Information, consent and treatment of patients with Morgellons disease: an ethical perspective.

Morgellons is a medically contested diagnosis with foremost dermatological symptoms. Patients experience fibers emerging from the skin, together with a range of other somatic, psychiatric, and neurological complaints. Within the medical community, it is generally held to be a variation of delusional parasitosis/delusional infestation, which is usually treated with antipsychotics. Little attention has been paid in the literature to the ethical aspects of treating patients with Morgellons disease. The communicative strategies suggested in the literature display significant ethical issues, primar

Delusions of disseminated fungosis.

Introduction. Delusional infestation is a rare monosymptomatic hypochondriacal psychosis according to The Diagnostic and Statistical Manual of Mental Disorders (5th ed.; DSM-5; American Psychiatric Association, 2013). It can be a primary disorder or associated with an underlying psychological or physical disorder. It commonly presents as delusional parasitosis, and less than 1% may be fungi related. We present this case as it is a rare presentation of a rare condition. Case Presentation. Our patient is a 60-year-old Caucasian man who presented with a 7-year history of delusional infestation ma

[Bipolar affective disorder and Ekbom syndrome: apropos of a case].

A delusion of parasitosis may be seen in patients with a variety of organic and psychiatric disorders. It may also be a primary delusion often referred to as the "Ekbom syndrome". The nosological categorization of this rare condition is still under debate. We report the case of a 81 year-old woman followed in our department during more than 20 years for a bipolar affective disorder. Five years ago she developed a persistent delusion of parasitosis displaying phenomenological characteristics that are typical of the Ekbom syndrome. Given the complete independence of the respective courses of aff

Delusional infestation.

This papers aims at familiarizing psychiatric and nonpsychiatric readers with delusional infestation (DI), also known as delusional parasitosis. It is characterized by the fixed belief of being infested with pathogens against all medical evidence. DI is no single disorder but can occur as a delusional disorder of the somatic type (primary DI) or secondary to numerous other conditions. A set of minimal diagnostic criteria and a classification are provided. Patients with DI pose a truly interdisciplinary problem to the medical system. They avoid psychiatrists and consult dermatologists, microbio

Psychosis in a Military Veteran in the Context of Hallucinogen Use: A Case Report.

N, N-dimethyltryptamine (DMT) is a potent hallucinogenic compound with increasing recreational use despite limited safety data. While DMT use has been linked to psychosis, delusional parasitosis as a specific manifestation remains unreported. We present a novel case of a 60-year-old female veteran with chronic post-traumatic stress disorder (PTSD), a history of major depressive disorder, past marijuana use, and no history of prior psychiatric hospitalizations, who developed persistent delusions of parasitic infestation following prolonged recreational intravenous (IV) DMT. She was referred to

Psycho-cutaneous disorders: an epidemiologic study.

BACKGROUND AND AIMS: Psycho-dermatology addresses the interaction between mind and the skin. Effective management of at least one third of the patients attending the skin department depends, to some extent, upon the recognition of emotional and psychologic factors. The aim of this study was to provide epidemiologic data about common psycho-dermatoses and their prevalence in dermatologic patients in Iran. MATERIALS AND METHODS: Patients suspicious to have psychocutaneous problem with primary dermatologic chief complaint were included. After detailed dermatologic evaluation, all patients were v

[Post-scabies itch: an overview of causes and treatment strategies].

The main manifestation of scabies infection is intense itching. This itch is experienced by nearly every individual affected by the infestation and may persist even after successful treatment of scabies. In certain cases, this post-scabies itch can persist for several weeks to months. In rare cases, it can even progress into a delusional parasitosis related to scabies. This article highlights three cases and explores the underlying causes of itch as well as treatment strategies.

Regional gray matter volume and structural network strength in somatic vs. non-somatic delusional disorders.

BACKGROUND: Monothematic delusional disorders are characterized by a single tenacious belief. They provide a great opportunity to study underlying brain structures in the absence of confounding symptoms that accompany delusions in schizophrenia. Delusional beliefs include persecution, jealousy or somatic delusions including infestation. It is unclear whether specific delusional content is associated with distinct neural substrates. METHODS: We used magnetic resonance imaging in patients presenting with somatic vs. non-somatic delusional disorders. Patients with delusional infestation (DI, n=1

Delusional infestations: clinical presentation, diagnosis and treatment.

Patients with delusional infestations (DI), previously named delusions of parasitosis, have a fixed, false belief that they are infested with living or non-living pathogens. Patients have abnormal cutaneous symptoms such as itching, biting, or crawling sensations. They often demonstrate self-destructive behavior in an effort to rid the pathogens from under their skin, leading to excoriations, ulcerations, and serious secondary infections. This review article aims to provide an overview of DI including its clinical presentation, diagnosis, and treatment. Strategies on how to establish a strong

Nonrashes. Part I: The Koebner nonreaction.

