Deciding treatment for miscarriage--experiences of women and healthcare professionals.

INTRODUCTION: Women experiencing miscarriage are offered a choice of different treatments to terminate their wanted pregnancy at a time when they are often shocked and distressed. Women's and healthcare professionals' experiences of the decision-making process are not well described. We aimed to gain insight into this process and the circumstances that may affect it. METHOD: A qualitative study using a grounded theory approach. Data were obtained through semi-structured interviews with six women who had chosen and completed either surgical, medical or expectant treatment for miscarriage and f

Optimizing immersive learning in crisis management: cognitive and psychological mechanisms of VR training.

BACKGROUND: Virtual Reality (VR) training holds significant promise for crisis education, yet the cognitive and psychological mechanisms through which it enhances higher-order decision-making skills remain inadequately explained. Grounded in Presence Theory and Cognitive Load Theory, this study investigates how VR training improves decision-making competence, examining the mediating role of immersion and the moderating effect of training duration. METHODS: A cross-sectional survey was conducted with 352 emergency response professionals from China, selected via stratified random sampling. Data

H.M., word knowledge, and aging: support for a new theory of long-term retrograde amnesia.

This study develops a new theory of long-term retrograde amnesia that encompasses episodic and semantic memory, including word knowledge. Under the theory, retrograde amnesia in both normal individuals and hippocampal amnesics reflects transmission deficits caused by aging, nonrecent use of connections, and infrequent use of connections over the life span. However, transmission deficits cause severe and irreversible retrograde amnesia only in amnesics who (unlike normal persons) cannot readily form new connections to replace nonfunctioning ones. The results of this study are consistent with th

Attachment goes to court in Sweden: perception and application of attachment concepts in child removal court decisions.

Concerns have been raised regarding misconceptions about attachment theory in child protection settings, but the application of attachment concepts in judicial child protection decisions has not been systematically explored. This study therefore examined the perception and application of attachment concepts in Swedish judicial decision protocols concerning involuntary removals of children (aged 0-2 years) where emotional neglect was a notable concern (n=28). Attachment concepts were frequently misunderstood, and imprecisely articulated. Unsystematic observations of child behavior were used to

Uncertainty reduction in environmental data with conflicting information.

The assessment of ecotoxicological effects of chemicals for regulatory purposes requires large amounts of experimental data which are expensive to obtain and eventually might entail exhaustive animal testing. The required decision-making processes in this regulatory context, must often be carried out with limited or even contradictory sources of information. To benefit from all sources of information without compromising the quality of the decision process, uncertainty management and reduction techniques, such as the Dempster-Shafer theory of evidence, have to be applied. This theory was appli

Gamble with Your Head and Not Your Heart: A Conceptual Model for How Thinking-Style Promotes Irrational Gambling Beliefs.

Dual process theory suggests that people use two processing systems to filter information and form judgments that direct a course of action: an intuitive and an analytic system. While the intuitive system is necessary for efficient and effective daily functioning, reliance on fast, intuitive thinking when gambling is likely to result in biased or flawed decision-making. Those who gamble tend to endorse an array of fallacious or irrational beliefs that contribute to risky decision-making and excessive gambling. This paper argues that gambling beliefs may be developed and reinforced through unde

Relational and symbolic process: decisions regarding the place of death in the context of advanced cancer.

OBJECTIVE: To construct a substantive theory regarding the meanings attributed to and the factors influencing the decision regarding the place of death in the context of advanced cancer. METHODOLOGY: Symbolic Interactionism and Grounded Theory were used as theoretical frameworks. Audio-recorded interviews, conducted in person or remotely between 2023 and 2024, were carried out with 96 participants from nine states located in four Brazilian regions, who were organized into five sample groups. RESULTS: The hospital is perceived as a space offering protection, relief from pain and suffering, an

System theory in medical diagnostic devices: an overview.

Medical diagnostics refers to testing conducted either in vitro or in vivo to provide critical health care information for risk assessment, early diagnosis, treatment, or disease management. Typical in vivo diagnostic tests include the computed tomography scan, magnetic resonance imaging, and blood pressure screening. Typical in vitro diagnostic tests include cholesterol, Papanicolaou smear, and conventional glucose monitoring tests. Historically, devices associated with both types of diagnostics have used heuristic curve fitting during signal analysis. However, since the early 1990s, a few en

Commentary on "Consciousness as a Memory System" by Budson, Richman, and Kensinger (2022).

Consciouness is a phenomenon that has eluded explanation by generations of physicians and scientists. Many discussions, experiments, and theories about consciousness have been published, but none has adequately explained the phenomenon. In the previous issue, Budson and colleagues (2022) present a theory of consciousness based on explicit memory processes, with consciousness developing in the context of memory function. In the authors' view, consciousness accompanying other cortical processes such as language or visual-spatial function developed only later in evolution. The evidence presented

On the composition of risk preference and belief.

Prospect theory assumes nonadditive decision weights for preferences over risky gambles. Such decision weights generalize additive probabilities. This article proposes a decomposition of decision weights into a component reflecting risk attitude and a new component depending on belief. The decomposition is based on an observable preference condition and does not use other empirical primitives such as statements of judged probabilities. The preference condition is confirmed by most of the experimental findings in the literature. The implied properties of the belief component suggest that, besid

Bridging Theory and Practice in Emergency Nursing: A Discussion Paper on Peripheral intravenous catheter Management Using Tanner's and Chinn & Kramer's Models.

