Mucolytics and the critically ill patient: help or hindrance?

Patients in the intensive care unit (ICU) with compromised lung function often have excessive pulmonary secretions and have difficulty clearing mucus. A variety of pharmacotherapeutics have been used in an attempt to change the rate, amount, and viscosity of respiratory secretions. These drugs are known as mucokinetic or mucolytic drugs, but their efficacy remains controversial in the research literature. Most commonly used methods of mucolysis/mucokinesis include: bland aerosols (saline), pH adjustment (sodium bicarbonate), and disulfide disruption (N-acetylcysteine, MUCOMYST). Clinical resea

The effect of melatonin on the liver of rats exposed to microwave radiation.

OBJECTIVES: We aimed to clarify if melatonin treatment (2 mg/kg i.p.) may favorably impact the liver tissue in rats exposed to microwave radiation. The experiment was performed on 84 six-weeks-old Wistar male rats exposed for 4h a day, for 20, 40 and 60 days, respectively, to microwaves (900 MHz, 100-300 microT, 54-160 V/m). Rats were divided in to four groups: I (control) - rats treated with saline, II (Mel) - rats treated with melatonin, III (MWs) - microwave exposed rats, IV (MWs + Mel) - MWs exposed rats treated with melatonin. We evaluated oxidative stress parameters (malondialdehyde and

Microalbuminuria as renal marker in recurrent acute tonsillitis and tonsillar hypertrophy in children.

Raised levels of microalbuminuria pointing out glomerular abnormality and indicate renal damage. Glomerulonephritis is caused by immune reaction leading to the formation of circulating immune complexes that are deposited on the basal membrane of the glomerulus. The time course and the appearance of antibodies against infectious agents both play very important roles in its clinical presentation. Antibodies against streptococci have not a protective role, but offers a useful marker of the presence or absence of recent infection. This work studies the presence of microalbuminuria and circulating

Elucidating the functional dynamics of DNASE1L2 intron retention in tuberculosis progression.

BACKGROUND: Intron retention (IR) is an important biological process associated with disease development. However, the role of IR in the progression of tuberculosis (TB) remains unexplored. Therefore, this study aimed to characterize the dynamic IR landscape during TB progression and elucidate the role of DNASE1L2 -IR in this process. METHODS: We conducted a comprehensive high-throughput sequencing analysis using 1729 samples from 19 public datasets and identified candidate IR events associated with TB progression. We then validated these candidate IR events in three macrophage infection mode

One-off streptococcal serologic testing in young children with recurrent tonsillitis.

OBJECTIVES: Recurrent acute tonsillitis in children under 4 years of age is usually viral, making antibiotic therapy inappropriate and the indication for tonsillectomy uncertain. Identifying those young children with bacterial infections is therefore important. The purpose of this study was to determine whether one-off streptococcal serologic testing is a useful tool in assessing recurrent acute tonsillitis in young children. METHODS: We performed a retrospective study of 45 children (35 male and 10 female) under the age of 4 years who were found by a staff otolaryngologist to have recurrent

Nonoperative management of full-thickness intravenous extravasation injuries in premature neonates using enzymatic debridement.

Peripheral intravenous fluid extravasation is a common occurrence among neonatal intensive care unit patients. Fifteen high-risk neonates, averaging less than 35 weeks' gestation and less than 1,500 g birth weight, with full-thickness extravasation injuries were successfully treated nonoperatively by a topical fibrinolysin/deoxyribonuclease ointment. All wounds healed without delaying hospital discharge, and no significant scar contractures were observed in patients followed up to 16 months after injury.

Enzymatic digestion of DNA gamma-irradiated in aqueous solution separation of the digests by ion-exchange chromatography.

Aqueous solutions of DNA were gamma-irradiated in the presence and absence of oxygen and enzymatically hydrolysed by the combined action of pancreatic deoxyribonuclease (DNase I), snake-venom phosphodiesterase (PDE I), spleen phosphodiesterase (PDE II) and alkaline phosphatase. In contrast to unirradiated DNA, which is fully hydrolysed to nucleosides by these enzymes, gamma-irradiated DNA yields a series of oligonucleotides. Their isolation might enalbe the future identification of the chemical nature of DNA lesions.

Multifunctional DNA Nanoplatform Based on CuInS/ZnS QDs and Rolling Circle Amplification for Synergistic PTT/CDT/CT of Multidrug Resistant Cancer Cells.

