Beyond depression symptoms: the default mode network as a predictor of antidepressant response.

Antidepressant efficacy for major depressive disorder (MDD) remains limited, with the neural mechanisms underlying treatment response poorly understood. The default mode network (DMN), particularly the connectivity between the medial prefrontal cortex (mPFC) and posterior cingulate cortex (PCC), has been implicated in MDD pathophysiology and may be linked to treatment outcomes. However, its potential as a biomarker for antidepressant response has not been validated. Here, we investigate the relationship between DMN connectivity and antidepressant treatment response in MDD. Transcranial focused ultrasound targeting the default mode network for the treatment of depression.

INTRODUCTION: Up to 50% of individuals fail to respond to current depression treatments. Repetitive negative thought and default mode network hyperconnectivity are central in depression and can potentially be targeted using novel neuromodulation techniques. This community-based study assessed whether a treatment using non-invasive transcranial focused ultrasound targeting the default mode network can decrease depression symptoms and repetitive negative thought, and improve quality of life. METHODS: Study recruitment began in August 2023 and ended in February 2024. Twenty individuals aged 18 -

Hierarchical Representation of Multistep Tasks in Multiple-Demand and Default Mode Networks.

Task episodes consist of sequences of steps that are performed to achieve a goal. We used fMRI to examine neural representation of task identity, component items, and sequential position, focusing on two major cortical systems-the multiple-demand (MD) and default mode networks (DMN). Human participants (20 males, 22 females) learned six tasks each consisting of four steps. Inside the scanner, participants were cued which task to perform and then sequentially identified the target item of each step in the correct order. Univariate time course analyses indicated that intra-episode progress was t

Characteristic changes in the default mode network in hypertensive patients with cognitive impairment.

Hypertension has a close affinity to brain degeneration and cognitive decline during the aging process. The default mode network (DMN) is usually affected in various diseases related to cognitive impairment (CI). The present research aimed to explore the alterations in the DMN and its subcomponents in hypertensive patients with and without CI and to investigate the associations between cognitive performance and network abnormalities. Resting-state functional magnetic resonance imaging and neuropsychological tests were performed in 74 subjects, namely, 30 hypertensive patients with normal cogni

Differential failure to deactivate the default mode network in unipolar and bipolar depression.

OBJECTIVES: Neuroimaging studies have revealed evidence of brain functional abnormalities in bipolar depressive disorder (BDD) and major depressive disorder (MDD). However, few studies to date have compared these two mood disorders directly. METHODS: Matched groups of 26 BDD type I patients, 26 MDD patients and 26 healthy controls underwent functional magnetic resonance imaging (fMRI) while performing the n-back working memory task. A whole-brain ANOVA was used to compare the three groups and clusters of significant difference were examined further using region-of-interest (ROI) analysis. RE

The Effects of Gene Mutations on Default Mode Network in Familial Alzheimer's Disease.

Familial Alzheimer's disease (FAD) mutations have very high penetrance but age at onset and rate of disease progression differ. Neuroimaging and cerebrospinal fluid (CSF) examinations in mutation carriers (MCs) may provide an opportunity to identify early biomarkers that can be used to track disease progression from presymptomatic to the dementia stages of disease. The default mode network (DMN) is a resting state neuronal network composed of regions known to associate with amyloid deposition in AD. We hypothesized that functional connectivity in the DMN might change at pre-clinical stages in

Depressive Rumination, the Default-Mode Network, and the Dark Matter of Clinical Neuroscience.

The intuitive association between self-focused rumination in major depressive disorder (MDD) and the self-referential operations performed by the brain's default-mode network (DMN) has prompted interest in examining the role of the DMN in MDD. In this article, we present meta-analytic findings showing reliably increased functional connectivity between the DMN and subgenual prefrontal cortex (sgPFC)-connectivity that often predicts levels of depressive rumination. We also present meta-analytic findings that, while there is reliably increased regional cerebral blood flow in sgPFC in MDD, no such

Modulation of the default-mode network between rest and task in Alzheimer's Disease.

Default-mode network (DMN) connectivity at rest is disrupted in Alzheimer's Disease (AD), but it is unknown whether this abnormality is a static feature, or if it varies across cognitive states. We measured DMN integrity in 16 patients with mild AD and 18 controls during resting state and a simple visual task. Patients showed resting-state deficits in the parahippocampal gyrus and posterior cingulate. No group differences were found during the task. Controls exhibited higher DMN connectivity of multiple regions during rest than task, while the patient group showed no modulation of the DMN betw

Connectivity of default-mode network is associated with cerebral edema in hepatic encephalopathy.

Cerebral edema, a well-known feature of acute liver disease, can occur in cirrhotic patients regardless of hepatic encephalopathy (HE) and adversely affect prognosis. This study characterized and correlated functional HE abnormalities in the brain to cerebral edema using resting-state functional magnetic resonance imaging (rs-fMRI) and diffusion tensor imaging (DTI). Forty-one cirrhotic patients (16 without HE, 14 minimal HE, 11 overt HE) and 32 healthy controls were assessed. The HE grade in cirrhotic patients was evaluated by the West Haven criteria and neuro-psychological examinations. Func

Disrupted functional connectivity within the default mode network and salience network in unmedicated bipolar II disorder.

