respiratory and cardiovascular functions.

Dantrolene sodium, a peripherally acting skeletal muscle relaxant, at doses up to 30 mg/kg iv had no effect on respiratory volume, respiratory rate, blood pressure, or heart rate in anesthetized dogs. The ED50 for inhibition of skeletal muscle contractions was 4.5 mg/kg in anesthetized dogs. In anesthetized sheep, the ED50 for skeletal muscle relaxation was 3.2 mg/kg under methoxyflurane anesthesia and 1.7 mg/kg under pentobarbital anesthesia. Unanesthetized sheep administered doses up to 30 mg/kg iv evidenced no dose-related cardiovascular effects. Respiratory volume decreased and respiratory

Probe and Assay Dependent Pharmacology of Dantrolene Analogues at the GPR35.

The G-protein-coupled receptor 35 (GPR35) plays a key role in various physiological and pathological processes and has emerged as a potential therapeutic target for the treatment of pain, inflammation, and metabolic diseases. Herein, we report the discovery and characterization of dantrolene analogues as GPR35 ligands. Pharmacological profiling with label-free dynamic mass redistribution and Tango arrestin assays showed that the 2,4-imidazolidinedione-containing analogues of dantrolene act as partial agonists of the GPR35, while its urea, thiourea, or amido derivatives display probe and assay

Approaches to Optimizing Dantrolene Neuroprotection for the Treatment of Alzheimer's Disease.

Alzheimer's Disease (AD), a neurodegenerative disorder with high incidence and mortality, is leading its way to the top of the list of the deadliest diseases without an effective disease-modifying drug. Ca2+ dysregulation, specifically abnormal release of Ca2+ via over activated ryanodine receptor (RyR), has been increasingly considered as an alternative upstream mechanism in AD pathology. Consequently, dantrolene, a RyR antagonist and FDA approved drug to treat malignant hyperthermia and chronic muscle spasms, has been shown to ameliorate memory loss in AD transgenic mice. However, the ineffi

Dantrolene Requires Mg2+ and ATP To Inhibit the Ryanodine Receptor.

Dantrolene is a ryanodine receptor (RyR) inhibitor, which is used to relax muscles in malignant hyperthermia syndrome. Although dantrolene binds to the RyR protein, its mechanism of action is unknown, mainly because of the controversial results showing that dantrolene inhibited Ca2+ release from intact fibers and sarcoplasmic reticulum (SR) vesicles, but failed to inhibit single RyR channel currents in bilayers. Accordingly, it was concluded that an important factor for dantrolene's action was lost during the purification procedure of RyR. Recently, Mg2+ was demonstrated to be the essential fa

A Prospective Repurposing of Dantrolene as a Multitarget Agent for Alzheimer's Disease.

The orphan drug dantrolene (DAN) is the only therapeutic treatment for malignant hyperthermia (MH), a pharmacogenetic pathology affecting 0.2 over 10,000 people in the EU. It acts by inhibiting ryanodine receptors, which are responsible for calcium recruitment in striatal muscles and brain. Because of its involvement in calcium homeostasis, DAN has been successfully investigated for its potential as neuroprotecting small molecule in several animal models of Alzheimer's disease (AD). Nevertheless, its effects at a molecular level, namely on putative targets involved in neurodegeneration, are st
