The radiographic outcomes of direct pulp-capping procedures performed by dental students: a retrospective study.

BACKGROUND: The decision between pulp capping and root canal therapy after pulp exposure is a clinical issue. The aim of the authors' study was to evaluate the outcome of direct pulp-capping procedures performed by dental students. METHODS: The authors followed the treatment outcomes of 193 patients with 204 pulp exposures with direct pulp capping. They determined the outcome of pulp capping radiographically using periapical radiographs taken at least three years after pulp exposure. The outcome was considered as successful if the tooth was present and not associated with periapical radioluce

Human Primary Tooth Histology Six Months after Treatment with Silver Diamine Fluoride.

OBJECTIVE: To describe the histological characteristics of a human primary tooth with deep caries next to the dental pulp 6 months after being treated with silver diamine fluoride (SDF). STUDY DESIGN: A tooth that was considered to be not restorable 6 months after being treated with SDF was examined with light microscopy. RESULTS: the histologic examination revealed no carious pulp exposure, tertiary dentin, a flattened odontoblastic layer adjacent to irregular tertiary dentin, dentinal tubules with silver deposits to a depth of 1 mm and no bacteria, and a pulp with no significant inflammati

Indirect pulp capping and primary teeth: is the primary tooth pulpotomy out of date?

Formocresol pulpotomy (FP) in the United States is most frequently used to treat asymptomatic caries near the pulp in primary teeth. Indirect pulp therapy (IPT) is also indicated and has a significantly higher long-term success. Pulpotomy is thought to be indicated for primary teeth with carious pulp exposures, but research shows the majority of such teeth are nonvital or questionable for treatment with vital pulp therapy. IPT has a significantly higher success in treating all primary first molars, but especially those with reversible pulpitis compared with FP. The purpose of this article was

Radiographic evidence of traumatic injuries to primary incisors without accompanying clinical signs.

BACKGROUND/AIMS: In epidemiological studies, traumatic dental injuries in children are usually determined from clinical signs or parents' reports. Radiographs are used in dental trauma centers but not in field surveys. The aim of this study was to evaluate the proportion of patients who present with radiographic evidence of trauma without any clinical signs of a history of TDI. METHODS: The prevalence of traumatic dental injuries was assessed from the records of 674 preschool children at their first visit to a private dental practice. Evidence of the history of traumatic dental injuries was b

Histopathology of the pulp in primary incisors with deep dentinal caries.

The purpose of this study was to assess the histological appearance of the pulp of human primary incisors extracted because of deep, unrestorable caries, and to determine how clinical pulp exposures affected the histological status of the pulp compared to nonexposures. Caries was removed carefully from all teeth after fixation; 24 incisors had pulp exposures, and 29 teeth had no pulp exposures. Histological examination showed normal pulps in 69% of the teeth without pulp exposures, compared to 33% of teeth with exposed pulps (P < 0.05). Microabscesses were observed in 33% of cases with pulp ex

Therapeutic-ion-doped bioactive glasses for vital pulp therapy: Linking glass chemistry, release dynamics, immunobiology, and clinical translation.

Vital pulp therapy (VPT) requires a material to control residual microbial challenge without sustaining injury in a confined, vascularized pulp, while preserving endogenous pulp cells, supporting angiogenesis, and enabling a continuous reparative dentin barrier and durable coronal seal. This critical narrative review asks under what composition-release-exposure conditions therapeutic ion-doped bioactive glasses (BGs) can provide antimicrobial and immune-regulatory effects while preserving pulp cell viability and supporting vascularized reparative dentin formation, and what minimum evidence is

BRCC3 aggravates pulpitis by activating the NF-κB signaling pathway in dental pulp cells.

BRCA1/BRCA2-containing complex subunit 3 (BRCC3) has been proved to exert pro-inflammatory effect in various inflammatory diseases through different mechanisms, but its involvement in pulpitis remains unclear. This study aims to investigate the regulatory role and mechanisms of BRCC3 in modulating dental pulp cell inflammation and pulpitis progression. The expression of BRCC3 was observed to be elevated in human/mouse pulpitis samples and lipopolysaccharide-stimulated human dental pulp cells (hDPCs). Manipulation of BRCC3 expression revealed that BRCC3 facilitated the expression of pro-inflamm

Bacterial invasion of non-exposed dental pulp.

