Advancing Intensive Care Unit Early Mobility Measurement, Practice and Outcomes: The Out-of-Bed Index.

ICU early mobility improves some patient outcomes, yet practice adoption remains inconsistent owing in part to variable terminology, documentation and measurement. This article describes a decade-long learning health system approach to the study and practice of ICU early mobility measurement, leading to the development of the Out-of-Bed Mobility Eligible Index (OOB-E Index), an automated metric quantifying the frequency of out-of-bed events per mobility-eligible ICU day. Integrated into a mobility dashboard alongside other key metrics, the OOB-E Index supports supports standardized assessment,

Early Mobilization in Chronic Subdural Hematoma: A Systematic Review and Meta-Analysis.

BACKGROUND: There is variation in timing of postoperative mobilization in patients with chronic subdural hematoma (cSDH). Some studies suggest that early postoperative mobilization may reduce complications, without increase in recurrence. This review aims to evaluate whether timing of postoperative mobilization impacts such outcomes. METHODS: Preferred Reporting Items for Systematic Reviews and Meta-Analyses-adherent systematic review and meta-analysis. Full-text articles comparing early mobilization (EM) and elevated head position against conventional methods were included. Search string dev

Early mobilization in neurocritical care as a threshold-regulated intervention: a narrative review of mechanistic integration and implications for neurological recovery and physiological stability.

The integration of early mobilization into neurocritical care remains constrained by uncertainty regarding how heterogeneous physiological systems respond to graded activity during neurological instability. Although early mobilization is increasingly used in intensive care, the relationships between intervention timing, intensity, neurological recovery, functional outcomes, and physiological tolerance remain incompletely defined in acute neurocritical populations. This narrative review integrates clinical and mechanistic evidence to examine how immobility and reactivation may influence interco

Treatment Outcomes of Simple Elbow Dislocations: A Systematic Review of 1,081 Cases.

BACKGROUND: The treatment of simple elbow dislocations (SEDs) has become more functional last decade with a tendency to shorter immobilization of the elbow, whereas simultaneously, surgical stabilization has been promoted by some authors. The primary aim of this study was to systematically review the literature and analyze the outcomes and complications of different treatment options for acute and persistent SEDs, including operative and nonoperative treatments with varying immobilization periods. METHODS: A literature search was performed based on the online medical databases MEDLINE, Embase

An Integrative Review: Early Mobilization of Patients With External Ventriculostomy Drains in the Neurological Intensive Care Unit.

BACKGROUND: Patients in the intensive care unit (ICU) are at a high risk for immobility due to their high acuity and need for invasive devices including external ventriculostomy drains (EVDs). Prolonged patient immobilization is associated with poor outcomes. METHODS: Whittemore and Knafl's 5-stage framework was used to conduct an integrative review to synthesize findings from quantitative research studies on early patient mobilization for patients with EVDs in the neurological ICU. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses checklist was used as the reporting guide

Enhanced Recovery After Pediatric Scoliosis Surgery: Key Components and Current Practice.

With the goal of safety and efficiency in health care delivery, enhanced recovery protocols (ERPs) continue to gain traction throughout various surgical disciplines, including in pediatric scoliosis surgery. The growing body of literature reporting decreased length of stay and cost with no change in readmissions or complications has brought these protocols to the forefront. The key components of ERPs include preoperative patient counseling, perioperative pain management, and early patient mobilization. In this review, the authors aim to describe the foundational history and major components of

[ANESTHESIA MANAGEMENT FOR ENHANCED RECOVERY AFTER SURGERY].

In programs to enhance patient recovery after surgery, anesthesiologists play an important role. In conventional management, patients managed with the "starvation, stress, and drowning" regimen experience delays in the recovery process. In contrast, anesthesiologists in the current perioperative management scenario reduce the fasting period, maintain sufficient depth of anesthesia, and administer appropriate infusion levels. In particular, three major factors inhibiting postoperative recovery are known to be pain, gastrointestinal dysfunction, and immobility. To prevent these, anesthesiologist

A study of function after tibial cast bracing.

103 patients with fresh unilateral fractures of the tibial shaft were reviewed clinically and radiologically to assess the results of treatment by functional bracing. At review, 84 had returned to their previous employment and all but 22 could run. All had full knee movements, 85 per cent had a normal range or a mild loss of subtalar movement, and only 11 patients had greater than 25 per cent reduction of measured ankle movement. The average shortening measured radiologically was 0.5 cm. Angulation greater than 10 degrees was present in 1 patient. Factors leading to delayed union include commi

Effects of delayed ambulation following posterior spinal fusion for adolescent idiopathic scoliosis: a single institutional study.

OBJECTIVE: Mobilizing out of bed and ambulation are key components of recovery following posterior spinal fusion (PSF) for adolescent idiopathic scoliosis (AIS). However, there remains a paucity of studies identifying risk factors associated with delayed ambulation and its impact on postoperative outcomes. The aim of this study was to investigate patient- and surgical-level risk factors associated with delayed ambulation and the ramifications of delayed ambulation on healthcare utilization for patients undergoing PSF for AIS. METHODS: The medical records of 129 adolescent (10-18 years) patien

A scoping review of postoperative early rehabilitation programs after dysvascular-related amputations.

BACKGROUND: After amputation, people face challenges including wound healing and decreased functional mobility. Early mobilization in acute hospital care has proved safe, improved function, and sped discharge. Still, loss of a leg complicates standing and early mobilization after amputation. Approaches to early mobilization and rehabilitation after amputation surgery have not been widely studied. OBJECTIVES: To map the evidence regarding early postoperative mobilization after dysvascular amputation. Specific aims included identifying research designs and populations, describing rehabilitation

Early mobilisation is associated with lower subacute blood pressure and variability in ICH: A retrospective cohort study✰.

BACKGROUND: Very early rehabilitation after stroke appears to worsen outcome, particularly in intracerebral haemorrhage (ICH). Plausible mechanisms include increased mean blood pressure (BP) and BP variability. AIMS: To test associations between early mobilisation, subacute BP and survival, in observational data of ICH patients during routine clinical care. METHODS: We collected demographic, clinical and imaging data from 1372 consecutive spontaneous ICH patients admitted between 2 June 2013 and 28 September 2018.
