Receipt of social services intervention in childhood, educational attainment and emergency hospital admissions: longitudinal analyses of national administrative health, social care, and education data in Wales, UK.

BACKGROUND: Research consistently finds poorer health and educational outcomes for children who have experienced out-of-home care relative to the general population. Few studies have explored differences between those in care and those in receipt of intervention from social services but not in care. Children receiving social services interventions often experience Adverse Childhood Experiences (ACEs), and deprivation, which are known to negatively impact outcomes. We aimed to estimate the association of different social services interventions with educational outcomes and hospital admissions,

Association of the distance travelled, and the call to hospital arrival time with early neonatal mortality in neonates born to mothers using emergency medical services at term gestation: a retrospective observational study.

BACKGROUND: Most neonatal deaths in India occur due to a lack of timely and proper care just before and during delivery. Timely availability of an ambulance for institutional delivery has led to some decrease in this. However, the distance travelled and time taken may affect the outcome. OBJECTIVE: The objective of this retrospective observational study was to determine if the distance travelled and the call to hospital arrival time are associated with deaths in the early neonatal period (0-7 days postnatal age), in neonates born to mothers who used a centralised emergency medical services (E

Better outcomes for the older surgical patient trial (BOOST): protocol for a hybrid type 1 effectiveness-implementation trial of an embedded geriatric perioperative service within emergency and elective surgery in an Australian tertiary referral hospital.

INTRODUCTION: Older patients admitted under surgical care have longer length of stay (LOS) and are at risk of functional decline, hospital-acquired complications and geriatric syndromes. Embedded specialist geriatrician models within surgical care teams can reduce length of stay and perioperative complications. Evidence gaps remain regarding the implementation of these models of care and their impact on patient outcomes. This study aims to measure hospital, patient and implementation outcomes of an embedded perioperative geriatric service in a large Australian tertiary referral hospital. METH

Beyond Clinical Acuity: Cardiac Comorbidity and Complication Profiles and the Prehospital Hospitalization/Transport Decision in Ischemic Heart Disease-A Five-Year Retrospective Emergency Medical Service Study in Astana, Kazakhstan.

Background/Objectives: Ischemic heart disease (IHD) is a major cause of cardiovascular mortality, and emergency medical service (EMS) crews often make the first decision on whether an IHD-coded patient should be transported to hospital or managed at the scene. Evidence from Central Asian EMS systems is sparse, and it remains unclear whether routinely coded comorbidity information adds decision-relevant information beyond acute presentation. We aimed to identify predictors of the EMS hospitalization/transport decision among IHD calls in Astana, Kazakhstan. Materials and Methods: We conducted a

A retrospective medical record audit of the management of cannabis-related emergency department presentations, hospital admissions and hyperemesis of pregnant women who self-reported non-medicinal cannabis use to a substance use in parenting and pregnancy service.

INTRODUCTION: Research on cannabis-related hyperemesis and its identification and management during pregnancy is scarce. This study aimed to investigate the management of cannabis-related emergency department (ED) presentations, hospital admissions and hyperemesis of pregnant women who self-reported non-medicinal cannabis use to a substance use in pregnancy and parenting service (SUPPS). METHODS: A retrospective medical record audit was conducted on 75 SUPPS patients who identified cannabis as their primary drug of concern and gave birth between January 2015 and May 2020 in Australia. Trends and characteristics in pre-hospital emergency care in Beijing from 2008 to 2017.

BACKGROUND: We aimed to describe and analyze the pre-hospital emergency medical service (EMS) in Beijing and provide information for the government and medical institutions to optimize EMS. METHODS: We collected all pre-hospital emergency data in Beijing from 2008 to 2017. The chief complaint in each case was classified according to the Medical Priority Dispatch System (MPDS). The sites' administrative districts were determined through geo-encoding of addresses and then classified into four functional regions. We analyzed the demand for EMS, emergency response times (ERT), and disease spectru

Medicare program; revisions to payment policies under the physician fee schedule for calendar year 2003 and inclusion of registered nurses in the personnel provision of the critical access hospital emergency services requirement for frontier areas and remote locations. Final rule with comment period.

This final rule with comment period refines the resource-based practice expense relative value units (RVUs) and makes other changes to Medicare Part B payment policy. In addition, as required by statute, we are announcing the physician fee schedule update for CY 2003. The update to the physician fee schedule occurs as a result of a calculation methodology specified by law. That law required the Department to set annual updates based in part on estimates of several factors. Although subsequent after-the-fact data indicate that actual increases were different to some degree from earlier estimate

Deployment of Alternative Response Units in a High-Volume, Urban EMS System.

