Violence in schizophrenia: role of hallucinations and delusions.

The study examines the relationship between hallucinations/delusions and violent behaviour in a sample of long-stay inpatients with chronic schizophrenia. Thirty-one subjects defined as violent and meeting DSM-111-R criteria for schizophrenia were compared with 31 matched non-violent schizophrenia patients with respect to detailed phenomenologies of auditory hallucinations using the Mental Health Research Institute Unusual Perceptions Schedule (Carter and Copolov, 1993; Carter et al., 1995) and delusions using the Maudsley Assessment of Delusions Schedule (Taylor et al., 1994). Patients in the

[Delusion of mercury poisoning in multiple sclerosis].

We confirmed the diagnosis of multiple sclerosis in a 54-year-old man. The patient rejected this diagnosis and was completely convinced that all his symptoms resulted from a chronic mercury intoxication. We found evidence that this delusional disorder was an independent illness, and not a "symptomatic psychosis in multiple sclerosis." So far, little notice has been taken in psychiatry of delusions of environmental poisoning, which cause difficulties in applying common criteria of delusion.

Delusions of disfigurement in a woman with acne rosacea.

A 36-year-old woman with newly diagnosed acne rosacea is presented. Her skin changes were noticeable only under closest scrutiny, but she quit her job, became despondent about her acne, and developed suicidal ideation. The diagnosis and treatment of this patient allow a broader discussion of the somatically focused patient whose ideation reaches delusional intensity.

[Laboratory diagnosis of pseudoparasites, artifacts and parasitic delusions].

Common practice in a diagnostic parasitology laboratory involves distinguishing parasitic organisms from various artifacts. Artifacts mean pseudoparasites, supposed parasites and parasitic delusions. Pseudoparasites include undigested leftovers or coincidentally or purposely ingested nonparasitic organisms or their parts. Supposed parasites are wild organisms which were incorrectly identified as the components of faeces. In parasitic delusions, it is impossible to find any kind of parasites while patients describe imaginary parasites in detail. All the above categories of nonparasitic findings

Delusions in Alzheimer's disease and multi-infarct dementia.

Neuropsychiatric symptoms such as delusions and misidentifications have been reported in dementia ranging from 10% to 73% in Alzheimer's disease (AD) patients and up to 40% in multi-infarct dementia (MID) patients. The aim of this study was to investigate in 61 AD and 31 MID patients both the frequency and the content of delusions during the course of illness and to evaluate the relationship between these and both functional and mental decline. The results indicated that delusion experiences had occurred in 45% of AD patients and in 38% of MID patients, occurring most frequently during the fir

Sexual hallucinations and delusions in borderline personality disorder.

INTRODUCTION: Sexual hallucinations have lifetime prevalence rates of 0.4% in the general population and up to 44% in schizophrenia spectrum disorders. For borderline personality disorder (BPD) these rates are unknown. We therefore studied prevalence rates of sexual hallucinations and delusions in this group, as well as their phenomenological characteristics and their connection with childhood trauma. METHODS: We carried out a cross-sectional study among outpatients with BPD, using the Sexual Hallucinations and Delusions Questionnaire (SHDQ), the Psychotic Symptom Rating Scales-Auditory Verba

Delusions in postpartum psychosis: Implications for cognitive theories.

Postpartum psychosis is a rare but serious condition that can affect women after childbirth. We present a case study of an individual with no comorbidities or psychiatric history who developed postpartum psychosis characterised by prominent misidentification delusions whilst admitted to hospital. The woman recovered quickly with medication and showed no evidence of relapse over the following three years. Whilst still symptomatic and after recovery, the patient was able to provide a detailed description of her experiences. Contemporaneous interviews and observations during her hospital admissio

Accidental colourful mushroom poisoning - delirium, delusions and dreams.

We report a case of a woman presenting with acute onset of delirium following ingestion of Amanita muscaria mushrooms for anxiolytic effect. Immediate diagnosis was obtained after taking a detailed history and confirmation with online poison database images. She fully recovered with supportive treatment and was discharged 24 hours after presentation. To the best of our knowledge, this is the first case report on A muscaria mushroom poisoning in the UK.

Tacrolimus-induced paranoid delusions and fugue-like state.