"Nonrashes" range from pruritus that may signal an underlying systemic illness to delusional parasitosis associated with severe psychological disturbance. We report another kind of nonrash, the Koebner nonreaction or isomorphic nonresponse, that was manifested by the absence of a drug rash at the site of recently administered x-irradiation. We also present a compilation of a number of other nonrashes, such as the mitempfindungen (referred itch) and the prodrome of herpes zoster. The recognition of certain types of nonrashes may be lifesaving.

qEEG in the Diagnosis and Prognosis of a Case with Delusional Infestation.

Stroke can have neuropsychological consequences, such as poststroke psychosis. One aspect of poststroke psychosis is delusional infestation (DI), also known as delusional parasitosis. Patients with DI have fixed sensations that they get infested by lice. Some explanations of DI indicate that striatal and fronto-cortical structural and functional abnormalities are related to DI symptomatology. In this case report, we present a patient with DI due to right intracarotid artery occlusion, which is detected radiologically. Before treatment with escitalopram and risperidone, the case had a frontal s

Improvement of trigeminal neurotrophic ulceration with pimozide in a cognitively impaired elderly woman--a case report.

Neurotrophic ulceration of the face is a rare but well recognized sequel to division of the trigeminal nerve. Trauma is an important contributory factor and thought to be due in part to paraesthesiae, which encourages picking and scratching, with resultant chronic and persistent ulceration. A case is described of an 82-year-old woman with severe trigeminal neurotrophic ulceration which improved substantially with pimozide, given for treatment of unrelated paranoid symptoms. The possible relevance of this to the established use of pimozide in delusional parasitosis is briefly discussed.

Delusional infestation: are you being bugged?

This case report documents a 58-year-old male who presented to the clinic with a 12-month history of a burrowing sensation in his eyelids that he attributed to a parasitic infestation. After being extensively investigated and reviewed by relevant specialties, no evidence of parasitic infestation was found. He was diagnosed with and treated for blepharitis. Psychiatric referral for presumed delusional infestation (DI) was recommended. Despite this, he remained insistent in his belief of infestation, and was inevitably lost to follow-up. DI, previously known as delusional parasitosis, is a rare

Concurrent Delusions of Ocular Parasitosis and Complex Visual Hallucinations from Charles Bonnet Syndrome Treated Successfully with Aripiprazole in an Elderly Male: A Case Report.

INTRODUCTION: Delusional parasitosis (DP) has been described as among the most challenging diagnosis to manage in dermatology and psychiatry literature. Patients with this perplexing and enigmatic condition present potentially to a wide range of specialties including primary or emergency care, dermatology, infectious diseases, neurology, and psychiatry. DP is probably underdiagnosed from patients' underreporting of symptoms of being infested with parasites, resulting from the associated social stigma. In addition, specialists who most often encounter these patients often possess low familiarit

Psychocutaneous disease: Clinical perspectives.

Psychocutaneous disease, defined in this review as primary psychiatric disease with skin manifestations, is commonly encountered in dermatology. Dermatologists can play an important role in the management of psychocutaneous disease because patients visit dermatology for treatment of their skin problems but often refuse psychiatric intervention. This review describes common psychocutaneous syndromes, including delusional, factitious, obsessive-compulsive and related, and eating disorders, as well as psychogenic pruritus, cutaneous sensory (pain) syndromes, posttraumatic stress disorder, and sle

Body image disturbance in body dysmorphic disorder and eating disorders. Obsessions or delusions?

At this time, the question posed by this article's title--body image disturbance in body dysmorphic disorder and eating disorders: obsessions or delusions?--is probably best answered "both." Both disorders appear to be characterized by obsessional and delusional thinking. In addition, it is likely that their nondelusional and delusional variants constitute a single disorder encompassing a spectrum of insight, with the entire spectrum characterized by obsessional thinking. This view represents a considerable departure from DSM-III-R, in which the psychotic disorders were encapsulated in a separ

Delusional infestation: a clinical profile.

OBJECTIVE: Delusional infestation or delusional parasitosis is a form of monodelusional disorder, a condition sometimes encountered in psychiatric or primary care practice. The outcome of this condition is good when compliance can be ensured. PATIENTS AND METHODS: In the present study, a series of 50 consecutive cases of delusional infestation is reported. RESULTS: A majority of cases (94%) had insidious onset. The duration of symptoms in all but 3 cases was 6 months or more. Twenty-eight cases presented with a delusion of infestation by insects over the body and 20 cases with a delusion of

Cocaine abuse: dermatologic manifestations and therapeutic approaches.

Cocaine affects the cutaneous system and other organ systems. Cocaine use is associated with vasculitides, infectious complications, and numerous dermatologic conditions. It has been associated with formication (ie, tactile hallucinations of insects crawling underneath the skin), which leads to delusions of parasitosis and other psychosis-related dermatologic disorders. When a patient presents to a dermatology clinic with chronic skin lesions, a vague medical history, negative findings from previous evaluations, labile affect, and delusional behavior, drug screening should be performed to iden
