[Clinical studies on the furosemide VOR test].

In order to detect vestibular endolymphatic hydrops, the directional preponderance of the VOR gain (VOR-DP%) during sinusoidal rotation (freq. 0.1Hz; Amp. 120deg) was calculated and compared between pre- and post-intravenous injections of furosemide (20mg). Abnormal changes in VOR-DP% (> = 10%) from the pre-injection level were referred to as positive in this furosemide VOR test (FVOR). The following results were obtained from this study. (1) Positive responses were obtained in 31 of 61 (51%) patients with definite Meniere's disease, in 11 of 21 (52%) patients with delayed endolymphatic hydrop

Streptomycin perfusion of the labyrinth.

The combination of fluctuant hearing loss, fullness, tinnitus and dizzy spells we call Meniere's disease is thought to be caused by endolymphatic hydrops. Most patients with the clinical picture of Meniere's disease do have endolymphatic hydrops but some patients with endolymphatic hydrops do not have the clinical picture of Meniere's disease. It would appear there is an, as yet unknown, immune-mediated, cause for Meniere's disease, in addition to endolymphatic hydrops, and this immune-mediated cause may aggravate those ears with endolymphatic hydrops. While medical treatment with a low-salt d

Variations in surgical anatomy of the endolymphatic sac.

Twenty-nine specimens of the extraosseous portion of the human endolymphatic sac (ES) were serially sectioned longitudinally. The length and width of the extraosseous ES were measured and the surface area was calculated. As the specimens included the sigmoid sinus (SS), the relationship between the ES and the SS was analyzed. The extraosseous ES varied considerably in size. The lumen either consisted of a single tube or was subdivided into several tubules. The distal part of the ES overlapped the SS in one third of the specimens. The results indicate that a minute extraosseous ES could explain

Menière's disease.

Menière's disease is one of the most common etiologies for dizziness in the United States. The mainstay of therapy for Menière's is medical therapy. Symptomatic treatment is employed, but long-term prophylaxis is emphasized, with salt restriction and diuretic administration as needed. A prophylactic approach can prevent many attacks and ameliorate those that do occur. Patients with moderate to severe symptoms who fail medical therapy may be candidates for endolymphatic sac surgery, since this procedure is successful in a majority of patients and has low morbidity. Those patients in whom an end

The Mondini dysplasia - a clinical study.

The authors describe a clinical approach to the diagnosis and treatment of sensory-neural hearing loss associated to the Mondini dysplasia. The first element of diagnostic suspicion was represented either by a history of fluctuant and progressive hearing loss or by electrocochleography patterns showing multiple-peaked acoustic nerve action potentials similar to those observed in Ménière's disease. Confirmation of the diagnosis was obtained by temporal bone polytomography. An endolymphatic subarachnoid shunt was performed in 22 ears of 18 patients. The results, although preliminary, suggest tha

Factors influencing results with streptomycin perfusion of the labyrinth.

Streptomycin perfusion of the labyrinth is the logical choice of treatment for the third stage of Meniere's disease with intractable vertigo. The results of streptomycin perfusion of the labyrinth are comparable to those of other treatments, including endolymphatic shunt and vestibular nerve section. Refinements must be made in the process of selecting candidates for the operation. To study the factors influencing the results of streptomycin perfusion of the labyrinth, 144 patients who had streptomycin perfusion of the labyrinth and were followed for 1 year were studied using the 1972 AAOO cla

[Menière's disease : evidence and controversies].

Menière's disease is recognized as the idiopathic form of recurrent inner ear disease with the trias of hearing loss, tinnitus and vertigo with aural fullness and Menière's syndrome as the non-idiopathic form. Subentities with unknown pathogenesis are Lermoyez' s syndrome and Tumarkin crises. A common pathogenetic factor is the disturbance of endolymphatic and perilymphatic osmotic and hydrostatic pressure due to defined etiologies or to idiopathic attacks. Etiologies of Menière's syndrome can be pathologic middle ear pressure, anomalies of the vestibular and cochlear aqueducts, round window t

Effects of endolymphatic-perilymphatic fistula on endolymphatic hydrops in guinea pig.

The endolymphatic-perilymphatic shunt operation between the scala media and scala tympani was performed in 22 guinea pigs with endolymphatic hydrops induced by the silver nitrate injection method 2 months before the operation. Two (n = 10), 7 (n = 6), and 28 (n = 6) days after the operation, the condition of the fistula and the inner ear pathology were studied histopathologically. In half of the animals, the fistula was open, whereas in the other half, the fistula was closed. In the group examined after 2 postoperative days, the animals with open fistulas showed a slight collapse of Reissner's

Endolymphatic sac surgery for Ménière's disease: a systematic review and meta-analysis.