BACKGROUND: Recent studies demonstrate that functional disruption in resting-state networks contributes to cognitive and affective symptoms of bipolar disorder (BD), however, the functional connectivity (FC) pattern underlying BD II depression within the default mode network (DMN), salience network (SN), and frontoparietal network (FPN) is still not well understood. The primary aim of this study was to explore whether the pathophysiology of BD II derived from the pattern of FC within the DMN, SN, and FPN by using seed-based FC approach of resting-state functional magnetic resonance imaging (rs

Language processing in emergencies recruits both language and default mode networks.

Effective language processing in emergencies is crucial for professionals, including firefighters, soldiers, and doctors. Substantial research has been undertaken on language processing in silence, with several studies indicating the impact of noise on language processing in non-emergencies. However, it remains unclear about the neural mechanisms involved in language processing during emergencies, especially the role of the language network (LN) and the default mode network (DMN) in such contexts. In this study, we adopted functional near-infrared spectroscopy (fNIRS) to investigate brain acti

Default mode and frontoparietal control networks bridge memory and choice consistency.

Choice consistency denotes the capacity to maintain stable, coherent preferences across diverse contexts-a cornerstone of rational decision-making. However, real-world decisions frequently diverge from normative models, marked by inconsistencies and irrationalities. Memory processes may underlie this variability, influencing the formation and maintenance of choice consistency. Yet, the interplay between memory and choice consistency, particularly their shared neural substrates, remains poorly understood. To address these gaps, we developed a novel behavioral paradigm integrating memory retriev

Functional connectivity of semantic and default mode networks during narrative comprehension.

Understanding narratives requires at least transient access to the semantic system, to decode incoming content, and prolonged access to the default mode network to maintain and manipulate the narrative model. Subregions within the integrative semantic hubs in bilateral anterior temporal lobe appear differentially sensitive to the need to rapidly decode external input (exogenous processing) versus reflecting on context stored in the narrative model (endogenous processing). The latter is most consistently reported in the middle temporal gyrus portion of the hub, suggesting that this region serve

Functional MRI for Assessment of the Default Mode Network in Acute Brain Injury.

BACKGROUND: Assessment of the default mode network (DMN) using resting-state functional magnetic resonance imaging (fMRI) may improve assessment of the level of consciousness in chronic brain injury, and therefore, fMRI may also have prognostic value in acute brain injury. However, fMRI is much more challenging in critically ill patients because of cardiovascular vulnerability, intravenous sedation, and artificial ventilation. METHODS: Using resting-state fMRI, we investigated the DMN in a convenience sample of patients with acute brain injury admitted to the intensive care unit. The DMN was

Cortical morphometry in frontoparietal and default mode networks in math-gifted adolescents.

Math-gifted subjects are characterized by above-age performance in intelligence tests, exceptional creativity, and high task commitment. Neuroimaging studies reveal enhanced functional brain organization and white matter microstructure in the frontoparietal executive network of math-gifted individuals. However, the cortical morphometry of these subjects remains largely unknown. The main goal of this study was to compare the cortical morphometry of math-gifted adolescents with that of an age- and IQ-matched control group. We used surface-based methods to perform a vertex-wise analysis of cortic

Meditation experience is associated with differences in default mode network activity and connectivity.

Many philosophical and contemplative traditions teach that "living in the moment" increases happiness. However, the default mode of humans appears to be that of mind-wandering, which correlates with unhappiness, and with activation in a network of brain areas associated with self-referential processing. We investigated brain activity in experienced meditators and matched meditation-naive controls as they performed several different meditations (Concentration, Loving-Kindness, Choiceless Awareness). We found that the main nodes of the default-mode network (medial prefrontal and posterior cingul

Default mode network functional connectivity is associated with intention interference in healthy aging.

Prospective memory (PM), the ability to fulfil an intended action, is important in daily life. However, completed intentions can interfere with subsequent behaviors, known as the aftereffect. The neural basis of this phenomenon remains unclear, particularly from a network perspective. Based on cognitive theories, the present study aimed to examine the underlying neural mechanisms, focusing on the default mode network (DMN) and the frontoparietal network (FPN) to resolve previously inconsistent findings. Thirty-six middle-to-older adults completed a memory-for-intention task, with outdated cues

Alterations of Default Mode Network Effective Connectivity in Youth with Social Anxiety Disorder.

BACKGROUND: Often emerging in early adolescence, social anxiety disorder (SAD) is a mental health condition characterised by persistent fears of negative evaluation. Neuroimaging studies implicate disruptions in the default mode network (DMN) in SAD, however, it remains unclear whether specific DMN regions are driving broader network alterations during this developmental period. METHODS: Thirty-six adolescents and young adults (aged 16-25) with SAD and 66 age- and gender-matched controls underwent resting-state functional magnetic resonance imaging.