Faced with increasing demand for their services, Emergency Medical Services (EMS) agencies must find more efficient ways to use their limited resources. This includes moving beyond the traditional response and transport model. Alternative Response Units (ARUs) are one way to meet the prehospital medical needs of some members of the public while reducing ambulance transports. They are non-transport vehicles tasked with very specific medical missions. These can include acute management of low-acuity complaints, ongoing home care for chronic medical conditions, preventive medicine, and post-hospi

[Emergency medicine : what's new in 2024].

Emergency medicine plays a crucial vital role as the gateway to the Swiss healthcare system. Although it has not yet been officially recognized with a specialist title, unlike most European countries - emergency medicine in Switzerland is characterized by robust research activity. This scientific article demonstrates a dynamic and rigorous evolution. In this edition, we present six articles authored by Swiss emergency physicians, each examining different aspects of our field. Topics include gender stereotypes, prehospital cardiac arrest, the unique needs of the older patients in emergency depa

[Urological emergency management : Detection and treatment].

As a result of the restructuring of emergency care provided by statutory health insurance companies and demographic change, the number of people presenting to emergency departments in hospitals is continuously increasing. A significant proportion of these emergencies concern the specialist discipline of urology. The clinical manifestations in emergency medical care range from trauma-related injuries to the urogenital tract and urinary transport disorders to hematuria and life-threatening infections. Due to the high incidence of urological tumors and the urogenital tract's affection in other ma

Emergency department medical care during civil unrest: a narrative review.

BACKGROUND: Civil unrest is a critical societal challenge in many parts of the world. Crowd control weapons (including conducted energy devices, chemical irritants, kinetic impact projectiles, acoustic devices, and water cannons) produce agent-specific injuries that do not precisely map onto standard trauma frameworks. This narrative review synthesises the available evidence on the mechanisms, health effects, and emergency department management of casualties inflicted by crowd-control weapons. MAIN BODY: Emergency department preparedness for civil unrest rests on surge capacity planning acros

[The concept of an emergency admission unit in a major community hospital].

The main task of a central interdisciplinary emergency admission unit (EAU) staffed and administered by the anesthesia department is to provide primary hospital treatment for emergency patients. The EAU serves also as a base for the emergency ambulance and functions as a 24-hour resuscitation unit for the whole hospital. The placement of the unit in the underground floor of the hospital makes it easy accessible for ambulances but excludes walk-in patients. The shock room has a floor area of 152 m2 and is optimally equipped, while two smaller emergency rooms provide reserve accommodation. On th

Understanding oncologic emergencies and related emergency department visits and hospitalizations: a systematic review.

BACKGROUND: Patients with cancer frequently visit the emergency department (ED) and are at high risk for hospitalization due to severe illness from cancer progression or treatment side effects. With an aging population and rising cancer incidence rates worldwide, it is crucial to understand how EDs and other acute care venues manage oncologic emergencies. Insights from other nations and health systems may inform resources necessary for optimal ED management and novel care delivery pathways. We described clinical management of oncologic emergencies and their contribution to ED visits and hospit

Emergencies in Otorhinolaryngology: Diagnostic Evaluation, Assessment of Urgency, and Treatment.

BACKGROUND: Emergencies in otorhinolaryngology are common: According to a Spanish study, approximately 12% of patients in a general emergency room have symptoms relating to the ear, nose, or throat (ENT). Such situations range in severity from minimal to lifethreatening and affect persons of all ages. These patients may pre - sent first to a general practitioner or to an emergency room without specialized ENT coverage. In this article, we discuss the assessment of clinical urgency based on symptoms, and the ensuing treatments. METHODS: This narrative review is based on pertinent publications

[Syncope in prehospital emergency medicine].

Loss of consciousness is a frequent cause for an emergency call to the emergency medical services (EMS). It can be associated with life-threatening conditions. A distinction must be made between transient loss of consciousness (TLOC) and syncope, which is of cardiovascular origin by definition. Initial assessment in prehospital emergency care should follow the ABCDE algorithm including a 12-lead ECG. The presence of important risk factors such as occurrence in supine position, physical stress, palpitations, history of heart diseases, and any abnormalities in the ECG warrants hospital admission
