Elbow dislocation with arterial injury.

Elbow dislocations are most often the result of a hyperextension mechanism with the forearm levered and displaced posterior to the humerus. Recurrent instability and arterial disruption are uncommon sequelae of this injury. Any asymmetry in distal pulses after joint reduction demands a study of arterial anatomy (arteriogram or duplex scan) to rule out avulsion or intimal damage.

[Complex injury of the elbow joint].

High-velocity trauma now often results in complex injuries to the upper extremity, and especially the elbow joint. These can lead to both an enormous reduction in the range of motion of the shoulder, elbow and wrist joints, in severe cases with complete loss of upper extremity function. A complex injury is defined as a fracture and/or dislocation of the elbow in association with (1) a serial injury of the upper extremity, (2) a severe soft tissue trauma, or (3) concomitant injury to vessels or nerves. Serial fractures, in particular can lead to enormous problems with treatment and are often as

[Total elbow arthroplasty as a treatment option in complex injuries of the elbow in elderly patients].

AIM: The aim of this study was to evaluate the functional outcome of patients with severe elbow injuries treated by primary and secondary implantation of an elbow prosthesis using two different functional scores. METHOD: To determine whether or not total elbow replacement is an acceptable treatment option in such cases, we analysed the functional outcome of patients after primary or secondary implantation of the semi-constrained Coonrad-Morrey prosthesis. Between March 2005 and September 2009 we implanted 25 Coonrad-Morrey prostheses in 24 patients with acute or chronic elbow injuries. Eight

Black and white: how good are clinicians at diagnosing elbow injuries from paediatric elbow radiographs alone?

OBJECTIVES: Paediatric trauma elbow radiographs are difficult to interpret and there is a potential for harm if misdiagnosed. The primary goal of this study was to assess the ability of healthcare professionals internationally to interpret paediatric trauma elbow radiographs from the radiograph alone by formulating the correct diagnosis. METHODS: This prospective international study was conducted online via the Free Open Access Medical Education platform, Don't Forget the Bubbles (DFTB, ISSN 2754-5407). Participants were recruited via the DFTB social media accounts between 17 August and 14 Se

Management of elbow ankylosis in a head injured patient with floating elbow injury.

The development of heterotopic ossification around the elbow has been linked to trauma (particularly fractures or dislocations), closed head injury, spinal cord injury, burns of the extremity, genetic traits, adult respiratory distress syndrome, and orthotopic liver transplant. Pain and lack of motion are the most common complaints during the progression of ectopic bone about the joint. Opposite to common credence, fracture healing is not accelerated in patients with traumatic brain injury. Formation of heterotopic bone is not a clinically significant form of healing, therefore healing rates o

Options for Treating Complex Soft Tissue Injuries Around the Elbow.

Comprehensive preoperative assessments and multidisciplinary collaboration are crucial for crafting tailored treatment plans for complex elbow soft tissue injuries. Generally, surgeons should prioritize the simplest effective method from the reconstruction ladder, ensuring durable wound coverage and joint function preservation. Advance postoperative rehabilitation planning is integral, emphasizing early mobilization and a patient-centered approach for optimal healing and functional restoration.

Atypical Injuries of the Elbow in Children: A Case Series.

Fractures occurring in the elbow region of children and adolescents pose a notable challenge for orthopedic surgeons due to the complex anatomy and developmental stages of the humerus. Capitulum fractures, a rare form of elbow injury, are classified into three types according to the Bryan and Morrey classification system. Monteggia fractures, a rare occurrence in children, involve a combination of ulna break and radial head dislocation, with Monteggia equivalent fractures expanding upon traditional classifications. The distinction lies in Monteggia equivalent fractures (MEFs) not always involv

Understanding Shoulder and Elbow Injuries in the Windmill Softball Pitcher.