OBJECTIVE: To analyze current endolymphatic surgery techniques and quantify their efficacy in controlling vertigo and maintaining hearing in the short and long term. DATA SOURCES: A comprehensive literature search using the PubMed-NCBI database from 1970 to 2013. STUDY SELECTION: Articles on sac decompression and mastoid shunt (with and without silastic) were included. Included studies had to report data using the 1985 or 1995 American Academy of Otolaryngology-Head and Neck Foundation (AAO-HNS) guidelines, describe surgical technique in detail, include a minimum of 10 patients, and have min

Histopathologic findings in surgical specimens of endolymphatic sac in Meniere's disease.

Two types of abnormality in the endolymphatic sac acquired at shunt surgery are presented. One was deep brown pigmentation in the subepithelial connective tissue. The pigment did not react to Prussian blue, thus ruling out hemosiderin. The other, in seven out of eight specimens, was the perisaccular fibrosis. The fibrotic foci in four cases were assumed, from the findings under ordinary illumination and polarized light microscopy, to be reorganized scar tissue.

Revision endolymphatic sac surgery for recurrent Meniére's disease.

A total of 44 cases of revision endolymphatic sac surgery (RESS) were studied. Overall, results of RESS were comparable to the primary endolymphatic sac surgery (PESS). Furthermore, early failure rate (around 6 months after PESS) is significantly higher compared to late failure (approximately 2 years after PESS). In addition, important revision findings which showed saccular fibrosis and new bone growth in the long-term make possible the theory that there is a difference in pathological locus between early and late failures of PESS. Bulky implant devices (such as inner ear valve) had the effec

Meniere's disease.

Meniere's disease is an idiopathic disorder of the inner ear characterized by the syndrome of endolymphatic hydrops, episodic vertigo, fluctuating hearing loss, tinnitus, and aural fullness. People with this disorder may be severely disabled. Medical therapy exists in the form of diuretics and dietary restriction of salt to minimize the fluid pressure in the labyrinth and cochlea. Treatment of allergies with desensitization and steroids has also shown to be effective in selected patients. Surgical therapies exist in two categories, conservative and ablative. [Results of surgery of the endolymphatic sac].

Surgery of the endolymphatic sac is controversial. Some consider it a placebo and others consider it the surgical treatment of choice in Ménière's disease. We studied the medical records of 87 patients who underwent surgery between 1978 and 1996. Simple decompression was practiced in 89% and a House shunt in 27.6%. The 1-year results were improvement or recovery from vertigo in 65.4%, no improvement in 25%, and reoperation in 9.5%. Tinnitus improved in only 11.9% and hearing loss improved in 9.5% and worsened in 19%. We reviewed the cases of 50 patients with a 5-year follow-up(60%). Vertigo im

Meniere's disease at the University of Iowa: 1973 to 1980.

The goals of treatment in Meniere's disease are hearing preservation and control of vertigo. Controversy abounds with regard to the most expeditious and efficacious means of attaining these goals. Low-salt diet, diuretic therapy, endolymphatic-mastoid shunt, and middle cranial fossa vestibular neurectomy are all modes of treatment employed for Meniere's disease at the University of Iowa. This review of our experience from 1973 to 1980 summarizes the efficacy of each therapeutic option.

Shunt surgery in Meniere's disease.

This is a clinical study of the results of endolymphatic sac shunt surgery for Meniere's disease in 28 patients. Postoperative audiometric results and clinical evaluation of vertigo and tinnitus were analyzed to determine the effectiveness of this form of treatment. Although this is a small study and the time interval since operation is of only moderate duration (6 to 46 months), the results are encouraging: 79% of the patients showed improvement or stabilization of hearing as well as control of vertigo.

Retrolabyrinthine section of the vestibular nerve.

When symptoms of dizziness and episodic vertigo cannot be controlled through medical management or drainage procedures such as endolymphatic subarachnoid shunt operations, selective vestibular nerve section may be necessary. In the 1920s eighth cranial nerve sections were performed by neurosurgeons through the suboccipital approach but were frequently associated with hearing loss and facial paralysis. The middle fossa approach has been popularized by Dr. William House and others as a method of selectively sectioning the vestibular nerve and preserving facial and cochlear function. More recentl
