[Postoperative brain infarction in a patient with the previous asymptomatic brain infarction].

We report a case of a postoperative brain infarction, in which an asymptomatic preoperative brain infarction was also revealed postoperatively. A 63-year-old man with bladder carcinoma was scheduled for the cystoplasty. The patient had no prominent preoperative abnormality. Anesthesia was maintained with isoflurane in N2O/oxygen combined with a spinal block and a continuous epidural block. Anesthesia lasted uneventfully for 16.5 hours. However, emergence from anesthesia was delayed. On the first postoperative day, motor aphasia and right hemiparalysis was confirmed. The computed tomography (CT

Anesthetic management of a patient with mandibular hypoplasia, deafness, progeroid features, lipodystrophy syndrome: a case report.

BACKGROUND: Mandibular hypoplasia, deafness, progeroid features, and lipodystrophy (MDPL) syndrome is a rare autosomal dominant disorder that presents unique challenges for anesthetic management due to its multisystemic manifestations. This report outlines the anesthetic considerations for MDPL patients based on our case experience. CASE PRESENTATION: A 15-year-old male with MDPL syndrome underwent testicular extraction under general anesthesia. Insertion of a peripheral venous catheter was challenging due to scleroderma-like skin. Although the facial features of MDPL syndrome suggested a dif

An unusual foreign body in the oral cavity: a case report from a patient safety point of view and literature review.

Accidental foreign bodies (FBs) in the oral cavity, airway, esophagus and breathing circuit associated with anesthetic procedures are rare but can cause serious and life-threatening complications. We here present a case in which an unusual FB in the oral cavity was found after emergence from general anesthesia. The FB was later identified as a melted cap of a felt-tip pen. We investigated the cleaning process for reusable materials and concluded that the FB was accidentally placed in the inner lumen of the reusable bite block during the cleaning process. We then performed a review of the liter

Multiple systemic embolization after endoscopic cyanoacrylate injection for gastric variceal bleeding: a case report.

BACKGROUND: Endoscopic cyanoacrylate injection (ECI) is an established first-line treatment for gastric variceal bleeding (GVB). However, a rare but potentially fatal complication of this therapy is systemic embolization of the cyanoacrylate, most often to the lungs or spleen, while emboli to systemic circulation are exceedingly uncommon. CASE PRESENTATION: We report the case of a 71-year-old Asian male patient with cirrhosis who was found to have gastric variceal bleeding. He underwent endoscopic variceal obliteration using histoacryl (N-butyl cyanoacrylate) injection under intravenous anest

Fatal hemorrhagic infarction of posterior fossa meningioma during cardiopulmonary bypass.

Few publications address cardiac surgery in the presence of meningioma. Individual complications include transient visual loss from a suprasellar meningioma, hemiparesis after mitral valve replacement with recovery after resection, and non-fatal hemorrhage into a posterior fossa meningioma. The largest report of 16 patients with known meningiomas over 11 years suggested a benign course, with no new neurologic symptoms and no required resection of a meningioma over an average follow-up of 31 months. In 2 cases we report a presumed posterior fossa meningioma led to fatal outcome after cardiac su

Current and Developing Approaches for Facilitating Emergence from General Anesthesia.

Despite the widespread use of clinical anesthesia, the process of emergence from general anesthesia remains primarily driven by anesthetic elimination. Although emergence from general anesthesia is typically safe, prolonged delays strain resource-intensive settings and contribute to increased healthcare costs. In addition to improving access to care, providing clinicians with more precise control over emergence could offer diagnostic potential and improve patient outcomes. For decades, this unmet need has motivated research into the mechanisms underlying anesthetic emergence. Now, the first ag

Cerebral Arterial Gas Embolism in Cardiovascular and Aortic Procedures: A Narrative Review of the Mechanisms, Diagnosis, Management, and Prevention.

Cerebral arterial gas embolism (CAGE) is an uncommon but potentially catastrophic complication of cardiothoracic surgery, extracorporeal circulation, mechanical circulatory support, and catheter-based cardiac interventions. During these procedures, gas may be introduced to the cardiac chambers, aorta, and cerebral arterial circulation through different mechanisms. Clinical presentation ranges from abrupt intraoperative hemodynamic and neurologic deterioration to delayed, subtle focal deficits emerging after an apparently uneventful emergence from anesthesia. As intracranial air is detectable o

Cholinergic Modulation of General Anesthesia.

Acetylcholine in the brain promotes arousal and facilitates cognitive functions. Cholinergic neurons in the mesopontine brainstem and basal forebrain are important for activation of the cerebral cortex, which is characterized by the suppression of irregular slow waves, an increase in gamma (30- 100 Hz) activity in the electroencephalogram, and the appearance of a hippocampal theta rhythm. During general anesthesia, a decrease in acetylcholine release and cholinergic functions contribute to the desired outcomes of general anesthesia, such as amnesia, loss of awareness and consciousness, and imm

Intravenous infusion of rocuronium bromide prolongs emergence from propofol anesthesia in rats.

BACKGROUND: Neuromuscular blocking agents induce muscle paralysis via the prevention of synaptic transmission at the neuromuscular junction and may have additional effects at other sites of action. With regard to potential effects of neuromuscular blocking agents on the central nervous system, a definitive view has not been established. We investigated whether intravenous infusion of rocuronium bromide affects the emergence from propofol anesthesia. METHODS: Using an in vivo rat model, we performed propofol infusion for 60 minutes, along with rocuronium bromide at various infusion rates or no

Bilateral supratentorial epidural hematomas: a rare complication in adolescent spine surgery.

A 14-year-old girl presented with a rare case of spontaneous bilateral supratentorial epidural hematomas which developed rapidly following cervical surgery. The hematomas presumably resulted from dural dynamics changes secondary to cerebrospinal fluid loss and intracranial hypotension. Intracranial epidural hemorrhage after spinal surgery is extremely uncommon with only one previous case report. Spontaneous intracranial epidural hematoma is an extremely rare complication, but should be considered as a possible complication of spine surgery, especially in adolescents complicated by delayed cons

[Maternal and infant prognosis of emergency cesarean section: prospective study of the Principal Hospital in Dakar, Senegal].

The prognosis of emergency cesarean section is poor for both the mother and child in developing countries. The respective impact of obstetrical and surgical factors has rarely been analyzed. This prospective study was carried out in 370 women (mean age, 30.5 years) who underwent emergency cesarean section at Principal Hospital in Dakar, Senegal, between January 1 and December 31, 1997. Fifty percent of these women had been transferred from an outside maternity clinic. Indications related to the mother (75% of cases) or fetus (25% of cases) were divided into two groups according to degree of em
