A rare cause of dysphonia.

Although dysphonia is a common complaint among patients seen in an otolaryngology clinic, an autoimmune cause of the problem is less common. Autoimmune disease can be the source of various laryngologic manifestations, but diagnosis depends on a high index of suspicion, careful patient evaluation and testing, and videolaryngoscopy (videostroboscopy if available) to uncover specific identifying findings. This report describes a patient presenting with upper-range vocal difficulties who was found to have early systemic lupus erythematosus.

Attitudes of children with dysphonia.

Because voice disorders in childhood may have a negative impact on communicative effectiveness, social development, and self-esteem, the objective was to determine the impact of voice disorders on lives of children from the perspective of chronically dysphonic children and their parents. This study consisted of focused interviews with chronically dysphonic children and their caregivers. Focused interviews were conducted with 10 children in each of the following age groups: Toddler (2-4 years old), Young Child (5-7 years old), School-Aged Child (8-12 years old), and Adolescent (13-18 years old)

The quantitative analysis of dysphonia.

A clinical method of measuring 'hoarseness' is presented. Its value is assessed in 3 different types of vocal cord lesion and compared against normal values. There are shown to be significant differences between the normal and the abnormal voice and also differences between each type of vocal cord lesion. We feel that this quantification of hoarseness is useful in the treatment of patients as well as in monitoring their progress under therapy.

Predictive value of globus pharyngeus in patients with functional dysphonia versus organic dysphonia.

OBJECTIVES/HYPOTHESIS: This is a retrospective study investigating the prevalence of globus pharyngeus in patients with dysphonia. STUDY DESIGN: Retrospective chart review. METHODS: The study examined the prevalence of globus pharyngeus in patients presenting with history of dysphonia at the American University of Beirut Medical Center Voice Center was performed. The etiology of dysphonia was categorized as organic in the presence of laryngeal pathology versus functional in the absence of any laryngeal pathology on laryngeal videostroboscopic examination. Functional dysphonia was further str

A pattern recognition approach to spasmodic dysphonia and muscle tension dysphonia automatic classification.

Spasmodic dysphonia (SD) and muscle tension dysphonia (MTD) are two voice disorders that present similar characteristics. Usually, they can be differentiated only by experienced voice clinicians. There are many reasons that support the idea that SD is a neurological disease, requiring surgical treatments or, more usually, laryngeal botulinum toxin A injections as a therapeutic option. On the other hand, MTD is a functional disorder correctable with voice therapy. The importance of a correct diagnosis of these two disorders is critical at the treatment-selection moment. In this article, we pres

Automated acoustic analysis of task dependency in adductor spasmodic dysphonia versus muscle tension dysphonia.

OBJECTIVES/HYPOTHESIS: Distinguishing muscle tension dysphonia (MTD) from adductor spasmodic dysphonia (ADSD) can be difficult. Unlike MTD, ADSD is described as "task-dependent," implying that dysphonia severity varies depending upon the demands of the vocal task, with connected speech thought to be more symptomatic than sustained vowels. This study used an acoustic index of dysphonia severity (i.e., the Cepstral Spectral Index of Dysphonia [CSID]) to: 1) assess the value of "task dependency" to distinguish ADSD from MTD, and to 2) examine associations between the CSID and listener ratings. Quality of life, self-perceived dysphonia, and diagnosed dysphonia through clinical tests in teachers.

OBJECTIVES: To examine the impact of voice on the quality of life of teachers and to assess whether the degree of dysphonia and otorhinolaryngologists' (ORL) diagnostics are correlated with the quality of life. METHODS: Eighty-eight female teachers from the municipal schools of Belo Horizonte who were in speech therapy at the Speech Therapy Clinic of the Hospital das Clínicas of Minas Gerais participated in the study. The variables studied were age, ORL diagnosis, perceptual-hearing assessment of voice through GRBAS scale, and vocal activities and participation profile (VAPP) protocol. Statis

Differential diagnosis of adductor spasmodic dysphonia and muscle tension dysphonia using phonatory break analysis.

OBJECTIVES: Muscle tension dysphonia (MTD) can masquerade as adductor spasmodic dysphonia (ADSD) leading to diagnostic confusion. Intraword phonatory breaks have been offered as the sine qua non of ADSD, however, little is known regarding the presence of phonatory breaks in MTD. This investigation assessed the diagnostic worth of acoustic analysis of phonatory breaks as a possible objective test to distinguish ADSD from MTD. STUDY DESIGN: Case-control comparison. METHODS: Voice samples from patients with confirmed ADSD (n = 41) and MTD (n = 59) were analyzed acoustically to determine the pre

Comparing health locus of control in patients with Spasmodic Dysphonia, Functional Dysphonia and Nonlaryngeal Dystonia.

