UNLABELLED: The pre-diagnostic period of 252 children (median age 6.3 years, range 0-16.9 years) with primary brain tumours was assessed to analyse their clinical presentation and reasons for any delay in diagnosis. The median pre-diagnostic symptomatic interval (PSI) was 60 days (range 0-3010 days) with a parental delay of 14 days (range 0-2310 days) and a doctor's delay of 30 days (range 0-3010 days). Posterior dislocation of the shoulder is rare. It may follow trauma, electroconvulsive treatment or an epileptic attack. Correct diagnosis may be delayed, as there is no obvious deformity in the shoulder region and the arm is held in a natural sling position. AP X-ray of the shoulder shows only a few minor signs. A high index of suspicion, proper physical examination, and oblique X-ray views lead to correct diagnosis. Closed reduction is possible if the dislocation is of less than 3 weeks duration. Open reduction is indicated in young patients, and in older patients when there is disability.

The delay in diagnosis of 106 patients with adult coeliac disease, diagnosed between 1976 and 1980, was studied. Overall, delay in diagnosis was 11 +/- 10.6 years, being considerably greater in females (12.8 +/- 7.8 years) than in males (3.5 +/- 9.8 years). In a large series of patients with carcinoma of the sigmoid colon and rectum there was a delay in diagnosis due to misinterpretation of the physical signs in 36 cases (2.3%). The causes of the misdiagnosis were haemorrhoids, radiological misinterpretation, and chronic inflammatory disease. A miscellaneous variety completed the series. A study of the survival of those patients in whom there was a delay in diagnosis shown that it is most unlikely that it actually affected adversely the prognosis of these patients.

Bordetella pertussis is a gram-negative bacterium that primarily infects the respiratory tract. We report the case of a 13-month-old male patient who developed pertussis. At the initial clinic presentation, the diagnosis was not immediately considered, as the early manifestations closely mimicked those of a viral upper respiratory infection. As the illness progressed, the classic paroxysmal cough with inspiratory "whoop" became apparent, prompting the correct diagnosis. OBJECTIVES: Ataxia-telangiectasia (AT) is a rare, autosomal recessive neurodegenerative disorder in which the diagnosis is obvious when ataxia and telangiectasia are both present. However, the diagnosis can be made upon the onset of ataxia and before the appearance of telangiectasia if confirmed by laboratory tests. Early diagnosis is important for genetic counseling, appropriate care, and avoidance of unnecessary tests. It is known that delayed endolymphatic hydrops (DEH) occurs in patients with unilateral and/or bilateral profound hearing loss.
