Ethical issues in research and innovative therapy in children with mood disorders.

Research involving children with mood disorders should generally offer a reasonable prospect of benefit to those involved, unless the risks are minimal. While federal regulations require a prospect of direct benefit, from an ethical perspective indirect benefits, such as the advantages of being in a clinical trial, are relevant. Standard care should not be presumed to be preferable to treatment in the context of a clinical trial, particularly if it has not been evaluated in well designed studies involving groups comparable to the patient. Similarly, active treatments in a controlled trial shou

Researching domestic violence against women: methodological and ethical considerations.

The results of three population-based studies on violence against women in Nicaragua are compared in this article. Two of the studies were regional in scope (León and Managua) and focused specifically on women's experiences of violence, whereas the third study was a Demographic and Health Survey (DHS) conducted with a nationally representative sample of women. The lifetime prevalence estimates for women's undergoing physical violence from a partner were significantly higher in the León study (52 percent) and Managua study (69 percent), compared with that given in the DHS (28 percent). Possible

Pregnant women & vaccines against emerging epidemic threats: Ethics guidance for preparedness, research, and response.

Zika virus, influenza, and Ebola have called attention to the ways in which infectious disease outbreaks can severely - and at times uniquely - affect the health interests of pregnant women and their offspring. These examples also highlight the critical need to proactively consider pregnant women and their offspring in vaccine research and response efforts to combat emerging and re-emerging infectious diseases. Historically, pregnant women and their offspring have been largely excluded from research agendas and investment strategies for vaccines against epidemic threats, which in turn can lead

Ethics in AI Research: Emerging Governance Challenges for Institutional Review Boards in Health Systems.

Artificial intelligence (AI) is increasingly integrated into health research, creating ethical questions that extend beyond privacy, consent, bias, transparency, accountability, and safety. Existing bioethical principles and AI governance frameworks provide essential foundations but complex health-data ecosystems also raise institutional questions concerning reviewer capability, stewardship, reciprocity, authority, and downstream value. This Perspective proposes a stewardship-centered extension organized around four non-exhaustive governance domains: (1) IRB capability asymmetry; (2) data stew

Relational ethics meets principled practice in community research engagements to understand and address homelessness.

Growing homelessness in Aotearoa New Zealand stems primarily from rising inequalities and poverty. Drawing from scholarship on relational ethics, principled practice and Māori cultural concepts, this paper offers our reflections on nearly two decades of collective work to document and address homelessness. Central to the approach outlined are enduring community partnerships, the cultivation of reciprocal relations, and time spent with homeless people and those trying to work with them. We present exemplars for how we draw on everyday interactions with homeless people and agency staff to enhanc

Human Infection Challenge Studies: a Test for the Social Value Criterion of Research Ethics.

Human infection challenge studies involving the intentional infection of research participants with a disease-causing agent have recently been suggested as a means to speed up the search for a vaccine for the ongoing coronavirus disease 2019 (COVID-19) outbreak. Calls for challenge studies, however, rely on the expected social value of these studies. This value represents more than the simple possibility that a successful study will lead to the rapid development and dissemination of vaccines but also some expectation that this will actually occur. I show how this expectation may not be realist

A call for a streamlined ethics review process for multijurisdictional, child health research studies.

To be time and resource efficient in neonatal research and to answer clinically relevant questions with validity and generalizability, large numbers of infants from multiple hospitals need to be included. Multijurisdictional research in Canada is currently fraught with research ethics review process hurdles that lead to delays, administrative costs, and possibly termination of projects. We describe our experience applying for ethics review to 13 sites in 7 provinces for a project comparing two standard of care therapies for preterm born infants with respiratory distress syndrome. We welcome th

How strong is the administrative backbone of ethical governance? A descriptive cross-sectional study of health research ethics committees in Tanzania.

BACKGROUND: Health research ethics committees (HRECs) provide the administrative backbone of ethical governance by safeguarding the rights, safety and well-being of research participants and ensuring the ethical conduct of health research. Effective ethical oversight depends not only on rigorous protocol review but also on robust governance, competent personnel, adequate infrastructure, sustainable institutional support and efficient administrative systems. Despite their critical role, evidence on the administrative functionality of HRECs in Tanzania remains limited. OBJECTIVE: To assess the

Determining Ethical Incentives in Biomedical HIV Research: Insights from Researchers, Regulators, and People Living with HIV.

