SARS-CoV-2 pneumonia with subcutaneous emphysema, mediastinal emphysema, and pneumothorax: A case report.

INTRODUCTION: Since the end of 2019, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection has affected more than 1,000,000 population in the world. Subcutaneous emphysema and pneumothorax are uncommon complications of SARS-CoV-2 pneumonia. Herein, we describe a fatal case of SARS-CoV-2 pneumonia with subcutaneous emphysema and pneumothorax. PATIENT CONCERNS: Subcutaneous emphysema was found in neck, bilateral chest walls, abdomen wall, groin area, and scrotum of a 67-year-old man. Extensive air-space opacities, subcutaneous emphysema and a small amount of pneumothorax were f

Severe pneumoperitoneum, mediastinal emphysema, and thoracoabdominal wall emphysema during flexible bronchoscopy: a case report.

This case presents a rare complication that occurred during flexible bronchoscopy, including severe pneumoperitoneum, mediastinal emphysema, and thoracoabdominal wall emphysema. This study provides a profound analysis of the most probable causes of the aforementioned complications, hoping to offer some valuable insights for clinical practice. The patient developed abdominal distension, abdominal bulge, decreased oxygen saturation, and hemodynamic instability during flexible bronchoscopy under intravenous anesthesia with oxygen delivery via a nasopharyngeal oxygen cannula at a flow rate of 5 L/

ADRB2 gene polymorphism and emphysema heterogeneity can modulate bronchodilator response in patients with emphysema.

BACKGROUND: Genetic variation in the β2-adrenergic receptor (ADRB2) gene has been thought to have an important role in the differential response to β2-agonist therapy for asthma. However, previous studies have shown little evidence for an association between these ADRB2 variants and the bronchial dilator response (BDR) in chronic obstructive pulmonary disease (COPD) patients. This discrepancy could be explained by differences in the distribution and heterogeneity of pulmonary emphysema in COPD patients, since emphysema distribution and heterogeneity are thought to have a role in pulmonary func

Panlobular emphysema is associated with COPD disease severity: A study of emphysema subtype by computed tomography.

BACKGROUND: Computed tomography has the potential to inform COPD prognosis. We sought to determine associations of emphysema phenotype with clinical parameters including lung function, inflammatory markers, and quality of life. METHODS: Participants of this single-center observational cohort (n = 83) were 40-80 years old, had ≥10 pack-year smoking, and a diagnosis of COPD confirmed by spirometry. All participants had available historic chest CT scans which were systematically reviewed by a single expert radiologist and scored for emphysema subtype, extent, and distribution. Associations betwe

Bullous emphysema - Not always nicotine-related!: Unusual distribution of emphysema in a patient with a rare hobby.

In a patient admitted for further investigation of haemoptysis and dyspnoea and known emphysema of the lung, a remarkable distribution of emphysematous bullae could be detected on CT-imaging. Further history, besides smoking, revealed apnoea diving-activity during younger adult age. The distinct appearance of partially septated pleura-based bullae lead to the suspicion of a positive-pressure barotrauma of the lungs in the past, now complicated by infection and bleeding. This case highlights the importance of thorough questioning of the patient and underlines the consideration of differential d

Man with massive subcutaneous emphysema.

Subcutaneous emphysema is a well-described and frequently self-limited complication of multiple medical conditions including patients who have undergone the insertion or removal of chest tubes. While typically benign, the extensive collection of soft tissue emphysema in select patients can lead to significant respiratory distress secondary to restricted thoracic wall excursion or laryngotracheal dysfunction due external compression from the subcutaneous gas. In these patients, release of this trapped air via gill-slit incisions of the thoracic wall can be a life-saving intervention. We present

Pulmonary interstitial emphysema in newborns.

Pulmonary interstitial emphysema (PIE) is a rare case of air leak syndrome in newborns, with air escaping from the tracheobronchial tree into the lung tissue. We observed 3 cases of PIE. The three newborns were moderately premature. The common initial presentation was respiratory distress, which subsequently worsened. The treatment was conservative, with positioning and oxygen therapy. Medium- and long-term outcomes were good, with patients remaining clinically symptom-free. However, follow-up CT scans revealed the persistence of sequelae bullae.Systematic medium- and long-term follow-up may be

Cervicofacial emphysema: A systematic review.