We report the case of a 43 year old male with no prior psychiatric history with apparent tacrolimus-induced psychosis. Previous reports have identified other neurotoxic adverse effects due to tacrolimus, however, to our knowledge, there are few reports that describe psychosis induced by the immunosuppressant drug. Although psychosis may be a rare adverse effect, it can have significant impact on the long-term prognosis and treatment in transplant recipients. It is imperative to quickly identify patients who develop a mental status change while on tacrolimus and to work with the appropriate tra

[Transsexualism or delusions of sex change? Avoiding misdiagnosis].

The aim of this paper was to present basic data about gender identity disorders and psychotic transsexual desires. From time to time in scientific literature there are descriptions of a diagnosis of psychotic disorders in persons previously diagnosed and treated as transsexuals, in whom the transsexual thinking disappears after using antipsychotic agents. Coexistence of transsexualism and schizophrenia causes a lot of doubt--it is observed in scientists opinions but also in the diagnostic criteria of DSM-IV and ICD-10. Moreover, delusions of sex change are probably more frequent than it is tho

Hallucinations, delusions, and cognitive decline in Alzheimer's disease.

OBJECTIVES: To examine the occurrence of hallucinations and delusions in Alzheimer's disease over a 4 year period and their association with rate of cognitive decline. METHODS: A cohort of 410 persons with clinically diagnosed Alzheimer's disease underwent annual clinical evaluations over a 4 year period. Participation in follow up exceeded 90% in survivors. Evaluations included structured informant interview, from which the presence or absence of hallucinations and delusions was ascertained, and detailed testing of cognitive function. The primary cognitive outcome measure was a composite cog

Misidentification delusions, facial misrecognition, and right brain injury.

Individuals suffering from misidentification syndromes may present with right hemispheric pathology and deficits in facial recognition. In addition, misidentification delusions have been associated with aggressive behaviour. The possible linkage between misidentification phenomena, facial recognition, and aggression is discussed, illustrated by the case of a patient suffering from an organic delusional disorder.

Delusions in newly admitted residents of nursing homes.

Two psychiatrists, using standardized clinical examinations, found that 21% of 125 patients newly admitted to nursing homes had delusions. The psychiatric disorders associated with these delusions are described. Delusional patients were more behaviorally disturbed before admission than nondelusional patients. These behavior problems were often the reason for admission, and they persisted after admission. Nursing home staff members infrequently identified and often inappropriately treated delusional patients. Since delusions are an important potentially treatable cause of behavior problems in n

Artificial intelligence-associated delusions and large language models: risks, mechanisms of delusion co-creation, and safeguarding strategies.

Large language models (LLMs) are poised to become a ubiquitous feature of everyday life, mediating communication, decision making, and information curation across nearly every domain. Within psychiatry and psychology, the attention has largely been on bespoke therapeutic applications, sometimes narrowly focused and often diagnostically siloed, rather than on the broader reality that individuals with mental illness will increasingly engage in agential interactions with artificial intelligence (AI) systems. Although the capacity of these systems to model therapeutic dialogue, provide companionsh

Understanding, treating, and renaming grandiose delusions: A qualitative study.

BACKGROUND: Grandiose delusions are arguably the most neglected psychotic experience in research. OBJECTIVES: We aimed to discover from patients: whether grandiose delusions have harmful consequences; the psychological mechanisms that maintain them; and what help patients may want from clinical services. DESIGN: A qualitative interview design was used to explore patients' experiences of grandiose delusions. METHOD: Fifteen patients with past or present experiences of grandiose delusions who were attending psychiatric services were interviewed. Thematic analysis and grounded theory were used

Treating Hallucinations and Delusions Associated With Parkinson's Disease Psychosis.

PURPOSE OF REVIEW: We discuss features of Parkinson's disease psychosis (PDP) including symptomology and pathophysiology. Treatment options, including non-pharmacologic strategies, dose reduction of offending agents, and the addition of non-dopaminergic antipsychotics, are addressed. The efficacy of second-generation antipsychotics and novel agents is examined. RECENT FINDINGS: Pimavanserin, a 5-HT2A/C receptor inverse agonist with no other receptor activity, has shown efficacy and tolerability and is now FDA approved for PDP treatment. Research into novel targets is ongoing. PDP is a morbid
