Metta-Based Therapy for Chronic Depression: a Wait List Control Trial.

OBJECTIVES: Current treatments for chronic depression have focused on reducing interpersonal problems and negative affect, but paid little attention to promoting prosocial motivation and positive affect. Following this treatment focus, the objective of the present study was to examine whether the combination of metta (Loving Kindness) group meditation and subsequent tailored individual therapy focusing on kindness towards oneself and others (metta-based therapy, MBT) shows greater improvements in depressive symptoms than a wait list control group in patients with chronic depression. METHODS:

Emotion Regulation and Relationship Satisfaction in Couple Therapy: What Happens Across Sessions?

While previous studies have demonstrated significant associations between partners' emotion regulation (ER) and overall well-being, very few studies provide knowledge on ER in clinical couples, especially across couple therapy sessions. The sample consisted of 174 mixed-sex couples, predominantly white and from the western region of the United States, who attended at least the initial eight sessions of couple therapy. The current study examined the intra- and inter-personal effects of ER difficulties (i.e., lack of awareness, nonacceptance, limited strategies, and impulsivity) on the trajector

Emotion regulation and heart rate variability may identify the optimal posttraumatic stress disorder treatment: analyses from a randomized controlled trial.

INTRODUCTION: High variability in response and retention rates for posttraumatic stress disorder (PTSD) treatment highlights the need to identify "personalized" or "precision" medicine factors that can inform optimal intervention selection before an individual commences treatment. In secondary analyses from a non-inferiority randomized controlled trial, behavioral and physiological emotion regulation were examined as non-specific predictors (that identify which individuals are more likely to respond to treatment, regardless of treatment type) and treatment moderators (that identify which treat

The impact of brief intensive trauma-focused treatment for PTSD on symptoms of borderline personality disorder.

Background: It is generally recommended to exercise caution in applying trauma-focused treatment to individuals with posttraumatic stress disorder (PTSD) and comorbid borderline personality disorder (BPD). Objective: To investigate the effects of a brief, intensive, direct trauma-focused treatment programme for individuals with PTSD on BPD symptom severity. Methods: Individuals (n = 72) with severe PTSD (87.5% had one or more comorbidities; 52.8% fulfilled the criteria for the dissociative subtype of PTSD) due to multiple traumas (e.g. 90.3% sexual abuse) participated in an intensive eight-day

Psychological intervention in patients with central sensitization syndromes and comorbid psychosocial trauma: a systematic review and meta-analysis.

The co-occurrence of central sensitization syndromes (CSS) and post-traumatic symptoms (PTSS) or post-traumatic stress disorder (PTSD) exacerbates impairment, and treatment is unsatisfactory in many cases. This systematic review and meta-analysis examined the effectiveness of psychological therapy in individuals with CSS and comorbid PTSS/PTSD. A search of Medline, PsycInfo, Web of Science, Pubmed, and Scopus databases (2000 to 2023) was conducted following PRISMA guidelines. Nineteen articles were selected, including nine for meta-analysis. The main psychological therapies were pain-focused c

Compassion-focused therapy enhances outcomes of PTSD pharmacotherapy: A randomized controlled trial.

traumatic stress disorder is a debilitating condition often resistant to pharmacotherapy alone. Compassion-focused therapy may offer additional benefits in the treatment of post-traumatic stress disorder. We evaluated the effectiveness of compassion-focused therapy as an adjunct to standard pharmacological treatment for reducing post-traumatic stress disorder symptom severity in male veterans. A randomized controlled trial was conducted involving 50 male veterans with post-traumatic stress disorder diagnosed according to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition.

Is the reexperience of painful emotion therapeutic?

Recently, a great deal of attention has been focused on the health promoting benefits that can accrue from revisiting painful emotion. The rationales for revisiting painful emotions include those that assume reexperiencing emotion per se can be health-promoting. Another view stipulates that revisiting painful emotion will only yield benefit if there is some recasting/restructuring of the emotional memory. Research pertinent to the various rationales is discussed. Then research on the impact of emotional expression and outcomes studies of therapies designed to enhance emotional experience are r

A scoping review of therapies to support marriages and partnerships after acquired brain injury.

