Relation Between Reach Distance and Center of Pressure Distance for Multidirectional Reach in Hemiplegic Patients.

Background Sitting balance training is an important therapy used by physical and occupational therapists who work with hemiplegic patients. Objective This study compared reach distance (Rd) and center of pressure distance (COPd) for postural control during a multidirectional reaching task in 26 patients with hemiplegia. Method Following the evaluation of trunk function, we classified subjects according to their Chedoke-McMaster assessment stage: group A, stages 5 and 6; group B, stage <4. We used a functional reach apparatus with a body pressure distribution measurement system and a video came

[Needs, uses, cons-pros, good practices and opportunities about walker in elderly with loss of autonomy].

Non-use of the walker may be secondary to an initial inappropriate prescribing, a lack of adequate training, a lack of monitoring and side effects of using. Improving both stability and mobility in users is due to several biomechanical mechanisms. The benefits of walker are: general physiological effects, more confidence, better social life and decrease in the burden of care. The disadvantages of walker are: technical or practical aspects criticized by users, musculoskeletal disorders, delayed reaction time, fall risk and stigma. Few scientific data evaluating the interest of the walker concer

Energy expenditure during everyday activities--a study comparing people with varying mobility limitations due to multiple sclerosis and healthy controls.

PURPOSE: To investigate energy expenditure of people with multiple sclerosis (MS) during everyday activities. METHODS: Fifteen healthy controls, 19 people with MS who used at most a stick to walk outdoors (MS-A), and 11 people with MS who used bilateral support for gait (MS-B) completed scripted everyday activities. A portable indirect calorimetry unit calculated energy expenditure. Steps were counted from video. RESULTS: There was no significant difference in kcal between the three groups (ANOVA: F(2, 42) = 2.877, p = 0.067). There was a significant difference in steps: F(2, 42) = 17.93, p

Resumption of ambulatory status after surgery for nonambulatory patients with epidural spinal metastasis.

BACKGROUND CONTEXT: Improving the surgical outcome of nonambulatory patients with metastatic epidural spinal compression has been of great interest lately. Although there have been many reports regarding the surgical outcome of spinal metastasis, the surgical outcome in terms of the probability of operative success for nonambulatory patients has not been thoroughly described. If the probability of ambulatory recovery is known, the optimal surgical indications can be determined and implemented. PURPOSE: To predict the surgical outcome and probability of ambulatory resumption for nonambulatory

[Acoustic contamination in urgent medical transportation by road].

BACKGROUND: To determine levels of exposure to noise during urgent medical transportation by road and to describe the perception of hearing function impairment in workers and the use of hearing protection devices. MATERIAL AND METHODS: An observational study was performed in Bizkaia (Spain). The instantaneous maximum noise levels (L(peak)) and its continuous equivalents (L(eq)) were recorded through sonometric dosimetries, measured in a "sample ambulance" of the emergency network. In parallel, a survey was conducted on a representative sample of 127 workers in which self-reported hearing loss

Measuring patient dependency--performance of the Jones Dependency Tool in an Australian emergency department.

BACKGROUND: The emergency department (ED) is a dynamic environment, in which a high volume of undifferentiated patients with varying levels of urgency must be assessed and treated in a timely manner, primarily by nurses. Nurses not only provide initial but also continuing care for all patients presenting to the ED. Predicting the number of nurses required and the ideal ratio of nursing staff to patients presenting is complex. Current methods of predicting ED nursing workforce fail to reflect the severity and variability of illness/injury or case mix. In addition, they exclude the realities of

Heart rate responses to moderate linear body accelerations: clinical implications in aeromedical evacuation.

Linear accelerations during take-off and landing were measured in both civilian and military aircraft usually involved in aeromedical evacuations in Denmark. Accelerations of similar durations and magnitudes were induced in six healthy subjects transported in an ambulance in different supine positions. Heart rate responses recorded depended significantly on the position of the subject. It was concluded that seriously ill patients must be positioned transversely to the axis of acceleration during aeromedical evacuation.

Stroke Symptoms Present in Ambulance Records and Prehospital Time Delay-A Retrospective Cohort Study.

BACKGROUND: The outcome of a stroke is highly dependent on time to treatment. Reducing prehospital delay is crucial to increase access to effective stroke treatment. OBJECTIVE: We aimed to examine how stroke symptoms are recorded and to assess the association between prehospital registration of stroke symptoms and stroke triage and prehospital delay. METHODS: We included patients with a final diagnosis of acute stroke, admitted to Nordland Hospital Trust and Helgeland Hospital Trust in Norway from June 2018 to May 2019. Data were extracted from the Emergency Medical Communication Center reco

Prosthetic rehabilitation for bilateral transfemoral amputees using microprocessor-controlled knees with a novel training program: A study of two cases.

