Chelators controlling metal metabolism and toxicity pathways: applications in cancer prevention, diagnosis and treatment.

Chelating drugs and chelator metal complexes are used for the prevention, diagnosis and treatment of cancer. Cancer cells and normal cells require essential metal ions such as iron, copper and zinc for growth and proliferation. Chelators can target the metabolic pathways of cancer cells through the control of proteins involved in the regulation of these metals and also of other molecules involved in cell cycle control, angiogenesis and metastatic suppression. Other targets include the inhibition of specific proteins such as ribonucleotide reductase involved in DNA synthesis, the inhibition of

Arctigenin discovered by high-throughput screening ameliorates doxorubicin-induced cardiotoxicity as a novel natural KEAP1-NRF2 inhibitor.

Doxorubicin (DOX), a widely used chemotherapeutic, is significantly limited by dose-dependent cardiotoxicity characterized by oxidative stress, mitochondrial dysfunction, and ferroptosis. Despite decades of research, dexrazoxane (DXZ) remains the only approved cardioprotectant, yet its clinical utility is constrained by potential reduction in efficacy and systemic toxicity. Effective alternatives are therefore urgently needed. Through high-throughput screening of a 960-compound natural product library, we identified arctigenin (ATG), a major bioactive compound from the Asian medicinal plant Ar

Doxorubicin-Induced Cardiotoxicity in Breast Cancer: Mechanistic Pathways, Pharmacogenomic Modifiers, and Translational Strategies.

Doxorubicin, a prototypical anthracycline, remains an important component of breast cancer therapy, although its use has declined with the availability of effective non-anthracycline regimens and is increasingly restricted to selected high-risk settings. At the same time, anthracyclines continue to serve as first-line therapy for multiple malignancies, including lymphomas, leukemias, and sarcomas, sustaining the broad clinical relevance of anthracycline-associated cardiotoxicity. This timely review synthesizes mechanistic, pharmacogenomic, and translational evidence on doxorubicin-induced card

Cytogenetic evidence that DNA topoisomerase II is not involved in radiation induced chromsome-type aberrations.

ICRF-187 (Cardioxane™, Chiron) is a catalytic inhibitor of DNA topoisomerase II (Topo II), proposed to act by blocking Topo II-mediated DNA cleavage without stabilizing DNA-Topo II-"cleavable complexes". In this study ICRF-187 was used to evaluate the potential involvement of DNA topoisomerase II in the formation of the radiation-induced chromosome-type aberrations in the G0 phase of the cell cycle in human lymphocytes from three healthy male donors. This is based on many evidences that DNA topoisomerases are involved in DNA recombination, mainly of illegitimate type (non-homologous) both in v

[Anthracycline-Induced Cardiotoxicity and Exploration of Cardioprotective Drugs].

Many anticancer drugs, including anthracycline drugs, pose a risk of cardiovascular damage as an adverse reaction. This can detrimentally impact the prognosis and quality of life of patients, potentially leading to the interruption of cancer chemotherapy and compromising cancer treatment. Recently, onco-cardiology (or cardio-oncology) has developed as a new interdisciplinary field that focuses on the prevention and treatment of cardiovascular toxicity of anticancer drugs. In this review, we explore the mechanism underlying the cardiotoxicity of anthracyclines and examine pharmacological agents

Bone marrow mesenchymal stem cells alleviate the daunorubicin-induced subacute myocardial injury in rats through inhibiting infiltration of T lymphocytes and antigen-presenting cells.

INTRODUCTION: Bone marrow mesenchymal stem cells (BMSCs) have been extensively investigated from a perspective on cardiac regeneration therapy. The current study aimed to investigate the protective effect conferred by BMSCs in subacute myocardial injury, and to identify an appropriate BMSC reinfusion time. METHODS: BMSCs were isolated from human bone marrow blood. Daunorubicin (DNR)-induced subacute myocardial models were subsequently established. The rats with DNR-induced subacute myocardial injury were injected with dexrazoxane (DZR) and/or BMSCs at varying time points, after which cardiac

Late-onset cardiomyopathy among survivors of childhood lymphoma treated with anthracyclines: a systematic review.

Medical advances in pediatric oncology have led to increases in survival but the long-term adverse effects of treatment in childhood cancer survivors have not yet been examined in depth. In this systematic review, we aimed to study the prevalence and risk factors of late-onset cardiomyopathy (LOCM) among survivors of childhood lymphoma treated with anthracyclines. Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines we searched Pubmed/Medline, abstracted data and rated studies on quality regarding late-onset (>1 year following treatment) cardioto

Comparison of various iron chelators used in clinical practice as protecting agents against catecholamine-induced oxidative injury and cardiotoxicity.

Catecholamines are stress hormones and sympathetic neurotransmitters essential for control of cardiac function and metabolism. However, pathologically increased catecholamine levels may be cardiotoxic by mechanism that includes iron-catalyzed formation of reactive oxygen species. In this study, five iron chelators used in clinical practice were examined for their potential to protect cardiomyoblast-derived cell line H9c2 from the oxidative stress and toxicity induced by catecholamines epinephrine and isoprenaline and their oxidation products. Hydroxamate iron chelator desferrioxamine (DFO) sig

Anthracyclines in the treatment of cancer. An overview.