Cervicofacial Emphysema (CFE) is a self-limiting condition, defined by the presence of air in face and neck. The purpose of the manuscript is to systematically review the existing literature on CFE evaluation and management for updated clinical understanding of this condition. A literature search was conducted of publications about CFE on PubMed and Google Scholar by identifying all the articles with key search terms "Cervicofacial Emphysema" and "Sub Cutaneous Emphysema". Inclusion criteria were case series published in English between 1980 and 2024. In total, 241 case series were selected an

Emphysema and lung cancer risk.

BACKGROUND: With increasing significance of lung cancer screening programs, it is essential to determine the group of participants, who would benefit the most from screening. In our study, we aimed to establish the correlation between lung emphysema and lung cancer risk. METHODS: The study design was cross-sectional. Low-dose computed tomography (LDCT) scans of 896 subjects from MOLTEST-BIS lung cancer screening program, including 100 subjects with detected lung cancer, were visually evaluated for the presence, type and severity of emphysema. Quantitative emphysema evaluation was performed wi

Cocaine-Induced Giant Bullous Emphysema.

BACKGROUND: Emphysematous bullae, defined as airspaces of greater than or equal to one centimeter in diameter, have a variety of etiologies such as tobacco use and alpha-1 antitrypsin being the most common. Emphysematous bullae have also been reported in patients using cocaine usually involving the lung periphery and sparing the central lung parenchyma. We present a case of a male with a history of cocaine abuse found to have a singular giant emphysematous bulla occupying >95% of the right hemithorax requiring video-assisted thoracic surgery (VATS) with a favorable outcome. Case Presentation.

Endobronchial valves for severe emphysema.

The results of the randomised controlled trials investigating the bronchoscopic lung volume reduction treatment using endobronchial valves (EBV) are promising, and have led to their inclusion in treatment guidelines, US Food and Drug Administration approval and inclusion in routine care in an increasing number of countries. The one-way valve treatment has advanced and is now a regular treatment option. However, this new phase will lead to new challenges in terms of implementation. We believe that key issues in future research concern advanced patient selection, improved methods for target lobe

Extensive subcutaneous emphysema following lobectomy.

We present a case report of extensive subcutaneous emphysema secondary to an elective left upper lobectomy. A 65-year-old gentleman was brought into a London teaching hospital's Accident and Emergency department following report of severe swelling. He was mistakenly treated by the paramedics as an allergic reaction and given hydrocortisone and salbutamol nebulisers with no effect. Upon arrival, the patient had widespread crepitus extending from his peri-orbital muscles down to mid-torso. A computer tomography scan revealed a pleuro-cutaneous fistula at the site of a recently sited chest drain,

Postpartum pneumomediastinum and subcutaneous emphysema.

We present the case of a 22-year-old primigravida with symptoms of neck swelling and difficulty breathing 3 hours after uneventful vaginal delivery. On examination, there was surgical emphysema. A CT scan confirmed air in the subcutaneous tissues and also revealed pneumomediastinum. There was no evidence of pneumothorax and gastro-oesophageal endoscopy showed no signs of oesophageal rupture. The patient remained clinically stable throughout her admission and was discharged home after 48 hours of observation. Pneumomediastinum is one of the rare causes of shortness of breath and chest pain duri

Regression of giant bullous emphysema.

Spontaneous resolution of bulla associated with infection or tumor is occasionally observed. However, resolution of progressively enlarging giant bullous emphysema (GBE) after medical therapy has not been reported. A 51-year-old man smoker with GBE on the right lung was referred to Samsung Medical Center for the consideration of bullectomy. A review of his medical records revealed that right-side bullous emphysema had been detected 4 years previously and it had progressively enlarged with a concomitant deterioration of lung function. Although he had a history of chronic asthma, he had never be

Unilateral orbital emphysema after sneezing.

We report a case who developed unilateral orbital emphysema after sneezing in a 30-year-old man presented with left crepitant eyelid swelling and progressive ptosis. Sneezing may cause orbital emphysema in cases with a history of minor periorbital trauma. While periodical followup without any medication is considered satisfactory, needle decompression may provide early cure.
