PURPOSE OF REVIEW: The subcutaneous ICD (S-ICD) and the extravascular ICD (EV-ICD) both avoid intravascular hardware, but they solve sensing and detection differently. This review compares their sensing architectures, detection and discrimination algorithms, therapy capabilities, and system-specific complications, along with practical considerations including extraction, MRI compatibility, and patient-reported outcomes.
RECENT FINDINGS: Inappropriate shock rates have declined substantially for both platforms through algorithmic refinements, two-zone programming and SMART Pass for S-ICD, and i

We report the case of a 50 year-old woman referred for extraction of an infected biventricular implantable cardioverter defibrillator (ICD) who underwent device and leads extraction. Due to adhesions at the level of the subclavian-innominate vein angle, manual traction with locking stylet was ineffective. A new mechanical sheath provided with a stainless steel bladed tip, the Evolution Mechanical Dilator Sheath (Cook Medical), was successfully used to complete lead extraction. As far as we know, this is the first report on the use of this new tool; its safety should be ascertained by increasin

BACKGROUND: Transvenous lead extraction carries a small risk of vascular tears and tricuspid valve (TV) damage.
CASE SUMMARY: A 68-year-old woman with dilated cardiomyopathy and a primary prevention cardiac resynchronization therapy with a defibrillator presented with device infection and was referred for system extraction. During the procedure, she developed acute hemodynamic instability due to an anterior papillary muscle rupture, resulting in a flail anterior TV leaflet and severe tricuspid regurgitation. Emergent TV transcatheter edge-to-edge repair was performed successfully within 48 ho

BACKGROUND: The reported failure rate of the Sprint Fidelis defibrillator lead (SFDL) has increased more than initially expected, with emerging evidence of accelerating fracture rates. Current consensus guidelines continue to discourage prophylactic lead extraction, citing major complication rates of 1.4-7.3%. Therefore, data relating to the risks of systematic SFDL extraction are lacking, with no methodical extraction protocol reported to date. Moreover, few statistical analyses have identified predictors of SFDL failure.
OBJECTIVES: The aims of this single-centre study were: to examine the

With the expanding use of cardiac implantable electronic device (CIED) therapy, intravascular device infections are becoming more common. In the case of transvenous implantable cardioverter-defibrillator (ICD) infections requiring extraction for bacterial clearance, there remains no standard method to deliver temporary ICD therapy following device removal. We present a case of persistent bacteremia complicated by monomorphic ventricular tachycardia (VT) electrical storm where biventricular ICD system extraction was performed and a temporary transvenous dual-coil lead with an externalized ICD g

Background: Rates of pacemaker implantation are steadily increasing and as patients are living longer, endovenous leads remain implanted for an extended period of time thereby increasing the risk of cardiac implantable electronic device (CIED) infection. Investigating fever of unknown origin in patients with implanted pacemakers can be challenging. Recently, 18F-fluorodeoxyglucose positron emission tomography/computerised tomography (18F-FDG-PET/CT) scanning has been used as a diagnostic tool for lead endocarditis in small studies. Objectives: ENDOTEP is a prospective and multicentre study des

BACKGROUND: Right-sided (RS) implanted leads represent a small but clinically relevant subset of transvenous lead extraction (TLE). Anatomical differences and greater lead burden may increase procedural complexity during extraction.
OBJECTIVE: To compare procedural complexity, complications, and outcomes TLE for RS and bilateral-sided vs left-sided only (LS) implanted leads.
METHODS: We analyzed 4288 consecutive patients undergoing TLE, including 121 (2.8%) with extraction of at least 1 RS lead and 4167 with LS leads only. Within the RS cohort, 76 procedures were unilateral and 45 (37%) invo

BACKGROUND: Implantation of cardiac pacemaker leads in the left bundle branch area (LBBA) has been increasingly adopted into clinical practice, driven by the goal of leveraging the natural conduction system to achieve physiologic cardiac pacing. Long-term management of pacemaker leads may require lead extraction to address infection or device malfunction, yet limited clinical data exist on the extraction of LBBA leads chronically implanted deep within the interventricular septum.
OBJECTIVE: The objective of the study is to assess the chronic extractability of a new stylet-driven lead implante

INTRODUCTION: Lead extraction and reimplantation in adult congenital heart disease (CHD) patients is challenging due to anatomical complexity.
CASE: A 76-year-old man with prior ventricular septal defect (VSD) patch repair and pacemaker implantation developed a device infection. Complete transvenous lead extraction (TLE) was achieved using laser and mechanical sheaths. A leadless pacemaker (Aveir VR) was implanted at the right ventricular outflow tract (RVOT) using pre-fixation mapping to avoid the VSD patch.
CONCLUSION: This case illustrates the effectiveness of pre-mapping in achieving saf

