Anal sphincter injury associated with shoulder dystocia.

Objective: Shoulder dystocia is an obstetric emergency, occurring in 0.2-3% of vaginal deliveries. Research has mainly focused on the neonatal morbidity arising from shoulder dystocia, such as brachial plexus injury and hypoxic-ischemic encephalopathy. Maternal morbidity is thought to be increased with shoulder dystocia though is much less commonly reported. Obstetric anal sphincter injury remains the leading cause of fecal incontinence in women and shares several antenatal and intrapartum risk factors with shoulder dystocia. The aim of this study was to identify risk factors for sphincter inj

Neonatal morbidity associated with shoulder dystocia maneuvers.

OBJECTIVE: We sought to examine neonatal morbidity associated with different maneuvers used among term patients who experience a shoulder dystocia. STUDY DESIGN: We conducted a retrospective cohort study of all women who experienced a clinically diagnosed shoulder dystocia at term requiring obstetric maneuvers at a single tertiary care hospital from 2005 through 2008. We excluded women with major fetal anomaly, intrauterine death, multiple gestation, and preterm. Women exposed to Rubin maneuver, Wood's screw maneuver, or delivery of the posterior arm were compared to women delivered by McRobe

[Shoulder dystocia--risk factors and fetal outcome].

The purpose of this retrospective study is to evaluate the perinatal outcome in cases with birth injuries, suggesting shoulder dystocia. This survey involves 92 cases of live newborns (gestational age between 37 and 42 weeks) with shoulder injuries. Another control group of 120 cases of live mature newborns without any shoulder injuries is studied for comparison. There are no significant differentialities between those two groups, regarding age, parity, weight and height of the parturients. Shoulder dystocia is most frequently found in newborns of 3500-4000 g birthweight. Antepartum, risk fact

Dystocia among women with symptomatic uterine rupture.

OBJECTIVE: The purpose of this study was to analyze cervical dilatation patterns among women with uterine rupture by means of a mathematic model and to use the results to determine optimal intervention criteria. STUDY DESIGN: This was a case-control review that compared a case patient group of 19 women with uterine rupture during labor with control groups with either no previous cesarean deliveries, vaginal birth after cesarean delivery, or failure of attempted vaginal birth after cesarean delivery. The mathematic model quantified dilatation and adjusted for conditions specific to each patien

Shoulder dystocia: incidence, mechanisms, and management strategies.

Shoulder dystocia can lead to death or brain damage for the baby. Traction on the head can damage the brachial plexus. The diagnosis should be made when the mother cannot push the shoulders out with her own efforts with the next contraction after delivery of the head. There should be no traction on the head to diagnose shoulder dystocia. McRoberts' position is acceptable but it should not be accompanied by any traction on the head. If the posterior shoulder is in the sacral hollow then the best approach is to use posterior axillary traction to deliver the posterior shoulder and arm. If both sh

Obstetric Emergencies: Shoulder Dystocia and Postpartum Hemorrhage.

Shoulder dystocia and postpartum hemorrhage represent two of the most common emergencies faced in obstetric clinical practice, both requiring prompt recognition and management to avoid significant morbidity or mortality. Shoulder dystocia is an uncommon, unpredictable, and unpreventable obstetric emergency and can be managed with appropriate intervention. Postpartum hemorrhage occurs more commonly and carries significant risk of maternal morbidity. Institutional protocols and algorithms for the prevention and management of shoulder dystocia and postpartum hemorrhage have become mainstays for c

Shoulder dystocia: risk factors, predictability, and preventability.

Shoulder dystocia remains an unpredictable obstetric emergency, striking fear in the hearts of obstetricians both novice and experienced. While outcomes that lead to permanent injury are rare, almost all obstetricians with enough years of practice have participated in a birth with a severe shoulder dystocia and are at least aware of cases that have resulted in significant neurologic injury or even neonatal death. This is despite many years of research trying to understand the risk factors associated with it, all in an attempt primarily to characterize when the risk is high enough to avoid vagi

Shoulder dystocia: nursing prevention and posttrauma care.

