Overdose with antiepileptic drugs: the efficacy of extracorporeal removal techniques.

Drug overdose is a growing problem among adolescents. Clinical severity depends on the drug and ingested amount, which in some cases may be life-threatening. We present a clinical case of a previously healthy teenage girl who ingested 16.4 g of carbamazepine and 14.5 g of valproic acid. She presented with profound disturbance of consciousness and toxic levels of both drugs, raised in the first hours after the ingestion. She was successfully treated with charcoal haemoperfusion followed by continuous venovenous hemodiafiltration. Overdose with the two drugs separately is common, but there are n

Drugs in blood samples from unconscious drug addicts after the intake of an overdose.

This investigation includes whole blood samples from 53 drug addicts found unconscious in the Copenhagen area with evidence of a heroin overdose. Heroin/morphine was detected in 85% of the patients and other opioids in 11%. One or more benzodiazepines, most often diazepam, were detected in 75% of the patients. A blood alcohol concentration higher than 1.00 mg/g was detected in 57% of the patients. Methadone was detected in seven patients, ketobemidone in four, amphetamine in five and cocaine in one. This investigation showed widespread multi-drug abuse and heroin/morphine alone was detected in

Assessing Prevalence of Nonmedically Used Prescription Drug Involvement in Overdose Deaths Through Linkage of State Unintentional Drug Overdose Reporting System and Controlled Substances Monitoring Program Data.

BACKGROUND: While illicit substances are commonly involved in the overdose crisis, prescription substances still play a role. Oftentimes, decedents do not have prescriptions for these substances at the time of death. As such, we sought to examine the prevalence of nonmedical drug use in Tennessee through linkage of fatal drug overdose and prescription data. METHODS: We used State Unintentional Drug Overdose Reporting System (SUDORS) data to identify fatal drug overdoses in Tennessee from 2019 to 2022. Deaths were linked to Controlled Substances Monitoring Program data deterministically using

Overdose mortality rates in Croatia and factors associated with self-reported drug overdose among persons who inject drugs in three Croatian cities.

BACKGROUND: Drug overdose is the major cause of morbidity and mortality among persons who inject drugs (PWID). We assessed factors associated with the non-fatal drug overdose among PWID in three Croatian cities and national trends of overdose-related mortality (OM), and rates of uptake of opioid agonist drug treatment (OAT). METHODS: We used a respondent-driven sampling method to recruit 830 PWID in Zagreb, Split and Rijeka in 2014/2015. Participants completed behavioural questionnaires that included questions about overdose history, and we used Poisson regression to assess factors associated

Nanosponges in Detoxification: Strategy for Toxin Removal and Drug Overdose Management.

A potential class of cutting-edge medication delivery devices, nanosponges have become an effective tool for managing drug overdoses and detoxification. The capacity to adsorb and sequester a variety of toxins, such as heavy metals, bacterial endotoxins, and medications, makes these highly porous, biocompatible, and biodegradable particles-which are usually made from crosslinked polymers-an effective way to remove toxins. Nanosponges' adaptability enables their use in a number of sectors, including drug-resistant illness treatment, environmental remediation, and acute poisoning treatment. Deto

Pregnancy-Associated Drug Overdose Mortality in the United States, 2018-2023.

OBJECTIVE: To evaluate changes in pregnancy-associated drug overdose mortality in the United States from 2018 to 2023 and compare this with drug overdose mortality among reproductive-aged nonpregnant female decedents. Secondarily, to assess types and combinations of drugs that contribute to overdose mortality, regional patterns, and overdose timing. METHODS: This was a cross-sectional study that used 2018-2023 National Center for Health Statistics death certificate data. Drug overdose deaths were identified using International Classification of Diseases, Tenth Revision codes, and specific dru

Predicting drug overdose and death after "before medically advised" hospital discharge.

BACKGROUND: "Before medically advised" (BMA) hospital discharge is associated with higher risks for subsequent death and drug overdose. We sought to develop prediction models to estimate the absolute risk of death from any cause and illicit drug overdose after BMA discharge for a given patient. METHODS: We used retrospective population-based administrative health data of a 20% random sample of all British Columbia residents to derive and internally validate regression models to estimate the risks of death (model A) and illicit drug overdose (model B) within 30 days after BMA discharge. Model

Increases in Drug and Opioid Overdose Deaths--United States, 2000-2014.

