OBJECTIVE: Analyze the incidence and findings of endoleak after thoracic endovascular aortic repair by using CT angiography
MATERIAL AND METHOD: Between August 2006 and December 2008, 68 patients diagnosed with thoracic aortic aneurysm underwent thoracic endovascular aortic repair and were included in the present study. The patients were 47 men and 21 women, with a mean age of 69 +/- 9.4 years old. Thoraco-abdominal CT angiographic images (64-slice MDCT) after operation of 68 patients were retrospectively reviewed to evaluate incidence of endoleak and classify findings of endoleak.

BACKGROUND: Endoleaks represent the most common complication after EVAR. Some types are associated with ongoing risk of aneurysm rupture and necessitate long-term surveillance and secondary interventions.
PURPOSE: This document, as with all CIRSE Standards of Practice documents, will recommend a reasonable approach to best practices of managing endoleaks. This will include imaging diagnosis, surveillance, indications for intervention, endovascular treatments and their outcomes. Our purpose is to provide recommendations based on up-to-date evidence, updating the guidelines previously published

BACKGROUND: Unlike periprocedural Type 1A endoleaks, late appearing proximal endoleaks have been poorly described.
METHODS: We studied all elective EVAR from 2010 -2018 in a single institution. Late endoleaks were defined as those appearing after 1 year. We used Cox regression to study factors associated with late Type 1A endoleaks and survival.
RESULTS: Of 477 EVAR during the study period, 411 (86%) had follow-up imaging, revealing 24 Type 1A endoleaks; 4 early and 20 late. Freedom from Type 1A endoleaks was 99%, 92-81% at 1, 5 and 8 years with a median time to occurrence of 2.5 years (.01-

Abdominal aortic aneurysm (AAA) is a disease of the elderly which may result in aneurysm rupture if not treated in a timely manner. The incidence of AAA has increased in part due to patient and physician education, ultrasound screening, and liberal use of computed tomography imaging in conjunction with an aging population. Endovascular aneurysm repair has become the preferred treatment for surgeons and interventionalists. When endografts are placed outside of device-specific instructions for use, the risk of endoleak development is significantly increased. Open surgical repair of Type Ia endol

PURPOSE: The majority of type 2 endoleaks (T2EL) are identified on computed tomography angiography (CTA) both on arterial and venous phase. There is a subset of T2EL that are demonstrated only on venous phase CTA. This study was done to report the outcomes of T2EL detected only on venous phase CTA.
MATERIALS AND METHODS: A total of 261 consecutive T2EL treated via embolization were reviewed for the presence of endoleak demonstrated only on venous phase CTA. A group of 16 patients (12 men, 4 women; mean age, 80.1 years) was identified who had pre-embolization venous phase T2EL. Patients were e

OBJECTIVE: Endovascular aneurysm repair (EVAR) is considered the standard therapy for most patients with abdominal aortic aneurysm (AAA). Endoleak is a well-known EVAR-related complication that requires long-term follow-up. However, patient follow-up is often challenging outside clinical trials. We sought to evaluate the incidence and the effect of delayed endoleaks in a Veterans Administration (VA) health care system where long-term follow-up is ensured.
METHODS: We retrospectively evaluated 213 consecutive patients who underwent EVAR at a referral Veterans Administration medical center. Age

BACKGROUND: Our prospective investigation aimed to determine and analyze the incidence and the determinants of endoleaks after thoracic stent graft.
METHODS: Sixty-one patients affected by thoracic aortic aneurysms were treated between January 2000 and March 2008. The study cohort contained 54 men, with a mean age of 63.6 +/- 17.9 years. The follow-up imaging protocol included chest radiographs and triple-phase computed tomographic angiography performed at 1, 4, and 12 postoperative months and annually thereafter.
RESULTS: Median follow-up was 32.4 months (range: 1-96 months). Endoleaks were

PURPOSE: Most clinical end points after endovascular aneurysm repair (EVAR) are endograft-specific, but type II endoleaks have been assumed to be an unavoidable consequence of the repair method and independent of the type of endograft used. Some recent data have suggested that the rate of type II endoleaks may also be graft-dependent. We reviewed a large clinical experience with six endografts to determine the behavior of type II endoleaks and whether they are graft-specific.
METHODS: All elective EVAR cases from five university institutions from 1996 to 2003 were retrospectively analyzed. En

Type II endoleak is the most common complication after endovascular abdominal aortic aneurysm repair. Type II endoleak with aneurysm sac growth is not benign for long-term outcomes of endovascular abdominal aortic aneurysm repair and should be treated to prevent secondary stent graft-related complications and aneurysm rupture. The current consensus is to consider treatments for persistent type II endoleak with significant aneurysm sac growth. For complete embolization of type II endoleak to obliterate the endoleak cavity with the elimination of all supplying arteries, it is necessary to select