BACKGROUND: Acquired brain injury (ABI) can undermine marriages/partnerships. Couples need access to relationship therapies proven to be effective in a brain injury context. OBJECTIVE: To provide an overview of relationship therapies evaluated for use in ABI. METHODS: Six databases were searched for studies that evaluated the impact on the relationship of interventions that were, at least partly, intended to improve the relationship. RESULTS: Twenty-seven papers were included. Some reported the use of therapies widely used with other populations (family therapy, emotion-focused therapy, beh

Treatment of multiple traumatized adolescents by enhancing regulation skills and reducing trauma related symptoms: rationale, study design, and methods of randomized controlled trial (the Mars-study).

BACKGROUND: There is ongoing debate regarding the treatment of severe and multiple traumatized children and adolescents with post-traumatic stress disorder (PTSD). Many clinicians favor a phase-based treatment approach (i.e., a stabilization phase prior to trauma-focused therapy) over immediate trauma-focused psychological treatment, despite the lack of scientific evidence. Research on the effects of different treatment approaches is needed for children and adolescents with (symptoms of complex) PTSD resulting from repeated sexual and/or physical abuse during childhood. OBJECTIVE: This paper

Differential therapeutic outcomes of community-based group interventions for women and children exposed to intimate partner violence.

Two community-based group therapies, emotion focused versus goal oriented, are compared among women exposed to intimate partner violence (n = 46) and their children ( n = 48) aged between 6 and 12 years. A series of repeated measures analyses are employed to evaluate the effects of time from baseline to postintervention following random assignment. Main and treatment effects for women provide support for the relative effectiveness in increasing quality of social support in the emotion-focused intervention and in the reduction of both family conflict and alcohol use for the goal-oriented interv

A healing herd: benefits of a psychodynamic group approach in treating body-focused repetitive behaviors.

Body-focused repetitive behaviors, such as trichotillomania and skin-picking, are not well understood in clinical communities. Research and practice with a population exhibiting these behaviors have focused almost exclusively on the usefulness of cognitive-behavioral therapy in symptom reduction. The author specializes in treating patients with body-focused repetitive behaviors and suggests that a holistic, psychodynamic model of treatment for this population creates possibilities for deeper change. Group therapy can be an important component of this work, helping clients to overcome resistanc

Risk of Depression Among MMT Patients: Does Coping Strategies and Perceived Social Support Play a Role?

INTRODUCTION: Patients receiving methadone maintenance therapy (MMT) experience higher level of stress and are at greater risk of developing mental health problems such as depression which could potentially affect both quality of life and treatment outcomes. This cross-sectional study is aimed at understanding the relationship between psychosocial factors such as social support, coping, and depression among patients receiving MMT in a Malaysian Hospital. METHODS: One hundred and ninety-six patients attending MMT program were recruited. The Patient Health Questionnaire-9 (PHQ-9) was used to sc

Rapid response to intensive treatment for bulimia nervosa and purging disorder: A randomized controlled trial of a CBT intervention to facilitate early behavior change.

OBJECTIVE: Rapid response to cognitive behavior therapy (CBT) for eating disorders (i.e., rapid and substantial change to key eating disorder behaviors in the initial weeks of treatment) robustly predicts good outcome at end-of-treatment and in follow up. The objective of this study was to determine whether rapid response to day hospital (DH) eating disorder treatment could be facilitated using a brief adjunctive CBT intervention focused on early change. METHOD: 44 women (average age 27.3 [8.4]; 75% White, 6.3% Black, 6.9% Asian) were randomly assigned to 1 of 2 4-session adjunctive intervent

Changes in functional connectivity of the amygdala during cognitive reappraisal predict symptom reduction during trauma-focused cognitive-behavioral therapy among adolescent girls with post-traumatic stress disorder.

