Emphysematous cholecystitis: diagnostic problems and differential diagnosis of gallbladder gas accumulations.

Acute emphysematous cholecystitis is an uncommon form of acute cholecystitis characterized by the presence of gas within the wall, lumen of the gallbladder or biliary ducts. Recognition of this gas is of utmost importance for an adequate therapy. Based on selected case studies the significance of plain abdominal radiographs, ultrasonography, CT and radionuclide studies is discussed. Differential diagnoses and diagnostic problems are mentioned to help understand this rare, but clinically important condition.

Emphysematous cholecystitis in a young male without predisposing factors: case report.

INTRODUCTION AND IMPORTANCE: Emphysematous cholecystitis (EC) is a rare, life-threatening form of acute cholecystitis, usually occurring in older patients with a history of diabetes mellitus and other comorbidities. CASE PRESENTATION: We present a 36-year-old male patient with no predisposing factors who presented with right upper quadrant pain, fever, nausea, vomiting, and loss of appetite, which was clinically indistinguishable from acute non-EC. Multimodality imaging revealed gas within the lumen and wall of the gallbladder, confirming the diagnosis. CLINICAL DISCUSSION: This case is unus

Upper gastrointestinal bleeding related to emphysematous cholecystitis due to Clostridium perfringens.

We describe the case of a 46-year-old man admitted for upper gastrointestinal bleeding in the context of cirrhosis. A deep bleeding duodenal ulcer was treated by sclerotherapy. Abdominal pain and fever lead us to perform an abdominal computed tomography, which demonstrated emphysematous cholecystitis. An emergency cholecystectomy was performed and antimicrobial therapy initiated. The patient recovered uneventfully. Links between ulcers and emphysematous cholecystitis are discussed.

Emphysematous cholecystitis. Review of five cases and report of septic musculoskeletal complications.

A variant of acute cholecystitis is emphysematous cholecystitis. Here in we reviewed useful clinical data of five cases of this entity. Clinical outcomes were uncomplicated for three patients who were treated with open cholecystectomy. But on the other hand we faced two septic musculoskeletal complications in two patients who were treated with percutaneous trans gallbladder drainage. We believe that we must be aware of musculoskeletal complications, whenever a patient with emphysematous cholecystitis is treated with percutaneous trans gallbladder drainage.

Case report: Whole genome sequence of Clostridium perfringens JUM001 causing acute emphysematous cholecystitis.

A strain of Clostridium perfringens was isolated from the bile sample of a patient with emphysematous cholecystitis who underwent a laparoscopic cholecystectomy, followed by treatment with meropenem and recovery. Metagenomic analysis of the bile sample showed that 99.73% of the bile microbiota consisted of C. perfringens, indicating that C. perfringens JUM001 was the causative pathogen of acute emphysematous cholecystitis in this patient. Complete genome sequencing showed that C. perfringens JUM001 contained a circular chromosome of 3,231,023 bp and two circular plasmids, pJUM001-1 of 49,289 b

Intravascular Hemolysis and Septicemia due to Clostridium perfringens Emphysematous Cholecystitis and Hepatic Abscesses.

Context. Clostridium perfringens septicemia is often associated with translocation from the gastrointestinal or gastrourinary tract and occurs in patients who have malignancy or are immunocompromised. Clostridium perfringens septicemia is usually fatal without early identification, source control, and antibiotics. Case. We present a case of a 65-year-old female with Clostridium perfringens septicemia secondary to emphysematous cholecystitis, with progression to hepatic abscesses. Conclusion. Septicemia secondary to Clostridium perfringens is generally fatal if not detected early. Source contro

Massive pneumoretroperitoneum arising from emphysematous cholecystitis: a case report and the literature review.

BACKGROUND: Emphysematous cholecystitis is a severe variant of acute cholecystitis caused by anaerobic bacteria. Although intraperitoneal air as a complication has been described in association with emphysematous cholecystitis, pneumoretroperitoneum arising from emphysematous cholecystitis is extremely rare. Herein, we describe a rare case of pneumoretroperitoneum arising from emphysematous cholecystitis that was successfully treated with emergency surgery. CASE PRESENTATION: An 84-year-old male was transported to the Emergency Department of our hospital for acute abdomen. Computed tomography

Post-ERCP Emphysematous Cholecystitis in a Young Woman: A Rare and Potentially Fatal Complication.

A 45-year-old woman with suspected Functional Biliary Sphincter Disorder (FBSD) developed Clostridium perfringens related emphysematous cholecystitis after ERCP. A low index of suspicion for emphysematous cholecystitis in this young, otherwise healthy woman led to a significant delay in making the correct diagnosis, and air in the gallbladder was wrongly attributed to a possible gallbladder perforation. ERCP is associated with significant risks, particularly in patients with FBSD, where diagnostic uncertainty renders the balance of risk versus benefit even more critical. Post-ERCP emphysematou

Case of emphysematous cholecystitis in a patient with type 2 diabetes mellitus associated with schizophrenia.

