Molecular Mechanisms of Eosinophilic Esophagitis.

Food hypersensitivity is a group of diseases arising from a specific immune response that reproduces on exposure to a given food. The current understanding of molecular mechanisms and immunopathology of non-IgE-mediated/mixed food hypersensitivity, e.g., eosinophilic esophagitis, contains many gaps in knowledge. This review aims to provide a modern classification and identify the primary diseases of non-IgE-mediated/mixed food hypersensitivity reactions, delineate the distinctive molecular features, and discuss recent findings in the immunopathology of eosinophilic esophagitis that may become

Endoscopy in Pediatric Eosinophilic Esophagitis.

Endoscopy and mucosal biopsies are essential to the diagnosis of EoE. Together they either confirm or exclude mucosal eosinophilia and provide a visual inspection of the esophagus that may be consistent with EoE or suggest other underlying etiologies. Endoscopy also plays an important therapeutic role in the management of EoE including the assessment of treatment response and treatment of associated complications including esophageal stricture and food impaction. Assessment of treatment response largely depends on endoscopy and mucosal biopsies although less invasive strategies may eventually

Emphysematous esophagitis with gastric perforation.

Emphysematous esophagitis is an extremely rare disease and there are very few previous reports in the literature. We report a case of emphysematous esophagitis and gastritis with complete affectation of the gastric and esophageal wall at diagnosis. Two surgical interventions were performed due to gastric perforation that was treated in both cases with primary closure. The post-operative recovery was satisfactory. Despite the large emphysematous esophago-gastritis affectation at diagnosis and the presence of gastric perforation, it is safe to perform the same management principles as with emphy

Epithelial origin of eosinophilic esophagitis.

Eosinophilic esophagitis (EoE) is a chronic, allergen-driven inflammatory disease of the esophagus characterized predominantly by eosinophilic inflammation, leading to esophageal dysfunction. Converging data have placed the esophageal epithelium at the center of disease pathogenesis. In particular, the main EoE disease susceptibility loci at 2p23 and 5p22 encode for gene products that are produced by the esophageal epithelium: the intracellular protease calpain 14 and thymic stromal lymphopoietin, respectively. Furthermore, genetic and functional data establish a primary role for impaired epit

Eosinophilic Esophagitis: Incidence and Prevalence.

Eosinophilic esophagitis (EoE) is a rapidly emerging allergy-mediated condition encountered frequently in clinical practice. It presents with failure to thrive, nausea, and vomiting in children and is a common reason for dysphagia and food impaction in adults. Several institution-based and population-based studies have reported the frequency of EoE over the past few years. The incidence and prevalence of EoE vary depending on the method of data collection. In population-based studies using national registries, the incidence appears to be increasing, currently estimated to be approximately 10 c

Eosinophilic Esophagitis: Pathophysiology and Definition.

Eosinophilic esophagitis is an adaptive immune response to patient-specific antigens, mostly foods. Eosinophilic esophagitis is not solely IgE-mediated and is likely characterized by Th2 lymphocytes with an impaired esophageal barrier function. The key cytokines and chemokines are thymic stromal lymphopoeitin, interleukin-13, CCL26/eotaxin-3, and transforming growth factor-β, all involved in eosinophil recruitment and remodeling. Chronic food dysphagia and food impactions, the feared late complications, are related in part to dense subepithelial fibrosis, likely induced by interleukin-13 and t

How to Approach Lymphocytic Esophagitis.

PURPOSE OF REVIEW: Lymphocytic esophagitis (LE) is an unusual esophageal condition defined by an increased number of lymphocytes in the esophageal epithelium. With few published studies of LE available, it is unclear whether LE is a truly distinct clinical entity or a histological manifestation of other known gastrointestinal disorders. This review summarizes recent studies of lymphocytic esophagitis. RECENT FINDINGS: Studies have suggested that LE may be related to eosinophilic esophagitis (EoE) or a manifestation of gastroesophageal reflux disease (GERD). There is an association between LE

[Endoscopic classification of reflux esophagitis].

