[Insanity and suicide, as described in the "Okuni-nikki (Diaries of our clan)" of the Hirosaki Clan Government].

The "Okuni-nikki (Diaries of our Clan)" of the Hirosaki Clan Government covers the years 1661-1867. Akitomo Matsuki and Yōichi Hanada picked up medical topics from these diaries, and edited them in two volumes of "Tsugaru-ijibunka-shiryōshūsei: Okuni-nikki" (1993, 1994). I selected cases of insanity and suicide from these volumes. Selected were (1) 12 cases of insanity (in its wider sense) (5 cases of insanity, 2 cases of depression, a case of mania, a case of hypochondriasis, a case of epilepsy, a case of pavor nocturnus, and a case of insomnia), (2) 16 cases of suicide and self injury, cased

A study of the clinical activity of a gel combining monocaprin and doxycycline: a novel treatment for herpes labialis.

BACKGROUND: Current treatment of herpes labialis is usually with topical antiviral drugs and early drug administration is required for effectiveness. Monocaprin, a 1-monoglyceride of capric acid, has high microbicidal activity in vitro and efficiently inactivates herpes simplex virus. Tetracyclines are inhibitors of matrix metalloproteinases that are part of the inflammatory response and contribute to the breakdown of tissue in ulcers. The study objective was to investigate the antiviral and wound-healing effect of a hydrogel containing either monocaprin or a combination of monocaprin and a lo

Linking Maternal Problematic Smartphone Use to Maternal Hostility and Warmth Toward Children: A Daily Diary Study of Chinese Families.

Although the implications of parental smartphone use for parenting have been discussed among researchers and practitioners, prior studies have mostly focused on the between-person associations of problematic smartphone use with negative parenting behaviors. Therefore, using 5 days of daily diary data, the present study examined whether day-to-day fluctuation in maternal problematic smartphone use was associated with day-to-day fluctuation in maternal hostility as well as warmth toward children, on the same day and on the following day. Data were collected from mothers of 74 fourth graders (mea

Reflective and brooding rumination as moderators of the associations between positive and negative social media experiences, depressed and anxious mood, and positive and negative affect among Hispanic adults: A daily diary approach.

INTRODUCTION: Beyond the frequency of social media use, the valence of social media experiences is important to consider. The current study examined how positive and negative social media experiences associate with mental health and well-being-related outcomes while also assessing how reflective and brooding rumination may moderate these associations given the role of rumination in mood, affect, and social experiences. Investigation among Hispanic groups is crucial as Hispanics report non-trivial rates of social media use and depressive and anxiety symptoms. METHOD: Seventy-five Hispanic adul

Validation of the HIV Risk Assessment of Sexual Partnerships (H-RASP): Comparison to a 2-Month Prospective Diary Study.

The HIV Risk Assessment of Sexual Partnerships (H-RASP) was developed in order to create a retrospective measure of sexual risk-taking that can account for the differing contexts of sexual partnership(s) within a specified period of time. In order to validate the H-RASP relative to other methods of measuring sexual risk-taking, measurements from the H-RASP were compared to data from a prospective diary study of 95 young men who have sex with men over the same two-month period. We found that the H-RASP was not significantly different at measuring participants' total number of sexual partners an

Stool patterns of Malaysian adults with functional constipation: association with diet and physical activity.

INTRODUCTION: Diet and lifestyle modification is commonly used in constipation management. As there is a dearth of studies on this topic in Malaysia, we aim to elucidate the relations between stool patterns, dietary intake and physical activity levels among adults with functional constipation. METHODS: From a database collected via surveys at public events, a convenience sample of 100 adults diagnosed with Rome II-defined functional constipation was enrolled in this cross-sectional study. After severity assessment using the Chinese Constipation Questionnaire, subjects completed 2-week bowel m

Comparison of a novel aluminum lactate-based with an aluminum chloride-based antiperspirant in excessive axillary and inguinal perspiration: first randomized controlled trial.

BACKGROUND: Topical aluminum salts are a commonly used remedy for excessive axillary perspiration. To the contrary, less is known about their anhidrotic potential in the groin. This study sought to compare the anhidrotic efficacy and tolerability of an aluminum chloride-based antiperspirant to an innovative aluminum lactate-based antiperspirant in healthy study participants presenting with excessive axillary and inguinal perspiration. PARTICIPANTS AND METHODS: Fifty participants were enrolled in this open open-labeled, randomized, controlled trial. Following a baseline assessment, efficacy wa

Re-evaluation of the diagnosis of porphyria cutanea tarda in Admiral Sir Francis Beaufort.