BACKGROUND: While trauma-focused cognitive-behavioral therapy (TF-CBT) is the 'gold standard' treatment for pediatric post-traumatic stress disorder (PTSD), little is known about the neural mechanisms by which TF-CBT produces clinical benefit. Here, we test the hypothesis that PTSD symptom reduction during TF-CBT among adolescent girls with PTSD is associated with changes in patterns of brain functional connectivity (FC) with the amygdala during cognitive reappraisal. METHOD: Adolescent girls with PTSD related to physical or sexual assault (n = 34) were enrolled in TF-CBT, delivered in an app

Neural predictors of treatment outcome through emotion regulation in internalizing disorders: a narrative review.

Internalizing disorders such as depressive and anxiety disorders are prevalent, disabling, and characterized by difficulty managing emotions particularly in the context of negative information. Despite the availability of empirically supported treatments, response to these treatments remains heterogeneous. Accruing data indicate neural predictors of treatment response have the potential to contribute to precision medicine to improve treatment outcomes. This review evaluates functional magnetic resonance imaging studies that examined explicit (e.g., effortful) and implicit (e.g., automatic) emo

A biopsychosocial safeness model: New directions for psychotherapy research and practice.

This article examines the distinction between safety and safeness and argues that safeness reflects a distinct biopsychosocial process with specific evolutionary functions, physiological signatures, and social manifestations. Whereas safety is primarily associated with threat detection and defensive regulation, safeness refers to affiliative states characterized by perceived security, connection, and soothing regulation. Building on attachment theory, social safeness research, emotion regulation models, polyvagal theory, and compassion-based approaches, we propose an integrative developmental

Emotion Processing and the Role of Compassion in Psychotherapy from the Perspective of Multiple Selves and the Compassionate Self.

Emotion processing is an important factor for successful psychotherapy. Clients tend to suffer from maladaptive emotions, which contribute to states of confusion, rumination, and stagnation. The therapist should demonstrate adequate empathy and understanding of the client's complaints to help the client to recognize and respect their own emotions and desires. In most cases, there is more than one desire, and each desire should be confronted. The compassionate self exercises are helpful to distinguish and integrate confused states. In this report, the author introduces a case in which the thera

Anorexia Nervosa and a Lost Emotional Self: A Psychological Formulation of the Development, Maintenance, and Treatment of Anorexia Nervosa.

In this paper, we argue that Anorexia Nervosa (AN) can be explained as arising from a 'lost sense of emotional self.' We begin by briefly reviewing evidence accumulated to date supporting the consensus that a complex range of genetic, biological, psychological, and socio-environmental risk and maintenance factors contribute to the development and maintenance of AN. We consider how current interventions seek to tackle these factors in psychotherapy and potential limitations. We then propose our theory that many risk and maintenance factors may be unified by an underpinning explanation of emotio

Neural correlates of mindful emotion regulation in high and low ruminators.

Depressive rumination is considered a prominent risk factor for the occurrence, severity, and duration of depressive episodes. A variety of treatment options have been developed to treat depressive rumination of which mindfulness based programs are especially promising. In the current study, we investigated the neural underpinnings of a short mindfulness intervention and mindful emotion regulation in high and low trait ruminators in an ecologically valid environment using functional near-infrared spectroscopy (fNIRS). Participants were randomly assigned to a mindfulness instruction (MT) group

Trauma-Focused Art Therapy in the Treatment of Posttraumatic Stress Disorder: A Pilot Study.

Research showed that more than 30% of patients with Posttraumatic Stress Disorder (PTSD) do not benefit from evidence-based treatments: Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) or Eye Movement Desensitization and Reprocessing (EMDR). These are patients with prolonged and multiple traumatization, with poor verbal memory, and patients with emotional over-modulation. Retelling traumatic experiences in detail is poorly tolerated by these patients and might be a reason for not starting or not completing the recommended treatments. Due to lack of evidence, no alternative treatments are r