The United States is experiencing an epidemic of drug overdose (poisoning) deaths. Since 2000, the rate of deaths from drug overdoses has increased 137%, including a 200% increase in the rate of overdose deaths involving opioids (opioid pain relievers and heroin). CDC analyzed recent multiple cause-of-death mortality data to examine current trends and characteristics of drug overdose deaths, including the types of opioids associated with drug overdose deaths. During 2014, a total of 47,055 drug overdose deaths occurred in the United States, representing a 1-year increase of 6.5%, from 13.8 per

Reliability of history of acetaminophen ingestion in intentional drug overdose patients.

The objective of this study was to determine the reliability of denial of acetaminophen ingestion in intentional drug overdose patients. All intentional drug overdose patients admitted to an emergency department who were able to provide a history were included. A detailed history was obtained on names, timing and number of medications ingested, and serum acetaminophen was assayed. Multidrug ingestion was defined as the reporting of ≥2 medications. Patients were considered 'reliable' if they reported acetaminophen ingestion and had detectable acetaminophen levels or the other way around. Validi

Prolonged gastric emptying half-time and gastric hypomotility after drug overdose.

A prospective study was undertaken to determine if gastric motility and emptying are altered by the ingestion of overdoses of tricyclic antidepressants, acetaminophen, opioid-acetaminophen mixtures, carbamazepine or phenytoin. Gastric scintigraphy was used to measure gastric emptying half-time and assess gastric motility in 104 patients at initial presentation and again at follow-up (n = 85). Patients were imaged for 5 hours after being given 20 MBq of 99mTc tin colloid to drink. Drug serum levels were measured on all patients at initial presentation and at follow-up. We observed markedly prol

Suspected central nervous system toxicity from inadvertent nonsteroidal antiinflammatory drug overdose.

This case illustrates a severe adverse reaction of the central nervous system involving a moderate overdose of diclofenac sodium along with therapeutic dosages of indomethacin and ibuprofen. Our patient became disoriented, hallucinated, lost consciousness, and suffered respiratory arrest after ingesting five diclofenac tablets (375 mg total), two ibuprofen tablets (400 mg total), and one indomethacin capsule (75 mg). Aggressive supportive therapy was instituted and the patient improved. She was awake, alert, and extubated in less than 24 hours with no serious sequelae.

Potential of liquid membranes for drug overdose treatment: in vitro studies.

The in vitro removal of six barbiturates from pH 2 donor solutions by liquid membranes with pH control was evaluated. More than 90% of amorbarbital, phenobarbital, and secobarbital were removed within 10 min by the liquid membranes. Drug transport obeyed first-order kinetics initially, and Fick's law was obeyed. The transport rate of phenobarbital by a liquid membrane was temperature dependent. An Arrhenius plot revealed that the activation energy was 10 kcal/mole. The liquid membranes showed some instability in the presence of bile salts.

Decline in US Drug Overdose Deaths by Region, Substance, and Demographics.

IMPORTANCE: Drug overdose deaths (DODs) surged with the advent of fentanyl. Recent US reports indicated a decline, but standard surveillance systems do not account for monthly variability or seasonality and require monthly population data to calculate DOD rates. OBJECTIVE: To identify when US DOD rates began to decelerate and to examine patterns by census region, drug type, and demographics. DESIGN, SETTING, AND PARTICIPANTS: This repeated cross-sectional study of DOD rates was conducted from January 2015 to October 2024, using data from the National Center for Health Statistics and US Censu

Experience of Personal Loss Due to Drug Overdose Among US Adults.

IMPORTANCE: Since 1999, over 1 million people have died of a drug overdose in the US. However, little is known about the bereaved, meaning their family, friends, and acquaintances, and their views on the importance of addiction as a policy priority. OBJECTIVES: To quantify the scope of the drug overdose crisis in terms of personal overdose loss (ie, knowing someone who died of a drug overdose) and to assess the policy implications of this loss.