The use of microprocessor-controlled prosthetic knees facilitates safe and efficient ambulation in individuals with unilateral transfemoral amputation, contributing to their reintegration into daily life. We report rehabilitation outcomes in two individuals with bilateral transfemoral amputations that used Kenevo (Ottobock, Germany), a microprocessor-controlled prosthetic knee designed for low-activity users (K1/K2), as the first prosthesis. Gait training began in the most stable mode (A) and then progressed to modes B, B+, or C depending on gait stability. One patient subsequently trained wit

Respiratory care in Duchenne muscular dystrophy.

Respiratory complications are amajor cause of morbidity and mortality in Duchenne muscular dystrophy (DMD). Progressive weakness of inspiratory and expiratory muscles leads to ineffective cough, mucus retention, sleep-disordered breathing, nocturnal hypoventilation, and ultimately chronic respiratory failure. Before the advent of mechanical ventilation, respiratory failure was the leading cause of early death. Advances in multidisciplinary care, including noninvasive ventilation (NIV), airway clearance techniques, corticosteroid therapy, scoliosis management, and cardioprotective treatments, h

Intensive, functional training leads to optimal outcomes in a young woman post brain stem hemorrhage due to cerebral cavernous malformation.

Cerebral cavernous malformations (CCMs) can cause intracranial hemorrhages and account for 5-15% of all cerebral vascular malformations. The purpose of this retrospective case report is to describe the unusual motor recovery of a young woman following a large hemorrhage of a previously unknown brainstem CCM, otherwise not reported in the literature. Case Description: The patient was a 29-year-old female who presented with severe hemiparesis on the left 6 weeks after a first-ever hemorrhage. She had demonstrated minimal improvement in her motor recovery to date, was dependent on a walker for am

Potential interactions of community-based acute care teams on the frequency of acute contacts with health-care systems: an interrupted time series analysis.

BACKGROUND: Ageing populations necessitate innovative approaches to health-care services. In 2018, Denmark introduced statutory municipal acute nursing care as part of community-based health care to improve care for adults with complex health needs. We evaluated the potential interactions of the acute care team Odense (ATO) on (1) acute hospital contacts; (2) acute hospital contacts caused by specific infection diagnoses, hypernatraemia, delirium, or confusion; and (3) acute ambulance transports. METHODS: In this retrospective, population-based, longitudinal study, we employed interrupted tim

Skeletal and body composition changes in hemiplegic patients.

Hemiplegic patients are prone to bone loss and alterations in fat and lean mass, which ultimately affect their rehabilitation status and propensity in bone fractures. The present study aimed to evaluate body composition and bone mineral density (BMD) in stroke patients within 1st year post-stroke. Fifty-eight hemiplegic patients (36 men and 22 women) were enrolled in this prospective study. Dual-energy X-ray absorptiometry was used to assess total-body and lower-extremity BMDs (g/cm(2)), lean mass (g), and fat mass (g) after 3, 6 and 12 mo of stroke that led to hemiplegia. The Modified Ashwort

Unilateral lower limb paralysis after aortobifemoral bypass graft for ruptured abdominal aortic aneurysm: a case report.

An 81-year-old man underwent an aortobifemoral bypass graft because of a ruptured abdominal aortic aneurysm. His postoperative recovery was complicated by unilateral lower limb paralysis caused by perioperative ischemia of the lumbosacral plexus. Ischemic lumbosacral plexopathy is an uncommon complication after infrarenal aortic surgery with serious morbidity. Despite a good surgical technique and knowledge of the vascularization of the spinal cord, its occurrence remains unpredictable.

Identifying factors associated with health care use: a hospital-based risk screening index.

The purpose of the current study was to identify variables near hospital admission that could effectively discriminate patients at risk for nursing home placement, long hospital stay, or readmission. Risk factors reported in the literature were used to predict hospital outcome for 532 admissions. Factors that discriminated type of outcome included: two or more chronic medical conditions, living alone or being admitted from a nursing home, dependent ambulation, poor mental status, psychiatric comorbidity, prior admission, age over 75, and being unmarried. Using these criteria, an index was deve

Factors influencing early admission in a French stroke unit.

BACKGROUND AND PURPOSE: Intravenous tissue plasminogen activator improves outcome after ischemic stroke when given within 3 hours of symptoms onset in carefully selected patients. However, only a small proportion of acute stroke patients are currently eligible for thrombolysis, mainly because of excessive delay to hospital presentation. We sought to determine the factors associated with early admission in a French stroke unit. METHODS: We prospectively studied the admission delay of acute stroke patients in a French stroke unit during a 12-month period ending July 1999. Univariate and multiva
