Adult-onset cyclic esotropia and optic atrophy.

A 37-year-old man developed periodic esotropia with 4-day cycles. He was unique because of his age, negative family history for strabismus, bilateral optic atrophy, and absence of light perception in the deviating eye. Adult-onset cyclic esotropia in a blind eye is reported. This afferent defect may lead to secondary changes in the central nervous system, and fusional potential is not necessary for cyclic esotropia to occur.

Acute-onset esotropia: should we look inside?

The authors report a case of acute onset of comitant esotropia with diplopia in a 5-year-old boy with a diffuse pontine glioma. On first presentation, the angle of esodeviation was 30 prism diopters at distance fixation and 25 prism diopters at near fixation. Neurological symptoms appeared 10 weeks after the first visit.

Consecutive exotropia following surgically corrected cyclic esotropia.

OBJECTIVES: This study examined the rates of sexual and physical abuse and suicide attempts among male and female patients and focused on the associations between sexual and physical abuse and recent suicide attempts among men. METHODS: Data were examined for a cohort of patients aged 19 years and older who were seeking treatment for substance use disorders, other psychiatric disorders, or both from the Department of Veterans Affairs (VA) between July 1997 and September 1997. Almost all the patients in the sample (more than 99 percent) had a substance use disorder. Patients were interviewed w

Acute acquired comitant esotropia after orbital cellulitis.

Acute extraocular muscle restriction in children is a rare condition; orbital myositis is the most common cause. Most neuropathic or myopathic causes of strabismus present as an incomitant deviation. We report a case of orbital cellulitis preceding acute comitant esotropia in a patient showing no evidence of myositis or neurologic pathology.

Acute acquired comitant esotropia: a prospective study.

PURPOSE: To define the clinical characteristics of patients presenting with acute onset esotropia and features suggestive of possible underlying central nervous system pathology. To assess the prognosis for the return of binocular function and to consider the most appropriate management. METHODS: A prospective clinical study was carried out of all patients presenting to the department of paediatric ophthalmology at a university teaching hospital over the period January 1994 to April 1997. Each patient underwent a full ophthalmological examination (including assessment of sensory status). All

Esotropia following posterior superior alveolar nerve block.

Adequate use of local anesthetics is an important phase of modern dentistry. Regardless of the care used in administration of local anesthetics, unusual reactions can occur. A case is presented in which posterior superior alveolar administration of two percent Lidocaine 1/100,000 epinephrine resulted in medial rotation of the orbit (esotropia).

Large bimedial rectus recessions in congenital esotropia.

The success rate of large (6 and 7 mm) bimedial rectus recessions in 45 congenital esotropes with deviations of 50 prism dioptres or greater was found to be 91%. Judgment of final alignment was made six weeks postoperatively, with an average follow-up of 13 months. Large bimedial rectus recessions are an effective surgical treatment for congenital esotropia. This procedure does not significantly alter adduction, and leaves other muscles available should further surgery be necessary. These findings show that initial surgery on three or more muscles is unnecessary in congenital esotropia.

Esotropia Induced by Optical Correction in Myopic Patients.

PURPOSE: To describe a specific pattern of esotropia observed in myopic patients occurring exclusively while wearing refractive correction despite a normal accommodative convergence / accommodation (AC/A) ratio and to evaluate the efficacy of its management. METHODS: This is a retrospective study which included children and young adults with high myopia who presented with esotropia with spectacles or contact lenses correction only, and orthophoria or near orthophoria without optical correction. AC/A ratio was measured using gradient method. Ductions and versions were evaluated. The angles of

[The timing and challenges of early intervention for infantile esotropia].

Infantile esotropia is a common ophthalmic disease in children. A lot of clinical and basic research evidence suggests that early surgery enhances sensory and ocular motor development. However, the proper timing of surgery has been debated for decades. In addition, there is more likely instability of deviation in the preoperative evaluation of infants, and even if the patient achieved alignment after surgery, the defects in binocular vision may accompany for a lifetime. This article analyzes the difficulties and key points of early intervention for infantile esotropia, aiming to provide scient

Evaluation and Management of Divergence Insufficiency-Type Esotropia.

Divergence insufficiency-type ET is a common cause of distance diplopia in elderly adults. A recent prospective multicenter data collection study has provided additional guidance on management. Either base-out prism glasses or strabismus surgery were found to have high success rates, based on patient report of diplopia, and health-related quality-of-life domain scores. It was concluded that either prism or surgery were reasonable initial treatment strategies. Although allocation bias precluded formal comparison of prism versus surgery (evident in baseline differences between groups), there wer

Surgical management of a patient with exercise-induced esotropia.

Exercise-induced esotropia is a rare form of strabismus characterized by paroxysms of esotropia and diplopia only during exercise. This report describes a 15-year-old boy who presented with a 1-year history of exercise-induced esotropia. He has been asymptomatic after recession of a single medial rectus muscle. It is hypothesized that the esotropia was attributable to a malfunction in the muscle tension-control circuit and that altering the small signal gain with surgery resulted in improvement.

Where the wild things are: when esotropia misbehaves.

Esotropia is the most common form of strabismus seen in clinic. Accommodation is known to be an important precipitating factor, and its management is usually not complex. In this paper, I have reviewed a dozen circumstances in which the presentation was complicated. For some patients, the cause of fleeting symptoms was elusive and had to be determined through careful sensory and motor testing. In others, the presentation was indicative of serious underlying pathology that warranted further investigation. Lastly, restrictive conditions may be erroneously thought to be neurological, and some of

A data-driven approach to the management of accommodative esotropia.

PURPOSE: To determine if key findings on initial examination are predictive of ability to achieve and maintain functional alignment (residual esotropia of less than 8 prism diopters) with single-vision spectacles, bifocals, or surgery in children with accommodative esotropia. DESIGN: Retrospective, observational case series. METHODS: Institutional review of 68 consecutive accommodative esotropia patients. Exclusion criteria included previous spectacles use, other ocular pathologic features or surgery, or follow-up of less than 2 years. The main outcome measure was the ability to achieve func

Acute comitant esotropia in a child with a cerebellar tumor.

We report a case of acute comitant esotropia in a child with a cerebellar tumor. A 3-year-old boy was referred for management of a 9 month history of acute acquired comitant esotropia. On first presentation, the patient's angle of esodeviation was 50 prism-diopters (PD) at distance and near fixation without any lateral incomitance. The cycloplegic refraction revealed +0.75 diopters in both eyes. Very mild bilateral papilledema was found on the fundus examination, but the neurological examination did not reveal any other pathological findings. Brain MRI showed a 5 cm mass located in the midline