Anthracyclines are widely used and effective antineoplastic drugs. Although active against a wide variety of solid tumours and haematological malignancies, their clinical use is hindered by tumour resistance and toxicity to healthy tissue. Modification of the general anthracycline ring structure results in analogues with different but overlapping antitumour and tolerability profiles. Activity of the anthracyclines is related to topoisomerase II inhibition, which occurs as a result of anthracycline intercalation between adjacent DNA base pairs. Production of hydroxyl free radicals is associated

Anthracyclines in a patient with acute leukemia and severe cardiomyopathy requiring mechanical support: A case report.

INTRODUCTION: For young adult patients with acute leukemia, both the efficacy and cardiotoxicity of anthracycline-based regimens have been documented. We report the case of a patient with severe cardiomyopathy, mechanically supported by a left ventricular assist device (LVAD), who subsequently developed Philadelphia-chromosome positive acute lymphoblastic leukemia (Ph + ALL). To our knowledge, this is the first report of anthracycline administration in a patient with heart failure requiring mechanical support. CASE REPORT: Our 27-year-old female patient was diagnosed with Ph + B-ALL as part o

Mechanisms and clinical course of cardiovascular toxicity of cancer treatment I. Oncology.

The opening session of Second International Colloquium on Cardio-Oncology addressed two areas of vital interest. The first reviewed new thoughts related to established agents. While anthracycline cardiotoxicity has been studied and reviewed extensively, ongoing research attempting to understand why it appears the mechanism(s) of toxicity differs from that of oncologic efficacy continue to evoke comment and intriguing speculation. Better understanding of the role of β-topoisomerase II in toxicity has advanced our understanding of the cascade of events that lead to heart failure. Additionally, t

[Cardiac toxicity in cancer therapy].

The aim of this article is to review (based on the literature data) the mechanism of chemotherapy- and radiation-induced cardiac toxicity, diagnostic procedures and methods of reducing this toxicity. Cardiac toxicity associated with chemotherapy and radiotherapy may be life threatening, can limit the dose and duration of the treatment and certainly adversely affect short-term and long-term quality of life. A development of new strategies for reduction and prophylaxis of cardiac toxicity has great clinical impact. Chemotherapeutic agents may cause acute myocardial injury or chronic complication

Chemotherapy-induced oxidative injury in pediatric acute lymphoblastic leukemia: The role of N-acetylcysteine (Review).

Pediatric acute lymphoblastic leukemia (ALL) is the most common childhood malignancy. However, intensive chemotherapy frequently leads to notable organ toxicity, much of which is mediated by treatment-induced oxidative stress. Reactive oxygen species (ROS) generated during cytotoxic therapy contribute to tissue damage, including the liver, heart and nervous system. Current adjunctive therapies provide drug-specific protection, such as dexrazoxane for anthracycline-induced cardiotoxicity, but they do not address the shared ROS-generating pathway, a common mechanism of chemotherapy-induced toxic

Emerging Research and Future Directions on Doxorubicin: A Snapshot.

Doxorubicin, a widely used anthracycline antibiotic, has been a cornerstone in cancer chemotherapy since the 1960s. In addition to doxorubicin, anthracycline chemotherapy medications include daunorubicin, idarubicin, and epirubicin. For many years, doxorubicin has been the chemotherapy drug of choice for treating a broad variety of cancers. Despite its efficacy, doxorubicin therapy is hindered by serious side effects, primarily cardiotoxicity, and the challenges of drug resistance. Recent research has focused on optimizing doxorubicin's therapeutic index by developing cardioprotective strategi

Clinical guidelines for the management of cytostatic extravasation in children.

Extravasation of cytostatic drugs is a rare but potentially devastating complication of chemotherapy in children, leading to severe pain, tissue necrosis, functional impairment, and cosmetic deformities. Despite its importance, management protocols remain inconsistent. This paper presents structured instructions to support rapid recognition, intervention, and prevention of cytostatic extravasation in paediatric patients, aiming to reduce morbidity and optimise outcomes. The guidance was developed from published recommendations, systematic reviews, and institutional experience, and it provides

Exploring the Pleiotropic Cardioprotective Effects of GLP-1 Receptor Agonists in Preventing Anthracycline-Induced Cardiotoxicity: A Theoretical Proposal for Future Research.

Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) have been shown to reduce morbidity and mortality associated with type II diabetes mellitus, and/or obesity, and/or cardiovascular disease in multiple clinical trials. Their efficacy in reversing cardiovascular disease and mitigating the risk of major adverse cardiac and vascular events has been well studied, with outcome trials consistently demonstrating benefits such as reduced systemic inflammation, improved endothelial function, and favorable metabolic effects. These pleiotropic actions have nearly innumerable potential applications, wi
