Severe pain as a possible cause of dropped head syndrome that was attenuated after amputation of an ischemic lower limb.

BACKGROUND: Dropped head syndrome (DHS) is defined as weakness of the neck extensor muscles causing a correctable chin-on-the-chest deformity. Here we report the case of a patient with severe pain from lower leg ischemia showing DHS whose symptoms were attenuated by pain relief after amputation of the severely ischemic lower leg. To our knowledge this is the first report indicating that severe pain can cause DHS. CASE PRESENTATION: A 64-year-old Asian woman was referred to our department with a 1-month history of DHS. She also suffered from severe right foot pain because of limb ischemia. She

A Novel Missense Variant in the AGRN Gene; Congenital Myasthenic Syndrome Presenting With Head Drop.

Congenital myasthenic syndromes (CMS) are a heterogeneous group of diseases of the neuromuscular junction caused by compromised synaptic transmission. Clinical features include early-onset weakness of limbs and oculobulbar muscles resulting in hypotonia, bulbar paresis, ptosis, and hypoventilation. The first dropped head syndrome in children were detected in 2 patients with LMNA and SEPN1 mutations. We report a 17-month-old boy with dropped head and limb-girdle weakness, who had no ptosis or ophthalmoplegia at presentation. We performed whole exome sequencing, which revealed a homozygous misse

A case of dropped head syndrome due to focal myositis that worsened with cellulitis and improved only by treatment of cellulitis.

Dropped head syndrome due to focal myositis is extremely rare. Due to the rarity of the disease, its clinical characteristics and prognosis remain unknown. We present a unique case of dropped head syndrome due to focal myositis that exacerbated following cellulitis and was dramatically improved along with the improvement of her cellulitis only treated with antibiotics. We should consider the possibility of preceding trigger event such as infection when we face with the exacerbation of focal myositis before making a decision of strengthening immunosuppressive therapy to avoid unnecessary increa

Neutral lipid storage disease with myopathy and dropped head syndrome. Report of a new variant susceptible of treatment with late diagnosis.

Neutral lipid storage disease with myopathy (NLSDM) is characterized by the accumulation of cytoplasmic triglyceride droplets in various tissues; this very rare condition is caused by mutations in the PNPLA2 gene, susceptible to specific pharmacological management that decreases clinical progression. We describe the clinical and biochemical characteristics of a Colombian patient with a previously unreported homozygous mutation in the PNPLA2 gene with a difficult to manage disease, who was diagnosed late by advances in molecular techniques.

Development and internal validation of a prognostic scoring system for Conservative treatment outcomes in idiopathic neck extensor Myopathy-Dropped head syndrome.

PURPOSE: Prognostic factors for conservative treatment of dropped head syndrome (DHS) remain unclear. The objectives of this study were to identify prognostic factors for conservative treatment outcomes in patients with isolated neck extensor myopathy-related dropped head syndrome (INEM-DHS) and to develop a prognostic scoring system to support clinical treatment decision-making. METHODS: One hundred consecutive INEM-DHS patients who visited our hospital and could be followed for six months of conservative treatment were included. Patients were classified into two groups: the improved group (I

Outcomes of Conservative Versus Surgical Treatment of Dropped Head Syndrome in a Single Institution: A Case Series and Review of the Literature.

Dropped head syndrome (DHS), also known as camptocephalia, is characterized by a mobile chin-on-chest deformity from hypotonia of the cervical extensor muscle or hypertonia of the anterior neck muscles. There remains a paucity of quality published literature on this topic, particularly its management. The objective of this study is to identify radiographic and clinical outcomes of surgery as compared to nonsurgical treatment, to summarize the literature, and to create a decision-making paradigm for managing patients with DHS. As such, we report outcomes of our retrospective chart review series

The Occurrence of Dropped Head Syndrome After a Cervical Medial Branch Nerve Block in a Patient With Cervicothoracic Kyphotic Deformity: A Case Report.

Complications from medial branch blocks (MBBs) are rare when following proper procedural protocol. Dropped head syndrome (DHS) is characterized by profound muscle weakness in the cervical spine, resulting in a failure to maintain a level horizontal gaze and, in the worst cases, a chin-on-chest deformity. In this case report, we described DHS developing after cervical MBBs using short-acting anesthetic agents and subsequent management. A 69-year-old woman with a previous C6-C7 anterior cervical discectomy and fusion (ACDF) underwent bilateral posterior cervical MBBs targeting the C4-C5 and C5-C

Dropped head syndrome as the sole presenting manifestation of probable post-cytomegalovirus immune-mediated brainstem encephalitis in an immunocompetent woman: a case report.

BACKGROUND: Dropped head syndrome (DHS) is a rare, disabling chin-on-chest deformity caused by severe cervical extensor weakness. The etiologic spectrum is dominated by peripheral myopathies, motor neuron disease, and myasthenia gravis; central nervous system causes are seldom considered, particularly in older adults where coexisting cervical spondylosis appears to offer a unifying structural explanation. To our knowledge, DHS as the sole presenting manifestation of brainstem encephalitis has not been previously reported. CASE PRESENTATION: A 60-year-old immunocompetent woman presented with f

Hyperbaric oxygen therapy for late onset dropped head syndrome following mantle field radiation therapy for Hodgkin lymphoma: a case report and literature review.