Anaerobic procedures were adopted to demonstrate the early bacterial invasion of non-exposed dental pulps, and to isolate and identify the bacteria. Of 19 freshly extracted teeth which originally exhibited deep dentinal lesions, clinical examination and electric pulp testing showed that nine of them had no pulpal exposure. Thus the pulps of these teeth were covered by clinically sound dentine beneath the carious lesion. Bacteria were found to have invaded the pulps of six of these nine teeth. The predominant bacteria were obligate anaerobes belonging to the genera Eubacterium, Propionibacteriu

Hypomineralized Enamel Alters Trigeminal Sensory Afferent Architecture, Transcriptome, and Dental Injury Responses.

At barrier tissues such as the skin and gut, sensory neurons interface with environmental stimuli and coordinate with neighboring epithelial and immune cells to detect tissue perturbations. In teeth, however, the sensory dentin-pulp complex is insulated from the oral environment by highly mineralized enamel. Although dentin-pulp responses to severe injury have been studied in models with direct pulp exposure, it remains unclear whether enamel barrier dysfunction alone alters pulpal and neuronal homeostasis. Using a kallikrein-related peptidase 4 knockout (KLK4 KO) mouse model of enamel hypomin

Cost-effectiveness of partial versus stepwise caries removal of deep caries lesions - a decision-analytic approach.

OBJECTIVE: The aim was to compare the cost-effectiveness of partial caries removal (PCR) and stepwise caries removal (SW) in mature permanent teeth diagnosed with deep caries lesions, in a Swedish Public Dental Care organization. MATERIALS AND METHODS: A decision tree model was used to calculate the cost-effectiveness for PCR and SW. Probability values were obtained through a literature search in PubMed and Scopus. Treatment times were estimated by a group of dentists practicing at public dental care clinics in Västra Götaland county, Sweden. RESULTS: The expected costs for PCR and SW were 1

Histological evaluation of hard tissue formation after direct pulp capping with a fast-setting mineral trioxide aggregate (RetroMTA) in humans.

OBJECTIVES: To histologically evaluate the morphology of the newly formed mineralized tissue and of the adjacent cells, in intact human teeth subjected to mechanical pulp exposure and capping with a fast-setting mineral trioxide aggregate (RetroMTA). MATERIALS AND METHODS: Seven caries-free third molars from three adults were subjected to pulp exposure, direct capping with RetroMTA, and restoration with a composite resin. Seven months later, the teeth were clinically and radiographically evaluated, extracted, and subjected to histological processing and evaluation. RESULTS: All teeth were cl

Neuronal-immune axis alters pain and sensory afferent damage during dental pulp injury.

Dental pulp tissue is densely innervated by afferent fibers of the trigeminal ganglion. When bacteria cause dental decay near the pulpal tissue, a strong neuronal and immune response occurs, creating pulpitis, which is associated with severe pain and pulp tissue damage. Neuroimmune interactions have the potential to modulate both the pain and pathological outcome of pulpitis. We first investigated the role of the neuropeptide calcitonin gene-related peptide (CGRP), released from peptidergic sensory afferents, in dental pain and immune responses by using Calca knockout (Calca -/- ) and wild-typ

Recent advances in drug repurposing for dentin repair.

OBJECTIVE: Exposure of a vital dental pulp following deep caries removal, accidental restorative procedures or trauma can severely affect the dentin-pulp complex in adult permanent teeth. To preserve a biologically functional healthy pulp, promote dentin repair, and minimize invasive and costly procedures, direct pulp capping is commonly used in vital pulp therapies. Despite its well-accepted therapeutic value, current pulp capping inorganic hydraulic calcium-silicate cements like mineral trioxide aggregate show limited targeted bioactive effects. Therefore, repurposing clinically approved dru

Controlled release of odontogenic exosomes from a biodegradable vehicle mediates dentinogenesis as a novel biomimetic pulp capping therapy.

Mineralized enamel and dentin provide protection to the dental pulp, which is vital tissue rich with cells, vasculature, and nerves in the inner tooth. Dental caries left untreated threaten exposure of the dental pulp, providing facile access for bacteria to cause severe infection both in the pulp and systemically. Dental materials which stimulate the formation of a protective dentin bridge after insult are necessary to seal the pulp chamber in an effort to maintain natural dentition and prevent pulpal infection. Dental materials to date including calcium hydroxide paste, mineral trioxide aggr
