Hernia : the journal of hernias and abdominal wall surgery2007eng
We present a rare occurrence of a perforated diverticulum of the sigmoid colon found within the contents of a strangulated left inguinal hernia in a 75-year-old man. The hernia was repaired using the Bassini technique. An end colostomy was established after resection of the diseased section of the sigmoid. We review the relevant literature and discuss the choice of repair techniques in such situations.

Giant colonic diverticulum, defined as a single diverticulum ≤ 4 cm, is rarely encountered. Due to the high incidence of complications related to the disease, obtaining the correct diagnosis early in the disease course is essential. Diagnosis is usually reached by conventional and cross-sectional abdominal radiography. Treatment decisions should be ideally made by a multidisciplinary discussion among surgeons, interventional radiologists, and the patient. The treatment of choice is the surgical management by open or laparoscopic approach.

INTRODUCTION: Giant colonic diverticulum is a rare complication of colonic diverticulosis that occurs when a single diverticulum enlarges to over 4cm in diameter. There have been fewer than 200 cases reported worldwide since it was first described in 1946.

Diverticulum of the female urethra has an incidence of 1.4% to 4.7%. It is generally agreed that the majority of these are acquired lesions. There is, however, evidence that some are of congenital origin. Documented cases of diverticula with colon-type tissue are rare. A polypoid elevation of the colonic wall was identified on air contrast barium enema in six patients. The abnormality in each case was found to be an inverted colonic diverticulum. Inverted diverticula appeared as broad-based, smooth, sessile polyps measuring 1.5-2.0 cm. In five of the six patients a characteristic central umbilication and/or evidence of barium within the polyp could be identified. A case is reported in which bleeding from a Meckel's diverticulum was mistakenly localized to the hepatic flexure of the colon on superior mesenteric angiography. An unusually long and mobile mesentery, allowing the bleeding Meckel's diverticulum to be positioned high in the right upper quadrant, led to this misdiagnosis.
