Angulation error assessment for the trajectory in the anteroposterior and lateral fluoroscopic views during percutaneous endoscopic transforaminal lumbar discectomy.

BACKGROUND: Anteroposterior (AP) and lateral fluoroscopies are often used to evaluate the intraoperative location and angulation of the trajectory in percutaneous endoscopic transforaminal lumbar discectomy (PETLD). Although the location of the trajectory shown in fluoroscopy is absolutely accurate, the angulation is not always reliable. This study aimed to evaluate the accuracy of the angle shown in the AP and lateral fluoroscopic views. METHODS: A technical study was performed to assess the angulation errors of PETLD trajectories shown in AP and lateral fluoroscopic views. After reconstruct

Percutaneous Disc Decompression for Lumbar Radicular Pain: A Review Article.

BACKGROUND: Open discectomy remains the standard of treatment for patients with lumbar radicular pain secondary to a prolapsed intervertebral disc. Open discectomy performed in patients with small, contained herniations may result in poor outcomes. The various techniques of percutaneous disc decompression (PDD) have been developed to address this population. METHODS: A literature search was conducted on articles, which address PDD for lumbar radicular pain. Published techniques include chymopapain chemonucleolysis, percutaneous laser disc decompression (PLDD), automated percutaneous lumbar di

"Minimally invasive" lumbar spine surgery: a critical review.

BACKGROUND: Minimal-access technology has evolved rapidly with "tubular" or "percutaneous" approaches for decompression and stabilization in the lumbar spine. Potential benefits (smaller scars, diminished local pain, reduced blood loss, reduced postoperative wound pain, shorter hospital stays) have to be weighed against possible drawbacks (reduced orientation, steep learning curve, increased radiation exposure, dependency on technology, cost). While non-comparative case series are often rather enthusiastic, comparative studies and particularly RCTs are scarce and might convey a more realistic

Percutaneous spinal interventions.

Interventional neuroradiology procedures of the spine are being performed with increasing frequency. These therapies complement and, in some cases, replace more conventional operations of the vertebral column and its contents. This article surveys the background, present application, and future horizons of several minimally invasive spinal interventions, including vertebroplasty and kyphoplasty, microcatheterization of the cervical epidural space via lumbar puncture for drug delivery, percutaneous intraspinal navigation, and percutaneous spinal fixation.

Percutaneous Endoscopic Resection of Lumbar Discal Cyst.

BACKGROUND: Discal cysts, an uncommon condition, can replicate the characteristic signs typically linked to a herniated lumbar disc, encompassing discomfort in the lumbar region and neuralgia that extends along the nerve paths, thereby complicating the process of distinguishing the discal cyst from other conditions. Consensus on the treatment of this disease remains elusive, and the best treatment for it is still a matter of controversy. In numerous past reports, this disease has been treated through either open or microscopic surgical approaches. OBJECTIVES: The aim of this study is to asses

Percutaneous cervical nucleoplasty using coblation technology. Clinical results in fifty consecutive cases.

Conventional open cervical discectomy, with or without bony fusion, in common neurosurgical knowledge is considered the standard treatment for cervical disc herniation. Percutaneous procedures are minimally invasive and offer decreased morbidity, require no bone graft and promise shorter recuperation time. Nevertheless, candidates for a percutaneous procedure as inclusion criteria must complain of symptoms related to contained herniated disc or focal protrusion. It does not substitute conventional open procedures required for extruded discs. We used the coblation technology for nucleoplasty of

Modified Posterior Percutaneous Endoscopic Cervical Discectomy for The treatment of Degenerative Cervical Spondylotic Myelopathy Caused by Vertebral Posterior Osteophytosis.

BACKGROUND: The clinical application of posterior percutaneous endoscopic cervical discectomy (PPECD) achieves stable curative effects and satisfactory results in patients with cervical spondylotic radiculopathy. However, the management of PPECD in the treatment of CSM is rarely discussed. CASE DESCRIPTION: A case of CSM in a 37-year-old woman with vertebral posterior marginal osteophytosis was managed by modified PPECD with anterior bony decompression. Here, we describe the first case report of CSM complicated by vertebral posterior marginal osteophytosis that was successfully treated by mod

Transpedicular Lumbar Endoscopic Surgery for Highly Migrated Disk Extrusions: Preliminary Series and Surgical Technique.

OBJECTIVE: Endoscopic surgery for highly migrated lumbar disk extrusions is a challenge even for spine surgeons who are familiar with using endoscopic techniques. Because of the anatomic constraints involved in transforaminal access in endoscopic surgery, an incomplete removal of a highly migrated disk extrusion can result in some cases. Here the authors describe a new technique for accessing extruded lumbar disks that have migrated into the canal directly through a transpedicular approach. METHODS: A technique for the endoscopic treatment of highly migrated lumbar disk extrusions is presente

Percutaneous laser nucleolysis of the intervertebral lumbar disc. An experimental study.

Percutaneous laser discectomy has developed as a new modality in the treatment of herniated intervertebral discs. An experimental study of percutaneous nucleolysis using a carbon dioxide laser beam was performed in 10 dogs, each dog having 2 intervertebral disc spaces treated, for a total of 20 discs. The intradiscal pressure decreased in all instances with the drop ranging from 10% to 55% at the L2-L3 disc and 40% to 69% at the L4-L5 disc. The macroscopic and microscopic studies showed vaporization of the nucleus pulposus, and in 8 spaces, thermal damage to the end plate was observed. This st

Endoscopic management of lumbar spinal tophaceous gout: six cases treated with percutaneous transforaminal (PTED) and interlaminar (PIED) discectomy and a literature review.

BACKGROUND: Gout is a metabolic disorder caused by the deposition of urate crystals in the joints and surrounding tissues. Prolonged hyperuricemia leads to the formation and accumulation of urate crystals, often resulting in acute and chronic joint damage. Spinal gout is a rare type caused by the deposition of urate crystals in spinal tissues. It leads to local inflammation, pain, and, in severe cases, neurological symptoms. The clinical presentation of spinal gout is varied and lacks specificity, making it easily confused with other spinal disorders, especially in elderly patients. METHODS:
