Midwifery students' reactions to ethical dilemmas encountered in outpatient clinics.

BACKGROUND: Midwives are required to make ethical decisions with the support of respective codes of professional ethics which provide a framework for decision making in clinical practice. While each midwife should be ethically aware and sensitive to the ever-changing issues within reproduction, few empirical studies have examined the views of student midwives in relation to reproductive ethical dilemmas. OBJECTIVE: The aim of this study was to explore midwifery students' reactions to a number of ethical dilemmas relating to women's experiences of reproductive decision making. DESIGN: A serie

Deep Brain Stimulation at End of Life: Clinical and Ethical Considerations.

Deep brain stimulation (DBS) is an implanted neurological device effective in treating motor symptoms of Parkinson disease (PD), such as tremor, rigidity, and bradykinesia. More than 150,000 patients worldwide have been implanted with DBS devices. Questions arise at the end of life concerning how to provide best care for patients with DBS, including its continued benefit or potential complications, yet, no published articles provide guidance for hospice providers regarding the management of DBS devices in end-of-life care. With contributions from hospice physicians, a neurosurgeon, and ethicis

A clinical and ethical review on late results and benefits after EVAR.

INTRODUCTION: The aim of this review is to assess if late mortality after endovascular repair (EVAR) of abdominal aortic aneurysms (AAA) is a real problem, and whether it could be an issue in the case of medical litigation. MATERIAL AND METHODS: A review of all English language literature was performed on PubMed web-site, looking for all papers reporting EVAR long-term mortality rate. EVAR performances were reviewed also from an ethical and medico-legal point of view, based on current Italian laws. RESULTS: Mono-centric studies, and international registers suggest that today EVAR offers simi

Ethical implications for clinical practice and future research in "at risk" individuals.

The last 15 years have witnessed a shift in schizophrenia research with increasing interest in earlier stages of illness with the hope of early intervention and ultimately prevention of psychotic illness. Large-scale longitudinal studies have identified clinical and biological risk factors associated with increased risk of psychotic conversion, which together with symptomatic and demographic risk factors may improve the power of prediction algorithms for psychotic transition. Despite these advances, 45-70% of at risk subjects in most samples do not convert to frank psychosis, but continue to f

Developmental, sexual and reproductive neuroendocrinology: historical, clinical and ethical considerations.

Different milestones have marked the development of the field of neuroendocrinology. This is an essay in which a new direction is proposed; one in which the field is marked more by a concern for ethical considerations than experimental procedures. Among the primary concerns is the need for a national registry for intersex and transsexual cases to follow how they are managed and to assess the long-term results. As in Europe, the data in the registries should be available to qualified and certified researchers and clinicians for analysis. A secondary focus is on the surgery often imposed in case

Ethical and clinical considerations in selecting patients who will receive clozapine.

In a large state hospital population, more treatment-resistant schizophrenic patients may be clinically eligible to receive clozapine than the hospital can afford to pay for, given the costs of the medication and its mandatory monitoring system. The authors review clinical criteria for selecting patients and discuss two ethical principles that might be useful: select patients on the principle of providing the greatest good for the greatest number of patients (based on expected outcomes such as likelihood of discharge and benefit to the milieu) and give priority to patients for whom the fewest

Clinical Research: A Review of Study Designs, Hypotheses, Errors, Sampling Types, Ethics, and Informed Consent.

Recently, we have been noticing an increase in the emergence and re-emergence of microbial infectious diseases. In the previous 100 years, there were several incidences of pandemics caused by different microbial species like the influenza virus, human immunodeficiency virus (HIV), dengue virus, severe acute respiratory syndrome Coronavirus (SARS-CoV), middle east respiratory syndrome coronavirus (MERS-CoV), and SARS-CoV-2 that were responsible for severe morbidity and mortality among humans. Moreover, non-communicable diseases, including malignancies, diabetes, heart, liver, kidney, and lung d

Monolithic moral frameworks: how are the ethics of palliative sedation discussed in the clinical literature?