Synergistic therapy holds significant potential in cancer diagnosis and treatment, yet faces substantial challenges such as nonspecific drug release and tumor multidrug resistance (MDR). Herein, we report a self-assembled DNA nanoplatform (ZCIS QDs@RD) based on CuInS/ZnS QDs (ZCIS QDs) and rolling circle amplification (RCA) for synergistic photothermal therapy (PTT)/chemodynamic therapy (CDT)/chemotherapy (CT). The nanoplatform achieved selective targeting of breast cancer cells by modifying the multivalent AS1411 aptamer. The hydrothermally synthesized ZCIS QDs exhibited excellent fluorescenc

DNase-mediated eDNA removal enhances D-LL-31 activity against biofilms of bacteria isolated from chronic rhinosinusitis patients.

Chronic rhinosinusitis (CRS) is a chronic infection of the nasal cavity and paranasal sinuses associated with the presence of a microbial biofilm. Extracellular DNA (eDNA) is an important component of the biofilm matrix. Antimicrobial peptides (AMPs) are natural peptides with the ability to kill microorganisms. D-LL-31 is a synthetic variant of the AMP cathelicidin with increased resistance to proteolytic breakdown. In this study it is shown for 3 clinical CRS isolates that treatment of 24 h biofilms with DNase I enhanced the antimicrobial activity of D-LL-31. Conversely, co-incubation of D-LL

Snake venom cardiotoxin induces G-actin polymerization.

Snake venom cardiotoxin showed the ability to induce polymerization of G-actin from rabbit skeletal muscle in a low ionic strength buffer composed of 0.2 mM CaCl2/0.2 mM ATP/0.5 mM mercaptoethanol/2.0 mM Tris-HCl, pH 8.0. The activity was enhanced greatly when 0.4 mM MgCl2 was present in the buffer and could be inhibited if G-actin was preincubated with deoxyribonuclease I. Furthermore, the DNAase could also partially depolymerize actin polymer previously formed by the interaction of G-actin with the toxin.

Secretory behaviour of hypertrophic and hyperplastic salivary gland.

The enzyme content and the secretory behaviour of normal rat salivary glands were compared with these properties in glands made hypertrophic and hyperplastic by the chronic administration of isoproterenol. The enlarged glands displayed reductions in the concentrations of ribonuclease, deoxyribonuclease and amylase. The secretory behaviour in vivo was similar for all enzymes in both types of glands, but the enlarged glands secreted a lower percentage of their content in vitro. The reduction in amylase activity was shown by immunological techniques to be due to a reduction in the number of enzym

Immunothrombosis and Complement Activation Contribute to Disease Severity and Adverse Outcome in COVID-19.

Severe COVID-19 is characterized by systemic inflammation and multiple organ dysfunction syndrome (MODS). Arterial and venous thrombosis are involved in the pathogenesis of MODS and fatality in COVID-19. There is evidence that complement and neutrophil activation in the form of neutrophil extracellular traps are main drivers for development of microvascular complications in COVID-19. Plasma and serum samples were collected from 83 patients infected by SARS-CoV-2 during the two first waves of COVID-19, before the availability of SARS-CoV-2 vaccination. Samples were collected at enrollment, day

Identification of surface proteins of Trichinella spiralis muscle larvae using immunoproteomics.

Trichinella spiralis surface proteins are directly exposed to the host's immune system, making them the main target antigens which induce the immune responses and may play an important role in the larval invasion and development process. The analysis and characterization of T. spiralis surface proteins could provide useful information to elucidate the host-parasite interaction, identify the early diagnostic antigens and the targets for vaccine. The purpose of this study was to identify the surface proteins of T. spiralis muscle larvae by two-dimensional gel electrophoresis (2-DE) Western-blot

Neutralizing Anti‒DNase 1 and ‒DNase 1L3 Antibodies Impair Neutrophil Extracellular Traps Degradation in Hidradenitis Suppurativa.

Hidradenitis suppurativa (HS) is a debilitating inflammatory skin disorder characterized by abscess-like nodules and boils resulting in fistulas and tissue scarring. We previously reported evidence of an autoimmune signature in HS, characterized by enhanced neutrophil extracellular trap (NET) infiltration in HS skin lesions and dysregulation of the adaptive immune system characterized by the presence of autoantibodies. Timely removal of NETs is critical for tissue homeostasis to prevent a dysregulated generation of modified autoantigens and tissue damage. DNases 1 and 1L3 play important roles

Development of a rectally administrable Dnase1 to treat septic shock by targeting NETs.

AIMS: Neutrophil extracellular trap (NET), which is formed by DNA threads, induces septic shock by aggravating systemic inflammation. An intravenous administration of deoxyribonuclease is regarded as a compelling modality for treating septic shock. However, alternative routes should be chosen when cutaneous veins are all collapsed due to hypotension. In this study, we genetically engineered this enzyme to develop a rectal suppository formulation to treat septic shock. MAIN METHODS: Dnase1 was mutated at two amino acid residues to increase its stability in the blood and fused with a cell-penet