With increasing volume of cardiac implantable electronic devices in the last decade, the indications for device extraction have increased. Multidisciplinary collaboration between cardiothoracic surgeons, cardiac anesthesiologists, and cardiac electrophysiologists has been recognized as an essential pre-requisite in pre-procedural planning to limit complications from this inherently risky procedure. Fortunately, the tools and techniques have continued to evolve to make extraction safer and more effective. This article discusses traditional and non-traditional techniques for transvenous lead ext

BACKGROUND: Increasingly, transvenous lead extractions (TLEs) have become more complex.
OBJECTIVE: This study aimed to describe a novel technique for femoral snaring using commercially available equipment and study the application of this technique for a tandem approach in TLE cases with long-dwelling leads compared with conventional TLE.
METHODS: We include 59 patients with long-dwelling transvenous leads who underwent powered sheath TLE with either upfront femoral snaring (n = 25) or conventional approach (n = 34). Our femoral snaring technique uses a grasping device within telescoped shea

BACKGROUND: Lead fragmentation is a common issue during transvenous lead extraction (TLE) leading to incomplete extraction and prolonged procedural time. Retaining the catheter connector pin has proven to increase the lead tensile strength at bench testing.
OBJECTIVE: This study aimed to investigate the feasibility of systematic connector retention ("No-cut technique" [NCT]) during TLE using powered sheaths.
METHODS: Consecutive patients undergoing TLE using the stepwise approach at 2 tertiary centers were included. The first center followed a conventional approach based on cutting the cathe

INTRODUCTION: Cardiac implantable electronic device infections often necessitate extraction, which can be complicated by calcified adhesions, leading to increased procedural risk and reduced efficacy. Given these challenges, intravascular lithotripsy (IVL) has been proposed as a tool for lead extraction to aid in overcoming calcification. This report presents the first reported case where shockwave IVL was successfully employed to extract a severely calcified lead system after an initial extraction attempt failed due to extensive calcification.

BACKGROUND: Cardiac implantable electronic device-related tricuspid regurgitation (TR) is increasingly recognized; however, TR induced primarily by atrial leads is rare.
CASE SUMMARY: A 64-year-old woman with idiopathic dilated cardiomyopathy underwent dual-chamber implantable cardioverter-defibrillator placement. Serial echocardiography demonstrated progressive TR and right atrial dilation caused by a redundant atrial lead loop prolapsing through the tricuspid valve. Over time, left ventricular systolic function improved, ventricular pacing burden remained low, and pulmonary hypertension did

INTRODUCTION: As the global population ages and the use of cardiac implantable electronic devices (CIEDs) increases, transvenous lead extraction (TLE) is being performed more frequently in elderly patients. Concerns remain regarding its safety and efficacy in octogenarians, and age‑specific recommendations are limited.
OBJECTIVES: We aimed to assess procedural success, complication rates, and mortality associated with TLE in patients aged 80 years or older in comparison with younger individuals.

BACKGROUND: Complications related to a cardiac implantable electronic device sometimes require transvenous lead extraction (TLE). We report long-term follow-up of patients undergoing TLE, particularly mortality, recurrent device infection, and need for repeat procedures.
METHODS AND RESULTS: Consecutive patients undergoing TLE at a high-volume center were assessed for characteristics, indications, and outcomes. One thousand and six leads were extracted from 510 patients. Clinical success rate was 98.2% and complete procedural success was 92.2%, with one intraprocedural death. The mean follow-

BACKGROUND: Transvenous cardiac implantable electronic device (CIED) lead extraction (TLE) is susceptible to superior vena cava (SVC) injury and can be performed in the operating room (OR) or electrophysiology lab via a mechanical device or laser-powered extraction. This study reflects a contemporary experience of mechanical right-left rotational extraction by cardiac surgeons in the OR.
METHODS: We conducted a retrospective single-center review of adult (age ≥18 years) TLE cases performed by cardiac surgeons between 2019 and 2021. Leads were extracted via a transvenous mechanical right-left

Transvenous lead extractions have been increasingly performed in the last decade, in line with the rise in cardiac implantable electronic device implantation. Contemporary transvenous lead extraction registries have demonstrated high procedural success rates and low major complication and mortality rates. However, procedural risk remains highly variable and is dependent on several factors. This highlights the importance of risk stratification to facilitate adequate procedural planning including procedural setting (operating room, hybrid laboratory, or device laboratory), presence of surgical b

BACKGROUND: Subclavian venous occlusion is a frequent complication of transvenous cardiac implantable electronic device (CIED) implantation, with reported incidences exceeding 30%. Although often asymptomatic, chronic total occlusion (CTO) creates a major technical barrier during device revision, particularly for cardiac resynchronization therapy (CRT) upgrades. Conventional strategies such as lead extraction or contralateral tunneling may increase procedural risk or compromise future vascular access. Subclavian CTO venoplasty is a minimally invasive alternative to re-establish ipsilateral acc
