[Evaluation of cochlear and vestibular functions of patients treated by dibekacin in an E.N.T. unit].

Cochlear and vestibular ototoxicity of dibekacin was evaluated in 27 patients treated with aminoglycoside after surgery on head and neck cancer. The treatment was administered intramuscularly or intravenously during 13 days at the schedule of 3 mg/kg per day. Cochlear and vestibular functions were tested by audiometry and electronystagmography before the administration of the antibiotic and on the 14th, 30th day, and for certain patients on the 4th month after the start of the treatment. No cochlear ototoxicity was found after the dibekacin treatment. On the other hand, the dibekacin treatment

Dysequilibrium and audiovestibular function in panic disorder: symptom profiles and test findings.

Patients with panic disorder commonly report symptoms of dizziness and imbalance. We studied the relationship between objective measures of audiovestibular function, phenomenologic, and self-report measures of dysequilibrium and related somatic symptoms in a sample of panic disorder patients with and without agoraphobia, unselected for the complaint of dysequilibrium. Of seventeen patients evaluated by electronystagmography, 71 percent exhibited abnormal vestibular test findings. These latter patients had higher total anxiety ratings than patients without vestibular abnormalities. We conclude

Rotatory recovery nystagmus: an important localizing sign in endolymphatic hydrops.

Localizing the ear responsible for vertigo attacks may be exceedingly difficult in patients with delayed endolymphatic hydrops, vestibular Ménière's disease or bilateral Ménière's disease. This has important clinical implications when planning operative treatment. We present a case of delayed endolymphatic hydrops to exemplify these difficulties and demonstrate the localizing value of recovery nystagmus. We stress the importance when possible of monitoring patients with endolymphatic hydrops for recovery nystagmus during acute vertigo attacks. Monitoring should include direct eye observation a

[Quantitative ocular-motor tests analysis in multiple sclerosis].

UNLABELLED: Abnormalities in the ocular-motor reflex recorded in electronystagmography may indicate central vestibular system impairment. Multiple sclerosis (MS) is the most common chronic, debilitating disease characterized by focal demyelinization that develops throughout the central nerves system at varying time. The aim of the study was to apply the quantitative analysis of ocular-motor tests to evaluation degree of the central system impairment in MS patients. MATERIAL AND METHODS: The study was carried on 60 MS patients consulted in Balance Division, Otolaryngology Clinic, Medical Unive

Changes in the Tullio phenomenon and the fistula sign in the course of endolymphatic hydrops.

We performed electronystagmography with caloric stimulation and studied vestibular evoked myogenic potentials (VEMPs) associated with changes in the Tullio phenomenon and the fistula sign during the clinical course of a patient with endolymphatic hydrops. The Tullio phenomenon and the fistula sign disappeared in association with a reduction in the caloric response, which implicates the ampullary function of the lateral semicircular canal. Even when no VEMP could be detected for the affected ear, the Tullio phenomenon and fistula signs were observed; thus, either these phenomena had a lower res

Audiological and vestibular findings in long-term hormonal treatment.

93 patients receiving long-term hormonal treatment at the Hormonal Outpatient Clinic of the Department of Gynecology were examined audiologically and electronystagmographically. The patients had different hormonal disturbances including Turner's syndrome. Electronystagmography showed normal values with the exception of lower slow-component speeds in Turner syndrome patients and a shift of all directional preponderances to the left. Audiologically 13% showed pathological values in the filtered speech test. This phenomenon may be partially connected to a central gnostic hearing lesion. More stud

Positional nystagmus in association with macroglobulinemia.

Summary--Waldenstrom's macroglobulinemia is a malignant lymphoproliferative disease associated with a monoclonal macroglobulinemia. Most explanations of the cochlear and vestibular symptoms in this disease have dealt with vascular thrombotic or hemorrhagic phenomena. Five patients currently being treated for macroglobulinemia have been reviewed. Three patients gave a history of periodic dizziness and underwent extensive neuro-otologic examination. The results suggestsed an alternate mechanism for the pathophysiology of the vestibular disturbance in macroglobulinemia. Vestibular examination by

[Epidemiology of benign paroxysmal positional vertigo].

Every ninth patient with a previous history of vertigo at the ENT Hospital of the University of Freiburg suffers from BPPV. The incidence, some important pathogenetic factors and the clinical picture of BPPV are discussed. In young men posttraumatic aetiology dominates, whereas idiopathic BPPV prevails in perimenopausal women. The affected ear tends to be hard of hearing in the high frequencies and is sometimes hypoexcitable by caloric stimulation. The diagnosis is best made by personal examination with Frenzel's glasses in darkness. Electronystagmography, brainstem evoked response audiometry,

[Objective evaluation of tolerance of ventriculography with different x-ray contrast media (according to the results of nystagmography)].

