Clinical Personality Patterns in Alcohol Use Disorder: A Study Focused on Sex Differences.

Background: Research on sex differences in personality disorders profiles among individuals with Alcohol Use Disorder (AUD) remains limited. This study aimed to examine sex differences in personality disorders in AUD individuals attending to an outpatient alcohol and drugs treatment unit. Methods: Persons seeking alcohol detoxification treatment were assessed with the Millon Clinical Multiaxial Inventory-III (MCMI-III) after abstinence. Both dimensional trait scores and cluster personality disorders types distribution were analyzed. A total of 216 subjects, 114 women (53%) and 102 men (47%), p

Early improvement in eating attitudes during cognitive behavioural therapy for eating disorders: the impact of personality disorder cognitions.

BACKGROUND: The personality disorders are commonly comorbid with the eating disorders. Personality disorder pathology is often suggested to impair the treatment of axis 1 disorders, including the eating disorders. AIMS: This study examined whether personality disorder cognitions reduce the impact of cognitive behavioural therapy (CBT) for eating disorders, in terms of treatment dropout and change in eating disorder attitudes in the early stages of treatment. METHOD: Participants were individuals with a diagnosed eating disorder, presenting for individual outpatient CBT. They completed measur

Pathology versus statistical infrequency: potential sources of gender bias in personality disorder criteria.

The antisocial, narcissistic, dependent, histrionic, and borderline personality disorders often obtain differential sex prevalence rates. One explanation has been that the diagnostic criteria for these personality disorders have different gender implications for maladaptivity (e.g., perhaps the dependent personality disorder diagnostic criteria are considered by clinicians to be more pathological for women than for men). This hypothesis was explored in two studies that obtained judgments by professional clinicians of the maladaptivity and statistical infrequency of personality disorder diagnos

Personality features of obsessive-compulsive disorder.

The authors evaluated personality dysfunction in 23 patients with primary obsessive-compulsive disorder and an age- and sex-matched group of patients with major depressive disorder. There were no significant differences between the two patient groups with respect to mean personality trait scores or the frequency or type of personality disorder diagnosis. A mixed personality disorder with avoidant, dependent, and passive-aggressive features was most commonly observed in the obsessive-compulsive group. There was a very low frequency of schizoid or compulsive personality disorder.

Dimensions of dependence and their influence on the outcome of cognitive behaviour therapy for health anxiety: randomized controlled trial.

BACKGROUND: The personality trait of dependence is common in health-seeking behaviour. We therefore examined its impact in a large randomized controlled trial of psychological treatment for health anxiety. AIMS: To test whether dependent personality traits were positive or negative in determining the outcome of an adapted form of cognitive behaviour therapy for health anxiety (CBT-HA) over the course of 5 years and whether dependent personality dysfunction could be viewed dimensionally in a similar way to the new ICD-11 diagnostic system for general personality disorder. METHOD: Dependent pe

Patterns of personality pathology in patients with generalized anxiety disorder, panic disorder with and without agoraphobia, and social phobia.

The relationship between co-occurring personality disorders and anxiety disorders (panic disorder with or without agoraphobia, social phobia, and generalized anxiety disorder) was examined, taking into account the effect of major depression. This article describes findings for 622 participants in the Harvard/Brown Anxiety Research Project, a longitudinal follow-up study of DSM-III-R-defined anxiety disorders. A total of 24% of participants had at least one personality disorder, with avoidant, obsessive compulsive, dependent, and borderline most common. Generalized anxiety disorder, social phob

Are personality disorders associated with social welfare burden in the United States?

This study examined the association between personality disorders and use of major social welfare services in a nationally representative sample of U.S. adults (N = 43,093). Social welfare services received and diagnoses of personality, substance use, mood, and anxiety disorders were assessed with the Alcohol Use Disorder and Associated Disabilities Interview Schedule-DSM-IV-version. Analyses quantified the association between personality disorders and forms of public assistance while controlling for numerous confounds. Logistic regression analyses revealed dependent personality disorder, para

The concurrent validity of a Web-based self-report assessment for personality disorders.

Incorporating online tools in clinical practice could help improve routine assessments of personality disorders and their co-occurring clinical disorders. TeleScreen, a Web-based self-report questionnaire for DSM-IV disorders, has not yet been compared with well-validated structured interviews for clinical and personality disorders. Patients with personality disorders (n = 89) were assessed with TeleScreen and independently interviewed with Structured Clinical Interview for DSM-IV Axis-I disorders (SCID-I) and personality disorders (SCID-II). The concurrent validity was examined using sensitiv

Bipolarity of maladaptive personality traits in the alternative model of personality disorders.

It has been posited that extremely high or extremely low levels of any personality trait in the five-factor model can be maladaptive. However, the Alternative Model of Personality Disorders in Section III of the Diagnostic and Statistical Manual of Mental Disorders-fifth edition (DSM-5) is composed almost exclusively of unipolar maladaptive traits. The lack of maladaptively low neuroticism and high extraversion fails to fully cover psychopathy; the lack of maladaptively high extraversion fails to cover histrionic personality disorder; the lack of maladaptively high agreeableness fails to cover

Personality disorders and premature dropout from psychological treatment for smoking cessation.