The authors present the first documented case of the apparent effective use of hyperbaric oxygen therapy (HBOT) for the treatment of radiation-induced dropped head syndrome (DHS). DHS is a condition associated with progressive and often severe weakness of cervical paraspinal muscles, especially the neck extensors. This results in loss of horizontal gaze and in advanced cases causes a chin-on-chest deformity. Radiation-induced DHS is a rare, primarily late-term complication first described in patients treated with mantle field radiotherapy for Hodgkin lymphoma, though there is an increasing bod

Paroxysmal head drops with ataxia-like symptoms presenting as Sandifer syndrome in a 3-year old girl.

We present a case of Sandifer syndrome in a 3-year-old girl who initially presented with a history of recurrent paroxysmal head drops associated with ataxia-like symptoms and recurrent falls sustaining a clavicular fracture on one occasion. She was referred to and seen by the paediatric neurologist. Physical examination, electroencephalogram, MRI brain, electromyograph single fibre study and blood tests were all normal. With the history of hiccups and choking-like episodes she was referred to the speech and language therapist (SALT). SALT assessment did not reveal indications of swallowing imp

Three-dimensional gait analysis reveals differences in spinal balance and muscle activity during prolonged walking in patients with dropped head syndrome based on global spinal alignment.

PURPOSE: No studies have elucidated the dynamic spinal balance in patients with dropped head syndrome (DHS) categorized by global spinal alignment. We investigated the differences in dynamic spinal balance and corresponding muscle activity during prolonged walking in patients with DHS based on their global spinal alignment. METHODS: Three-dimensional gait analysis combined with electromyography was conducted to evaluate kinematic spinal parameters during walking, including the sagittal vertical axis (SVA) in the cervical (C-SVA), thoracic (T-SVA), and lumbar (L-SVA) regions, along with the mu

Unravelling the receptor binding property of egg drop syndrome virus (EDSV) from the crystal structure of EDSV fiber head.

Egg drop syndrome virus (EDSV) is an avian adenovirus that causes markedly decrease in egg production, and in the quality of the eggs when it infects chickens. Until now, EDSV virus-cell interactions are poorly understood, and the cellular receptor is still unknown. In the present study, we determined the atomic structure of the fiber head of EDSV (residues 377-644) at 2.74 Å resolution. Structure comparison with the (chick embryo lethal orphan) CELO long fiber head and human adenovirus fiber heads reveals that the avian adenovirus may interact with the same attachment factor in a unique fashi

Bobble-head doll syndrome and drop attacks in a child with a cystic choroid plexus papilloma of the third ventricle. Case report.

The authors report an unusual case of a 2-year-old boy with a 3-month history of episodic rightward anterolateral head tilt and large-amplitude positional anteroposterior head bobbing reminiscent of bobble-head doll syndrome. This child experienced a sudden onset of drop attacks and then, within several hours, deep coma. The causative lesion was a contrast-enhancing, partially cystic third ventricular mass, which ultimately obstructed the aqueduct, producing profound obstructive hydrocephalus. An emergency ventriculostomy and endoscopic fenestration of the septum pellucidum was performed. Four

Dropped head in systemic sclerosis: a case based review.

Dropped head syndrome is a rare disease entity characterized by severe weakness of the cervical para-spinal muscles, resulting in a chin-on-chest deformity. Systemic sclerosis is one of the causes of dropped head syndrome, but its characteristics and prognosis remain unclear due to the extreme rarity of this condition. We present a case of dropped head which occurred in systemic sclerosis. He presented with severe dropped head and relatively mild weakness of the proximal limb muscles. Serum level of creatine kinase was elevated, myopathic change was observed in electromyography, and gadolinium

Dropped head presentation of mitochondrial myopathy.

Dropped head secondary to weakness of the neck extensors has been reported in a wide assortment of neuromuscular disorders. Infrequently, dropped head can be the first sign of disease. We describe two patients with dropped head as the presenting manifestation of mitochondrial myopathy. In both patients, serum lactate was elevated and muscle biopsy showed mitochondrial proliferation. Mitochondrial myopathy should be considered in the differential diagnosis of dropped head syndrome, particularly when other, more common causes such as myasthenia gravis, polymyositis, and amyotrophic lateral scler

Surgical Management of Postradiation, Dropped Head Spinal Deformity in Patients with Head and Neck Cancer.

BACKGROUND: Dropped head syndrome is a morbid condition that affects daily functionality, causing pain and dysphagia and respiratory compromise. Reported causes of dropped head syndrome include neuromuscular disorders, iatrogenic from cervical spine surgery, and idiopathic and postradiation for head and neck cancers. Management of this spinal disorder remains challenging, as the complication rates are high. We present our series of 7 patients who underwent surgical correction of dropped head syndrome, all resulting from radiation for head and neck cancers. METHODS: This was a retrospective re
