Surgical treatment of diaphragmatic eventration caused by phrenic nerve injury in the newborn.

BACKGROUND: Phrenic nerve palsy in infants and young children usually results from birth injury or iatrogenic damage. The newborn almost invariably presents with severe respiratory distress, diaphragmatic elevation, and paradoxical movement at the affected side. METHODS/RESULTS: In this retrospective analysis a group of 23 patients below the age of 1 year with an obstetric or postoperative phrenic nerve injury was studied and compared with cases in the literature. All patients were admitted between 1986 and 1997 to the Pediatric Surgical Center, Amsterdam. Thirteen of 18 patients with an obst

Uniportal thoracoscopic plication of diaphragmatic eventration: loop needle technique for better visualization.

Symptomatic unilateral diaphragmatic eventration require surgical intervention. A 56-year-old woman complained of dyspnoea on exertion and was noted to have left diaphragm elevation on chest radiographs. Dynamic magnetic resonance imaging showed paradoxical movement of the left diaphragm. We performed diaphragmatic plication by uniportal thoracoscopy with knifeless endostaplers and a loop needle device. Her symptoms significantly improved immediately after the operation, and this condition had been maintained for 6 months. We thus suggest this minimally invasive technique as an easy and safe m

Right-sided diaphragmatic eventration: a rare cause of non-immune hydrops fetalis.

We report 2 cases of non-immune hydrops fetalis (NIHF) in which autopsy findings revealed an association with right-sided congenital diaphragmatic eventration (CDE). Both patients born at 30 weeks of gestation presented with severe generalized skin oedema, pleural effusions and ascites. They both died shortly after birth of cardiorespiratory insufficiency due to lung hypoplasia and low output heart failure. The right thoracic cavity was filled with the membranous but intact right diaphragm covering the herniated visceral organs including parts of the liver, small bowel and colon surrounded by

An effective plication technique for diaphragmatic eventration under thoracoscopy with laparoscopic image display.

Diaphragmatic eventration causes respiratory distress with the development of severe diaphragmatic compression of lung volume. While non-surgical treatment, such as physical therapy and pulmonary rehabilitation, is adequate for mild cases, surgical intervention is critical for severe diaphragmatic eventration. A 45-year-old man with respiratory fatigue was diagnosed with left diaphragmatic eventration and underwent surgery with diaphragmatic plication with double-row stapling under a video-assisted approach. Thoracoscopy with laparoscopic image display avoided visceral tissue involvement, and

Surgical treatment of diaphragmatic eventration using video-assisted thoracic surgery: a prospective study.

BACKGROUND: This study seeks to evaluate results regarding the repair of diaphragmatic eventration using video-assisted thoracic surgery (VATS). METHODS: We performed a prospective observational study including patients referred to us for surgical treatment of diaphragmatic eventration during a 12-year period. Clinical, radiologic, and functional data were prospectively recorded. VATS was performed with two thoracoports and a 4-cm mini-thoracotomy. Diaphragmatic plication was performed using two nonresorbable running sutures from periphery to the cardio-phrenic angle. Follow-up data (clinical

Incidentally detected diaphragmatic eventration as para-cardiac thoracic activity on myocardial perfusion imaging.

Diaphragmatic eventration is the elevation of hemi-diaphragm without any disruption to diaphragmatic continuity which can be congenital or acquired. The most common acquired cause is phrenic nerve paralysis due to traumatic causes and is usually incidentally diagnosed on chest radiograph or computed tomography. We hereby report a case of a patient who had road traffic accident with fracture of the left proximal femur. Stress Myocardial Perfusion Imaging (MPI) done for pre-operative clearance showed an incidental tracer avidity adjoining to left myocardium in the thorax. It was confirmed on ana

Usefulness of liver scan using 99mTc-phytate in diagnosis of partial diaphragmatic eventration.

