[Newly approved drugs for Alzheimer disease: effectiveness and limitation].

In addition to donepezil, 2 other cholinesterase inhibitors galantamine and rivastigmine, and an N-methyl-D-aspartic acid receptor antagonist, memantine, have been recently approved for the treatment of Alzheimer disease patients at mild to moderately severe and moderately severe to severe stages of the disease, respectively, in Japan. These drugs were approved about a decade ago in Europe, US, and some other countries in Asia, and have been widely used since then. Many studies have been performed to evaluate the clinical effectiveness and cost-effectiveness of these drugs for treating cogniti

A year in pharmacology: new drugs approved by the US food and drug administration in 2025.

The US Food and Drug Administration approved 45 new drugs and 4 new cellular and gene therapy products in 2025, that is, a total of 49 new medical therapies. Oncology, hematology/immunotherapy, and cardiovascular/respiratory drugs remain most frequent. Among oncology drugs, there is a trend for targeting ever smaller subtypes of malignancies and/or rarer malignancies. Trends over the past 6 years include steady levels of first-in-indication (about 10%), first-in-class (about 38%), and next-in-class (about 51%) approvals. Along with a large share of approvals in orphan indications (> 50%), fast

A year in pharmacology: new drugs approved by the US Food and Drug Administration in 2023.

With 54 new drugs and seven cellular and gene therapy products, the approvals by the US Food and Drug Administration (FDA) recovered 2023 from the 2022 dent back to the levels of 2020-2021. As in previous years of this annual review, we assign these new drugs to one of three levels of innovation: first drug against a condition ("first-in-indication"), first drug using a novel molecular mechanism ("first-in-class"), and "next-in-class," i.e., a drug using an already exploited molecular mechanism. We identify four (7%) "first-in-indication," 22 (36%) "first-in-class," and 35 (57%) "next-in-class

Mechanisms of Action of the US Food and Drug Administration-Approved Antisense Oligonucleotide Drugs.

Antisense oligonucleotides (ASOs) are single stranded nucleic acids that target RNA. The US Food and Drug Administration has approved ASOs for several diseases. ASOs utilize three principal modes of action (MOA). The first MOA is initiated by base-pairing between the ASO and its target mRNA, followed by RNase H-dependent mRNA degradation. The second MOA is triggered by ASOs that occlude splice acceptor sites in pre-mRNAs leading to skipping of a mutation-bearing exon. The third MOA involves ASOs that sterically hinder mRNA function, often inhibiting translation. ASOs contain a variety of modif

Drug repurposing strategy II: from approved drugs to agri-fungicide leads.

Epidemic diseases of crops caused by fungi deeply affected the course of human history and processed a major restriction on social and economic development. However, with the enormous misuse of existing antimicrobial drugs, an increasing number of fungi have developed serious resistance to them, making the diseases caused by pathogenic fungi even more challenging to control. Drug repurposing is an attractive alternative, it requires less time and investment in the drug development process than traditional R&D strategies. In this work, we screened 600 existing commercially available drugs, some

A year in pharmacology: new drugs approved by the US Food and Drug Administration in 2021.

The second year of the COVID-19 pandemic had no adverse effect on the number of new drug approvals by the US Food and Drug Administration (FDA). Quite the contrary, with a total of 50 new drugs, 2021 belongs to the most successful FDA years. We assign these new drugs to one of three levels of innovation: (1) first drug against a condition ("first-in-indication"), (2) first drug using a novel molecular mechanism ("first-in-class"), and (3) "next-in-class", i.e., a drug using an already exploited molecular mechanism. Interactions of antiepileptic drugs with drugs approved for the treatment of indications other than epilepsy.

Introduction: Comorbidities of epilepsy may significantly interfere with its treatment as diseases in the general population are also encountered in epilepsy patients and some of them even more frequently (for instance, depression, anxiety, or heart disease). Obviously, some drugs approved for other than epilepsy indications can modify the anticonvulsant activity of antiepileptics. Areas covered: This review highlights the drug-drug interactions between antiepileptics and aminophylline, some antidepressant, antiarrhythmic (class I-IV), selected antihypertensive drugs and non-barbiturate inject

Antiprotozoal Activity Profiling of Approved Drugs: A Starting Point toward Drug Repositioning.

Neglected tropical diseases cause significant morbidity and mortality and are a source of poverty in endemic countries. Only a few drugs are available to treat diseases such as leishmaniasis, Chagas' disease, human African trypanosomiasis and malaria. Since drug development is lengthy and expensive, a drug repurposing strategy offers an attractive fast-track approach to speed up the process. A set of 100 registered drugs with drug repositioning potential for neglected diseases was assembled and tested in vitro against four protozoan parasites associated with the aforementioned diseases. Drug approval based on randomized phase 3 trials for relapsed malignancy: analysis of oncologic drugs granted accelerated approval, publications and clinical trial databases.

