Metastatic Serratia marcescens endophthalmitis.

Metastatic endophthalmitis in a ten-day-old baby who underwent colostomy for an unperforated anus and developed septicemia to Serratia marcescens is described. Although rapid isolation of the agent enabled efficacious specific antibiotic treatment and systemic eradication of the infective agent, the ocular condition continued to deteriorate. Because of the total loss of visual functions and the fear of possible development of sympathetic ophthalmia, enucleation rather than vitrectomy or evisceration was performed. Histopathologic examination of the globe revealed massive infiltrates within the

Endogenous Actinobacillus actinomycetemcomitans endophthalmitis.

A 56-year-old immunocompromised man had an acute decrease in vision that led to the detection of Actinobacillus actinomycetemcomitans endocarditis complicated by septic emboli to his eye and other organs. This case demonstrates that bacterial sepsis can mimic a fungal infection. To our knowledge, this is the first report of endophthalmitis resulting from infection with A actinomycetemcomitans.

Endophthalmitis after strabismus surgery.

We treated two cases of endophthalmitis caused by strabismus surgery. In one patient, a scleral perforation was recognized during surgery and treated with cryopexy. In the other, a scleral perforation was suspected but never confirmed. Despite intensive antibiotic and corticosteroid therapy, visual acuity decreased to light perception in Case 1 and to counting fingers in Case 2. The second eye was enucleated eight years later after it had become blind and painful.

Management and outcomes of postoperative endophthalmitis since the endophthalmitis vitrectomy study: the Endophthalmitis Population Study of Western Australia (EPSWA)'s fifth report.

PURPOSE: To examine if changes in the diagnosis and management of postoperative endophthalmitis have occurred since 1995, and to identify factors that might predict final visual outcome. DESIGN: Retrospective, population-based, noncomparative, consecutive case series. PARTICIPANTS: Patients with clinically diagnosed endophthalmitis after cataract surgery and lens-related surgery in Western Australia from 1980 to 2000. METHODS: Endophthalmitis cases were identified using record linkage and cross-referencing with the surgical logbooks of vitreoretinal surgeons before validation by medical rec

Nationwide incidence of endophthalmitis among the general population and the subjects at risk of endophthalmitis in Italy.

PURPOSE: To investigate the incidence of endophthalmitis in Italy. METHODS: The Ministry of Health Hospital Admissions online database was searched to identify cases of endophthalmitis and cases with risk factors for endophthalmitis (corneal ulcer, open wound of the eyeball, and intraocular surgery including retinal, iris, and lens procedures with or without vitrectomy). Annual Incidence rates (overall, and age- and gender-specific) were computed in the general population and in the population with one or more of the risk factors for endophthalmitis. RESULTS: The overall annual incidence of

Successful treatment of Fusarium endophthalmitis with voriconazole and Aspergillus endophthalmitis with voriconazole plus caspofungin.

PURPOSE: To report successful treatment of exogenous Fusarium and Aspergillus endophthalmitis with new antifungal agents. DESIGN: Interventional case report. METHODS: Treatment of two cases is reviewed. RESULTS: A 64-year-old man developed post-cataract Fusarium moniliforme endophthalmitis. Infection persisted despite removal of the intraocular lens, three vitrectomies, and five intravitreal injections of amphotericin. Inflammation resolved and vision improved from 20/80 to 20/40 on 6 months of oral voriconazole. A 55-year-old man developed post-cataract intraocular inflammation. After thre

When to declare an endophthalmitis outbreak: Poisson model of postcataract surgery endophthalmitis outbreak projections.

PURPOSE: To create an accessible, online tool that determines the likelihood of an endophthalmitis outbreak. SETTING: Surgical centers that perform cataract surgeries. DESIGN: Mathematical modelling study. METHODS: The global risk of acute postoperative endophthalmitis (POE), defined as occurring within 6 weeks after standalone cataract surgery, was determined from a systematic review. An online tool was then created using the Poisson model to determine the likelihood of a POE outbreak. The model used the number of cataract surgeries performed, time, and the number of POE cases observed. Si

Clinical Features Associated With Return to Pre-endophthalmitis Visual Acuity Following Presumed Infectious Endophthalmitis.

BACKGROUND AND OBJECTIVE: This study aimed to investigate clinical characteristics associated with return to pre-endophthalmitis visual acuity (VA) following treatment for presumed infectious endophthalmitis. PATIENTS AND METHODS: This was a retrospective study of 74 eyes with endophthalmitis and VA recorded ≤ 12 weeks before presentation and 6 months after presentation between 2009 and 2018. RESULTS: Thirty-six of 74 eyes (49%) returned to preendophthalmitis VA. Eyes with a positive intraocular fluid culture had 71.5% lower odds of returning to pre-endophthalmitis VA. Nine of 10 eyes (90%)

Eye-banking risk factors for fungal endophthalmitis compared with bacterial endophthalmitis after corneal transplantation.

