Evidence from two rounds of Population HIV Impact Assessments of progress towards HIV epidemic control in adolescents aged 15-19 years in Eswatini, Lesotho, Malawi, and Zimbabwe: a cross-sectional study.

BACKGROUND: There are more than 1 million adolescents aged 15-19 years living with HIV globally. Using two rounds of the Population HIV Impact Assessments (PHIAs), we describe incidence, prevalence, and UNAIDS 95-95-95 testing, treatment, and viral load suppression (VLS) indicators for adolescents living with HIV in four countries in southern Africa over time. METHODS: In this multinational cross-sectional study, we analysed data from two PHIA rounds in Eswatini (2016-17 and 2021), Lesotho (2016-17 and 2020), Malawi (2015-16 and 2020-21), and Zimbabwe (2015-16 and 2020). PHIAs are surveys dis

Prevalence of nonsuppressed viral load and associated factors among HIV-positive adults receiving antiretroviral therapy in Eswatini, Lesotho, Malawi, Zambia and Zimbabwe (2015 to 2017): results from population-based nationally representative surveys.

INTRODUCTION: The global target for 2020 is that ≥90% of people living with HIV (PLHIV) receiving antiretroviral therapy (ART) will achieve viral load suppression (VLS). We examined VLS and its determinants among adults receiving ART for at least four months. METHODS: We analysed data from the population-based HIV impact assessment (PHIA) surveys in Eswatini, Lesotho, Malawi, Zambia and Zimbabwe (2015 to 2017). PHIA surveys are nationally representative, cross-sectional household surveys. Data collection included structured interviews, home-based HIV testing and laboratory testing. Blood samp

"If I wasn't a pastor, I would be telling them about condoms, contraceptives and PrEP"-conflicts, dilemmas and credibility: qualitative insights from religious leaders and the promotion of pre-exposure prophylaxis (PrEP) in Eswatini.

BACKGROUND: Religious leaders (RLs) form an influential group in Eswatini. This study explores their understanding and acceptance of pre-exposure prophylaxis (PrEP), with a view to improve PrEP service delivery for those who would benefit from PrEP the most. METHODS: We conducted online and in-person in-depth interviews with (n=26) purposefully selected RLs (n=12 female and n=14 male) from Eswatini. Data were managed using NVivo V.12 and coded inductively. We used thematic analysis to analyse our data and structure our findings across the individual, interpersonal, community and institutional

Investing in health workers: a retrospective cost analysis of a cohort of return-of-service bursary recipients in Southern Africa.

BACKGROUND: Return-of-service (RoS) schemes are investment strategies that governments use to increase the pool of health professionals through the issuing of bursaries and scholarships to health sciences students in return for service after graduation. Despite using these schemes for many years, Eswatini, South Africa, Botswana and Lesotho have not assessed the costs and return on investment of these schemes. This study aimed to assess the costs and relative rates of contract defaulting in these four Southern African countries. METHODS: A retrospective cohort study was carried out by reviewi

Low sensitivity of the fourth-generation antigen/antibody HIV rapid diagnostic test Determine™ HIV Early Detect for detection of acute HIV infection at the point of care in rural Eswatini: a diagnostic accuracy study.

INTRODUCTION: The diagnosis of acute HIV infection (AHI) is challenging in routine settings because it cannot be detected by routine third-generation antibody rapid diagnostic tests (RDTs). The current fourth-generation antibody/antigen RDT, Determine™ HIV Early Detect, has demonstrated high sensitivity in laboratory studies, but field evaluations at the point of care are lacking. We nested a diagnostic accuracy study within a larger study of the burden of sexually transmitted infections in rural Eswatini. METHODS: Adults were enrolled at six routine HIV testing sites (HTS) in the Shiselweni

A cross-sectional study evaluating the frequency of HIV drug resistance mutations among individuals diagnosed with HIV-1 in tenofovir disoproxil fumarate-based pre-exposure prophylaxis rollout programmes in Kenya, Zimbabwe, Eswatini and South Africa.

INTRODUCTION: The ongoing rollout of oral tenofovir-based pre-exposure prophylaxis (PrEP) has the potential to reduce HIV-1 incidence, but HIV drug resistance (HIVDR) in individuals who acquire HIV-1 on PrEP could threaten the treatment effectiveness of overlapping antiretrovirals (tenofovir/emtricitabine), contribute to development of resistance, and undermine HIV control efforts. Accordingly, the Global Evaluation of Microbicide Sensitivity (GEMS) project was established to monitor HIVDR in PrEP rollout programmes in Southern and Eastern Africa. METHODS: GEMS monitored resistance in >100,00

An assessment of video-observed treatment as an adherence support tool for patients with drug-resistant TB.