Shoulder dystocia is a birth emergency that occurs in approximately 1% of all births. Shoulder dystocia can be followed by broken clavicle or humerus, brachial plexus injury, fetal hypoxia, or death. Although risk factors for shoulder dystocia include previous birth complicated by shoulder dystocia, maternal obesity, excessive prenatal weight gain, fetal macrosomia, gestational diabetes, and instrumental delivery, shoulder dystocia is not predictable. Perinatal nurses can reduce the risk for shoulder dystocia by teaching mothers about optimal weight gain in pregnancy and assisting mothers with

Prediction of shoulder dystocia using multivariate analysis.

We evaluated the use of multivariate analysis in the prediction of shoulder dystocia (SD). One hundred consecutive cases with SD were matched with 100 controls without dystocia. All patients had term, vaginal delivery. Multivariate analysis was used to identify independent variables significantly related to shoulder dystocia. The regression coefficients for the identified factors were used to calculate a composite score from which receiver operating characteristics (ROC) curves were derived. Birthweight (BW), 1-hour Glucola (GLU), operative vaginal delivery (OVD), and height of fundus (HOF) we

The McRoberts maneuver in shoulder dystocia: A critical reappraisal.

This review summarizes the current evidence on the strengths and limitations of the McRoberts maneuver for the management of shoulder dystocia. It examines reported success rates, safety, maternal and neonatal outcomes, biomechanical mechanisms, and training requirements. The McRoberts maneuver remains the recommended first-line intervention because it is simple, rapid, and minimally invasive. However, reported success rates vary widely, ranging from 25% to 61% in most studies. Its effectiveness depends on several key factors. First, clinicians should receive regular simulation-based training

The Hook & Roll Maneuver for Resolution of Shoulder Dystocia.

BACKGROUND: Shoulder dystocia is an obstetric emergency with the potential to cause maternal and neonatal injuries. TECHNIQUE: The hook & roll maneuver is a modification of other posterior axilla maneuvers that involves hooking the posterior axilla with one finger and providing gentle traction, elevating the posterior shoulder toward the vaginal opening while simultaneously rolling the anterior shoulder toward the fetal face and rotating approximately 30-90°. EXPERIENCE: At the time of this submission, the hook & roll maneuver has been used 54 times in our institution, successfully resolving

An Overview of Developmental Disorders Leading to Dystocia in Cattle.

Dystocia due to developmental disorders causing foetal malformation is a significant challenge in bovine obstetrics, often resulting in severe birth complications. This narrative review provides an updated overview of the most common congenital syndromes associated with dystocia in cattle, emphasising both their clinical implications and underlying causes. Congenital disorders such as schistosoma reflexum, perosomus elumbis, arthrogryposis, hydrocephalus, anasarca and embryonic duplication are reviewed in detail, along with their impact on the course of calving. While the management of dystoci

Fetal dystocia in guinea pigs: A case report.

Dystocia is a common complication in guinea pig pregnancies, presenting significant challenges in clinical management. The present case report describes the presentation, diagnosis, and surgical intervention in an 8 months old female guinea pig with dystocia. The subject is a primiparous guinea pig originating from a commercial breeder, exhibited prolonged labor with two pups, one of which was stillborn. Physical examination revealed a distended abdomen, lack of uterine contractions, signs of distress, and vulvar discharge. Radiographic and ultrasound tests confirmed obstruction due to large f

Schistosomus reflexus dystocia in a crossbred dairy cow.

KEY CLINICAL MESSAGE: Schistosomus reflexus (SR), an unusual congenital defect in calves, can be diagnosed grossly by exposed viscera and curved spine with hindquarters twisted up toward the head. SR is typically associated with dystocia and fetal deaths in cows. Hence, cattle breeding programs must be aware of these anomalies to avoid losses from abnormal, non-viable calves. ABSTRACT: Schistosomus reflexus (SR) is a rare and fatal congenital malformation in bovines from autosomal recessive disorders. We report a typical case of SR in a non-viable calf after the cesarean section of a crossbre

[Shoulder dystocia: Quality of retranscription in medical files].

OBJECTIVE: Shoulder dystocia is a rare obstetrical event and potentially serious. Apart from possible psychological implications, it may be responsible for maternal (haemorrhage and perineal tear) and neonatal complications (brachial plexus) leading to complaints and even lawsuits. The transcription of this event in medical files is essential as it is a reflection of the work in an emergency. It allows the obstetrician to defend him in case of trial. Our objective was to assess the quality of the transcription of shoulder dystocia situations in medical files. METHOD: Retrospective chart study
