Necrotizing fasciitis--the importance of early diagnosis and debridement.

Necrotizing fasciitis (NF) is a potentially life-threatening bacterial infection of the skin, deep subcutaneous tissue, and fascia. Early symptoms may be misdiagnosed as cellulitis. A hallmark symptom that distinguishes NF from cellulitis is severe local pain that is out of proportion to the size and type of the wound present. Early diagnosis and treatment of NF is imperative for a patient's survival. This article describes the pathophysiologic mechanisms, clinical manifestations, and treatment of NF, as well as implications for perioperative nursing.

Healing rates in diabetes-related foot ulcers using low frequency ultrasonic debridement versus non-surgical sharps debridement: a randomised controlled trial.

OBJECTIVE: Current clinical practice varies around debridement techniques used to promote healing of diabetes-related foot ulcers. This randomised controlled study will compare healing rates for diabetes-related foot ulcers treated with low frequency ultrasonic debridement versus non-surgical sharps debridement. Individuals with diabetes-related foot ulcers being managed by podiatry at a metropolitan hospital were screened against study criteria. Eligible participants were randomly allocated to either the non-surgical sharps debridement group or the low frequency ultrasonic debridement group a

Surgical debridement and maggot debridement therapy (MDT) bring the light of hope to patients with diabetic foot ulcers (DFUs): A case report.

INTRODUCTION AND IMPORTANCE: Management of diabetic foot ulcers (DFUs), one of the complications of diabetes mellitus, can lead to death and amputation, and it is one of the most critical challenges for the patients and their families. CASE PRESENTATION: The present case report concerns a 72-year-old man with a 5-year history of uncontrolled type 2 diabetes mellitus. The patient had antibiotic-resistant DFUs on two phalanges of his left foot, which were completely gangrenous, and a superficial ulcer of 1 × 1 cm under his left foot. Despite the routine DFU care, the phalanges of his DFU were a

Efficacy of debridement alone versus debridement combined with topical antifungal nail lacquer for the treatment of pedal onychomycosis: a randomized, controlled trial.

UNLABELLED: Pedal onychomycosis is a common malady caused by dermatophytes, saprophytes, and yeasts. Traditional treatment options for this condition include toenail debridement, and pharmacological therapies that range from the application of topical agents to the oral administration of antifungal medications. In this study, 55 patients (289 toenails) were randomly allocated to either nail debridement (27 [49.09%] patients) or debridement plus application of topical antifungal nail lacquer (28 [50.91%] patients). The primary outcome was mycological cure, and secondary outcomes included foot-r

Is 18 F-FDG PET-CT-guided geographic debridement superior to conventional debridement in appendicular chronic osteomyelitis? a retrospective comparative cohort study.

BACKGROUND: Determining the surgical extent of chronic osteomyelitis and fracture-related infection (FRI) remains challenging. Adequate resection margins prevent recurrence; however, no standardised preoperative tool objectively defines resection borders. OBJECTIVE: To compare PET‑CT-guided surgical planning versus the local standard‑of‑care imaging pathway (predominantly plain radiographs, with selective CT or MRI in a minority of patients) for chronic limb osteomyelitis and FRI outcomes. METHODS: Retrospective comparative cohort study of 126 patients treated 2019-2024. Group 1: 84 patients

Analysis of debrided and non-debrided invasive squamous cell carcinoma skin lesions by in vivo reflectance confocal microscopy before and after therapy.

Hyperkeratosis hinders the application of reflectance confocal microscopy (RCM) to image squamous cell carcinoma (SCC). Not all lesions with SCC show hyperkeratosis, and these lesions can be directly imaged. However, lesions with hyperkeratosis can be treated by debriding the hyperkeratotic surface for further imaging. RCM was used to investigate patients with suspected SCC. Lesions without obvious keratosis underwent direct RCM examinations. Lesions with obvious keratosis were treated by debriding the hyperkeratotic surface. The following main RCM criteria were used to diagnose invasive SCC:

Continuous integral debridement: implementation in the Asia-Pacific region.

