Ultra-low-dose recombinant human bone morphogenetic protein-2 for 3-level anterior cervical diskectomy and fusion.

This study evaluated the safety of 3-level anterior cervical diskectomy and fusion (ACDF) with ultra-low-dose recombinant bone morphogenetic protein-2 (rhBMP-2). Thirty-seven consecutive patients with cervical spondylotic myelopathy who were treated with 3-level ACDF and rhBMP-2 were evaluated. Complications such as airway or cervical swelling or hematoma were not observed. The rate of dysphagia was no different at 1, 2, and 6 months postoperatively compared with reports in the literature without rhBMP-2. There were significant improvements in VAS neck/arm pain, Oswestry Neck Disability Index,

Efficacy and outcomes of dynamic-plated single-level anterior diskectomy/fusion with additional analysis of comparative costs.

BACKGROUND: Few studies focus on the fusion rates and outcomes for single-level anterior cervical diskectomy/fusion (1-ACDF) utilizing iliac autograft and dynamic plates. METHODS: Fusion rates and outcomes were prospectively evaluated in 60 consecutive patients undergoing 1-ACDF utilizing iliac autograft and dynamic plates (ABC; Aesculap, Tuttlingen, Germany). Eighteen patients had radiculopathy, while 42 were myelopathic (average Nurick Score 3.3). Pathology included single-level disc disease/spondylosis (38 patients) and/or ossification of the posterior longitudinal ligament (OPLL, 22 patie

Relief of Cervicogenic Headaches after Single-Level and Multilevel Anterior Cervical Diskectomy: A 5-Year Post Hoc Analysis.

STUDY DESIGN: Prospective study. OBJECTIVE: Because single-level disk arthroplasty or arthrodesis in the lower subaxial spine improves headaches after surgery, we studied whether this effect may be better appreciated after two-level arthroplasty. METHODS: We performed an independent post hoc analysis of two concurrent prospective randomized investigational device exemption trials for cervical spondylosis, one for single-level treatment and the other for two adjacent-level treatments. RESULTS: For the one-level study, baseline mean headache scores significantly improved at 60 months for both

Prevertebral Soft-Tissue Swelling Following One-Level Anterior Cervical Diskectomy and Fusion: An Analysis Based on Surgical Level.

The purposes of this study were to identify the primary level at which prevertebral tissue swelling (PSTS) occurs following one-level anterior cervical diskectomy and fusion (ACDF) based on surgical level, and to quantify the degree to which it occurs. Although PSTS peaks at day 2 or 3 after ACDF, with swelling noted to be prominent at the C2-C4 levels, the way in which the features of PSTS vary according to surgical level has not been examined.Thirty-seven patients who underwent one-level ACDF were reviewed and classified into retropharyngeal and retrotracheal groups based on surgical level.

Preoperative Cannabis Use Is Associated With Elevated Risk for Complications and Revision Surgery After Anterior Cervical Diskectomy and Fusion.

BACKGROUND: Cannabis use has increased markedly in recent decades, with US adult usage doubling from 4.1% to 9.5% between 2001 and 2013, and 43% of young adults reporting use by 2021. As legalization expands and cannabis becomes more mainstream, its perioperative implications have become clinically relevant. Although the orthopaedic literature has begun exploring the effect of cannabis use, its effect on anterior cervical diskectomy and fusion (ACDF) remains underinvestigated. Given the widespread adoption of ACDF and the projected rise in spinal procedures, understanding cannabis-related risk

Prospective Comparison of Microsurgical, Tubular-Based Endoscopic, and Endoscopically Assisted Diskectomies: Clinical Effectiveness and Complications in Railway Workers.

OBJECTIVE: Although endoscopic diskectomy is superior to microsurgical diskectomy in terms of incision size, postoperative pain, and cosmetic appeal, the effectiveness and indications for endoscopic versus microsurgical diskectomy remain active discussion topics. Because of the increasing incidence of diskectomies being performed in Russia, further assessment of these techniques is needed. We performed a comparative analysis of 1-year clinical results and complications of microsurgical, tubular-based interlaminar endoscopic, and endoscopically assisted microsurgical diskectomies for patients w

Migraines as a Risk Factor for Many 90-Day Postoperative Complications Following Single-Level Anterior Cervical Diskectomy and Fusion.

INTRODUCTION: Anterior cervical diskectomy and fusion (ACDF) is a common spinal surgery for which patient factors may be associated with adverse outcomes. One such potential predisposing risk factor is a history of migraines. The relationship between migraines and postoperative adverse outcomes following ACDF procedures has not been studied. Appreciating such correlations may aid in patient counseling and care pathways. METHODS: Using the 2015-Q3 2022 PearlDiver Mariner161 database, adult patients undergoing single-level ACDF were identified. Patients who underwent concomitant spinal procedur

How I do it: total uncinatectomy during anterior diskectomy and fusion for cervical radiculopathy caused by uncovertebral joint hypertrophy.

BACKGROUND: Cervical radiculopathy from uncovertebral joint hypertrophy and foraminal stenosis is a common indication for anterior cervical diskectomy and fusion (ACDF). Often, the uncinate hypertrophy extends lateral to the foramen and impinges on the nerve close to the vertebral artery as it travels in between the transverse foramina. METHOD: Using an injected cadaveric specimen to highlight the vital neurovascular and bony structures pertinent to this procedure, we demonstrate the technical details of complete uncinatectomy for cervical foraminal stenosis. CONCLUSION: Total uncinatectomy

Longer Operative Time Is Associated With Increased Adverse Events After Anterior Cervical Diskectomy and Fusion: 15-Minute Intervals Matter.

Little is known about the impact of operative time, as an independent and interval variable, on general health perioperative outcomes following anterior cervical diskectomy and fusion. Therefore, patients undergoing a 1-level anterior cervical diskectomy and fusion were identified in the American College of Surgeons National Surgical Quality Improvement Program. Operative time (as an interval variable) was tested for association with perioperative outcomes using a multivariate regression that was adjusted for differences in baseline characteristics. A total of 15,241 patients were included. In