Locus of Control (LoC) refers to an individuals' perception of whether they are in control of life events. Health Locus of Control refers to whether someone feels they have influence over their health. Health Locus of Control has not been studied in any depth in voice-disordered patients. The objective of this study was to examine Health Locus of Control in three patient groups: (1) Spasmodic Dysphonia, (2) Functional Dysphonia and (3) a nondysphonic group with Nonlaryngeal Dystonia. LoC was measured and compared in a total of 57 patients using the Multidimensional Health Locus of Control Scal

Spontaneous Pneumomediastinum Presenting as Dysphonia.

Spontaneous pneumomediastinum is a rare condition describing air within the mediastinum from a non-traumatic etiology from an intrathoracic or extrathoracic source. It typically affects young men through a mechanism known as the Macklin effect, in which alveolar air travels along bronchovascular sheaths into the mediastinum. There are known predisposing conditions as well as activities that can precipitate spontaneous pneumomediastinum, which is generally self-limited, but in rare circumstances, can lead to serious complications such as tension pneumomediastinum, mediastinitis, and tension pne

Electrophysiological predictors of hyperfunctional dysphonia.

BACKGROUND: The relation between the autonomic nervous system (ANS) and muscles of the vocal tract is of particular importance when considering the pathomechanism of a functional voice disorder. AIMS: The aim of this study was to record electrophysiological indicators from the ANS as well as the tone of the external laryngeal muscle and test whether together they could point to an enhanced risk of primary functional voice disorder. MATERIALS AND METHODS: The study material consisted of 81 people, 27 of whom were professional opera singers. None reported any voice complaints. The research com

Spasmodic dysphonia: introductory phonetic analyses.

Adductor spasmodic dysphonia (ADSD) is a neurological dystonia characterised by involuntary adductor spasms of the larynx during speech. Spasm frequency is often reported to increase during syllables that begin with voiced speech sounds, especially glottal stops. Because of its underlying physical and acoustic complexities, the voicing contrast in American English (AE) appears unlikely to interact consistently with a singular physical phenomenon like laryngeal spasm. This retrospective study investigated additional phonetic contrasts and their relationship to spasm frequency. Standardised, 144

Otolaryngeal and Oropharyngeal Conditions: Dysphonia.

Dysphonia is any alteration of voice quality or vocal effort that impairs communication and affects quality of life. In patients with dysphonia, voice qualities often are described as tremulous, hoarse, strained, or raspy, with altered pitch, breathiness, or vocal fatigue. Dysphonia is a sign of an underlying disease process. Up to one-third of individuals will experience dysphonia in their lifetime. The evaluation includes a history, physical examination, and, in some cases, laryngoscopy. The most common cause of dysphonia is acute laryngitis, with the majority of cases lasting fewer than 3 w

Section four: laryngitis and dysphonia.

Acute laryngitis is most often caused by viral illnesses through direct inflammation of the vocal cords or from irritation due to postnasal drainage. Bacterial infections, such as acute epiglottitis, also can cause dysphonia but typically have other systemic symptoms as well as respiratory distress. Chronic laryngitis is characterized by symptoms lasting more than 3 weeks. Chronic vocal cord issues can be related to overuse or stress on the vocal cords resulting in nodules or polyps. Individuals in certain occupations, such as singers, school teachers, and chemical workers, are at greater risk

Brainstem pathology in spasmodic dysphonia.

Spasmodic dysphonia (SD) is a primary focal dystonia of unknown pathophysiology, characterized by involuntary spasms in the laryngeal muscles during speech production. We examined two rare cases of postmortem brainstem tissue from SD patients compared to four controls. In the SD patients, small clusters of inflammation were found in the reticular formation surrounding solitary tract, spinal trigeminal, and ambigual nuclei, inferior olive, and pyramids. Mild neuronal degeneration and depigmentation were observed in the substantia nigra and locus coeruleus. No abnormal protein accumulations and

Temporomandibular dysfunction and dysphonia (TMD).

Freedom of the manducatory apparatus is critical to economical and accurate vocal production. Two types of dysfunction will be discussed: global muscular hypertonia of the muscular manducatory system and dysfunction of the temporomandibular joint (TMJ). Hypertonia occurs in a high number of dysphonic patients. It impairs mandibular mobility and also induces chronic hypertonia of the tongue muscles, hyoid apparatus and occipitocervical joint through the neuromuscular facilitation process. TMJ can be a major source of mandibular kinetics impairment. Its pathogenic effects are most deeply felt in