The health-economic burden of hard-to-heal wounds is rising across the Asia-Pacific (APAC) region, yet regional data remain sparse, particularly for venous leg ulcers and pressure injuries. Effective wound bed preparation, including management of bioburden and biofilm, is central to healing outcomes, but debridement practice across APAC is inconsistent. In May 2026, the Journal of Wound Care published the continuous integral debridement (CID) concept, built around a four-step cleanse, debride, cleanse, dress (CDCD) cycle designed to standardise debridement in everyday clinical practice. This a

Endoscopic Marsupialization With Periodic Debridement for Petrous Apex Cholesteatoma.

In cases where an expanded cholesteatoma has extensively eroded the petroclival bones, marsupialization of the cholesteatoma cavity without matrix removal through an endoscopic transsphenoidal approach is a feasible and safe option. This approach avoids CSF rhinorrhea and injury to critical structures which may be underlying and adherent to the cholesteatoma matrix, and periodic debridements address the disease satisfactorily.

Biportal Endoscopic Intramedullary Debridement for Management of Tibial Osteomyelitis.

Chronic osteomyelitis occurs if the bone infection persists for more than 6 weeks. The presence of dead bone (sequestrum) and intramedullary abscess with chronic infected tissue may protect bacteria from the endogenous host immune response and limit the effectiveness of many antibiotics. Classically, removal of infected tissue inside the medullary cavity requires extensive osteotomy to create a bone window of adequate size. Bone endoscopy (medulloscopy) provides a minimally invasive approach to the medullary cavity of long bones. The purpose of this Technical Note is to describe the details of

Can we DAIR in FRI ? Debridement techniques in osteomyelitis.

The concept of Debridement, Antibiotics and Implant Retention (DAIR) is well known in periprosthetic joint infections. Extrapolating this concept to fracture related infections is mired in controversies. Characteristics of the metal implant, duration of infection, state of fracture healing, microbiological profile etc. appear to play a role in the decision making process whether or not to keep, adjust , exchange or remove (infected) metalwork. More than likely it is the quality of source control by meticulous debridement having a major impact whether a DAIR approach to FRI could result in a su

Innovative method for wound debridement in open globe injury.

BACKGROUND: Open globe injury is a serious sight threatening condition. Full-thickness, non-selfsealing corneal lacerations require repair in the operating room. During repair, debridement of the wound is an important step. Incarceration of the intraocular structures in the wound eg. Iris, lens capsule, vitreous leads to improper healing if not removed properly. PURPOSE: To demonstrate the technique of wound debridement in open globe injury. SYNOPSIS: : Manual removal of incarcerated tissue contents leads to incomplete cleaning & enormous traction on intra ocular contents. In this video, we

Coagulation side effects of enzymatic debridement in burned patients.

OBJECTIVES: Bromelain-based enzymatic debridement has emerged as a valuable option to the standard surgical intervention for debridement in burn injuries. Adverse effects on coagulation parameters after enzymatic debridement have been described. The purpose of this study was to compare the effect of enzymatic and surgical debridement on coagulation. METHODS: Between 03/2017 and 02/2021 patients with burn injuries with a total body surface area (TBSA) ≥ 1% were included in the study. Patients were categorized into two groups: the surgically debrided group and the enzymatically debrided group.

Robotic Puboprostatic Fistula Repair with Holmium Laser Pubic Debridement.

INTRODUCTION AND OBJECTIVE: Urosymphyseal fistula (UF) with osteomyelitis most commonly occurs as a result of prostate cancer and benign prostate hyperplasia therapy. UF presentation typically includes debilitating pelvic pain exacerbated with ambulation. Traditional management required open surgical genitourinary (GU) reconstruction with pubectomy leading to significant morbidity. However, progressive utilization of robotic approaches and advances in holmium laser technology has led to a less invasive alternative. Herein, we present our series of robotic-assisted holmium laser debridement of

Myiasis Phenomenon in a Patient Receiving Maggot Debridement Therapy.

OBJECTIVE: Classical treatment was administered on the wound on the left foot of the patient, but no improvement was observed. When advised to undergo amputation, the patient came to the wound care unit to treat the larval wound. It was observed that the wound had been wormed while the patient was undergoing larval treatment. Interestingly, this incidental encounter was thought to be appropriate for case presentation. METHODS: The larvae seen in the left hand of the patient were removed using forceps and sent to a laboratory. The larvae as well as adult flies obtained from them were examined
