The Penrose Effect and its acceleration by the war on drugs: a crisis of untranslated neuroscience and untreated addiction and mental illness.

In 1939, British psychiatrist Lionel Penrose described an inverse relationship between mental health treatment infrastructure and criminal incarcerations. This relationship, later termed the 'Penrose Effect', has proven remarkably predictive of modern trends which have manifested as reciprocal components, referred to as 'deinstitutionalization' and 'mass incarceration'. In this review, we consider how a third dynamic-the criminalization of addiction via the 'War on Drugs', although unanticipated by Penrose, has likely amplified the Penrose Effect over the last 30 years, with devastating social

Family Carers' Perspectives on Integrated Community Care in the Czech Republic.

AIM: The problem of family care for people dependent on another person has only recently become a focus of research. As demand for health and social services has not been adequately met by public service providers, growing attention has been given to informal care and the integration of families within systems of health and social care at the community level. This paper presents the results of a survey on informal carers' views and opinions under the current conditions of social support in the Czech Republic. The survey was based on theoretical concepts of caring societies, deinstitutionalizat

'Old' and 'new' institutions for persons with mental illness: treatment, punishment or preventive confinement?

Despite countless promises for a better life by national commissions, governments and the international community, there has evolved a vicious cycle of neglect, abandonment, indignity, cruel and inhuman treatment, and punishment of persons with mental illness. This shameful history of benign, and sometimes malignant, neglect of persons with mental illness is well understood, with the deep stigma and unredressed discrimination, the deplorable living conditions, and the physical and social barriers preventing their integration and full participation in society. The maltreatment of this vulnerabl

Profile of psychiatric emergency patients.

From this mass of data, one can assemble a statistical composite profile of the psychiatric emergency patient. This patient is young, unmarried, female, living near the emergency department, poor, diagnosed as psychotic, exhibiting some acutely disruptive or life-threatening behavior, having a history of prior contact with the psychiatric system, arriving at night by herself, and at risk for medical illness or substance abuse. Such a composite profile is misleading, however, for it is unlikely that there is a homogeneous population seeking help in psychiatric emergency services. Rather it seem

Exploring the Use of Special Housing Units by Men Released From New York Correctional Facilities: A Small Mixed-Methods Study.

This small, mixed-methods study sought to understand the use of special housing units among formerly incarcerated men. In the present study, 110 participants were placed in solitary confinement, ranging from ≤30 days to 20 years, with a mean of 2 years (SD = 2.55). These men ranged in age from 35 to 67 years, with a mean age of 46 years. Years of incarceration ranged from less than 30 days to 34 years; the mean number of years incarcerated was 4 (SD = 6.39). Of the 110 participants, qualitative interviews were conducted with 30 men to explore their experiences during time spent in solitary. Th

Using family psychoeducation when there is no family.

High familial expressed emotion, which may be a specific form of social overstimulation, has been linked to schizophrenic relapse and rehospitalization. Research on expressed emotion has led to a new generation of demonstrably effective family treatments for schizophrenia that rely heavily on psychoeducational techniques. Although most schizophrenic patients do not live with families, the family psychoeducational model has not been applied in nonfamilial living settings. The authors report two examples in which the family psychoeducational model was successfully used in group living situations

[Liberal or controlled approach--the management of violent patients in psychiatric clinics].

The traditional mental hospitals are said to hamper recovery and obstruct rehabilitation by an authoritarian therapeutic regimen. Nevertheless, the claim of community-oriented hospitals that a liberal therapeutic style improves prognosis and makes treatment more human requires scrutiny. The management of violent patients who call for controlling and disciplinary measures seems to be crucial. In the light of a case-history the article describes, how a misinterpretation of liberal principles complicates the treatment of dangerous patients and supports an attitude which does not admit the double

Mental development of adults with Down syndrome.

Measures of cognitive, verbal, and functional performance obtained at widely separated time periods were examined for adults with Down syndrome. Comparisons of first and last test profiles for Raven Coloured Progressive Matrices, Peabody Picture Vocabulary Test, and Adaptive Functioning Index (Social Education) Test all demonstrated significant increases over a mean time span of 5 years. These increases could not be attributed to year of entry into program, history of institutionalization, or age of the individuals. The findings indicate that mental development of adults with Down syndrome con

40th birthday of the Italian Mental Health Law 180 - perception and reputation abroad, and a personal suggestion.

How things are perceived from a distance may help better understand their nature. Perceptions at home are likely to shape perceptions abroad. The mutual cross-references between local and distant perspectives on the Italian Mental Health Law 180 may help understand the process which preceded and resulted in the reform. This editorial argues that Law 180 came about at a unique - enabling - time in history. It argues that the run-up to and passing of Law 180 constituted a great accomplishment by professionals, the wider public and politicians/administrators. This editorial goes on to argue that

Aging in institutions in East Asia: Care poverty pathways among older adults with intellectual disabilities.

