The aim of the present study was to compare in a prospective, multicenter trial the results early and late after pylorus-preserving gastrectomy (PPG) versus conventional distal gastrectomy (CDG) with Billroth I anastomosis for early gastric cancer. Eighty-one patients with early gastric cancer were randomized and then underwent either PPG or CDG. Duration of operation, intraoperative blood loss, days until removal of the nasogastric tube, days until start of oral intake, and decrease in body weight were studied as parameters for outcomes early after the surgery. Late results were studied in pa

After gastric operations about 20 to 30% of all patients complain of an abundance of postoperative gastrointestinal and vasomotor complaints. Research into these "postgastrectomy syndromes" is carried out periodically. In the beginning of this century concepts as precipitated evacuation, dumping stomach, too narrow or too wide anastomosis predominate. In the thirties the subdivision into a so-called early and late dumping syndrome follows. The description and treatment of the afferent loop syndrome in the middle of this century suggest the introduction of functional factors into the explanatio

INTRODUCTION: Reversal of Roux-en-Y gastric bypass (RYGB) may be indicated for possible side effects such as malnutrition, intolerance, dumping syndrome, or late dumping. Reversal can however induce significant gastro-esophageal reflux disease (GERD).
METHOD: We report the case of a 55 years old Caucasian woman, who had undergone conversion of Nissen fundoplication performed for GERD (Grade B) into RYGB because of recurrent reflux, though Grade A esophagitis and increasing obesity (37.5 kg/m2 BMI). She developed invalidating symptoms of late dumping syndrome and severe diarrhea. Pre- and post

The degree of a patient's emotional maturity and the supportive value of his home, working, and recreational environments were scored by psychosociological methods preoperatively in a group of 47 patients who were intensively investigated with regard to the dumping syndrome. After surgery, the result of the operation was independently assessed as a success or a failure. At that time, a careful review was performed of each patient's clinical state, with particular reference to postgastrectomy syndromes. Positive associations were found between surgical failure and emotional instability, recorde

Malabsorption following surgical procedures on the GI-tract happens only in rare cases, such as blindloop-syndrome, short gut-syndrome, internal fistulae and dumping syndrome after partial gastric resection. Pathophysiological background and therapeutical possibilities as well as parenteral nutrition are discussed in detail.

Bariatric surgery is most commonly carried out in women of childbearing age. Whilst fertility rates are improved, pregnancy following bariatric surgery poses several challenges. Whilst rates of many adverse maternal and foetal outcomes in obese women are reduced after bariatric surgery, pregnancy is best avoided for 12-24 months to reduce the potential risk of intrauterine growth retardation. Dumping syndromes are common after bariatric surgery and can present diagnostic and therapeutic challenges in pregnancy. Early dumping occurs due to osmotic fluid shifts resulting from rapid gastrointesti

Rapid transit of ingested nutrients into the small bowel following gastric or esophageal surgery can trigger dumping syndrome, a recognized postoperative complication seen with particular frequency after Roux-en-Y gastric bypass (RYGB). Under normal operative conditions, RYGB constructs a proximal gastric pouch that serves to limit meal volume and pace nutrient delivery into the jejunum. This report describes a 65-year-old woman with a history of RYGB and subsequent surgical revisions who presented with debilitating postprandial symptoms consistent with dumping syndrome alongside persistent cl

OBJECTIVE: To evaluate the immediate and long-term functional results of total gastrectomy (TGE) with restoration of duodenal passage through double tract reconstruction (DTR) in comparison with Roux-en-Y reconstruction.
MATERIAL AND METHODS: The study included 55 patients with gastric cancer who underwent TGE. They were divided into 2 groups: I - TGE with DTR (n=29); II - TGE with Roux-en-Y reconstruction (n=26). Follow-up examination was performed in 3, 6 and 12 months after surgery. Short-term results, nutritional status, morbidity and severity of dumping syndrome, as well as GSRS scores o

BACKGROUND Hypoglycemia is a common complication following total gastrectomy, primarily caused by dumping syndrome and severe malnutrition, with late dumping syndrome being particularly significant. However, for recurrent fasting hypoglycemia, the possibility of insulinoma should be considered. Hypoglycemia caused by insulinoma can lead to severe consequences, including seizures and even death. Thus, it is crucial to differentially diagnose hypoglycemia occurring after total gastrectomy. CASE REPORT In this report, we present the case of a 36-year-old Chinese woman who underwent total gastrect

