[Muscular dystrophy with central nervous system involvement. Apropos of 2 Spanish cases].

Two spanish male brothers with weakness and muscular dystrophy and affection of the CNS are presented. Muscular disturbances were noticeable from birth and, although generalized, they affected more severely proximal muscles. Both children presented joint contractures from an early stage. None of the patients got to walk and to stand. Muscular serum enzymes were slightly elevated. EMG and muscular histology were compatible with conventional pathology of PMD. Other features of severe alteration of CNS were observed in both patients, being the most significant lack of sphincter control at 13 and

A new simplified technique for pediatric anorectal manometry.

Using a new simple technique that is well tolerated and rapid for assessing anal sphincter functions in pediatric patients with fecal incontinence and constipation, 86 patients with varying degrees of anorectal dysfunction were tested. A microtip pressure transducer was used for recording anal sphincter responses to rectal balloon distension. Results were obtained within 15 to 30 minutes. In 17 patients, the internal anal sphincter showed no response or positive spike to balloon distension, consistent with the manometric diagnosis of aganglionic megacolon. These findings were confirmed by abse

Thyroid hormone receptor beta gene mutation (P453A) in a family producing resistance to thyroid hormone.

BACKGROUND: Resistance to thyroid hormone (RTH) is a dominantly inherited syndrome characterized by decreased responsiveness of target tissues to thyroid hormone. Two members of a Turkish family, a mother and son, had thyroid function tests suggestive of resistance to thyroid hormone (RTH). METHODS: The clinical presentation was, however, different. The mother (proposita) had palpitation, weakness, tiredness, nervousness, dry mouth and was misdiagnosed as having multinodular toxic goiter which was treated with antithyroid drugs and partial thyroidectomy. Her younger son had attention deficit

[Urinary incontinence in neurogenic defects and urogenital anomalies in childhood].

Dysraphic defects may cause neurogenic incontinence in childhood. Constipation and encopresis are often associated. Depending on the involved segment of the spinal cord hyperreflexia or atonia of the detrusor is observed. Similar findings, without anatomic correlation, can be seen in occult-neurogenic voiding dysfunctions. Therapeutic means aim at preservation of kidney function and the best possible continence. If the symptoms cannot be treated by anticholinergic drugs in a low-capacity, hypertonic bladder, augmentation by bowel segments or continent urinary diversion (e.g. Mainz I pouch) is

[Urodynamic investigations of bladder disturbances in imperforate anus and Hirschsprung's disease) (author's transl)].

104 children with imperforate anus and 68 children with megacolon congenitum were subjected to careful analysis in respect of postoperative incontinence of urine. This was observed in 20 patients with imperforate anus (19.2%) and 15 patients with Hirschsprung's disease (22%). In children with imperforate anus urine incontinence showed a slight tendency only to spontaneous cure. For example, 19.1% of the children with imperforate anus still showed signs of incontinence 11 years after the first operation. On the other hand, there was often a spontaneous cure of incontinence of urine in children

A case of late-onset anorexia nervosa with behavioural dysregulation in a 72-year-old woman, precipitated after olanzapine discontinuation.

This report details the case of a 72-year-old woman with a 5-year history of anorexia nervosa, whose presentation challenges the conventional presentation of anorexia nervosa. She was admitted under the Mental Health Act (MHA) of England and Wales because of escalating behavioural disturbance, including aggression and disinhibition, together with a long-standing pattern of restrictive eating and distorted body image. She presented with a markedly low body mass index, compulsive exercise and profound lack of insight, complicated by dilutional hyponatraemia, a less commonly reported but potentia

Cerebral folate deficiency presenting as adolescent catatonic schizophrenia: a case report.

Cerebral folate deficiency presents during infancy with irritability, deceleration of head growth, seizures, and progressive cognitive and motor impairment. Although low serum folate levels have been found in patients with schizophrenia, we describe the first case of cerebral folate deficiency presenting as catatonic schizophrenia. A 13-year-old previously healthy boy presented to our hospital with a 17-month history of schizophrenic symptoms with progressively worsening catatonia. On admission, he demonstrated near-complete mutism, frequent enuresis and encopresis, and severe psychomotor reta

[Disorders comorbid to attention deficit hyperactivity syndrome in children].