BACKGROUND: In 2017, WHO endorsed digital adherence technologies, including video-observed treatment (VOT), to support TB treatment adherence. In 2021, Eswatini implemented VOT to support adherence for drug-resistant TB (DR-TB) patients nationwide, although limited evidence exists regarding its feasibility and acceptability in low-resource settings. METHODS: We conducted 33 in-depth interviews with DR-TB patients and health care providers (HCPs), across all four regions of Eswatini to explore their experiences with VOT. RESULTS: The findings indicated broad acceptance of VOT among both HCPs

Six decades of malaria vector control in southern Africa: a review of the entomological evidence-base.

BACKGROUND: Countries in the southern Africa region have set targets for malaria elimination between 2020 and 2030. Malaria vector control is among the key strategies being implemented to achieve this goal. This paper critically reviews published entomological research over the past six decades in three frontline malaria elimination countries namely, Botswana Eswatini and Namibia, and three second-line malaria elimination countries including Mozambique, Zambia, and Zimbabwe. The objective of the review is to assess the current knowledge and highlight gaps that need further research attention t

Unawareness of HIV-Positive Status among Men in Seven Sub-Saharan African Countries: Age, Behavioral, and Structural Determinants.

Men in sub-Saharan Africa lag behind women in HIV testing and awareness. We pooled 2019-2023 Population-based HIV Impact Assessments from seven countries (Eswatini, Lesotho, Malawi, Mozambique, Tanzania, Uganda, Zimbabwe). Among 59,293 men, HIV prevalence was 5.9% (95% CI 5.7-6.2). Of 5,394 HIV-positive men, 22.1% (95% CI 20.0-24.3) were unaware of their status (8.4% in Eswatini; 31.5% in Mozambique), declining with age (41.0% at 15-29 vs. 11.5% at ≥ 50). Survey-weighted Poisson regression identified independent correlates: condom-free last sex (aPR 2.5), hazardous drinking (aPR 1.3), infreque

Population structure and genetic connectivity of Plasmodium falciparum in pre-elimination settings of Southern Africa.

To accelerate malaria elimination in the Southern African region by 2030, it is essential to prevent cross-border malaria transmission. However, countries within the region are highly interconnected due to human migration that aids in the movement of the parasite across geographical borders. It is therefore important to better understand Plasmodium falciparum transmission dynamics in the region, and identify major parasite source and sink populations, as well as cross-border blocks of high parasite connectivity. We performed a meta-analysis using collated parasite allelic data generated by mic

Some aspects of sexually transmitted disease in Swaziland.

A survey of sexually transmitted diseases was attempted in Swaziland. Time and the terms of reference limited its scope, but much interesting information was gathered. Neither the prevalence nor the relative frequency of the different diseases in the community could be ascertained, but there was evidence to support the local impression that the sexually transmitted diseases present an increasing problem in Swaziland. An important problem was that of case finding and two aspects of this are stressed: (1) A general lack of knowledge concerning the venereal diseases; (2) The large number of men i

Drivers of HIV-1 drug resistance to non-nucleoside reverse-transcriptase inhibitors (NNRTIs) in nine southern African countries: a modelling study.

INTRODUCTION: The rise of HIV-1 drug resistance to non-nucleoside reverse-transcriptase inhibitors (NNRTI) threatens antiretroviral therapy's long-term success (ART). NNRTIs will remain an essential drug for the management of HIV-1 due to safety concerns associated with integrase inhibitors. We fitted a dynamic transmission model to historical data from 2000 to 2018 in nine countries of southern Africa to understand the mechanisms that have shaped the HIV-1 epidemic and the rise of pretreatment NNRTI resistance. METHODS: We included data on HIV-1 prevalence, ART coverage, HIV-related mortalit

Screening and targeted sequencing of stool for microbiologic confirmation and drug resistance determination in paucibacillary tuberculosis.

In 2023, an estimated 10.8 million people developed tuberculosis, and 1.25 million people died from this disease, including 161,000 deaths in people with HIV (PWH) in whom tuberculosis remains the leading cause of death. Detecting Mycobacterium tuberculosis drug resistance remains a challenge among patients with paucibacillary tuberculosis; since there is such low bacterial load in their sputum it's unable to be detected via microscopy, there is also not enough bacteria for other sputum-based tests which could provide resistance testing. At an outpatient clinic in Eswatini from 2020-2023, stoo