Emphysematous cholecystitis is a rare, but life-threatening, form of acute cholecystitis caused by gas-forming organisms in the gallbladder. A 73-year-old male patient with type 2 diabetes mellitus complicated with neuropathy associated with schizophrenia was admitted to Okayama University Hospital, Okayama, Japan, because of a high fever and general malaise. On the fourth hospital day, despite normal liver function tests and little abdominal pain, his abdominal computed tomography showed huge gas formation in the gallbladder lumen along with a dilated gallbladder with a thickened wall, consis

Emphysematous Calculous Cholecystitis Diagnosed with Abdominal CT in a Diabetic Female Patient - Case Report.

Emphysematous cholecystitis is reported to have a low incidence of less than 1% in all cases of acute cholecystitis and yet a high mortality rate of up to 15%. It is most commonly seen in male diabetic patients with advanced age. The diagnosis is established with the presence of gas in the gallbladder lumen and/or within its wall which can be seen on plain abdominal radiography, abdominal ultrasound, and abdominal computerized tomography. The clinical presentation refers to one of acute cholecystitis, but the treatment requires prompt cholecystectomy since the patient's condition can deteriora

Emphysematous Cholecystitis Resulting in Secondary Biliary Cirrhosis: A Rare Complication of Endoscopic Retrograde Cholangiopancreatography.

A 48-year-old female developed acute emphysematous cholecystitis after an endoscopic retrograde cholangiopancreatography (ERCP) for evaluation of sphincter of Oddi dysfunction. Cholecystectomy was performed 2 days later. Cultures grew Clostridium perfringens. The patient received broad-spectrum antibiotics but developed recurrent cholangitic abscesses and intra- and extra-hepatic biliary necrosis. She was managed by percutaneous transhepatic biliary drains. For next 3 years, patient had recurrent episodes of biliary obstruction, cholangitis, and sepsis, resulting in secondary biliary cirrhosis

Emphysematous Cholecystitis and Necrotizing Pancreatitis: Unusual Combination of Critical Pathology in a Healthy Young Patient.

Emphysematous cholecystitis (EC) is a rare form of severe cholecystitis, more common among old, frail males, with comorbidities like diabetes mellitus and peripheral vascular disease. The coexistence of emphysematous cholecystitis with necrotizing pancreatitis is extremely uncommon and in the literature, though it has been reported, no case was of a young patient like the one we are describing. He was treated with laparoscopic cholecystectomy, and his pancreatitis managed conservatively.

[Case report: Mirizzi syndrome in a patient with emphysematous cholecystitis: ultrasonographic and computed tomographic findings].

Mirizzi syndrome is a very uncommon cause of extrahepatic bile duct obstruction. A stone impacts in the cystic duct or gallbladder neck and causes extrinsic compression and obstruction of the common hepatic duct, aided by a local inflammatory process. Preoperative diagnosis is very important because of the potential risk of bile duct injury during surgery and the local inflammatory process may suggest cholangiocarcinoma. Infection by specific coliform organisms may result in emphysematous cholecystitis in patients with diabetes or those who are immunosuppressed, with mural air being visible. H

Point of care ultrasound in rapid diagnosis of acute cholangitis and emphysematous cholecystitis: a case report.

BACKGROUND: Emphysematous cholecystitis is a rare and rapidly progressive disease that requires prompt diagnosis and intervention. Point of care ultrasound (POCUS) is a useful diagnostic imaging tool in the emergency department that can help expedite diagnosis and management of biliary etiology. CASE PRESENTATION: In our case, we describe an 85 year old female with a history of diabetes mellitus with a presentation consistent with undifferentiated cholecystitis. Point of care ultrasound performed in the emergency department showed a characteristic "champagne sign", along with other findings i

Laparoscopic treatment of an unruptured cystic artery pseudo-aneurysm in the presence of calculous emphysematous cholecystitis.

Cystic artery pseudoaneurysm is a rare complication of invasive biliary procedures or of acute or chronic cholecystitis and pancreatitis. Emphysematous cholecystitis is an acute inflammatory process of the gallbladder due to gas forming organisms such as Escherichia coli and Clostridium perfringens. We report the case of a 34-year-old gentleman admitted with a 3-day history of generalized abdominal pain, vomiting and markedly raised inflammatory markers. A computed tomography scan demonstrated acute calculus cholecystitis and an incidental CAP. This was successfully treated with an emergency l

Acute emphysematous cholecystitis due to clostridium perfringens: a rare entity requiring aggressive and early treatment.

Acute emphysematous cholecystitis is a rare entity (1% of acute cholecystitis) characterized by the presence of gas in the gallbladder lumen, gallbladder wall or even in the bile duct (pneumobilia). It is associated with arteriosclerosis, embolic events, and diabetes mellitus and as in other similar pathologies such as emphysematous gastritis or emphysematous pancreatitis the most frequently isolated microorganisms are Streptococcus pyogenes, Escherichia coli and Staphylococcus aureus, although others such as Proteus vulgaris, Pseudomonas aeruginosa, Clostridium perfringens and Klebsiella pneu