OBJECTIVES: Two biographies of Admiral Francis Beaufort (1774-1857) have stated that, aged 20-25 years, he suffered from porphyria cutanea tarda (PCT) that was 'cured' following severe blood loss during a naval skirmish. We have examined the evidence concerning the nature of his skin disease. DESIGN: Primary records, most notably Beaufort's correspondence with his family, his journals and his father's diaries were sought out and analysed. SETTING: This case report is discussed in the context of 18th-century naval medicine and concepts and treatment of skin disease. RESULTS: The description

Efficacy of the topical nasal steroid budesonide on improving sleep and daytime somnolence in patients with perennial allergic rhinitis.

BACKGROUND: Improving quality of life is considered to be a major endpoint and motivation for clinical intervention in patients with perennial allergic rhinitis (PAR). In addition to classical symptoms of congestion, pruritus, and rhinorrhea, patients will often complain of not being able to sleep well at night and of feeling fatigued during the day. Like sleep apnea, PAR has also been shown to cause sleep disturbance and consequently worsen daytime fatigue and somnolence. HYPOTHESIS: It is proposed that by decreasing nasal obstruction due to allergic rhinitis by treating with the topical ste

A randomized controlled pilot study investigating adherence to blood pressure diaries with personal pictures in stroke follow-up care.

INTRODUCTION: Blood pressure (BP) management is essential in secondary stroke prevention. Strategies to ensure continuous home BP monitoring are needed. Few studies investigated factors influencing adherence to home BP management. Therefore, we designed a pilot study to investigate the feasibility of keeping BP diaries (BPDs) with personal images. METHODS: In this prospective trial, we randomized persons with a diagnosis of stroke or transient ischemic attack into two groups: (i) 10 patients received a personalized BPD with pictures of their choosing and (ii) 10 patients received a BPD withou

Development of a Data-Enabled Mixed Method for Designing for Patients From a Human-Centered Design Perspective: Explorative Case Study.

BACKGROUND: In the early stage of human-centered design (HCD), qualitative and generative methods are commonly used to explore patients' contexts and needs, emphasizing active patient involvement to ensure that design insights accurately reflect real experiences and enhance both design effectiveness and patient empowerment. However, certain challenges arise in the early stage of the HCD process, including (1) high vulnerability of patient participants, (2) less diverse and representative patient groups due to recruitment challenges, and (3) insufficient problem framing across diverse patient e

Impact of Thumb Carpometacarpal Joint Osteoarthritis: A Pragmatic Qualitative Study.

OBJECTIVE: First carpometacarpal (CMC1) joint osteoarthritis (OA) is typically understood as part of the disease entity of hand OA. However, CMC1 joint OA often occurs in isolation or is a primary source of symptoms. The aim of the current study was to explore the experiences of New Zealanders with CMC1 joint OA to better understand the unique impact of this condition, ascertain outcomes of importance, and identify treatment targets. METHODS: In this pragmatic qualitative study, patients who either reported a history suggestive of CMC1 joint OA or had been diagnosed by a physician were recrui

Evidence for the management of mastalgia.

Mastalgia is the commonest breast symptom presenting to general practitioners and breast surgeons alike. To make a full assessment of the cause, all patients require a full history, examination and, sometimes, investigations. Diary cards are often helpful. The commonest cause is cyclical mastalgia. Most women require reassurance only and the pain often settles spontaneously after a few months. For the remainder, simple lifestyle changes should be suggested initially, such as wearing a well-fitted sports bra, weight reduction, regular exercise and a reduction in caffeine intake. Unfortunately,

Would a new definition and classification of nocturia and nocturnal polyuria improve our management of patients? ICI-RS 2014.

The following is a report of the proceedings of the Nocturia Think Tank sessions of the annual International Consultation on Incontinence-Research Society, which took place September 22-24, 2014 in Bristol, UK. The report is organized into sections pertaining to the main topic of discussion focussing on the question as to whether a new definition and classification of nocturia and nocturnal polyuria would improve the outcome of management in our patients. First, discussions identified theoretical and practical shortcomings of current definitions. Secondly, the utility of several nocturnal poly

Drinking and condom use: results from an event-based daily diary.

Although it is often assumed that drinking alcohol interferes with condom use, most studies on this topic do not meet the conditions required for causal interpretation. We examined the association of drinking to condom use using data from diaries of alcohol use and sexual encounters, collected over 8 weeks from college students and clients of a sexually transmitted disease clinic. This method establishes the temporal relationships between drinking and condom use and controls for individual differences by using a within-subjects analysis. Multilevel models that predicted condom use from alcohol