Background As relapsed disease is frequently the first target of newly developed therapies, it is vital to address the difficulty in demonstrating the efficacy of new drugs for relapsed malignancy in randomized phase 3 trials. Methods We analyzed the approved indications, target populations, and development status of post-marketing confirmatory trials of all oncology-related drugs that were granted accelerated approval for both hematological and solid malignancies. Furthermore, we searched for randomized phase 3 trials for adult patients with relapsed lymphoid malignancy, other than chronic ly

Factors influencing FDA drug approval decisions beyond clinical trial evidence: a scoping review.

INTRODUCTION: The Food and Drug Administration's (FDA) approval decisions are expected to rest primarily on clinical evidence, yet concerns persist about whether political pressure, commercial interests, or institutional dynamics also influence the approval process. METHODS: We conducted a scoping review of empirical studies published between 1990 and 2025 evaluating factors other than clinical evidence in FDA drug approval decisions, following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) framework. RESULTS: Fifteen studies

Roadmap for developing MRD as an early endpoint for drug approval in lymphoma.

Advances in lymphoma therapy have extended survival, but exposed limitations of traditional trial endpoints efficacy assessments. We propose two MRD-informed endpoints to accelerate drug development and improve regulatory decision-making: modified PFS (mPFS) and undetectable MRD (uMRD) rate. mPFS is a specific variant of event free survival (EFS) defined as the time from study enrollment until positive end-of-treatment MRD, clinical progression, or death and has potential as a validated surrogate endpoint for traditional approval. uMRD rate at a prespecified timepoint measures depth of respons

Expedited research and approval of medicines for drug-resistant bacterial infections in children.

Multidrug-resistant pathogens cause severe infections with substantial morbidity and mortality in neonates, infants and children, as they do in adults. However, the approval of therapies for multidrug-resistant infections in paediatric patients often lags years behind approval for adults, which creates an untenable situation for people who care for affected children. Many issues contribute to the current unacceptably long period between adult and paediatric new drug approval, including the requirement for sequential age group enrolment in clinical trials, the types of regulatory studies requir

Accelerated Approval Of Cancer Drugs: No Economic Reward For Drug Makers That Conduct Confirmatory Trials.

The Food and Drug Administration uses expedited approval of drugs to speed the development and assessment of drugs that address unmet needs related to serious or life-threatening conditions. Drugs approved via this route rely on surrogate endpoints or other clinical indicators that are not direct measures of benefits to patients, such as survival or quality of life. Companies are required to conduct a clinical trial confirming that a drug provides long-term benefits that are clinically meaningful, but prompt completion of these trials frequently does not occur. Theory suggests that because con

The Dawn of Allosteric BCR-ABL1 Drugs: From a Phenotypic Screening Hit to an Approved Drug.

Chronic myeloid leukemia (CML) is driven by the constitutive activity of the BCR-ABL1 fusion oncoprotein. Despite the great success of drugs that target the BCR-ABL1 ATP-binding site in transforming CML into a manageable disease, emerging resistance point mutations impair inhibitor binding, thereby limiting the effectiveness of these drugs. Recently, allosteric inhibitors that interact with the ABL1 myristate-binding site have been shown to awaken an endogenous regulatory mechanism and reset full-length BCR-ABL1 into an inactive assembled state. The discovery and development of these allosteri

Drug screening of food and drug administration-approved compounds against Babesia bovis in vitro.

Babesia (B.) bovis is one of the main etiological agents of bovine babesiosis, causes serious economic losses to the cattle industry. Control of bovine babesiosis has been hindered by the limited treatment selection for B. bovis, thus, new options are urgently needed. We explored the drug library and unbiasedly screened 640 food and drug administration (FDA) approved drug compounds for their inhibitory activities against B. bovis in vitro. The initial screening identified 13 potentially effective compounds. Four potent compounds, namely mycophenolic acid (MPA), pentamidine (PTD), doxorubicin h

Patents on Small-Molecule Drugs Approved by the US Food and Drug Administration, 1990-2019.

IMPORTANCE: Patent protection allows pharmaceutical companies to charge high prices for prescription drugs during periods of market exclusivity, and their expiration usually defines when generic competitors can enter the US market. Patents on drug active ingredients are classified as primary patents, while patents on formulations, methods of use, and delivery devices are nonprimary. OBJECTIVE: To evaluate how pharmaceutical patent portfolios for US Food and Drug Administration (FDA)-approved drugs have evolved over time. EVIDENCE REVIEW: Patents on small-molecule drugs approved by the FDA fr