Despite policy commitments to aging in place and deinstitutionalization, little research has examined older adults with intellectual disabilities who continue to age in long-stay institutions, particularly in East Asian and other non-Western contexts. Using Taiwan as an East Asian case, this study explores how place-specific care arrangements, unmet support needs, and welfare infrastructures shape pathways into institutional care and experiences of institutional aging. We conducted in-depth life-history interviews with 13 residents aged 56-74 in four long-stay institutions in Taiwan. Drawing o

Attitudes on Palliative Care for Adults with Developmental Disabilities.

Background: Intellectual and Developmental Disabilities (IDD) are defined as physical/mental impairments before age 18. Not only are many IDD patients living into adulthood, but deinstitutionalization has also led to most living in community settings. Little is known about end-of-life needs in these adults, and existing literature does not examine attitudes of Canadian providers. Objective: Thematic and content analysis examined attitudes of Canadian Pediatric Palliative Care (PPC) practitioners on caring for adults with IDD to identify components of care, which could be improved. Design: An a

The extent of support for ordinary living provided in staffed housing: the relationship between staffing levels, resident characteristics, staff: resident interactions and resident activity patterns.

Staffed housing has become an accepted alternative to institutional residential services for people with learning disabilities on the expectation that such provision will promote 'ordinary' patterns of living. Information on staffing levels and the behavioural characteristics of residents together with direct observational data on staff: resident interactions and resident engagement in activity were collected on 15 housing services in South Wales and analysed to explore the interrelationship between these key input and outcome variables. Staffing levels were found to be related to resident cha

[Current practice of psychiatric occupational rehabilitation. Results of a representative study in sheltered environments in the Westfalen area].

The number of patients suffering from chronic mental disorders but living outside the psychiatric hospital is undergoing a steady increase, which is reflected in an increasing demand for appropriate vocational and occupational facilities. This demand is met primarily by outpatient work therapy, self-help enterprises and enterprises providing jobs to supplement pensions, and by special departments for the mentally retarded in workshops for the handicapped. Within the framework of a prospective study on vocational rehabilitation of the mentally ill, 502 rehabilitants forming a representative sam

Social determinants of mental health care systems: intensive community based Care in the Veterans Health Administration.

BACKGROUND: Since deinstitutionalization in the 1950s-1970s, public mental health care has changed its focus from asylums to general hospitals, outpatient clinics and specialized community-based programs addressing both clinical and social determinants of mental health. Analysis of the place of community-based programs within a comprehensive health system such as the Veterans Health Administration (VHA) may illuminate the role of social forces in shaping contemporary public mental health systems. METHODS: National VHA administrative data were used to compare veterans who exclusively received

A Comparative Study of Family Caregiver Burden of Schizophrenia and Bipolar Affective Disorder.

BACKGROUND: The concept of family caregiving came into the limelight following the deinstitutionalization movement. The caregiver provides physical, emotional, and economic support, and in the process, they undergo considerable stress, like sleep deprivation, physical pain, emotional turmoil, financial issues, and social strains. Major chronic mental illnesses like Schizophrenia and bipolar disorder differ significantly in the symptomatology, course, outcome, response to medications, and degree of dependency, and thus impact the caregivers differently. AIM: This study aimed to compare the car

[Comparative analysis about hospitalization characteristics in the mental health unit of a general acute care hospital].

INTRODUCTION: In Argentina, like in the rest of the world, deinstitutionalization policies have been implemented. In our country, the shortage of epidemiologic data regarding admissions in mental health is a limitation for the discussion about how this changes should be implemented and its effect. OBJECTIVES: Epidemiologic data of inpatients of the Mental Health Unit of the Teodoro Álvarez General Hospital between June 2012 and May 2013 has been analyzed in order to elaborate a comparative analysis with a previous study that took place in the same unit on the years 2004-2005. Psychiatry in transition: outcomes of mental health policy shift in Italy.

OBJECTIVE: To assess the outcomes of changes in mental health policy introduced in Italy in 1978. METHODS: Data on psychiatric services, before and after the policy change, are presented. Effects of change are evaluated through indicators related to four issues: transfer of care, criminalisation of the mentally ill, suicides, and homelessness. RESULTS: Admissions of new patients to mental hospitals have been stopped and the size of the mental hospital population is now very low (26 per 100,000 population). Psychiatric care has been shifted to community services including general hospital psy

[Mental health of homeless persons. Critical review of the Anglo-Saxon literature].

Because the visibility of homeless persons congregating in urban areas has increased since the 1980's, the relationship between homelessness and mental illness has caused more and more concern. A multitude of epidemiological surveys have been organized in Great Britain, Australia, Canada, and mainly in United States, and have attempted to evaluate scientifically the psychiatric morbidity of this population. This literature review reveals disparity of epidemiological methods in assessing the type and extent of mental illness among homeless adults. The lack of consensual definition of homelessne