PURPOSE: To analyze the effectiveness of type II endoleaks (T2E) embolization using intra-operative contrast-enhanced ultrasound (CEUS).
METHODS: Consecutive patients treated for T2E underwent a standardized protocol with trans-arterial or trans-lumbar access, large volume embolization, onlay fusion, and intra-operative CEUS. Technical success was defined by exclusion of endoleak by CEUS.
RESULTS: Twenty-six patients (mean age 81 ± 11 years old; 89% male) were treated. The mean aneurysm sac enlargement was 11 ± 8 mm from T2E diagnosis. Embolization was performed using Onyx® 18 in all patient

OBJECTIVES: To evaluate the efficacy and clinical outcomes of accurate embolization of endoleaks after fenestrated thoracic endovascular aortic repair (F-TEVAR) for thoracic aortic dissections.
METHODS: Twenty patients with endoleaks (17 type I and 3 type II) after fenestrated thoracic endovascular aortic repair (F-TEVAR) were embolized using detachable and ordinary coils. We assessed the success rate and complications of the operation, and its effects, through clinical and CT follow-up.
RESULTS: The mean clinical follow-up duration was 25.68 ± 11.07 months (3-44 months). During follow-up, a

PURPOSE: While endovascular repair of aortic aneurysm (EVAR) has become the mainstay treatment for abdominal aortic aneurysm (AAA), it is not without its disadvantages. Feared complications include graft infections, fistulation and endoleak, the outcomes of which may be life limiting.
CASE REPORT: We present a case of a 57 year-old patient with human immunodeficiency virus (HIV) previously treated with EVAR for AAA complicated by endoleak post treatment. He developed an aorto-psoas abscess 2 years later which harboured Mycobacterium avium complex, and medical therapy was unsuccessful. He even

Arteriovenous fistula is a rare and often unrecognized complication of aneurysms, with a varied and frequently inconsistent presentation. We present the case of an ilio-iliac arteriovenous fistula formation in a 71-year-old man associated with a type III endoleak after endovascular iliac branch repair. Because of rapidly progressing congestive heart failure and hepatorenal syndrome, we performed urgent endovascular repair with successful endoleak exclusion. After the procedure, the patient demonstrated a remarkably rapid and complete recovery.

Type Ia endoleak after thoracic endovascular aortic repair is an ominous complication that requires an aggressive treatment. Total arch replacement with a distal suture taking the native distal aorta, the stent graft, and the arch graft, by allowing a perfect closure of the proximal endoleak, represents the most common treatment in this setting. Nevertheless, such intervention continues to carry significant mortality and morbidity that are mostly related to prolonged extracorporeal circulation time and hypothermic circulatory arrest. Here, we present a technique for total arch replacement that

INTRODUCTION: Type III endoleaks are a rare but potentially life-threatening complication post endovascular aortic aneurysm repair (EVAR).
CASE REPORT: A 91-year-old Chinese female, presented to our accident and emergency department for severe back and abdominal pain. She had previously undergone an EVAR procedure twenty years ago for a 6.5 cm diameter infra-renal abdominal aortic aneurysm. A CT aortogram revealed a type III endoleak, with the contralateral limb found to be disconnected from the main graft body. She was successfully treated by relining the graft using an endovascular techniqu

Type II endoleaks are a common complication after endovascular abdominal aortic aneurysm repair, with transarterial embolization using synthetic surgical glue being an established treatment option. We report a case of paraplegia due to spinal cord ischemia after lumbar arteries embolization by Glubran-lipiodol glue for a type II endoleak. Special attention must be given by interventional specialists when applying surgical diluted glues for the treatment of type II endoleaks to avoid distal embolization and subsequent spinal cord ischemia.

The Nellix endovascular aneurysm sealing system is a novel alternative to conventional endovascular aneurysm repair for aortic aneurysm management using paired balloon-expandable endografts supported by polymer-filled endobags to achieve sealing and anatomic fixation. Part of the promise of endovascular aneurysm sealing is increased resistance to lateral and longitudinal forces and a potential for reduced rates of device-related failures, particularly endoleaks. Initial efficacy data on this device are encouraging, but our knowledge of its associated complications and their management is limit

Endoleak is a common complication after thoracic endovascular aortic repair (TEVAR) or endovascular aortic repair (EVAR), occurring in up to 30% of cases, and therefore lifelong imaging surveillance is important. Endoleak is defined as the persistence of blood flow outside the lumen of the stent graft, but within the aneurysm, which can be classified into five categories. Typical features of the different types of endoleak are discussed as well as the imaging modalities to detect and classify an endoleak and the endovascular treatment options. Multidetector computed tomography angiography is t