It is important to accurately classify the severity of reflux esophagitis, because the clinical response to treatment is dependent on the severity of erosive lesions. In Japan we had used Savary-Miller classification for endoscopic grading. In this classification stage I to III encompass mucosal erosions of increasing extent, while stage IV includes all complications, which make it difficult to follow the evolution of the esophagitis and its response to treatment. A new endoscopic classification was proposed at the World Congress of Gastroenterology in Los Angeles in 1994. A mucosal break was

Management of refractory eosinophilic esophagitis.

BACKGROUND/AIMS: Whereas most children and adults respond to traditional eosinophilic esophagitis (EoE) treatments, such as exclusion of dietary allergens or the use of topical steroids, a small fraction may not. METHODS: Based on clinical experience and a review of the literature, the aim of this work is to provide practical advice for caring for 'refractory' patients with EoE. RESULTS: The approach to this type of patient continues to evolve and decision-making should consider a number of issues including the patient's age, lack of a complete understanding of the natural history of this di

Eosinophilic esophagitis: an Italian experience.

BACKGROUND: Eosinophilic esophagitis is an esophageal disorder characterized by esophageal and/or upper gastrointestinal tract symptoms, and by dense esophageal eosinophilia associated with a normal gastric and duodenal mucosa. Prevalently reported in children, eosinophilic esophagitis has recently been reported with increased frequency also in adults. AIMS: The purpose of this study was to report our experience with eosinophilic esophagitis in Italy, since there are only very few series of such patients in our country. PATIENTS AND METHODS: We retrospectively reviewed the histological data

Hiatus hernia and reflux esophagitis.

Usually, axial hiatus hernia is a common disorder that produces no symptoms and requires no treatment and in the few patients who do experience symptoms, simple nonoperative treatment measures suffice. The typical symptoms of reflux esophagitis are heartburn and nocturnal regurgitation. Most patients respond well to nonoperative treatment; any associated axial hiatus hernia is not the cause of the symptoms of reflux esophagitis and does not need treatment. In the very few patients who do need an operation, fundoplication conducted through the abdomen is recommended. In nonaxial hiatus hernia t

Bacterial esophagitis in immunocompromised patients.

We studied the clinical and pathologic features of bacterial esophagitis in three index cases identified by endoscopic biopsy and in 20 autopsy cases. Fourteen of the 23 patients had malignant hematologic conditions, aplastic anemia, or solid tumors; ten were profoundly neutropenic (white blood cell count, less than 100/mm3 [less than 0.1 X 10(9)/L]). The organisms involved in bacterial esophagitis were gram-positive cocci in 14, gram-negative bacilli in three, mixed gram-negative bacilli and gram-positive cocci in five, and gram-positive bacilli in one. Four patients had bacteremic bacterial

[Late sequelae of caustic esophagitis].

The caustic burns of the oesophagus still play a significant role in the framework of oesophageal conditions, both in terms of therapeutic approach in the acute phase, when high morbidity and mortality rates are recorded, and of long-term consequences. These are as follows: stenoses of varying extent, carcinoma, wall deformation, oesophago-bronchial fistulae and motor disturbances. This paper is concerned with the clinical, diagnostic and therapeutic implications of these late consequences.

Hiccups associated with reflux esophagitis.

A 67-yr-old man with protracted hiccups, as well as reflux esophagitis and a stricture, was found to have heartburn and water brash only during episodes of hiccups. Intraesophageal pH monitoring showed acid gastroesophageal reflux increased during these episodes and decreased with cessation of hiccups. After all attempts failed to relieve his hiccups, a Nissen fundoplication relieved heartburn, healed the esophagitis, and stopped excessive reflux of acid gastric juice, even though the hiccups persisted. These observations showed that reflux provoked by prolonged hiccups could either have adver