Attention deficit hyperactivity disorder (ADHD) is a common behavioral disorder found in children. Comorbid disorders, including oppositional/aggressive behaviors and emotional disorders, may be present in the majority of children diagnosed with ADHD thus complicating an optimal choice of pharmacotherapy. We evaluated the comorbid disorders in an out-patient sample of 76 children with ADHD (59 boys and 17 girls) aged 5-11 years using the Russian version of Kiddie-Shedule for Affective Disorders and Schizophrenia - Present and Lifetime Version. In all cases, a diagnosis met ICD-10 diagnostic cr

[АNORECTAL DYSFUNCTION, OCCURRING AFTER SURGICAL TREATMENT OF HIRSCHPRUNG’S DISEASE AND ITS CORRECTION].

Results of examination and treatment of 12 children, operated on in various clinics for Hirschprung’s disease, in whom chronic constipation and other signs of anorectal dysfunction have occurred, were analyzed. In 8 patients Duhamel-Bairov operation was conducted, in 2 – Soave-Lenyushkin operation, and in 2 – Soave-Boley operation. The main cause of postoperative chronic constipation occurrence after Duhamel operation is the colorectal septum preservation. The only one radical method of treatment of this complication is a transanal removal of colorectal septum. The cause of anastomotic narrowi

A case of metastatic spinal Ewing's sarcoma misdiagnosed as brucellosis and transverse myelitis.

An 11-year-old girl was admitted with back pain for 2 months, inability to walk for 15 days, and enuresis and encopresis for 2 days. She had been hospitalized with the diagnosis of brucellosis in another hospital. At presentation, she had paraplegia, sphincter dysfunction, and bilateral sensory loss below the T6 level, and was initially diagnosed with transverse myelitis caused by brucellosis. On the third day of hospitalization, however, agglutination test for brucella was negative, but it was positive for Salmonella. Therefore, transverse myelitis was considered to be due to salmonellosis. T

Pica: symptom or eating disorder? A historical assessment.

In DSM-III-R, pica, with anorexia nervosa, bulimia nervosa and rumination disorder of infancy, is accorded the status of a separate eating disorder. However, in the Draft of ICD-10, only anorexia nervosa and bulimia nervosa are listed under eating disorders. Pica in children, and feeding disorder in infancy and childhood, are incorporated with enuresis, encopresis, and feeding, movement and speech disorders in a separate "heterogeneous group of disorders". Extensive research on the history and terminology of eating disorders from the 16th to the 20th century suggests that, historically, pica w

Myxopapillary ependymoma in the third ventricle area and sacral canal: dropped or retrograde metastasis?

Myxopapillary ependymoma (MPE) is a rare type of central nervous system neoplasm mostly located in the cauda equina and filum terminale regions. A previously healthy 22-year-old Chinese man presented with the first case of MPE in the third ventricle area and sacral canal initially manifesting as spinal cord compression. The patient was admitted with pain in the right lower extremity for 5 months and encopresis for 3 months. Magnetic resonance imaging of the lumbar spine revealed an intradural lesion at the S2 level. The patient accordingly underwent lumbar laminectomy surgery and gross total r

Preschool behavior problems and subsequent risk of injury.

The hospital records of 951 children from a previously established birth cohort for which behavioral and extensive background information was available were checked for 3 years following the fifth birthday of the youngest child. The aim was to determine whether children with certain specific behavior disorders had a higher rate of injury than those without. Although the predicted relationships between overactive behavior, decreased concentration, and injury rate were not found, the hypothesis of an increased risk of injury for boys and for children with discipline problems was confirmed. Also,

Selective mutism: a review of etiology, comorbidities, and treatment.

Selective mutism is a rare and multidimensional childhood disorder that typically affects children entering school age. It is characterized by the persistent failure to speak in select social settings despite possessing the ability to speak and speak comfortably in more familiar settings. Many theories attempt to explain the etiology of selective mutism.Comorbidities and treatment. Selective mutism can present a variety of comorbidities including enuresis, encopresis, obsessive-compulsive disorder, depression, premorbid speech and language abnormalities, developmental delay, and Asperger's dis

Proximal segment histology of resected bowel in Hirschsprung's disease predicts postoperative bowel function.

The authors present an overview of 101 patients operated for Hirschsprung's disease by Rehbein procedure. Special attention is directed to the histomorphological findings in the proximal segment of the resected bowel specimens. A strong link was confirmed between these specific features and postoperative bowel function. A histologically regular proximal bowel segment generally predicted good postoperative bowel function with a constipation rate of 10.5%. In patients with intestinal neuronal dysplasia (IND) of the proximal segment the overall clinical result remained unchanged, although the con