Incentives are crucial for recruiting and retaining participants in research studies, yet researchers and regulators often struggle to determine appropriate incentives due to ethical concerns and a lack of clear guidelines. We conducted interviews and focus groups with three stakeholder groups-biomedical HIV researchers (n = 12), institutional review board (IRB) members (n = 12), and people living with HIV (PLWH) (n = 69)-to explore their perspectives on incentives and ethical decision-making in biomedical HIV research. Our findings revealed disparities in perspectives; researchers and IRB mem

Perspectives and ethical considerations for return of genetics and genomics research results: a qualitative study of genomics researchers in Uganda.

BACKGROUND: The return of genetics and genomics research results has been a subject of ongoing global debate. Such feedback is ethically desirable to update participants on research findings particularly those deemed clinically significant. Although there is limited literature, debate continues in African on what constitutes appropriate practice regarding the return of results for genetics and genomics research. This study explored perspectives and ethical considerations of Ugandan genomics researchers regarding the return of genetics and genomics research results. METHODS: This was a qualita

Public involvement and engagement in scientific research and higher education: the only way is ethics?

BACKGROUND: Involving and engaging the public in scientific research and higher education is slowly becoming the norm for academic institutions in the United Kingdom and elsewhere. Driven by a wide range of stakeholders including regulators, funders, research policymakers and charities public involvement and public engagement are increasingly seen as essential in delivering open and transparent activity that is relevant and positively impacts on our society. It is obvious that any activities involving and engaging members of the public should be conducted safely and ethically. However, it is n

Clinical Research: A Review of Study Designs, Hypotheses, Errors, Sampling Types, Ethics, and Informed Consent.

Recently, we have been noticing an increase in the emergence and re-emergence of microbial infectious diseases. In the previous 100 years, there were several incidences of pandemics caused by different microbial species like the influenza virus, human immunodeficiency virus (HIV), dengue virus, severe acute respiratory syndrome Coronavirus (SARS-CoV), middle east respiratory syndrome coronavirus (MERS-CoV), and SARS-CoV-2 that were responsible for severe morbidity and mortality among humans. Moreover, non-communicable diseases, including malignancies, diabetes, heart, liver, kidney, and lung d

Research ethics during infectious disease outbreaks: A survey of African research stakeholders using the Ebola virus disease outbreak as a case.

Conducting research during disease outbreaks can be ethically challenging as evidenced in the 2014-2016 Ebola outbreak in West Africa and COVID-19 pandemic. Yet, there has been little empirical research conducted for understanding the views and perspectives of different stakeholders regarding ethical issues in conducting research during disease outbreaks. This preliminary study was conducted to empirically explore African public health research stakeholders' views about research ethics issues during infectious disease outbreaks in Africa. We conducted an online survey of 330 participants atten

Deliberate infection in research: ethical perspective of controlled human infection studies.

A controlled human infection study is the deliberate exposure of a healthy volunteer to an infectious pathogen under a strict controlled environment. Although there has been noticeable contribution of such studies to the biomedical research involving understanding of disease pathogen interaction, vaccine development, etc, they are in direct contradiction of core ethical principles such as non-maleficence. The major ethical dilemma lies in the deliberate exposure without certain direct benefits. This narrative review discusses the ethical challenges and need for robust ethical framework taking

Ethical and social difficulties of addressing environmental harms related to health research.

Health research is vital to advance human well-being, but it is also a contributor to climate change and other environmental degradation. A growing bottom-up advocacy movement is engaged in developing measures (often called tools) to help researchers better understand the ways in which they can mitigate the environmental harms associated with research. While some evidence suggests benefits of using these tools, ethical and social challenges remain. These challenges include questions about: whether these tools will place undue burdens on researchers; whether the tools will be effective in suppo

Ethical Issues Conducting Research During War and Violent Conflict: A Review.

Global health engagement increasingly occurs in the context of war and violent conflict. Many health-related decisions are made which should be guided by evidence. Health research is needed to provide evidence, yet conducting research in conflict settings raises multiple ethical issues. However, the ethical issues involved in conducting research during war and violent conflict have received insufficient attention. The literature suggests that these ethical issues, while similar to those of research conducted in humanitarian crises, are also distinct. This article provides a review of the ethic

The Return of Biomarker Results in Research: Balancing Complexity, Precision, and Ethical Responsibility.

Recent research aimed at the discovery, integration, and communication of health outcome measures (or "biomarkers") in Alzheimer's disease has raised challenging questions related to whether, how and when results from these investigations should be disclosed to research participants. Reflecting the apparent heterogeneity of many neurodegenerative diseases, biomarker or other risk factor results are often probabilistic, interactive, multi-modal, and selective. Such characteristics make it very complex to summarize and communicate to clinicians, researchers, and research participants. Whereas th