The vestibulo-oculomotor reflex (nystagmus) was recorded by means of electronystagmography in 33 neurosurgical patients in dynamics prior to and after ventriculography. For contrasting the ventricular system a water-soluble medium (Conrey, dimer-X or Amipak) was used in 18 patients and contrast mixtures of water-soluble agents in combination with Myodil emulsion in 15. It was established that after ventriculography with water-soluble media the nystagmus hardly changed, while after the use of mixtures of radiocontrast media the trunk vestibular reactions in all types of nystagmus grew frequentl

Assessment of visual sensation, psychiatric profile and quality of life following vestibular schwannoma surgery in patients prehabituated by chemical vestibular ablation.

AIMS: Preoperative chemical vestibular ablation can reduce vestibular symptoms in patients who have gone through vestibular schwannoma resection. The goal of this study was to determine whether chemical vestibular prehabituation influences the patients' post-operative perception of visual stimulation, mental status and quality of life. We also tried to find out whether increases of optokinetic nystagmus, measured by routine electronystagmography, correlate with subjective symptoms. METHODS: We preoperatively administered (2 months prior to surgery) 0.5 - 1.0 mL of 40 mg/mL nonbuffered gentami

Endoscope-assisted vestibular neurectomy.

OBJECTIVE/HYPOTHESIS: In some instances endoscopes offer better visualization than the microscope and frequently allow less invasive surgery. This study was undertaken to determine whether endoscopy is safe and effective during neurectomy of the vestibular nerve. METHOD: Ten patients with intractable unilateral Meniere's disease underwent a retrosigmoid craniotomy for neurectomy of the vestibular nerve. Endoscopy with a Hopkins telescope was used during each procedure to study posterior fossa anatomic relationships and to assist the neurectomy. Preoperative and postoperative audiometric evalu

[Role of electrocochleography in identification of labyrinthine hydrops].

We studied potentialities of extratympanic electrocochleography (ETECG) in detection of labyrinthine hydrops (LH) in 50 patients who had an audiological picture of unilateral neurosensory hypoacusis. The patients were divided into two groups: group 1 patients suffered from attacks of systemic vertigo typical for Meniere's disease; group 2 patients had no vestibular complaints. ETECG results were compared to the dynamic range, results of otoacoustic emission and brain stem response, vestibulometry with electronystagmography (data of bithermal caloric test as most informative). The comparison in

The evaluation of dizziness in elderly patients.

Twenty-one elderly patients with dizziness underwent a comprehensive medical and otoneurological evaluation. The majority had vertigo, limited mobility and restricted neck movements. Poor visual acuity, postural hypotension and presbyacusis were also frequent findings. Electronystagmography revealed positional nystagmus in 12, disordered smooth pursuit in 18, and abnormal caloric responses in nine. Magnetic resonance imaging showed ischaemic changes in six out of eight patients. Although dizziness in the elderly is clearly multifactorial, the suggested importance of vertebrobasilar ischaemia w

Decrease of visual acuity in patients with clear media and normal fundi. Objective screening methods for differentiation and documentation.

For differentiation of unilateral reduction of visual acuity in patients with clear media and normal fundi, the Swinging Flashlight Test was used. Optic neuropathies as a rule give pathological results. On the other hand refraction scotomas, functional amblyopias and malingering produce a normal outcome of this test in general. For documentation of afferent pupillary defects a photographic method is described. Electronystagmography is advocated as a method for detection of malingerers and neurotics among patients complaining of acute uni- or bilateral visual acuity reduction. Optokinetic nysta

[Audio-vestibular changes in patients with rheumatoid arthritis].

A functional audio-vestibular investigation based on impedance metric techniques and electronystagmography was carried out in a group of patients with "classical" rheumatoid arthritis (RA) and with treated or untreated "definite" RA in various stages. Data obtained from these patients were compared with those obtained from a control group. Significant hypoacusis of the transmissive type was found in initial stages of RA while sensorineural or mixed type hypoacusis was found in later stages of RA. Significant vestibular alterations of the central type suggesting supratentorial involvement were

Clinical Video-Oculography and Video Head Impulse Testing.

Vestibular assessment has evolved from subjective bedside observation to quantitative measurement. Electronystagmography-enabled early objective recording of nystagmus, but its clinical use was limited by artifacts and practical challenges. Video-oculography advanced the field with the use of new technology to directly record eye movements providing a simple solution to record all classes of eye movements. The transition from the bedside head impulse test to the video head impulse test followed a similar trajectory, providing clinicians a new way to easily quantify the vestibulo-ocular reflex.

[Rare differential diagnosis in "vertigo" symptoms].

The diagnosis of vestibulocochlear nerve and statoacoustic organ lesions is based on the symptoms vertigo, hearing loss and tinnitus. The most common diseases are vestibular neuronitis, Ménière's disease, benign positional vertigo and sudden hearing loss. 6 patients are presented, complaining of at least one of the symptoms vertigo, hearing loss or tinnitus. They were suspected of suffering from a peripheral audiovestibular disease when admitted to the clinic, but all of them revealed other pathologies.