The relation between personality disorders and premature dropout (attending half of the sessions or fewer) from a psychological treatment for giving up smoking was examined in a sample of 202 smokers. Percent of premature dropout was significantly higher for smokers with personality disorder in general, specifically for smokers with dependent personality disorder and with Cluster C personality disorder, than in smokers without such psychopathology.

Personality disorders in obsessive compulsive disorder.

Standardized structured interview personality scales are now available that provide better reliability than clinician interview, but are still imperfect. These scales diagnose DSM III-R personality disorders, which are more illness-oriented than Freudian notions. Use of these scales has found that the majority of patients with OCD have at least one Axis II personality disorder, with most falling in cluster C. Obsessive compulsive personality disorder, as described in DSM-III-R, is, in most samples studied, present in the minority of patients with OCD, and is often less common than other person

Nightmare experience and personality disorder functioning styles in healthy volunteers and nightmare disorder patients.

Nightmares are prevalent in psychiatric disorders, and personality disorder features might be associated with nightmare experience, especially in nightmare disorder patients. The authors invited 219 healthy volunteers and 118 nightmare disorder patients to undergo tests of the Nightmare Experience Questionnaire (NEQ), the Parker Personality Measure (PERM), and the Plutchik-van Praag Depression Inventory. Compared to healthy volunteers, nightmare disorder patients scored significantly higher on annual nightmare frequency and NEQ Physical Effect, Negative Emotion, Meaning Interpretation, and Hor

Personality Pathology and Functional Impairment in Patients With Hypochondriasis.

BACKGROUND: Research indicates substantial co-occurance of personality pathology and hypochondriasis, which both involve significant psychosocial impairment. OBJECTIVE: This study sought to investigate the role of personality pathology for explaining functional impairment in patients with hypochondriasis, while accounting for the influence of health anxiety severity. METHODS: Patients diagnosed with hypochondriasis (N = 84; 60% women) were administered interview- and self-report instruments for personality pathology, health anxiety severity, and functional impairment (general, social, and ph

Alcohol-Dependent Subjects Show Different Personality Traits Compared With Subjects With Multiple Substance Dependence: Preliminary Data.

METHODS: We compared personality traits of 27 persons with multiple substance dependence with personality data of 52 alcohol-dependent persons regarding their personality traits and disorders (obtained by using SCID-II, TCI and NEO FFI). Both patient groups were free of any other mental disorder. RESULTS: In SKD-II, we found significant differences in the male group in dependent and scizotypic personality disorder. Prevalence of DSM-III-R self-defeating (masochistic) personality disorder in normal and outpatient populations.

The prevalence of DSM-III-R self defeating (masochistic) personality disorder was determined on an outpatient population (N = 82) and a normal population (N = 40) using the Personality Diagnostic Questionnaire (PDQ). Fifteen (18.3%) of the outpatients and two (5%) of normals were assessed as having the disorder. Overlap of greater than 50% with borderline, avoidant, and dependent personality disorders was found. Validation of masochistic personality disorder will require it to be of value in research and clinically in ways distinct from the other personality disorders with which it is associat

Neuroticism and personality disorder in depression.

Neuroticism and DSM-III personality disorder were studied in 39 depressed inpatients. Interrelationships between these variables and their relationship to depressive typology were compared. The relationship of neuroticism, DSM-III personality type and adequacy of personality to MAOI treatment are also examined. Neuroticism scores were unaffected by short-term treatment, and no differences in neuroticism were observed between melancholics and nonmelancholics, or between endogenous and nonendogenous depressives. Higher neuroticism scores were associated with DSM-III personality disorder. Persona

A comparison of DSM-III personality disorders in panic/agoraphobia and obsessive-compulsive disorder.

One hundred eighty-seven patients meeting DSM-III criteria for panic disorder (161 with agoraphobia) and 51 patients meeting DSM-III criteria for obsessive-compulsive disorder (OCD) were assessed with the Personality Diagnostic Questionnaire (PDQ), a self-rating scale designed to assess axis II personality disorders and traits. The results showed that the personality profiles were similar between the two diagnostic groups and that the major personality characteristics identified in panic/agoraphobic patients, e.g., avoidant, dependent, histrionic, and borderline, were more pronounced in patien

Asthenic personality, myth or reality.

One hundred and seventy five subjects admitted to hospital for neurotic conditions were assessed as to their personality types. Forty seven percent were judged as having personality traits consistent with the diagnosis of asthenic personality disorder. Thirteen of the 138 symptom, life history and psychological test items examined were found to be associated with the asthenic traits. Condensation of these thirteen items by factor analysis showed that two features--anxiety proneness and inability to cope with stress--together defined the disorder.

Validity of criteria for DSM-III self-defeating personality disorder.

Criteria for self-defeating personality disorder were examined using a standard measure in 148 psychiatric outpatients. The best individual criterion was self-defeating B (taken advantage of by others). Self-defeating A (sacrifices needs for others) and K (being hurt is sexually arousing) were of little value. Although some dependent, histronic, and avoidant criteria were also related to self-defeating personality disorder, the disorders appeared fairly clearly separate. There did appear to be more overlap (although not identity) with borderline personality disorder. It appears possible that i