Partial diaphragmatic eventration, when occurring at uncommon sites, is difficult to diagnose when only using a plain X-p and fluoroscopy. We experienced an unusual variety of this disorder. In order to diagnose this case, liver scintigraphy using 99mTc-phytate was performed, and we were able to make a definite diagnosis without other invasive examinations.

Clinical evaluation and surgical intervention for diaphragmatic eventration mimicking peritoneopericardial hernia in a cat.

A 2-year-old spayed female British Shorthair cat presented with an increased frequency and duration of cough since infant period. Based on radiographic, ultrasonographic, and computed tomography findings, peritoneopericardial diaphragmatic hernia was considered so that repair surgery was planned. During celiotomy, lax diaphragm was identified instead of defect. Transabdominal diaphragmatic plication was performed to resolve lax diaphragm and to prevent recurrence by overlapping relatively normal part of diaphragm. Diagnosed with diaphragmatic eventration postoperatively, the cat showed improve

Thoracoscopic Repair of a Rare Case of Congenital Diaphragmatic Hernia with Eventration (Mixed Lesion).

Congenital diaphragmatic malformations include congenital diaphragmatic hernia (CDH) and eventration of the diaphragm. The clinical presentation is variable, and the prognosis depends on multiple factors. The coexistence of CDH and diaphragmatic eventration in the same patient is extremely unusual and has not been reported previously in the literature. We report this rare association in a 3-month-old male infant who was managed successfully by minimally invasive approach.

Enrolment criteria for surgical plication in pediatric diaphragmatic eventration: a systematic review of the literature.

PURPOSE: Diaphragmatic eventration (DE) is an uncommon but clinically significant condition in children. Although diaphragmatic plication is widely performed in symptomatic patients, standardized criteria guiding surgical indication remain poorly defined. This review aimed to identify the clinical-radiological parameters most consistently used to select pediatric surgical candidates. METHODS: We performed a literature search for English-language studies (2000-2025) reporting pediatric DE cases. Studies limited to radiology, genetics, or unrelated diaphragmatic disorders were excluded. Extract

To Sink the Lifted: Selection, Technique, and Result of Laparoscopic Diaphragmatic Plication for Paralysis or Eventration.

Introduction Diaphragmatic eventration is a congenital defect of the muscular portion of a hemidiaphragm that eventually leads to hemidiaphragmatic elevation and dysfunction. The clinical diagnosis of diaphragmatic eventration or diaphragmatic paralysis may be indistinguishable and diaphragmatic plication is the treatment of choice for both conditions. Discussion We review the indications, patient selection, and surgical techniques for diaphragmatic plication. We explain our preferred technique and guide the reader step by step on our approach. Conclusion Minimally invasive diaphragm plication

Modified diaphragmatic plication prevents and rectifies dyspnea with diaphragmatic eventration caused by phrenicotomy or injuries associated with extended mediastinal surgeries.

BACKGROUND: Postoperative dyspnea caused by diaphragm eventration due to phrenic nerve injury after extended mediastinal tumor surgery severely affects postoperative recovery. This study describes a modified diaphragmatic plication (MDP) technique utilizing a stapler and evaluates the feasibility of this method in preventing and treating such dyspnea. METHODS: MDP using a cutting stapler consists of four steps: clamping and lifting, depressing and resecting, cutting and verifying, and hemostasis and reinforcement. This modified method serves three purposes: serves as a prophylactic measure ag

Thoracoscopic Plication for Neonatal Diaphragmatic Eventration With Intrathoracic Ectopic Kidney: A Rare Case Report.

Coexistence of congenital diaphragmatic eventration (CDE) and an intrathoracic ectopic kidney (ITEK) is extremely rare and may lead to substantial respiratory distress in neonates. This study reports the case of a small-for-gestational-age male neonate presenting with respiratory distress and requiring nasal continuous positive airway pressure. Computed tomography revealed right-sided CDE accompanied by an ITEK. At 44 days old, the infant underwent thoracoscopic diaphragmatic plication. CO2 insufflation during surgery led to spontaneous reduction of the ectopic kidney into the abdominal cavity