BACKGROUND: Post-prandial hypoglycemia is an uncommon but disabling late complication of Roux-en-Y gastric bypass (RYGB). Most patients can be treated with dietary interventions and medications; however, some patients develop refractory hypoglycemia that may lead to multiple daily episodes and seizures. While RYGB reversal surgery is an effective treatment, complication rates are high, and patients inevitably experience weight regain. Transoral gastric outlet reduction (TORe) is a minimally invasive treatment that is effective for early and late dumping syndrome. However, prior studies have no

The realisation that serotonin plays a role not only in the carcinoid syndrome but also in migraine, nociception, dumping syndrome, vascular disease and hypertension, has led to an enormous amount of activity in search of serotonin antagonists. Numerous such pharmacological agents have been identified but only few have found their way into clinical use. All of them are competitive serotonin inhibitors, in that they vie for the same receptor as the amine itself and are thus able to block its action as well as imitate its effects. By far the widest use of such inhibitors is in the prevention of

INTRODUCTION: Radical gastrectomy is associated with significant functional complications. In appropriate patients may be amenable to less invasive resection aimed at preserving the vagal trunks. The aim of this systematic review and meta-analysis was to assess the functional consequences and oncological safety of vagal sparing gastrectomy (VSG) compared to conventional non-vagal sparing gastrectomy (CG).
METHODS: A systematic review of four databases in accordance with PRISMA guidelines was undertaken for studies published between January 1, 1990, and December 15, 2021, comparing patients wh

Robotic surgery is increasingly gaining importance. While initial results suggest an advantage of the robotic over the minimally invasive approach in patients with gastric cancer, definitive proof of its superiority has yet to be provided. There are numerous approaches to recreate a gastric reservoir after a total gastrectomy. However, a major disadvantage of most conventional reconstructions are long term effects such as dumping syndrome, afferent loop syndrome and poor nutrition intake with severe impact on the patient quality of life. The jejunal pouch reconstruction is a beneficial reconst

In the period from 1981 to 1990, 43 patients were treated for the afferent loop syndrome. Their ages ranged from 20 to 76 years. Hofmeister-Finsterer gastric resection was the most frequent (81.40%) primary operation, in four patients (9.30%), primary resection was performed with anterocolonic anastomosis on a long loop after Braun. In two patients (4.65%) the syndrome had developed after Roux resection. In 12 patients (27.90%), the afferent loop syndrome was combined with the dumping syndrome, and in 24 (55.81%) with peptic ulcer of the gastrojejunoanastomosis, which along with other causes w

PURPOSE: Weight recurrence after Roux-en-Y gastric bypass affects 25% of patients. Gastrojejunal anastomosis dilation is a modifiable contributor. This study evaluates the efficacy and safety of endoscopic transoral outlet reduction (TORe), comparing suturing vs over-the-scope clips (OTSC).
METHODS: Retrospective analysis of consecutive patients undergoing TORe for weight recurrence (2012-2024) at a tertiary center. Treatments included endoscopic suturing (OverStitch, Boston Scientific), OTSC, or a combination of techniques. Outcomes included percentage total body weight loss (%TWL) and sympt

BACKGROUND: Postgastrectomy syndromes include reflux gastritis and oesophagitis, dumping syndrome, intractable diarrhoea and afferent loop syndrome. To prevent such syndromes, since January 1994 jejunal interposition has been used following distal gastrectomy. The aim of this study was to evaluate the benefit of this procedure.
METHODS: A consecutive series of 42 patients who underwent distal gastrectomy for gastric cancer was studied. Twenty-two patients had a Billroth I procedure before January 1994, and 20 patients had isoperistaltic jejunal interposition using a 10-12-cm segment after Jan

During the last years we have employed acid-suppression duodenal diversion procedures (truncal vagotomy-partial gastrectomy plus Roux-en-Y gastrojejunostomy) in addition to antireflux surgery in order to treat all the pathophysiological factors involved in the genesis of Barrett's esophagus. We have observed very good results concerning the clinical and objective control of GERD at the long-term follow up after this procedure. However, it could be associated with other nonesophageal symptoms or side-effects. This study was conducted to evaluate the presence of gastrointestinal symptoms (diarrh