PURPOSE: To track the relative frequency and explore possible risk factors of fungal compared with bacterial endophthalmitis after corneal transplantation. DESIGN: Case-comparison study nested in a surveillance registry. METHODS: We examined, among recipients who underwent surgery from January 1991 through December 2003, demographic and eye-banking characteristics of postkeratoplasty endophthalmitis cases that were reported to the Eye Bank Association of America by US eye banks. Potential predictors of fungal compared with bacterial endophthalmitis were examined using multiple logistic regre

Modern Endophthalmitis Control: The Complete Early Vitrectomy for Endophthalmitis (CEVE) Protocol and Surgical Prophylaxis.

This article presents an overview of endophthalmitis, a severe intraocular inflammatory condition that can develop following surgery, intravitreal injections (IVIs), or ocular trauma. Endogenous cases may also arise, particularly in patients with compromised immune systems. A broad spectrum of pathogens, including bacteria, fungi, and viruses, can be involved, with some displaying high virulence. Given the rapid progression and potential for vision-threatening complications or systemic involvement such as sepsis, immediate and appropriate intervention is essential. Surgical management, especia

Review of endogenous endophthalmitis caused by Salmonella species including delayed onset Salmonella typhi endophthalmitis.

Endogenous endophthalmitis is a rare but serious condition that occurs when bacteria cross the blood-ocular barrier and multiply within the eye. We provide a literature review of Salmonella endogenous endophthalmitis by reviewing eight reported cases and integrating this with our experience of one case in which a four-year-old child presented with acute endophthalmitis of four days duration. He had suffered typhoid fever two months back. Vitreous biopsy revealed Salmonella typhi. Despite aggressive surgical and medical treatment, the eye developed retinal necrosis, detachment, and phthisis. Bl

Literature- and Experience-Based Consensus for Acute Post-operative Endophthalmitis and Endogenous Endophthalmitis in Taiwan.

Clinical practices on acute post-operative and endogenous endophthalmitis (EnE) are highly variable among clinicians due to a lack of up-to-date, high-quality evidential support. An expert consensus is thus much needed. A panel consisting of ten retinal specialists in Taiwan was organized. They evaluated relevant literature and developed key questions regarding acute post-operative and EnE that are cardinal for practice but yet to have conclusive evidence. The panel then attempted to reach consensus on all the key questions accordingly. There were eight key questions proposed and their respect

Stenotrophomonas maltophilia endophthalmitis following keratoplasty.

Endophthalmitis is among the most sight-threatening infections in ophthalmology practice. Many microorganisms causing endophthalmitis have been reported. Stenotrophomonas maltophilia is among the rare causes of endophthalmitis and has been reported after cataract surgery, intravitreal injections and ocular trauma. We report a case of S. maltophilia endophthalmitis after keratoplasty, which is a rare entity, in a 63-year-old female patient.

[Acanthamoeba endophthalmitis-a case report].

Diagnosis of acanthamoeba keratitis is often a clinical challenge. In most cases it is primarily confused with herpes keratitis. A coinfection with bacteria or fungi can also lead to a delayed diagnosis and initiation of treatment. A few cases reported the detection of acanthamoeba in the anterior chamber. In the case of endophthalmitis without the detection of bacteria or fungi acanthamoeba should therefore always be considered as a potential differential diagnosis. The local and systemic administration of voriconazole can be successfully used to treat acanthamoeba endophthalmitis.

Endophthalmitis Prophylaxis for Cataract Surgery.

Endophthalmitis after cataract surgery is a rare but potentially devastating complication. There is great variability in endophthalmitis prophylaxis practice patterns worldwide. Treatment varies globally and is based on the microbiological profile and availability of formulations. Periocular povidone-iodine antisepsis is universally adopted and considered the standard of care in most practices. Perioperative topical antibiotics are also very popular despite the lack of level 1 evidence confirming efficacy. Based on growing observational evidence, routine intracameral antibiotic prophylaxis is

MR imaging findings of endophthalmitis.

Endophthalmitis is a sight-threatening ophthalmologic emergency. The clinical diagnosis is often challenging, and delayed diagnosis may exacerbate the poor visual prognosis. B-scan ultrasonography or spectral domain optical coherence tomography are imaging aids at the clinician's office. Cross-sectional imaging such as CT and particularly MRI can also help in the assessment of disease extent or complications. MR imaging findings are rarely described in the literature. Here, we discuss the spectrum of imaging findings of endophthalmitis and correlate them with key anatomic and pathophysiologic
