The analysis of the gained experience in treatment of patients with pathology of the arch and descending portion of the aorta is suggestive that the 'frozen elephant trunk' technique is an effective and safe method. In this connection, there is an increasing need for universal hybrid devices characterized by ease of use and durability in various aortic pathologies. Discussed in the article is the state of the art of the technology of treatment of patient with aortic arch pathology by means of hybrid grafts, followed by comparative analysis of currently used vascular hybrid prostheses, and alth

Treatment of type IB endoleak after thoracic endovascular aortic repair (TEVAR) for post-dissection aortic aneurysm usually includes attempts of endovascular interventions using coils or plugs to occlude the false lumen or placement of a distal fenestrated endograft. Open conversion usually requires deep hypothermia and circulatory arrest with the associated increased mortality and complications. We present a case of a young patient with a 90 mm descending thoracic aneurysm caused by a chronic type B aortic dissection. A type II endoleak after TEVAR was successfully treated with left subclavia

Several successful cases of endovascular treatment of type B dissections in patients with Marfan syndrome have been reported. In our patient with Marfan syndrome, a type B dissection was successfully treated endovascularly. Three weeks after this procedure, a computed tomographic angiography (CTA) revealed a perforation of the aortic wall distal to the left subclavian artery by a bare strut of the stent graft. A second stent graft was placed to treat this complication. In patients with Marfan syndrome, complications might be prevented by using stent grafts specifically developed to treat disse

Staged repair of extensive thoracic aortic aneurysms puts certain patients at risk of rupture. We report the case of a patient with Marfan syndrome who presented with subacute type-A aortic dissection and a large descending aortic aneurysm. We used the arch-first technique with a commercially available Dacron T-graft. A clamshell incision was used for exposure. A button of arch vessels was anastomosed to the T-graft. Antegrade cerebral perfusion was established through the side branch. The distal end of the graft was anastomosed to the descending aorta and the proximal end to a composite graft

In surgical treatment of aortic arch aneurysm or aortic dissection, we have been employing retrograde cerebral perfusion (RCP). In the present study, we have developed "arch vessels first technique" to shorten perfusion time and achieved an excellent clinical outcome. The surgical procedure "arch vessels first technique" is outlined as follows. RCP is initiated following circulatory arrest, while a woven dacron graft (12 mm in diameter, the dacron graft must be prepared in advance) with two side arms (12 mm in diameter, i.e., with the same diameter) is anastomosed left subclavian artery, left

The purpose of this study was to examine differences in blood supply to facial flaps created by three rhyditectomy techniques. The techniques chosen for comparison included a two-layer technique, consisting of separate subcutaneous and extended submuscular aponeurotic system (SMAS) dissections, the Composite dissections as described by Hamra, and a subperiosteal dissection. Six cadavers were injected with lead oxide before dissection, and eight were injected after dissection. After allowing the lead oxide to set, the soft tissues were removed from the face. Vascular patterns of the face were i

Valve-sparing aortic root replacement is increasingly used in selected patients with acute aortic dissection and preserved valve anatomy. We present the case of a 52-year-old man with acute Stanford type A aortic dissection extending from the aortic root to the iliac bifurcation with arch involvement; this was complicated by severe aortic insufficiency and right lower extremity malperfusion. The patient underwent emergent valve-sparing root replacement using the David technique combined with zone 2 arch reconstruction under hypothermic circulatory arrest and selective antegrade cerebral perfus

Dissecting aneurysms occur when blood extrudes into the wall of a vessel. Posterior circulation dissections are recognised as an important cause of cerbral infarction and subarachnoid haemorrhage(SAH), however posterior inferior cerebellar artery (PICA) aneurysmal dissections are rare. A 49-year-old man who presented with SAH was found to have a left PICA dissection on cerebral angiography. The lesion was treated with surgical clipping proximal to the dissection and a distal PICA to PICA anastomosis. The pathology, diagnosis, presentation and treatment of these difficult lesions is discussed.

BACKGROUND: The introduction of the single-step total arch replacement (TAR) with frozen elephant trunk (FET) has revolutionised the field of aortic surgery. TAR is indicated when the aortic arch is involved in aortic pathologies such as Type A aortic dissection and thoracic aortic aneurysms. Several FET devices are available commercially for global use, and example is the E-Vita Open NEO hybrid prosthesis (HP) developed by CryoLife-JOTEC. Unlike other FET devices available, this HP in particular features a design that puts it at a disadvantage as it does not incorporate gelatine or collagen i

Historically, a variety of non-dissection proximal aortic pathologies with suitable anatomy have been treated with thoracic endovascular aortic repair (TEVAR) with high technical success rates. However, TEVAR use for acute type A aortic dissection has been extremely limited due to the unique challenging anatomic and clinical features and the lack of specifically-designed endoprostheses. These features include: proximity of the aortic valve, coronary arteries, and supra-aortic vessels, related aortic insufficiency, diffusely dilated aorta diameter, entry tear extension into the aortic root and/

PURPOSE: To describe the management of complications from an unsuccessful hybrid repair of an aortic arch aneurysm.
CASE REPORT: A 63-year-old man with acute type B dissection and retrograde dissection into the aortic arch underwent emergent hybrid repair, with partial debranching (ascending aorta to left carotid artery bypass) and proximal stent-graft deployment. At 3 months, computed tomography (CT) showed stent-graft migration, causing a large type I endoleak; flow through the patent left subclavian artery (LSA) caused a large type II endoleak. At a second operation, a bilateral subclavian

BACKGROUND: Patients with chronic aortic dissection require monitoring for indications of disease progression. In present study, inflammation adjacent to associated aortic wall was evaluated by indium-111-oxine-labeled leukocyte scintigraphy, scince inflammation of the blood vessel wall often associates with progression of chronic aortic dissection.
METHODS AND RESULTS: Fifteen patients with aortic dissection underwent indium-111-oxine-labeled leukocyte scintigraphy. Seven showed positive images at sites corresponding to the actual sites of the dissociated aorta. Four patients with positive i
