BET 2: Tubular bandage, plaster or aircast boot for avulsion fractures of the base of the fifth metatarsal?

A shortcut review was carried out to investigate whether avulsion fractures of the base of the fifth metatarsal were best treated with tubular bandage, a walking boot or a short leg plaster cast. One paper presented the only evidence to answer one of the clinical questions (plaster cast or walking boot) but no evidence was found comparing tubular bandage and a walking boot. The author, date and country of publication, patient group studied, study type, relevant outcomes, results and study weaknesses of this paper are tabulated. It is concluded that while the only evidence available shows margi

Timing of surgical intervention in cauda equina syndrome: a systematic critical review.

OBJECTIVE: Cauda equina syndrome (CES) is a rare but important neurosurgical emergency. Despite being a recognized clinical entity since 1934, there remains significant uncertainty in the literature regarding the urgency for surgical intervention. The past decade has seen the emergence of the much-referred-to 48-hour limit as a possible window of safety. The ramifications of this time point are significant for early patients who may subsequently have urgent treatment delayed, and for litigation cases, after which adverse decisions are more likely to occur. METHODS: A systematic principally qu

BET 1: Intravenous tranexamic acid in the treatment of acute epistaxis.

A short-cut review was carried out to establish whether intravenous tranexamic acid is beneficial in managing acute epistaxis. Seven papers were found in Medline, Embase and the Cochrane Library using the reported searches, but none presented any evidence to answer the clinical question. It is concluded that there is no evidence to support or refute the use of intravenous tranexamic acid in acute epistaxis and that local advice should be followed.

BET 1: bedside ultrasound for the diagnosis of necrotising fasciitis.

A short-cut review was carried out to establish whether bedside ultrasound was accurate at diagnosing necrotising fasciitis in patients with limb infections. A total of 187 papers were found using the reported searches, of which one presented the best evidence to answer the clinical question. The author, date and country of publication, patient group studied, study type, relevant outcomes, results and study weaknesses of this paper is tabulated. It is concluded that there is limited evidence supporting the use of bedside ultrasound as an adjunct diagnostic tool in differentiating cellulitis fr

[Hyperbaric oxygen therapy for neurologic emergency and neurosurgical diseases: a systematic review of the literature].

I have reviewed reports concerning neurologic emergency and neurosurgical diseases adjunctively treated with hyperbaric oxygen (HBO) therapy. Some clinical studies indicate the favorable effects of HBO therapy on the events at an ultra-acute stage of brain ischemia. This therapy has been used in the treatments of other neurologic emergency diseases, such as brain and spinal injury, carbon monoxide intoxication, decompression illness involving the central nervous system and so on. Some reports clarify that patients with malignant gliomas have a prolonged survival by radiotherapy after HBO thera

Evidence-based suction management in accident and emergency: a vital component of airway care.

Effective airway management aims to establish and maintain a patent airway to ensure adequate alveolar ventilation and patient survival. Suction therapy must be regarded as a vital component of airway care, with all staff who are required to perform this procedure being aware of the principles of safe, effective suctioning. This paper provides an outline of the types of suction catheters available for use in the emergency care setting, and describes the safe procedures to be followed for both endotracheal and oro-pharyngeal suctioning. Potential complications are discussed other headings of re

Effectiveness of Implementing Evidence-based Interventions to Reduce C-spine Image Ordering in the Emergency Department: A Systematic Review.

OBJECTIVES: Appropriate use of imaging for adult patients with cervical spine (C-spine) injuries in the emergency department (ED) is a longstanding issue. Guidance for C-spine ordering exists; however, the effectiveness of the decision support implementation in the ED is not well studied. This systematic review examines the implementation and effectiveness of evidence-based interventions aimed at reducing C-spine imaging in adults presenting to the ED with neck trauma. METHODS: Six electronic databases and the gray literature were searched. Comparative intervention studies were eligible for i

[Hyperbaric therapy and diving medicine - diving medicine - present state and prospects].

The diving accident (decompression incident, DCI) occurs in the decompression phase of dives. The DCI can either be caused by an arterial gas embolism (AGE) subsequent to a pulmonary barotrauma or by the formation of inert gas bubbles subsequent to a reduction of ambient pressure during the ascent from depth. In contrast to the traditional assumption that decompression incidents only occur if decompression rules are neglected, recent data indicate that a vast amount of diving accidents occur even though divers adhered to the rules. Hence, there is a large inter- and intraindividual variability

Using clinical pathways to aid in the diagnosis of necrotizing soft tissue infections synthesis of evidence.

BACKGROUND: Necrotizing soft tissue infections (NSTIs) are rare, rapidly spreading infections that occur in the soft tissue compartments. The mortality rate is high and has been found to decrease if patients are treated early and aggressively with surgical debridement and broad-spectrum antibiotics. Unfortunately these infections present similarly to other types of skin and soft tissue infections (SSTIs) making diagnosis difficult. AIMS: This paper reviews the evidence surrounding the early diagnosis of NSTIs. This was used to develop a clinical practice guideline (CPG) for implementation in

BET 1: Does inhaled tranexamic acid reduce morbidity in adults with haemoptysis?

A short cut review was carried out to establish whether inhaled tranexamic acid is more effective than placebo at controlling bleeding in patients with haemoptysis. Thirty-four papers were found using the reported searches, of which one presented the best available evidence to answer the clinical question. The author, date and country of publication, patient group studied, study type, relevant outcomes, results and study weaknesses of this paper is tabulated. It is concluded that in patients with non-massive haemoptysis, management with nebulised TXA leads to fast resolution.

Research Gaps in Wilderness Medicine.

INTRODUCTION: Wilderness medicine involves the treatment of individuals in remote, austere environments. Given the high potential for injuries as well as the unique treatment modalities required in wilderness medicine, evidence-based clinical practice guidelines are necessary to provide optimal care. In this study, we identify evidence gaps from low-quality recommendations in wilderness medicine clinical practice guidelines and identify new/ongoing research addressing them. METHODS: We included relevant clinical practice guidelines from the Wilderness Medical Society and obtained all 1C or 2C

How do I find a point-of-care answer to my clinical question?

Emergency physicians often need point-of-care access to current, valid information to guide patient management. Most emergency physicians do not work in a hospital with a computerized decision support system that prompts and provides them with information to answer their clinical questions. Searching for answers to clinical questions online, especially those related to diagnosis and treatment, can be challenging, in part because determining the validity and clinical applicability of the results of individual studies is beyond the time constraints of most emergency physicians. This article desc