Although pitching-related injuries in the overhead athlete have been studied extensively, injuries associated with windmill pitching are not as clearly elucidated. Windmill pitching produces high forces and torques in the upper extremity, and studies have shown it creates similar shoulder and elbow joint loads to those reported in baseball pitchers. Studies have shown that the windmill pitching motion generates high levels of biceps activation with an eccentric load, placing the biceps at increased risk for overuse injuries. Although the American Orthopaedic Society for Sports Medicine publish

Prevention and management of early treatment failures in elbow injuries.

The appropriate management of early treatment failures in patients with elbow injuries requires the identification of the cause of failure. In this work, six types of elbow injury are considered: (i) identification of early failed reduction of a dislocation or fracture-dislocation of the elbow should lead to a repeat reduction procedure, testing for elbow instability, and ligament repair, followed by the use of a hinged external fixator to allow early mobilisation. Differentiating an isolated dislocation from a dislocation combined with a fracture of the coracoid process is crucial. Re-implant

Current treatment concepts for "terrible triad" injuries of the elbow.

Elbow fracture-dislocations destabilize the elbow, preventing functional rehabilitation. If left untreated, they commonly result in functional compromise and poor outcomes. The "terrible triad" injury is classically described as a combination of a coronoid process and radial head fractures, as well as a posterolateral elbow dislocation. Surgical treatment to restore stable elbow range of motion has evolved in the past few decades based on increased understanding of elbow biomechanics and the anatomy of these injuries. This article highlights current concepts in the treatment of these complicat

Steindler flexorplasty of the elbow in obstetric brachial plexus injuries.

A Steindler flexorplasty at the elbow was performed in 26 patients with an obstetric brachial plexus lesion because of deficient elbow flexion. Follow-up after a mean of 2.9 years (range, 1-7 years) showed a good functional result in 23 cases, whereas 3 primary failures were noted. In one case, a recurrent dislocation of the elbow was seen 3 years after the initial flexorplasty. Steindler flexorplasty is a comparatively simple operation and gives very reliable results. It should be preferred above other, more elaborate muscle transfers for loss of elbow flexion.

Sports and occupational injuries of the elbow: MR imaging findings.

Elbow injuries may limit participation in sports and occupational endeavors, as well as activities of daily living. Conventional radiography is the appropriate initial screening technique for evaluation of possible osseous injury or arthritis/arthrosis, but often it offers little information concerning soft-tissue derangement, which is a common source of dysfunction. MR imaging can evaluate these soft tissue to assist in diagnosis, as well as in planning treatment and assessing response to treatment. This essay illustrates MR findings of both the chronic overuse and the more acute traumatic so

Valgus Extension Injuries of the Elbow in the Throwing Athlete.

Valgus extension injuries of the elbow are common among throwing athletes. There is a wide spectrum of these injuries, ranging from early soft-tissue inflammatory changes to attenuation and incompetence of the ulnar collateral ligament, as well as bone changes, including chondromalacia, osteochondritis, and osteophyte formation. Early treatment should be directed toward decreasing pain and inflammation, followed by global strengthening and conditioning of the elbow with special emphasis on plyometric strengthening of the flexor-pronator musculature. In patients who remain symptomatic after an

Surgical Approaches to the Elbow in Fixation of Traumatic Injuries.

Elbow fractures and fracture-instability injuries are increasingly prevalent. Gaining adequate exposure to the joint surface poses considerable challenges due to the unique articular anatomy and intricacies of the neurovascular structures traversing the elbow. Laterally, the radial head and capitellum are accessed through a classic Kocher or variations of the Kaplan technique. Posteriorly, the distal humerus is exposed by retracting the extensor mechanism through triceps split, reflection, or olecranon osteotomy. The medial approach may expose the coronoid through or around the flexor carpi ul

[Elbow joint dislocation : Frequent concomitant injuries and current treatment concepts].

Dislocations of the elbow joint are among the most prevalent dislocation injuries in the human body after shoulder joint dislocations and represent a challenge in the clinical routine because of the concomitant injuries and complications. They predominantly affect young adults who become injured during athletic or daily activities. A distinction is generally made between a simple elbow dislocation and a dislocation fracture of the elbow; however, a uniform classification or treatment algorithm has not yet been established, especially for simple elbow dislocations with associated ligamentous, m
