Recent advances in microneedle technology in transdermal drug delivery systems.

INTRODUCTION: Transdermal drug delivery systems (TDDs) offer significant advantages over traditional drug administration routes (oral and parenteral); however, the barrier function of the stratum corneum limits its application to compounds possessing the obligatory physicochemical characteristics. To overcome the limitations of TDD, micro-sized needles can be used. Microneedles (MN), a non-invasive drug delivery system, can disrupt the skin's integrity, generating microchannels that facilitate the efficient infusion of drugs. AREAS COVERED: The review highlights the designing, (bio)materials

Intrathecal drug delivery systems: an overview of cost-effectiveness evidence.

Economic evaluations, which systematically compare the costs and health-related outcomes of health interventions, are essential for informing resource allocation decisions, particularly in the context of financial constraints and limited healthcare budgets. Intrathecal drug delivery (ITDD) systems are widely used for the management of cancer pain, chronic non-cancer pain, and spasticity. However, ITDD involves substantial upfront costs, ongoing maintenance expenses, and potential complications. This article provides an overview of health-related economic evaluation principles and of the eviden

Biomimetic delivery systems for overcoming drug resistance in gastrointestinal cancers.

Gastrointestinal (GI) cancers remain a major cause of cancer-related mortality worldwide, and multidrug resistance (MDR) is a critical barrier to durable therapeutic response. In GI malignancies, MDR arises from multilayered and interdependent factors, including insufficient intracellular drug accumulation, altered drug activation and metabolic detoxification, target rewiring and phenotypic plasticity, apoptosis-autophagy-mediated stress adaptation, and extrinsic protection from the tumor microenvironment, cancer stem cells, metastatic niches, and microbiome-associated mechanisms. Biomimetic d

Blood-Brain Barrier: Structure, Function, Diseases, and Drug Delivery Systems.

The blood-brain barrier (BBB) is a highly selective and dynamic neurovascular interface essential for maintaining central nervous system homeostasis. This specialized barrier comprises brain microvascular endothelial cells interconnected by tight junctions, supported by pericytes and astrocytic end-feet within the neurovascular unit. While protecting the brain from circulating pathogens and toxins, the BBB presents formidable obstacles to drug delivery, restricting approximately 98% of small-molecule therapeutics and nearly all large biomolecules from reaching the brain parenchyma. BBB dysfunc

Drug delivery systems in glaucoma - Current innovations and future perspectives.

Glaucoma is the leading cause of irreversible visual loss worldwide, and as yet, there is no cure. The only evidence-based treatment to slow progression is by lowering intraocular pressure (IOP). Despite the development of new topical medications to reduce IOP, the major limitation of eyedrops lies in human and anatomical factors, namely patient compliance and poor bioavailability, making current medical glaucoma treatment ineffective. In this manuscript, we summarise the limitations of traditional topical anti-glaucoma therapy and study current drug delivery systems to lower IOP, with focus o

Nano-conjugated drug delivery systems for enhanced tumour-specific targeting.

Cancer remains among the major causes of death in the world, yet the treatment has been hampered and hindered by the issue of nonspecific drug toxicity, lack of selectivity and resistance. Drug delivery systems based on nanocarriers have become one of the promising platforms to enhance the precision and efficacy of therapeutic options. Of particular interest among them are glycan-functionalized nanocarriers since they can utilize cancer-related glycosylation patterns to achieve better tumor-specific targeting, greater biocompatibility, and lesser systemic toxicity. Precision nanomedicine is ba

Advances in cerebral edema research and targeted drug delivery systems.

Cerebral edema, marked by excessive brain fluid accumulation, hinders stroke recovery and impacts survival, highlighting the need for effective therapies. This review examines the glymphatic system's role in post-stroke edema pathogenesis, explores edema formation mechanisms, and identifies therapeutic targets. While small molecule drugs show promise, their limited solubility and brain targeting necessitate advanced delivery approaches. Nanodrug delivery systems, capable of crossing the blood-brain barrier (BBB) and targeting cells via ligands, offer a compelling solution. We discuss the appli

Smart Stimuli-Responsive Drug Delivery Systems for Advanced Diabetes Management.