INTRODUCTION: Nursing management of peripheral intravenous catheters remains suboptimal and often lacks integration with underpinning theory. Therefore, it is essential to reflect on the persistent gap between theory and practice to enhance patients' outcomes in this area. OBJECTIVES: To conceptually evaluate the integration of Tanner's Clinical Judgment Model using Chinn and Kramer's criteria, and to explore the integration of these two complementary frameworks with Kramer's Ways of Knowing for peripheral intravenous catheter decision-making and patient outcomes improvement in the emergency

Deciding Under Uncertainty: The Case of Refractory Intracranial Hypertension.

A challenging clinical conundrum arises in severe traumatic brain injury patients who develop intractable intracranial hypertension. For these patients, high morbidity interventions such as surgical decompression and barbiturate coma have to be considered against a backdrop of uncertain outcomes including prolonged states of disordered consciousness and severe disability. The clinical evidence available to guide shared decision-making is mainly limited to one randomized controlled trial, the RESCUEicp. However, since the publication of this trial significant controversy has been ongoing over t

A game theoretical approach to optimal control of dual drug delivery for HIV infection treatment.

For human immunodeficiency virus (HIV) infection treatment, the host immune system and antiviral drugs form a coalition to fight against HIV. Evidence suggests that drug therapy with Highly Active Antiretroviral Therapy can effectively prolong the life of the patient because it can reduce the replication of HIV while, at the same time, protecting the CD4+ T cells. The dosages of different drugs, however, are important in determining the success of the treatment. In addition, drug therapy for HIV is complicated by the fact that viruses can mutate, thereby effectively escaping from the attack of

A model for decision making regarding periodontal treatment needs.

The aim of this study was to present a tentative model for decision making regarding periodontal treatment needs, based on the amount of remaining periodontal bone support. In the model presented, a maintained alveolar bone height of one third of the root length at the age of 75 years was regarded to be a reasonable goal. The data analyzed were derived from a patient sample of 194 dentate individuals aged 25-70 years from whom 2 full mouth series of intraoral radiographs, approximately 10 years apart, were available. For teeth present at both examinations, the radiographs were evaluated with r

Future time perspective drives younger and older adults to continue investing in failing activities.

Inspired by socioemotional selectivity theory, we investigated age differences in further investment in failing situations by considering goal type and future time perspective. In Experiment 1, 32 younger and 30 older adults reported their likelihood of continued investing following an unsuccessful investment. Older adults were more willing to invest with emotional goals as opposed to knowledge and unspecified goals, while younger adults showed a greater willingness to invest with emotional and knowledge goals rather than unspecified goals. In Experiment 2, another sample of 32 younger and 34

Neural response to reward anticipation under risk is nonlinear in probabilities.

A widely observed phenomenon in decision making under risk is the apparent overweighting of unlikely events and the underweighting of nearly certain events. This violates standard assumptions in expected utility theory, which requires that expected utility be linear (objective) in probabilities. Models such as prospect theory have relaxed this assumption and introduced the notion of a "probability weighting function," which captures the key properties found in experimental data. This study reports functional magnetic resonance imaging (fMRI) data that neural response to expected reward is nonl

The role of expecting feedback during decision-making under risk.

Sometimes choice is followed by outcome feedback and other times it is not. It remains unknown whether humans prefer gambling when they expect feedback to be revealed. Regarding this question, decision-making theories make alternative predictions. Some theories have proposed that choice is influenced by whether one expects to be disappointed in the future. Given that feedback is sometimes disappointing, these theories predict increased aversion towards gambling when feedback is expected compared to when feedback is not expected. The opposite effect is predicted by theories of curiosity, which

The Role of Regret in Extracorporeal Membrane Oxygenation Candidacy Decision-Making.

BACKGROUND: Veno-venous extracorporeal membrane oxygenation (VV ECMO) candidacy decisions are made under substantial uncertainty and are inconsistent. With a team including economist decision-scientists, we applied Regret Theory to hypothesize that ECMO clinicians' perceived past and anticipated regret is an important influence on these decisions and contributes to inconsistency. RESEARCH QUESTION: Does regret influence ECMO candidacy decisions? STUDY DESIGN AND METHODS: This is a secondary analysis of a qualitative semi-structured interview study with 24 ECMO clinicians from various interna

Improving communication of prognostic uncertainty in advanced illness: a realist review.

OBJECTIVES: Prognostic uncertainty is common in advanced illness, driven by multimorbidity, treatment complexities and unpredictable disease trajectories. When inadequately acknowledged or discussed, it can lead to patient and family distress and inhibit decision-making.We aimed to build a programme theory of how, why and for whom communication of prognostic uncertainty 'works' (or not) in advanced illness and palliative care contexts. DESIGN: Realist systematic review, reported according to RAMESES criteria. DATA SOURCES: Embase, MEDLINE, CINAHL, Cochrane Database of Systematic Reviews, SCI