A variety of moral frameworks can assist clinicians in making ethical decisions. In examining articles on palliative sedation and terminal extubation, we were struck that bioethical discussions uniformly appealed to principlism and especially to the rule of double effect. Other moral frameworks were rarely invoked, an observation consistent with Daniel Callahan's assertion that principlism has a "blocking effect" on broader ethical deliberation. We review here the principle of double effect as it applies to clinical acts that may hasten death, and present one radically different ethical formul

Sequential simulation-based clinical ethics education for clerkship students: A mixed-methods pre-post study.

INTRODUCTION: Clerkship students encounter ethically significant dilemmas but have limited opportunities to rehearse ethically grounded decision-making and difficult conversations in realistic clinical contexts. We evaluated an immersive, sequential simulation-based clinical ethics education program. METHODS: We conducted an explanatory sequential mixed-methods pre-post study with clerkship students at Kaohsiung Medical University from September 1, 2022, to May 31, 2023. Participants completed a core e-learning curriculum (digital modules/videos) before a two-day immersive simulation. The sim

Relevance of clinical ethics support services in specialized outpatient palliative care teams and inpatient hospices.

BACKGROUND AND OBJECTIVES: Clinical ethics support services (CES) may support palliative care teams in ethical conflicts. However, it is not known to what extent these services are available, used and how their relevance is perceived by the teams. The study focuses on the accessibility and evaluation of ethics consultations in specialized outpatient palliative care teams and inpatient hospices. METHODS: This study conducted a cross-sectional 21-item anonymous survey among teams of registered specialized outpatient palliative care services (n = 304) and inpatient hospices (n = 260) in Germany

Considerations raised during the regulatory and ethics review of platform clinical trials in infectious diseases.

BACKGROUND: Platform trials can address more than one clinical research question simultaneously and allow for the addition or removal of arms over time. The scientific and operational complexity of platform trials may give rise to specific challenges. In this study, we evaluate the considerations raised during the regulatory and ethics review of five infectious disease platform trials. METHODS: Considerations raised during the review of five platform trials (ECRAID-Prime, RECOVERY, RECLAIM, REMAP-CAP, and SNAP) between 31 January 2022 and 28 March 2025, and sponsor responses were analyzed usi

Clinical ethics dilemmas in a low-income setting - a national survey among physicians in Ethiopia.

BACKGROUND: Ethical dilemmas are part of medicine, but the type of challenges, the frequency of their occurrence and the nuances in the difficulties have not been systematically studied in low-income settings. The objective of this paper was to map out the ethical dilemmas from the perspective of Ethiopian physicians working in public hospitals. METHOD: A national survey of physicians from 49 public hospitals using stratified, multi-stage sampling was conducted in six of the 11 regions in Ethiopia. Descriptive statistics were used and the responses to the open-ended question "If you have expe

How can clinical ethics guide the management of comorbidities in the child with Rett syndrome?

Rett syndrome is a rare disorder caused by a mutation in the MECP2 gene. Those affected generally have severe functional impairments, and medical comorbidities such as scoliosis and poor growth are common. There is a paucity of information on the natural history of many rare disorders and an even greater deficit of evidence to guide best practice. The population-based and longitudinal Australian Rett Syndrome Database established in 1993 has supported investigations of the natural history of Rett syndrome and effectiveness of treatments. This paper reviews the disorder Rett syndrome and eviden

Ethics, emergencies and Ebola clinical trials: the role of governments and communities in offshored research.

The Ebola Virus Disease (EVD) in West Africa has stimulated investments in EVD research. While these research efforts are most welcome, we are concerned about the potential to ignore effective community ethics engagement programmes and critical government regulatory agencies in light of the urgency to conduct clinical trials for EVD therapies and vaccines. We discuss the reasons why community engagement with various research stakeholders is essential, how community engagement should be conducted, and the potential consequences of failing to engage both communities and regulatory agencies by dr