Traditional treatment methods for the management of diabetes, such as oral hypoglycemic medications and insulin injections, include drawbacks like systemic adverse effects, inconsistent medication levels, and low compliance. To avoid difficulties, glycemic levels in diabetic patients, a long-term metabolic condition, must be precisely and consistently controlled. Smart therapeutic systems allow for precise, on-demand medication release in response to local physiological or environmental cues, such as glucose levels, pH, temperature, or enzyme activity. They provide a possible substitute for co

Advances in the Drug Delivery Systems for Psoriasis Topical Therapy.

Psoriasis (PSO) is an immune-related skin disease featured by rapid and excessive cell growth leading to thick red scaly skin plaques. The main treatment strategy for PSO involves using anti-inflammatory drugs, immunosuppressants, etc, to relieve symptoms, suppress inflammation, and retard the overgrowth and division of skin cells. However, these drugs generally have disadvantages such as poor targeting, short half-life, low bioavailability, significant toxic side effects, etc. which impose a huge physiological and pathological burden on patients. Recently, the use of delivery systems to topic

Advanced drug delivery systems for the management of local conditions.

Localized disorders, even though originally confined to a specific body part, can progress into potentially life-threatening systemic disorders if treated inappropriately. Local treatment is often highly challenging due to poor penetration of therapeutic agents from their vehicles into the affected body site. Systemic treatment on the other hand often comes with unspecific side effects. The skin is the largest organ of the body, and conditions such as wounds and bacterial or fungal infections disrupt its natural barrier properties, important for the homeostasis of the human body. Transdermal Drug Delivery Systems (TDDS): Recent Advances and Failure Modes.

Transdermal drug delivery systems (TDDS), commonly refered to as "patches", present a nonintrusive technique to provide medication without the need for invasive procedures. These products adhere to the skin and gradually release a specific dosage of medicine at a defined rate into the bloodstream. Compared with other methods of drug delivery, TDDS offer benefits such as reduced invasiveness, convenience for patients, and avoidance of the metabolic processes that occur when drugs are orally consumed. Throughout time, TDDS have been used to provide medications for various medical conditions (suc

Novel Drug Delivery Systems for the Management of Fungal Keratitis.

Fungal keratitis (FK) is a dangerous corneal infection that is common in tropical and subtropical areas. Its incidence is extremely high, and ocular trauma and contact lenses can lead to FK, but its common treatment such as using topical antifungal eye drop instillation is often less effective because of several drawbacks of the drugs typically used, including limited ocular penetration, high frequency of dosing, poor biocompatibility, and the potential for severe drug reactions. Therefore, the development of novel drug delivery devices for the treatment of FK is urgent. The urgent need for no

Are Local Drug Delivery Systems a Challenge in Clinical Periodontology?

Placing antimicrobial treatments directly in periodontal pockets is an example of the local administration of antimicrobial drugs to treat periodontitis. This method of therapy is advantageous since the drug concentration after application far surpasses the minimum inhibitory concentration (MIC) and lasts for a number of weeks. As a result, numerous local drug delivery systems (LDDSs) utilizing various antibiotics or antiseptics have been created. There is constant effort to develop novel formulations for the localized administration of periodontitis treatments, some of which have failed to sh

Conjugated polymer nano-systems for hyperthermia, imaging and drug delivery.

Hyperthermia has shown tremendous therapeutic efficiency in the treatment of cancer due to its controllability, minimal invasiveness and limited side effects compared to the conventional treatment techniques like surgery, radiotherapy and chemotherapy. To improve the precision of hyperthermia specifically to a tumor location, near infra-red (NIR) light activatable inorganic metal nanoparticles have served as effective photothermal therapy materials, but toxicity and non-biodegradability have limited their clinical applications. Conjugated polymer nanoparticles have overcome these limitations a

Hyaluronan-decorated liposomes as drug delivery systems for cutaneous administration.

The work aimed to evaluate the feasibility to design hyaluronic acid (HA) decorated flexible liposomes to enhance the skin penetration of nifedipine. Egg phosphatidylcholine (e-PC) based transfersomes (Tween 80) and transethosomes (ethanol) were prepared. HA was reacted with 1,2-dipalmitoyl-sn-glycero-3-phosphoethanolamine (HA-DPPE) and two molar ratios (0.5 and 3%) of conjugate with respect to e-PC were tested. The presence of HA significantly increased the packing order of the bilayer (as verified by differential scanning calorimetry), reducing both the encapsulation efficiency and the flexi
